Development of consensus recommendations for the management of post-operative chylothorax in paediatric CHD

Objective:
A standardised multi-site approach to manage paediatric post-operative chylothorax does not exist and leads to unnecessary practice variation. The Chylothorax Work Group utilised the Pediatric Critical Care Consortium infrastructure to address this gap.
Methods:Over 60 multi-disciplinary providers representing 22 centres convened virtually as a quality initiative to develop an algorithm to manage paediatric post-operative chylothorax. Agreement was objectively quantified for each recommendation in the algorithm by utilising an anonymous survey. “Consensus” was defined as ≥ 80% of responses as “agree” or “strongly agree” to a recommendation. In order to determine if the algorithm recommendations would be correctly interpreted in the clinical environment, we developed ex vivo simulations and surveyed patients who developed the algorithm and patients who did not.
Results:The algorithm is intended for all children (<18 years of age) within 30 days of cardiac surgery. It contains rationale for 11 central chylothorax management recommendations; diagnostic criteria and evaluation, trial of fat-modified diet, stratification by volume of daily output, timing of first-line medical therapy for “low” and “high” volume patients, and timing and duration of fat-modified diet. All recommendations achieved “consensus” (agreement >80%) by the workgroup (range 81–100%). Ex vivo simulations demonstrated good understanding by developers (range 94–100%) and non-developers (73%–100%).
Conclusions:The quality improvement effort represents the first multi-site algorithm for the management of paediatric post-operative chylothorax. The algorithm includes transparent and objective measures of agreement and understanding. Agreement to the algorithm recommendations was >80%, and overall understanding was 94%.
Archaeometric analysis of a fragment of molded stucco cornice with rope from the House of the Mithraeum (Mérida, Spain)

During the excavations carried out since 2017 in the House of the Mithraeum (Casa del Mitreo) in Mérida a collection of paintings was recovered from Room 11, which had been abandoned in the late 3rd c. CE after a fire. The remains included fragments of molded stucco cornices, with braided esparto grass ropes on the reverse that were used to attach them to the ceiling. This article presents the descriptive and technical study of the finds and their compositional analysis using scanning electron microscopy and X-ray diffraction. Data resulting from these analyses allow us to understand the fragments’ composition and technical execution, and even the possible circulation of workshops and raw materials.
Quality of life of institutionalized older adults by dementia severity

Background:
The goal of the study was to analyze the factors associated with quality of life (QoL) in institutionalized older adults with dementia, based on self and proxy ratings, and if these characteristics differ by dementia severity.
Methods:Cross-sectional study of 525 people with dementia (PwD) and their caregivers (professional or family caregivers). Two different QoL questionnaires, leading to three measures, were used: QoL in Alzheimer's disease scale (QOL-AD), self and proxy-rated, and QoL in late-stage dementia scale (QUALID), proxy-rated. Multivariate linear regression models were tested for each QoL measure and for mild/moderate and severe stages of dementia.
Results:Multiple regression analyses showed a significant association between the three QoL measures and depression. Functional ability was significantly associated with QoL when assessed by proxy. Other factors such as education level, leisure activities and frequency of visits were significantly related with QOL-AD by proxy. The associated factors that differed by dementia severity were education level for moderate dementia, and frequency of visits and who answered the questionnaire (professional vs. family member) for severe dementia.
Conclusions:QoL was consistently associated with depressive symptoms independently of the measures as well as functional ability and social leisure activities when the QoL questionnaire was rated by proxy. Treating depressive symptoms, increasing social activities and maintaining the functional ability may decrease the deterioration of QoL in institutionalized older adults with dementia.
Effects of Aloe vera coating on postharvest quality of tomato

Introduction. Edible coatings are traditionally used to improve food appearance and preservation. They act as barriers during processing, handling and storage, and do not solely retard food deterioration, enhancing its quality, but are safe due to natural biocide activity, or to the incorporation of antimicrobial compounds. The aim of this work was to evaluate the effect of A. vera, applied as an edible coating, on the changes in physicochemical parameters related to tomato fruit quality during storage in ambient conditions (22 °C and 80% RH), as well as its role in controlling microbial spoilage. Materials and methods. The changes in physical, chemical and textural properties of commercial tomato cv. Charleston were evaluated during a storage period of 17 days in ambient conditions according to their coating (treatments), or not (control), with an aqueous extract of pure A. vera or diluted (2:1) in distilled water. Results. The pure aqueous extract of Aloe vera totally inhibited the growth of Rhizoctonia solani and Alternaria alternata, while it did not show antimicrobial activity against Phytophthora parasitica. The diluted aqueous extract of Aloe vera coating maintained the quality of the tomatoes during storage in ambient conditions, delaying ripening, although it did not constitute an effective barrier against weight loss. Conclusion. Our fundamental results may be useful to breeders and postharvest technologists, as well as distributors, importers and exporters, in handling and processing tomatoes.

Right pulmonary artery to left atrial fistula is a rare pathology characterised by a right to left shunt. Another important aspect of this pathology is the difficulty encountered in making a diagnosis, which is why the diagnosis is frequently delayed into adulthood. A description of two cases is used to emphasise the importance of the different modes of echocardiography as a diagnostic tool in diagnosis, as well as the two different clinical forms that it adopts: a group of patients suffering cardiac failure and cyanosis without apparent cause generally in neonates and a second group of mostly older patients with dyspnoea and cyanosis without apparent cause. Symptoms thus differ depending on the time of presentation and are related to the size of the fistula.
Productivity and susceptibility analysis for species caught in Atlantic tuna fisheries

Ecological risk assessment is a useful methodology for assisting the management of fisheries from an ecosystem perspective. Atlantic tuna fisheries, managed by the International Commission for the Conservation of Atlantic Tunas (ICCAT), are economically important and interact with several bycatch species. In spite of these interactions, no comprehensive ecological risk assessment has been conducted for bycatch species caught in ICCAT fisheries. In this paper, we followed a two stage approach with the objective of assessing the relative risk of species being negatively impacted by Atlantic tuna fisheries. An analysis of the ICCAT bycatch species list (which includes all species reported to have interacted with different tuna fishing gears operating in the Atlantic) revealed that most of these species are caught in longline fisheries, followed by gillnets and purse seines. According to the IUCN red list, 7 species of the ICCAT bycatch list (3 coastal sharks, 3 sea turtles and one seabird) are categorized as critically endangered. In our study, and based on their life history characteristics, marine mammals and coastal sharks caught in ICCAT fisheries showed the highest intrinsic vulnerability values. A productivity susceptibility analysis for the European Union (EU) tropical tuna purse seine fleet and the United States (US) pelagic longline fleet revealed two groups with high relative risk scores. The first one included pelagic and coastal sharks, characterized by relatively low productivities, and the second one included teleosts, characterized by higher productivities but high susceptibility to purse seine and longline gears. Some alternative approaches to conduct productivity susceptibility analyses in the context of ecological risk assessments are discussed.

The primary purpose of the study reported here was to explore the effects of the complexity of syllable structure and the effects of task differences in the explanation of deficit in phonological awareness (PA). A sample of 97 subjects was selected and organized into three different groups: 29 reading-disabled (RD) children, 41 normal readers matched in age with the former, and 27 younger normal readers at the same reading level as those with reading disabilities. We administered PA tasks which included items with different complexity of syllable structure. The results showed that the complexity of syllable structure had no particularly marked effect on the dyslexic children. Rather, the isolation task revealed the phonological deficit across all syllable structures.
