The objective is to determine if a practical face-to-face emergency disaster incident response training program delivered in the clinical setting will improve self-reported confidence and assessed knowledge of emergency department (ED) nurses to respond to disasters.
Methods
A single site prospective pre-test and post-test randomized controlled trial was adopted for this study. The intervention was a practical face-to-face training program, while the control group completed the required annual mandatory hospital online training.
Results
There was a large difference in post-test median self-reported confidence between groups. There was also a large difference in the proportion of subjects who reached satisfactory levels of self-reported confidence post-test. Regarding assessed knowledge, there was a moderate difference in post-test median knowledge between groups. There was also a moderate difference in the proportion who reached satisfactory levels of knowledge post-test.
Conclusions
This study has shown that ED nurses who undertake a practical face-to-face disaster preparedness education program in the clinical setting, are better prepared to respond to emergency disaster incidents. Organizations should consider the use of a practical structured face-to-face emergency disaster incident response education program to complement and enhance any online emergency and disaster training.
The subdiscipline of economic geography has a long and varied history, and recent work has pushed the field to diversify even further. This collection takes this agenda forward by showcasing inspiring, critical and plural perspectives for contemporary economic geographies. Highlighting the contributions of global scholars, the thirty chapters showcase fresh ways of approaching economic geography in research, teaching and praxis. With sections on thought leaders, contemporary critical debates and future research agendas, this collection calls for greater openness and inclusivity.
The clinical course of major depressive disorder (MDD) is heterogeneous, and early-onset MDD often has a more severe and complex clinical course. Our goal was to determine whether polygenic scores (PGSs) for psychiatric disorders are associated with treatment trajectories in early-onset MDD treated in secondary care.
Methods
Data were drawn from the iPSYCH2015 sample, which includes all individuals born in Denmark between 1981 and 2008 who were treated in secondary care for depression between 1995 and 2015. We selected unrelated individuals of European ancestry with an MDD diagnosis between ages 10–25 (N = 10577). Seven-year trajectories of hospital contacts for depression were modeled using Latent Class Growth Analysis. Associations between PGS for MDD, bipolar disorder, schizophrenia, ADHD, and anorexia and trajectories of MDD contacts were modeled using multinomial logistic regressions.
Results
We identified four trajectory patterns: brief contact (65%), prolonged initial contact (20%), later re-entry (8%), and persistent contact (7%). Relative to the brief contact trajectory, higher PGS for ADHD was associated with a decreased odds of membership in the prolonged initial contact (odds ratio = 1.06, 95% confidence interval = 1.01–1.11) and persistent contact (1.12, 1.03–1.21) trajectories, while PGS-AN was associated with increased odds of membership in the persistent contact trajectory (1.12, 1.03–1.21).
Conclusions
We found significant associations between polygenic liabilities for psychiatric disorders and treatment trajectories in patients with secondary-treated early-onset MDD. These findings help elucidate the relationship between a patient's genetics and their clinical course; however, the effect sizes are small and therefore unlikely to have predictive value in clinical settings.
Medical resuscitations in rugged prehospital settings require emergency personnel to perform high-risk procedures in low-resource conditions. Just-in-Time Guidance (JITG) utilizing augmented reality (AR) guidance may be a solution. There is little literature on the utility of AR-mediated JITG tools for facilitating the performance of emergent field care.
Study Objective:
The objective of this study was to investigate the feasibility and efficacy of a novel AR-mediated JITG tool for emergency field procedures.
Methods:
Emergency medical technician-basic (EMT-B) and paramedic cohorts were randomized to either video training (control) or JITG-AR guidance (intervention) groups for performing bag-valve-mask (BVM) ventilation, intraosseous (IO) line placement, and needle-decompression (Needle-d) in a medium-fidelity simulation environment. For the interventional condition, subjects used an AR technology platform to perform the tasks. The primary outcome was participant task performance; the secondary outcomes were participant-reported acceptability. Participant task score, task time, and acceptability ratings were reported descriptively and compared between the control and intervention groups using chi-square analysis for binary variables and unpaired t-testing for continuous variables.
Results:
Sixty participants were enrolled (mean age 34.8 years; 72% male). In the EMT-B cohort, there was no difference in average task performance score between the control and JITG groups for the BVM and IO tasks; however, the control group had higher performance scores for the Needle-d task (mean score difference 22%; P = .01). In the paramedic cohort, there was no difference in performance scores between the control and JITG group for the BVM and Needle-d tasks, but the control group had higher task scores for the IO task (mean score difference 23%; P = .01). For all task and participant types, the control group performed tasks more quickly than in the JITG group. There was no difference in participant usability or usefulness ratings between the JITG or control conditions for any of the tasks, although paramedics reported they were less likely to use the JITG equipment again (mean difference 1.96 rating points; P = .02).
Conclusions:
This study demonstrated preliminary evidence that AR-mediated guidance for emergency medical procedures is feasible and acceptable. These observations, coupled with AR’s promise for real-time interaction and on-going technological advancements, suggest the potential for this modality in training and practice that justifies future investigation.
Prebiotic fibre represents a promising and efficacious treatment to manage pre-diabetes, acting via complementary pathways involving the gut microbiome and viscosity-related properties. In this study, we evaluated the effect of using a diverse prebiotic fibre supplement on glycaemic, lipid and inflammatory biomarkers in patients with pre-diabetes. Sixty-six patients diagnosed with pre-diabetes (yet not receiving glucose-lowering medications) were randomised into treatment (thirty-three) and placebo (thirty-three) interventions. Participants in the treatment arm consumed 20 g/d of a diverse prebiotic fibre supplement, and participants in the placebo arm consumed 2 g/d of cellulose for 24 weeks. A total of fifty-one and forty-eight participants completed the week 16 and week 24 visits, respectively. The intervention was well tolerated, with a high average adherence rate across groups. Our results extend upon previous work, showing a significant change in glycated haemoglobin (HbA1c) in the treatment group but only in participants with lower baseline HbA1c levels (< 6 % HbA1c) (P = 0·05; treatment –0·17 ± 0·27 v. placebo 0·07 ± 0·29, mean ± sd). Within the whole cohort, we showed significant improvements in insulin sensitivity (P = 0·03; treatment 1·62 ± 5·79 v. placebo –0·77 ± 2·11) and C-reactive protein (PFWE = 0·03; treatment –2·02 ± 6·42 v. placebo 0·94 ± 2·28) in the treatment group compared with the placebo. Together, our results support the use of a diverse prebiotic fibre supplement for physiologically relevant biomarkers in pre-diabetes.
Little is known about the life cycle and mode of transmission of Dientamoeba fragilis. Recently it was suggested that fecal–oral transmission of cysts may play a role in the transmission of D. fragilis. In order to establish an infection, D. fragilis is required to remain viable when exposed to the pH of the stomach. In this study, we investigated the ability of cultured trophozoites to withstand the extremes of pH. We provide evidence that trophozoites of D. fragilis are vulnerable to highly acidic conditions. We also investigated further the ultrastructure of D. fragilis cysts obtained from mice and rats by transmission electron microscopy. These studies of cysts showed a clear cyst wall surrounding an encysted parasite. The cyst wall was double layered with an outer fibrillar layer and an inner layer enclosing the parasite. Hydrogenosomes, endoplasmic reticulum and nuclei were present in the cysts. Pelta-axostyle structures, costa and axonemes were identifiable and internal flagellar axonemes were present. This study therefore provides additional novel details and knowledge of the ultrastructure of the cyst stage of D. fragilis.
What could economic geographies be? What should economic geographies be? Who might be included in this project, what might they contribute, and how can we ensure that this work is valued? These are not new questions, and yet they remain as pertinent as ever.
This collection adopts a fresh perspective to these debates, and to economic geographies more broadly, with a focus on plurality. We show how contemporary economic geographies are already plural, as they are critical and inspiring. However, this remains to be widely recognized and celebrated. Such pluralism is, we argue, essential. It includes building upon economic geographies that acknowledge the deeply ingrained racial, gendered and classed power differentials inherent within the economy across space, scale and time; and that propose ways to address these problems. It involves expanding upon the areas that are considered the ‘heartlands’ of economic geography (such as a focus on regional and national scales, agglomeration and clustering, financial processes and industrial sectors), and advancing the theoretical devices deployed to understand these worlds. Pluralism likewise extends to empirical and methodological imagination, in terms of how, where and with whom economic geographies engage, include and empower. This involves wider engagements across international fields of study, going beyond Anglocentric sites, writings and perspectives, and broadening methodological expertise to encourage innovation and creativity.
Working towards more plural economic geographies also means tackling and addressing long-standing concerns about the overbearing heteronormativity of who ‘does’ and who is ‘recognized’ within the subdiscipline (Christopherson, 1989; McDowell, 1992). By this we refer to historical over-representation of White, male, Western, middle-class, able-bodied, older scholars, which is both an aesthetic problem and an epistemic one, and which requires constant maintenance. We are not asking for space to be made or given. This is not the economic geography we seek to expand. To our mind, there is no singular ‘project’, no ‘one’ economic geography, no particular set of gatekeepers. Rather it is a collective agenda, with varied voices, positions and approaches, and this collection celebrates this in all its diversity.
We have approached this edited collection with an ambitious agenda; to create further space to develop and extend pluralized contemporary economic geographies. This is a task that can only be achieved collectively, and so it is fitting to start our ending by thanking all the contributors to the collection. The work of pluralizing and diversifying is not an equal or shared task, and we are mindful that the labour of doing ‘diversity work’ often falls to those individuals who are most deeply affected by the problems of privilege and exclusivity. Thus, a collection such as this is more than the sum of its parts, and more than can be seen written on the pages. It represents a collective vision for how to do things differently. In this postscript we take the opportunity to outline what we hope may come from this collection as part of a broader project which brings in you, the readers. This is less of a ‘last word’ and more of a call to action.
One of the main ways in which we anticipate this book will be adopted is as a teaching resource across multiple disciplines and learning stages. With a wide range of topics and themes, concepts and approaches, from across disciplinary and empirical contexts, the collection offers rich material for teaching pluralistic economic geographies. The chapters are designed to be pithy, accessible and inspiring. As such, they are useful as key readings and introductory texts for foundational undergraduate modules across the social sciences. At the same time, due to the depth and breadth of the contributing chapters, the collection also offers materials for optional courses which explore contemporary economic issues in more detail.
Moreover, with a focus on previous, current and future research agendas, we expect the collection will appeal to students and scholars alike in developing ideas for their own studies (for example, scoping projects, dissertations, funded research). The act of pluralizing any discipline involves active participation and learning, regardless of educational or career stage. Indeed, this collection brings together scholars from a wide range of approaches, perspectives and backgrounds.
Added to this, pluralizing any discipline is a process that requires time and effort. This includes thinking about not just what we write about but how we write.
The central core of the work of Adam Smith is identified here, with particular reference to his own words. His argumentation is full of surprises and paradoxes, and it offers key insights for sociology, especially as it allows us to better understand key features of the modern world.
We review impacts of climate change, energy scarcity, and economic frameworks on sustainability of natural and human systems in coastal zones, areas of high biodiversity, productivity, population density, and economic activity. More than 50% of the global population lives within 200 km of a coast, mostly in tropical developing countries. These systems developed during stable Holocene conditions. Changes in global forcings are threatening sustainability of coastal ecosystems and populations. During the Holocene, the earth warmed and became wetter and more productive. Climate changes are impacting coastal systems via sea level rise, stronger tropical cyclones, changes in basin inputs, and extreme weather events. These impacts are passing tipping points as the fossil fuel-powered industrial-technological-agricultural revolution has overwhelmed the source–sink functions of the biosphere and degraded natural systems. The current status of industrialized society is primarily the result of fossil fuel (FF) use. FFs provided more than 80% of global primary energy and are projected to decline to 50% by mid-century. This has profound implications for societal energy requirements, including the transition to a renewable economy. The development of the industrial economy allowed coastal social systems to become spatially separated from their dominant energy and food sources. This will become more difficult to maintain with the fading of cheap energy. It seems inevitable that past growth in energy use, resource consumption, and economic growth cannot be sustained, and coastal areas are in the forefront of these challenges. Rapid planning and cooperation are necessary to minimize impacts of the changes associated with the coming transition. There is an urgent need for a new economic framework to guide society through the transition as mainstream neoclassical economics is not based on natural sciences and does not adequately consider either the importance of energy or the work of nature.
