OBJECTIVES/GOALS: We modeled risk of reintubation within 48 hours of cardiac surgery using variables available in the electronic health record (EHR). This model will guide recruitment for a prospective, pragmatic clinical trial entirely embedded within the EHR among those at high risk of reintubation. METHODS/STUDY POPULATION: All adult patients admitted to the cardiac intensive care unit following cardiac surgery involving thoracotomy or sternotomy were eligible for inclusion. Data were obtained from operational and analytical databases integrated into the Epic EHR, as well as institutional and departmental-derived data warehouses, using structured query language. Variables were screened for inclusion in the model based on clinical relevance, availability in the EHR as structured data, and likelihood of timely documentation during routine clinical care, in the hopes of obtaining a maximally-pragmatic model. RESULTS/ANTICIPATED RESULTS: A total of 2325 patients met inclusion criteria between November 2, 2017 and November 2, 2019. Of these patients, 68.4% were male. Median age was 63.0. The primary outcome of reintubation occurred in 112/2325 (4.8%) of patients within 48 hours and 177/2325 (7.6%) at any point in the subsequent hospital encounter. Univariate screening and iterative model development revealed numerous strong candidate predictors (ANOVA plot, figure 1), resulting in a model with acceptable calibration (calibration plot, figure 2), c = 0.666. DISCUSSION/SIGNIFICANCE OF IMPACT: Reintubation is common after cardiac surgery. Risk factors are available in the EHR. We are integrating this model into the EHR to support real-time risk estimation and to recruit and randomize high-risk patients into a clinical trial comparing post-extubation high flow nasal cannula with usual care. The role of explicit information (EI) as an independent variable in instructed SLA is largely underresearched. Using the framework of processing instruction, however, a series of offline studies has found no effect for EI (e.g., Benati, 2004; Sanz & Morgan-Short, 2004; VanPatten & Oikkenon, 1996). Fernández (2008) presented two online experiments with mixed results. She found an effect for EI with processing instruction on one target structure (subjunctive in Spanish) but not the other structures (object pronouns and word order in Spanish). Thus, the effects of EI could be related to the target structure or to a processing problem, or both. The present study is a conceptual replication of one of Fernández’s experiments. The target was German accusative case markings on articles with both subject (S)- verb (V)- object (O) and OVS word orders. As shown by Jackson (2007) and LoCoco (1987), learners of German as a second language misinterpret OVS sentences as SVO, ignoring case markings as a cue of who does what to whom. Thus, the goal of the instructional intervention was to push learners to process case markings and word order correctly. The treatment consisted of structured input items (Farley, 2005; Lee & VanPatten, 2003) under two conditions: +/−EI. Following Fernández, the treatment was conducted via computer using e-Prime, and learners’ responses were recorded as they made their way through the items. Whereas Fernández did not find an effect for EI for word order and object pronouns in Spanish, we found an effect for word order and case markings in German: (a) Twice as many learners in the +EI group reached criterion (began to process input strings correctly) compared with the −EI group, and (b) learners in the +EI group began processing word order and case markings sooner than in the −EI group. Even though the processing problem was the same in both Fernández’s and our experiments, we attribute the difference in results to the interaction of particular structures with the processing problem and call for additional research on the role of EI not just in processing instruction but in all formal interventions.

Deficits in “executive” neuropsychological functions have been proposed to underlie the development of antisocial behavior such as juvenile delinquency. Results of research into the executive functions of delinquents have been mixed, and studies have been hampered by reliance on small samples of adjudicated subjects and questionable validity of the tests administered. This research examined the performance of a large unselected birth cohort of adolescent boys and girls on five tests of executive function that have documented reliability and validity. It is the first such study to use self-reports of antisocial behavior. Executive deficits were shown only by a subgroup of delinquent subjects with childhood comorbidity of antisocial behavior and attention deficit disorder; that subgroup's behavior was also rated as more aggressive and impulsive than comparison groups'. Group differences on executive measures remained significant after the effects of overall IQ were statistically controlled. Also, delinquents who had been detected by police did not show poorer executive functions than subjects with equivalent self-reports of delinquent behavior who had evaded official detection, suggesting that executive deficits are related to the development of antisocial behavior itself, and not simply to risk of detection.

Summary
This chapter examines the accumulated evidence for the relation between neuropsychology and conduct problems in children and adolescents. It provides evidence for a link between neuropsychological health and conduct disorder. One of the most robust correlates of severe conduct problems is impaired verbal ability. Verbal deficits have been found in conduct-disordered children, serious adolescent delinquents and adult criminals. Evidence of the relation between executive deficits and conduct problems has been found among incarcerated subjects, among non-conduct-disordered subjects in laboratory situations, and among general-population samples. The cumulative effect of the neuropsychological deficits and the consequent social and academic failures results in a pattern of undercontrolled, explosive behaviour which persists across the life course. The relations between poor neuropsychological functioning and conduct problems were robust across diverse samples, particularly strong for severely anti-social subgroups, and survived conservative controls for possible third variables.
