Prevalence and evaluation of oropharyngeal dysphagia in patients with severe acute respiratory syndrome coronavirus 2 infection in the intensive care unit

Objective
The main objective was to assess the prevalence of dysphagia in the intensive care unit in patients with coronavirus disease 2019.
Methods. A cohort, observational, retrospective study was conducted of patients admitted to the intensive care unit for severe acute respiratory syndrome coronavirus 2 pneumonia at the University Hospital of Rouen in France.
ResultsOver 4 months, 58 patients were intubated and ventilated, 43 of whom were evaluated. Screening revealed post-extubation dysphagia in 62.7 per cent of patients. In univariate analysis, a significant association was found between the presence of dysphagia and: the severity of the initial pathology, the duration of intubation, the duration of curare use, the degree of muscle weakness and the severity indicated on the initial scan. At the end of intensive care unit treatment, 22 per cent of the dysphagic patients had a normal diet, 56 per cent had an adapted diet and 22 per cent still received exclusive tube feeding.
ConclusionPost-extubation dysphagia is frequent and needs to be investigated.

Introduction
Medication non-adherence is a significant risk factor for rehospitalisation in schizophrenia patients. Delayed release formulations like R-LAI may reduce rehospitalisation.
ObjectivesTo examine the association between R-LAI use and hospitalisation in schizophrenia patients.
AimsTo assess the effect of R-LAI, compared to non-use and use of other antipsychotic drugs, on the risk of hospitalization in real-life settings.
MethodThe CGS study recruited schizophrenia patients from 177 public and private wards of psychiatric hospitals across France. Inclusion criteria were schizophrenia (DSM-IV), age 15–65 years, ambulatory/hospitalised for < 93 days at entry. Patients were followed up to 12 months for antipsychotic use and hospitalisation. The recruitment was stratified for long-acting second generation antipsychotic use. Multivariate Poisson regression adjusted for confounding with propensity scores and allowing for autocorrelation was used to estimate relative rates of hospitalisation.
ResultsOf 2092 eligible patients, 1859 were included. Their mean age was 38.1 ± 11.1 years, 68.6% were male and 37.8% were hospitalised for < 93 days at entry. A total of 1659 patients (89.2%) were followed up for 12 months, accumulating 933 hospital stays (53.0 per 100 person-years). Compared to other schizophrenia patients, patients on R-LAI were younger, had more often a history of previous hospitalisation for equivalent severity, living conditions and other characteristics. The adjusted relative rate of hospitalisation for R-LAI use against non-use was 0.66 [95% CI 0.46–0.96], and 0.53 [95% CI 0.32–0.88] against long-acting first generation antipsychotics.
ConclusionsUse of R-LAI was associated with lower rates of hospitalization compared to non-use of R-LAI.

Contexte et objectif
La non-observance des traitements représente un risque d’exacerbation chez les patients schizophrènes [1,2,3]. L’objectif était d’évaluer l’impact d’un programme d’accompagnement des patients recevant la rispéridone à libération prolongée (RLP) sur le taux d’hospitalisation psychiatrique.
MéthodesUne cohorte de 506 patients schizophrènes traités par RLP a été recrutée dans 36 centres et suivie 12 mois. Le programme d’accompagnement consistait en un rappel téléphonique 48 heures avant la date prévue de l’injection et dans les trois jours suivants, au besoin. Les centres observants étaient ceux ayant appliqué le programme pour 50 % ou plus des injections prévues. Les patients observants étaient ceux qui s’étaient présentés dans les cinq jours de la date prévue de l’injection pour au moins 80 % des injections. L’effet de l’observance (centre, patient, ou les deux combinés) sur le taux d’hospitalisation a été estimé par un modèle de Poisson avec appariement, ajusté sur un score de propension résumant les facteurs de risque.
RésultatsEnviron 75 % des patients ont été observants et leur taux d’hospitalisation était similaire aux non-observants (32,8 et 31,7 par 100 patients-années [PA], respectivement). Les centres observants traitaient des patients moins sévères (hospitalisation dans l’année précédente [64,2 vs. 44,1 %] et meilleurs scores sur les échelles psychiatriques [CGI maximum, BPRS, EGF]). L’observance des centres avait un effet moins important sur le taux d’hospitalisation des patients observants (28,2/100 PA pour les centres observants vs 38,3/100 PA pour les centres non observants) que chez les patients non observants (11,7/100 PA vs 41,0/100 PA, respectivement). L’effet chez les patients non observants était maintenu dans les analyses ajustées.
ConclusionLe taux d’hospitalisation chez les patients traités par RLP était comparable entre les patients observants et non observants, mais ce taux était amélioré dans les centres observants au programme d’accompagnement des patients à observance faible.

Cardiac output measurements in off-pump coronary surgery: comparison between NICO and the Swan-Ganz catheter

Summary
Background: The aim of this prospective study was to compare continuous cardiac output measurements of the non-invasive cardiac output system (NICO) with the pulmonary artery catheter during off-pump coronary bypass surgery. Methods: Twenty-two patients enrolled for off-pump coronary surgery received both a pulmonary artery catheter and a non-invasive cardiac output system for measurement of cardiac output. Data were compared by the Bland–Altman method to calculate the degree of agreement and to analyse if a significant difference existed between the two methods of cardiac output measurements. Results: Perioperatively, the non-invasive cardiac output underestimated cardiac output, but postoperatively overestimated it. The limits of agreement were larger during surgery compared to the postoperative period (−3.1; +2.5 vs. −1.4; +2.2 L min−1). Perioperatively, cardiac output measured with the pulmonary artery catheter varied from 0.5 to 7.5 L min−1 (mean 3.6 L min−1) and with the non-invasive cardiac output from 0.5 to 8.4 L min−1 (mean 3.9 L min−1). Postoperatively, these were 2.5–7.7 L min−1 (mean 4.5 L min−1) and 2.3–8.4 L min−1 (mean 4.9 L min−1), respectively. Conclusion: During off-pump cardiac surgery, the non-invasive cardiac output reliably measures cardiac output and does it more rapidly than a pulmonary artery catheter and may be more useful in order to detect rapid haemodynamic changes.
