When citizens hold multiple values relevant to their policy opinions, they might experience value conflict, value reconciliation or make a value trade-off. Yet, it is unclear which individuals are able to manage their multiple values in these ways. We posit a sophistication-interaction theory of value pluralism where the most politically sophisticated individuals are able to reconcile the existence of multiple values, thus increasing the stability of their policy opinions. We test this hypothesis using a series of heteroskedastic graded item response theory models of public opinion toward policies related to climate change. We find that people structure their policy preferences toward climate change policies in values toward the environment and the economy, but only the most sophisticated citizens are able to reconcile the potential conflict between these values.

Despite the hypothesized importance of the tropics in the global climate system, few tropical paleoclimatic records extend to periods earlier than the last glacial maximum (LGM), about 20,000 years before present. We present a well-dated 170,000-year time series of hydrologic variation from the southern hemisphere tropics of South America that extends from modern times through most of the penultimate glacial period. Alternating mud and salt units in a core from Salar de Uyuni, Bolivia reflect alternations between wet and dry periods. The most striking feature of the sequence is that the duration of paleolakes increased in the late Quaternary. This change may reflect increased precipitation, geomorphic or tectonic processes that affected basin hydrology, or some combination of both. The dominance of salt between 170,000 and 140,000 yr ago indicates that much of the penultimate glacial period was dry, in contrast to wet conditions in the LGM. Our analyses also suggest that the relative influence of insolation forcing on regional moisture budgets may have been stronger during the past 50,000 years than in earlier times.

Studies using 24 h urine collections need to incorporate ways to validate the completeness of the urine samples. Models to predict urinary creatinine excretion (UCE) have been developed for this purpose; however, information on their usefulness to identify incomplete urine collections is limited. We aimed to develop a model for predicting UCE and to assess the performance of a creatinine index using para-aminobenzoic acid (PABA) as a reference. Data were taken from the European Food Consumption Validation study comprising two non-consecutive 24 h urine collections from 600 subjects in five European countries. Data from one collection were used to build a multiple linear regression model to predict UCE, and data from the other collection were used for performance testing of a creatinine index-based strategy to identify incomplete collections. Multiple linear regression (n 458) of UCE showed a significant positive association for body weight (β = 0·07), the interaction term sex × weight (β = 0·09, reference women) and protein intake (β = 0·02). A significant negative association was found for age (β = − 0·09) and sex (β = − 3·14, reference women). An index of observed-to-predicted creatinine resulted in a sensitivity to identify incomplete collections of 0·06 (95 % CI 0·01, 0·20) and 0·11 (95 % CI 0·03, 0·22) in men and women, respectively. Specificity was 0·97 (95 % CI 0·97, 0·98) in men and 0·98 (95 % CI 0·98, 0·99) in women. The present study shows that UCE can be predicted from weight, age and sex. However, the results revealed that a creatinine index based on these predictions is not sufficiently sensitive to exclude incomplete 24 h urine collections.

Summary
The Expanded Disability Status Scale (EDSS) represents the most widely accepted measure of disease progression in multiple sclerosis (MS), and is used in many natural history studies. This chapter discusses natural history of relapsing onset MS, primary progressive MS, changes in natural history and impact on clinical trial design, traditional prognostic factors, and other factors which may influence prognosis such as race, comorbid diseases, and health behaviors. Pharmacoepidemiological studies using real world data derived from clinical practice represent a cost-effective means of evaluating the long-term effectiveness of immunomodulatory drug treatments for MS. Along with evidence from the basic sciences, epidemiological studies can provide insights into potentially novel treatments, as well as the rationale and hypotheses for testing these treatments in clinical trials. For example, vitamin D and estrogen are being evaluated in clinical trials based partly on epidemiological observations. Heterogeneity remains the hallmark of MS.
