Summary
This chapter introduces the multiphase optimization strategy (MOST), an engineering-inspired framework for optimizing multicomponent behavioral interventions. The chapter contrasts the typical development of interventions with the MOST framework and discusses the potential to achieve steady, systematic progress in intervention science. In contrast to the typical development of a multicomponent intervention wherein the intervention is tested en bloc via a randomized controlled trial (RCT), MOST introduces a phase of optimization prior to the RCT wherein the effect of individual components and the interaction between components is empirically tested. The objective of MOST is to arrive at an optimized intervention that is highly effective, but also efficient, economical, and scalable. This chapter provides an overview of the MOST framework and demonstrates its application using a hypothetical example of the optimization of a clinical intervention targeting posttraumatic stress disorder symptoms among children who have experienced maltreatment.

OBJECTIVES/SPECIFIC AIMS: The goal of this study is to develop an effective and efficient STI preventive intervention among college students following the principles and phases of MOST. METHODS/STUDY POPULATION As part of the preparation phase, an explicit conceptual model, drawing heavily on theory and prior research, was used to translate the existing science into 5 candidate intervention components (ie, descriptive norms, injunctive norms, expectancies, perceived benefits of protective behavioral strategies, and self-efficacy). For the optimization phase, in Fall 2016 all first-year students (n=3547) from 4 universities were recruited to participate. Students were randomized to 1 of 32 different experimental conditions that included a combination of the candidate intervention components. Component effectiveness was evaluated using data from an immediate post-intervention survey on respective component mediators (eg, alcohol and sex-related descriptive norms). After a second factorial experiment (Fall 2017), only those intervention components that meet the pre-specified criteria of day ≥0.15 will be included in the optimized intervention. The evaluation phase will evaluate the effectiveness of the optimized STI preventive intervention via a randomized-control trial (Fall 2018). RESULTS/ANTICIPATED RESULTS: Preliminary results from the first factorial experiment suggest that descriptive norms and injunctive norms intervention components were significantly effective in reducing post-intervention perceived alcohol prevalence (β=−0.28, p<0.001) and approval of alcohol (β=−0.33, p<0.001), and sex-related norms (β=−0.23, p<.001). These results, in combination with process data, are being used to inform revisions of the intervention components to be included in a second factorial screening experiment. DISCUSSION/SIGNIFICANCE OF IMPACT: This study demonstrates how an iterative approach to engineering an STI preventive intervention using MOST can affect the behaviors of college students and serve as a foundation for other translational science.

Summary
The work described in this chapter comes from a perspective rooted in psychological research, where there is considerable interest in characteristics of the individual that cannot be directly measured, such as intelligence, abilities in various domains, attitudes, and complex private behaviors. The focus is on using manifest variables, usually subject self-reports or ratings by trained observers, to measure latent variables representing these unmeasured characteristics of individuals. In this chapter, we will discuss the use of latent class models to measure change over time in latent variables.
We will begin by drawing a conceptually important distinction between static and dynamic latent variables (Collins, 1996). Static latent variables are not expected to change over time, or else the change is not of interest in a particular study, as when group differences at a particular time are examined. In contrast, dynamic latent variables do change in systematic and important ways over time. Often the same variable may be thought of as static in one context and dynamic in another, depending on the objectives and interests of a particular research project. For example, drug use in eighth grade, cognitive ability in an elderly population, and gender role identification are all static latent variables. Change in drug use from seventh to ninth grade, age-related decline in cognitive ability, and gender role development throughout high school are all dynamic latent variables.
Growth in continuous dynamic variables proceeds in a strictly quantitative fashion over time.

According to the Gateway Hypothesis, in some cases one substance serves as a gateway for another, more advanced substance. In other words, it is necessary to go through this gateway in order to reach the more advanced substance. In a series of studies (Kandel & Faust, 1975; Yamaguchi & Kandel, 1984a, 1984b; Kandel, 1988; Kandel & Yamaguchi, 1993), Kandel and colleagues have examined the Gateway Hypothesis in detail. In general, these researchers have found that the legal drugs alcohol and tobacco appear to act as gateways for more advanced substances; alcohol plays a somewhat more important role for men and cigarettes play a somewhat more important role for women. Marijuana comes next, followed by cocaine and crack. Collins and colleagues (Graham, Collins, Wugalter, Chung, & Hansen, 1991; Collins, Graham, Long, & Hansen, 1994; Collins, Graham, Rousculp, & Hansen, 1997; Collins, Hyatt, & Graham, 2000; Hyatt & Collins, 2000) have examined the very early part of the onset process, looking at a sample of junior high and high school students. They have found that the onset process usually starts with alcohol; individuals go on either to try tobacco and then have a first experience with drunkenness or to have a first experience with drunkenness followed by trying tobacco. However, a small subset of individuals start their substance use experience with tobacco, go on to try alcohol, and then have a first experience with drunkenness.
