Parallel processing is a way to use resources efficiently by processing several jobs simultaneously on different servers. In a well-controlled environment where the status of the servers and the jobs are well known, everything is nearly deterministic and replicating jobs on different servers is obviously a waste of resources. However, in a poorly controlled environment where the servers are unreliable and/or their capacity is highly variable, it is desirable to design a system that is robust in the sense that it is not affected by the poorly performing servers. By replicating jobs and assigning them to several different servers simultaneously, we not only achieve robustness but we can also make the system more efficient under certain conditions so that the jobs are processed at a faster rate overall. In this paper we consider the option of replicating jobs and study how the performance of different ‘degrees’ of replication, ranging from no replication to full replication, affects the performance of a system of parallel servers.

Purpose: To report a case of adverse reaction to nicotine gum in a Chinese female with poor nicotine metabolic status after chewing a piece of 2 mg nicotine gum (NG). Background: NG is prescribed in smoking cessation programs to help individuals stop smoking. In smoking cessation treatment, the dose of NG should control withdrawal symptoms but avoid adverse effects. Case results: We describe a case of an adverse reaction following the administration of 2 mg NG to a female Chinese subject. This study compared a single dose of 2 mg NG with a single low dose cigarette (0.7 mg nicotine). Blood (5mL) was collected at baseline and 2h post exposure to NG. Blood pressure and pulse rate were also recorded. Nicotine metabolic rate was determined by calculation of the ratio of plasma cotinine (ng/mL)/plasma nicotine (ng/mL). The subject developed nausea and severe vomiting with raised blood pressure and pulse rate following administration of NG. The ratio of plasma cotinine/nicotine was 0.4 indicative of poor nicotine metabolic rate. Exposure to nicotine using the subject's normal cigarette did not produce similar adverse effects. Conclusion: NG may be associated with adverse reactions in smokers with low nicotine metabolic rate. Physicians in smoking cessation programs should be aware that some individuals may not tolerate this form of pharmacological treatment.

Summary
Summary Much of the drug testing available today is able to determine the presence or absence of a variety of psychoactive substances in a range of body fluids and tissues. For the results of such tests to be confidently interpreted, additional information is required, including general assessment and history-taking. In a wide range of large psychiatric surveys, substance dependence emerges as one of the most common mental health-related disorders, and it is also the one that is least likely to be treated. The range of available tests can be best considered as acting to support and complement a broader assessment and diagnostic procedure.
The aim of this chapter is to outline the variety of laboratory investigations available that could be considered as biological indicators of substance use and misuse. We will review the strengths and weaknesses of different approaches and different body matrices (urine, blood/plasma monitoring, oral fluid and hair testing), along with other possible materials not usually considered, with a focus on the clinical usefulness of these procedures.
A drug may be detected in any body fluid or tissue, but there are practical limitations to the extent to which samples can be and are used, and the mechanism of collection and supervision of samples are critical to the procedure.
Chain of custody
The procedure of ensuring that an identified sample was provided by a specified individual (chain of custody), and has subsequently been correctly labelled to ensure accuracy, must be properly documented. In the case of urine samples, chain of custody procedures usually require the collection to be witnessed by a designated member of staff (clinician, nurse, drug worker, therapist, etc.) and written confirmation of its validity from the individual voiding the sample, as detailed in Box 11.1 (Wolff et al, 1999a).
Box 11.1 Chain of custody procedures for urine samples
• Before, during and after urination, collection site personnel must keep the urine specimen in sight
• The urine container must be tightly capped, properly sealed and labelled with the patient/client's name and the time and date of collection
• To maintain accountability the approved chain of custody form must be used and it must accompany the sample from initial collection to final disposition
• Reliable transportation (courier system) to the laboratory must be used

Much of the drug testing available today is able to determine the presence or absence of a variety of psychoactive substances in a range of body fluids and tissues. For the results of such tests to be confidently interpreted, additional information is required, including general assessment and history-taking. In a wide range of large psychiatric surveys, substance dependence emerges as one of the most common mental health-related disorders, and it is also the one that is least likely to be treated. The range of available tests can be best considered as acting to support and complement a broader assessment and diagnostic procedure.
