Current evidence underscores a need to transform how we do clinical research, shifting from academic-driven priorities to co-led community partnership focused programs, accessible and relevant career pathway programs that expand opportunities for career development, and design of trainings and practices to develop cultural competence among research teams. Failures of equitable research translation contribute to health disparities. Drivers of this failed translation include lack of diversity in both researchers and participants, lack of alignment between research institutions and the communities they serve, and lack of attention to structural sources of inequity and drivers of mistrust for science and research. The Duke University Research Equity and Diversity Initiative (READI) is a program designed to better align clinical research programs with community health priorities through community engagement. Organized around three specific aims, READI-supported programs targeting increased workforce diversity, workforce training in community engagement and cultural competence, inclusive research engagement principles, and development of trustworthy partnerships.
High workforce turbulence has plagued clinical research, becoming intensified during the COVID-19 pandemic, especially for patient-facing workers. In a time of great uncertainty and risk among healthcare workers, researchers included, the pandemic also brought increased demand for research studies in volume, speed, and complexity, triggering elevated staff turnover. This has posed significant hurdles for employers, especially research sites, where retaining skilled patient-facing clinical research professionals (CRPs) is pivotal for sustaining medical innovation. Lack of job standardization and advancement pathways has been noted to play an important role both in turnover and contributes to the inability to accurately measure workforce trends. To address these factors, Duke University adopted a competency-based job classification system for CRPs in 2016.
Methods:
Since that adoption of competency-based jobs, employee-level staffing data for all CRPs have been tracked monthly, creating a master data file from September 2016 through June 2024. This study updates previous analyses, evaluating turnover and turbulence rates, and demographic changes in the CRP workforce over this period.
Results:
Over the last six years, the Duke CRP workforce remained relatively stable. Voluntary turnover rates fluctuated, peaking at 19.1% in FY 2021 during the COVID-19 pandemic, and have steadily declined each year since then.
Conclusions:
Despite national workforce challenges exacerbated by the pandemic, our data indicate that proactive measures to standardize clinical research jobs and assess the resultant well-defined site-based employee data may have mitigated extremes in workforce turnover at Duke University. Turbulence rates, while stabilizing, signal areas for further study.
This Element, written for researchers and students in philosophy and the behavioral sciences, reviews and critically assesses extant work on number concepts in developmental psychology and cognitive science. It has four main aims. First, it characterizes the core commitments of mainstream number cognition research, including the commitment to representationalism, the hypothesis that there exist certain number-specific cognitive systems, and the key milestones in the development of number cognition. Second, it provides a taxonomy of influential views within mainstream number cognition research, along with the central challenges these views face. Third, it identifies and critically assesses a series of core philosophical assumptions often adopted by number cognition researchers. Finally, the Element articulates and defends a novel version of pluralism about number concepts.
A survey of academic medical-center hospital epidemiologists indicated substantial deviation from Centers for Disease Control and Prevention guidance regarding healthcare providers (HCPs) recovering from coronavirus disease 2019 (COVID-19) returning to work. Many hospitals continue to operate under contingency status and have HCPs return to work earlier than recommended.
To assess the impact of a 24-hour autocancellation of uncollected Clostridioides difficile samples in reducing reported healthcare-associated infections (HAIs).
The study was conducted in 17 hospitals in Pennsylvania.
Interventions:
Clostridioides difficile tests that are not collected within 24 hours are automatically canceled (“autocancel”) through the electronic health record. The intervention took place at 2 facilities (intervention period November 2021–July 2022) and subsequently at 15 additional facilities (April 2022–July 2022). Quality measures included percentage of orders canceled, C. difficile HAI rate, percent positivity of completed tests, and potential adverse outcomes of canceled or delayed testing.
Results:
Of 6,101 orders, 1,090 (17.9%) were automatically canceled after not being collected for 24 hours during the intervention periods. The reported C. difficile HAI rates per 10,000 patient days did not significantly change. These rates were 8.07 in the 6-month preintervention period and 8.77 in the intervention period for facilities A and B combined (incidence rate ratio [IRR], 1.09; 95% CI, 0.88–1.34; P = .43), and were 5.23 HAIs per 10,000 patient days in the 6-month preintervention period and 5.33 in the intervention period for facilities C–Q combined (IRR, 1.02; 95% CI, 0.79–1.32; P = .87). From the preintervention to the intervention periods, the percent positivity rates of completed C. difficile tests increased by 1.1% for facilities A and B and by 1.4% for facilities C–Q. No adverse outcomes were observed.
Conclusions:
The 24-hour autocancellation of uncollected C. difficile orders reduced testing but did not result in reported HAI reduction.
Mitigating climate change requires a global transition from fossil fuels to a “green economy” driven by renewable energies. This shift has fostered massive investments in mining resources, notably lithium in South America, needed to store renewable energies. These mining ventures often produce harmful externalities where lithium is located. In Argentina, a major producer, striking variation has occurred in the fortunes of lithium-mining projects. In some instances, mining companies offered concessions that mitigated environmental damage and improved local socioeconomic conditions. In others, companies made minimal concessions, and in a third set they halted projects in response to local resistance. Why do mining ventures result alternatively in negotiated, unnegotiated, or aborted extraction? The article proposes a new typology of modes of extraction together with a multilevel explanatory framework that centers on the strengths and strategies of transnational mining companies, subnational governments, and local communities in setting the terms for extracting lithium.
Hospitals are increasingly consolidating into health systems. Some systems have appointed healthcare epidemiologists to lead system-level infection prevention programs. Ideal program infrastructure and support resources have not been described. We informally surveyed 7 healthcare epidemiologists with recent experience building and leading system-level infection prevention programs. Key facilitators and barriers for program structure and implementation are described.
Clarke and Beck import certain assumptions about the nature of numbers. Although these are widespread within research on number cognition, they are highly contentious among philosophers of mathematics. In this commentary, we isolate and critically evaluate one core assumption: the identity thesis.
Kostochka and Thomason independently showed that any graph with average degree $\Omega(r\sqrt{\log r})$ contains a $K_r$ minor. In particular, any graph with chromatic number $\Omega(r\sqrt{\log r})$ contains a $K_r$ minor, a partial result towards Hadwiger’s famous conjecture. In this paper, we investigate analogues of these results in the directed setting. There are several ways to define a minor in a digraph. One natural way is as follows. A strong$\overrightarrow{K}_{\!\!r}$minor is a digraph whose vertex set is partitioned into r parts such that each part induces a strongly connected subdigraph, and there is at least one edge in each direction between any two distinct parts. We investigate bounds on the dichromatic number and minimum out-degree of a digraph that force the existence of strong $\overrightarrow{K}_{\!\!r}$ minors as subdigraphs. In particular, we show that any tournament with dichromatic number at least 2r contains a strong $\overrightarrow{K}_{\!\!r}$ minor, and any tournament with minimum out-degree $\Omega(r\sqrt{\log r})$ also contains a strong $\overrightarrow{K}_{\!\!r}$ minor. The latter result is tight up to the implied constant and may be viewed as a strong-minor analogue to the classical result of Kostochka and Thomason. Lastly, we show that there is no function $f\;:\;\mathbb{N} \rightarrow \mathbb{N}$ such that any digraph with minimum out-degree at least f(r) contains a strong $\overrightarrow{K}_{\!\!r}$ minor, but such a function exists when considering dichromatic number.
Although comparative politics is conventionally seen as the study of politics across countries, the field has a longstanding and increasingly prominent tradition in national contexts; focusing on subnational units, institutions, actors and processes. This book offers the first comprehensive assessment of the substantive, theoretical, and methodological contributions. With empirical chapters from across the contemporary Global South, including India, Mexico, and China, as well as Russia, the contributors show how subnational research provides useful insights about substantive themes in political science, from regimes and representation, to states and security, to social and economic development. In addition to methodological chapters with specific guidance about best practices for doing subnational research, this volume also proposes a set of strategies for subnational research, assesses their strengths and weaknesses, and offers illustrative empirical applications.
