Effect of ethnicity on care pathway and outcomes in patients hospitalized with influenza A(H1N1)pdm09 in the UK

Data were extracted from the case records of UK patients admitted with laboratory-confirmed influenza A(H1N1)pdm09. White and non-White patients were characterized by age, sex, socioeconomic status, pandemic wave and indicators of pre-morbid health status. Logistic regression examined differences by ethnicity in patient characteristics, care pathway and clinical outcomes; multivariable models controlled for potential confounders. Whites (n = 630) and non-Whites (n = 510) differed by age, socioeconomic status, pandemic wave of admission, pregnancy, recorded obesity, previous and current smoking, and presence of chronic obstructive pulmonary disease. After adjustment for a priori confounders non-Whites were less likely to have received pre-admission antibiotics [adjusted odds ratio (aOR) 0·43, 95% confidence interval (CI) 0·28–0·68, P < 0·001) but more likely to receive antiviral drugs as in-patients (aOR 1·53, 95% CI 1·08–2·18, P = 0·018). However, there were no significant differences by ethnicity in delayed admission, severity at presentation for admission, or likelihood of severe outcome.

Immunity to attenuated influenza virus WRL 105 infection induced by heterologous, inactivated influenza A virus vaccines

Groups of student volunteers were immunized with one of five different inactivated influenza virus vaccines. The concentration of virus in the various vaccines differed by both the international unitage test and by the concentration of haemagglutinin, as measured by the single radial diffusion test; the results of the two methods of standardization showed no correlation. The serum HI response to immunization was variable; volunteers given A/England/72 showed a 16·6-fold increase in homologous serum antibody titre whilst volunteers given A/Hong Kong/68 vaccine showed a 4·2-fold increase. The variable response of volunteers to immunization could not be explained by the varied concentration of virus in the vaccines, as measured by either test, the titres of serum HI antibody present before immunization, or a combination of these two factors.
The ability to infect volunteers with WRL 105 virus 4 weeks after immunization with heterologous, inactivated virus vaccine was directly related to the degree of cross-reactivity between the haemagglutinins of this vaccine virus and WRL 105 virus. Thus, the greatest number of infections by the challenge virus were seen in volunteers given A/Hong Kong/68 vaccine, less were observed in volunteers given A/England/72 vaccine, and least were found in groups given A/Port Chalmers/73 or A/Scotland/74 vaccine. However, compared with the incidence of infection in volunteers given B/Hong Kong/73 vaccine, all the heterologous influenza A vaccine gave some immunity to challenge infection.

This article examines the experimental and clinical evidence of the effect of growth factor signalling in an attempt to reconcile the seeming paradox of, on the one hand, evidence that growth factor signalling can facilitate the activity of oestrogen receptor-α (ER-α), and on the other hand, evidence that extreme growth factor signalling can promote loss of ER function and expression, thereby promoting an endocrine insensitive and ultimately ER− phenotype. The results of this analysis lead to the therapeutic possibility that ER negativity may, in some instances, be reversible to generate endocrine response and improve patient prognosis.

The electronic structure and bonding character of intermetallic alloys are investigated by a combination of electron energy loss spectroscopy (EELS) experiments and ab initio electronic structure calculations. A detailed comparison is made between experimental spectra and calculations. The changes in electronic structure within a transition metal alurninide series and also due to alloying are studied using EELS spectra. The Korringa-Kohn-Rostoker coherent-potential-approximation method and large supercell models are used to investigate changes in composition and the effect of dopants on the electronic structure.

We show that the Kubo formula can be used to calculate the non-local electrical conductivity of layered systems from first principles. We use the Layer Korringa Kohn Rostoker method to calculate the electronic structure and the Green function of Co/Cu/Co trilayers within the local density approximation to density functional theory. This Green function is used to calculate the conductivity through the Kubo formula for both majority and minority spins and for alignment and anti-alignment of the Co moments on either side of the Cu spacer layer. This allows us to determine the giant magnetoresistance from first principles. We investigate three possibilities for the scattering in Co/Cu/Co: (1) equal electron lifetimes for Cu, majority spin Co, and minority spin Co, (2) equal electron lifetimes for majority and minority Co, weaker scattering in Cu and spin dependent interfacial scattering, (3) electron lifetimes for majority and minority spin cobalt proportional to their Fermi energy densities of states and spin dependent interfacial scattering.

We have performed calculations of the electronic structure of the random substitutional bcc Fe1-xCrx alloys, using the spin-polarized, self-consistent Korringa, Kohn and Rostoker coherent potential approximation (KKR-CPA) method. This is a first principles method based on a local spin density approximation for electron exchange and correlation energy. For the iron-rich alloys, we find that the average moment decreases linearly with Cr concentration, although the individual moments show a different concentration dependence and the Cr moment is anti-parallel to the Fe moment. This system is similar to Fe1-xVx system, although some details are different.

It has been established that the coherent potential approximation can successfully describe the energy of random alloys [1]. It has also served as the basis of generalized perturbation method [2] and concentration wave [3] xscalculations of the energy of short range ordered alloys. The multisublattice coherent potential, (MCPA) is the natural extension of the CPA with which to address long range order (LRO). Using the recently developed multisublattice coherent potential approximation Korringa Kohn Rostoker [4], (MCPA-KKR) code the elgenvalue sum can be calculated as a function of LRO. This allows the evaluation of the ordering energy by either of two approximations. The frozen potential approximation (FPA) [5] assumes that the muffintin single site potentials do not change as the long range order is varied; the Harris Approximation, (HA) [6], as applied in this work, assumes that the single site charge densities do not change as the long range order is changed. These two methods of calculating the ordering energy will be compared with each other and to experiment for several systems including CuZn, NiAl, and NiAl with zinc additions.

The Deep Space Network (DSN) [operated by JPL under contract to the National Aeronautics and Space Administration] is implementing a Very Long Baseline Interferometry (VLBI) capability at DSS 63 (Spain), DSS 14 (California, USA), and DSS 43 (Australia) to support the navigation requirements of planetary space missions. The early development work for this system has already demonstrated the capability of measuring UT1 with a formal accuracy as low as 0.6 msec with only 6 hours of data. Further, a radio astrometric catalog of approximately 45 sources whose positions are known to better than
has been constructed. In addition to these measurements, this paper describes the characteristics and anticipated performance of the complete VLBI system being implemented within the DSN for operational use in mid-1979. In particular, one of the capabilities of this system will be the measurement of UT1 and polar motion at weekly intervals. Although the navigation accuracy requirement is only 50 cm for the Voyager mission, this system should be capable of delivering UT1 and polar motion determinations with decimeter accuracy if it is operated at maximum performance. An additional requirement of this operational system is that it have the capability of providing these results within 24 hours of the actual observations.
Some effects of unsaturated oils given to dairy cows with rations of different roughage content

Three experiments were conducted to determine the effect of feeding polyunsaturated fish oils to dairy cows receiving rations of high, medium or low roughage content. In the rumen, the decreases in the proportion of acetic acid and increases in propionic acid induced by the oils became greater as the amount of oil given was increased but the magnitude of the response to any dose depended upon the composition of the basal diet. The effects on volatile fatty acids (VFA) proportions of small amounts of the oils (125–150 ml/day) were greatest with the low-roughage rations. With large doses of oil (375–450 ml/day) the responses were variable but it is concluded that, in general, changes in VFA proportions are least with low-roughage rations.
The fat content of milk was more sensitive to dietary oil supplementation than were the rumen VFA proportions. When the unsaturated oils were given there was a decrease in milk fat percentage and an increase in the proportion of unsaturated fatty acids in the fat; there was also increased incorporation of fatty acids with more than 18 carbon atoms in the milk fat. The metabolism in the rumen tended to become adapted to the feeding of 150 ml/day of oil, the VFA pattern returning during the second and third week of supplementation to that observed before the addition of oil.

The amounts of total lipid and fatty acids consumed, leaving the stomach and excreted in the faeces were examined in 4 sheep fitted with rumen and re-entrant duodenal cannulas. Diets of high (HM1) and low (CM1) roughage content were given at 0·9 times maintenance and the low-roughage diet was also given at 1·7 and 2·3 times maintenance. With all the rations more fatty acid left the abomasum than was consumed in the food, the difference being greater on ration CM1 than on ration HM1 and increasing irregularly with the amount of the low-roughage ration fed. Of the fatty acid entering the duodenum in the chyme, 72–89% was digested in the intestine. About 80% of the increase in fatty acids in the stomach was stearic acid and most of the remainder was palmitic acid. Almost all the polyunsaturated C18 acids ingested in the food were hydrogenated in the stomach, and the amounts of oleic acid were also greatly reduced, although more oleic acid entered the duodenum in the chyme with ration CM1 than with ration HM1. The possible origins of the increase in fatty acids in the stomach are discussed.
The effect of diet composition and level of feeding on digestion in the stomach and intestines of sheep

1. Four wether sheep, each fitted with re-entrant duodenal cannulas and a rumen cannula, were used to determine the relative quantitative importance of digestion in the stomach compared with that in the intestines when diets of low (HM1) and high starch (CM1) content were fed at 0.9 times maintenance and when the high-starch diet was fed at 1.7 (CM2) and 2.3 (CM3) times maintenance. Paper, impregnated with chromic oxide, and polyethylene glycol (PEG) were administered.
2. An apparatus is described which allowed one operator to collect, record, sample and return the duodenal digesta from two sheep simultaneously.
3. Total digestion was determined by faeces collections over 7–10 day periods, and digestion in the stomach by measuring the total flow of digesta from the abomasum to the duodenum over two 24 h periods with two or three sheep on each ration. Flow values were adjusted to give 100% recovery of chromic oxide.
4. The recoveries of chromic oxide and PEG were similar and it was concluded that either marker was satisfactory for adjusting 24 h flow values. Disadvantages of using PEG are discussed.
5. There was relatively little difference between rations HM1 and CM1 in the proportion of digestible organic matter and energy digested in the stomach. Only 57.1% of the total dry-matter digestion occurred in the stomach for ration HM1 compared with 65.0% for ration CM1 reflecting a smaller net gain in ash for ration CM1. Doubling the level of intake (ration CM2) resulted in a decrease in the proportion of digestible dry matter, organic matter and energy digested in the stomach. When the level was increased still further with ration CM3, the proportion continued to decrease with one sheep but increased with the other.
6. All but 5–11% of the digestible starch (measured as α-linked glucose polymer) was fermented in the stomach, the lowest proportion being with ration HM1.
7. The amount of nitrogen reaching the duodenum was approximately equal to the amount ingested with rations HM1 and CM1 but considerably greater with the other two rations. All rations contained approximately 2% nitrogen.
8. Calculation of the estimated loss of energy as methane and heat of fermentation suggested that 50–54% of the digestible energy was absorbed as volatile fatty acids.
