The Wisconsin Twin Research Program comprises multiple longitudinal studies that utilize a panel recruited from statewide birth records for the years 1989 through 2004. Our research foci are the etiology and developmental course of early emotions, temperament, childhood anxiety and impulsivity, autism, sensory over-responsivity, and related topics. A signature feature of this research program is the breadth and depth of assessment during key periods of development. The assessments include extensive home- and laboratory-based behavioral batteries, recorded sibling and caregiver interactions, structured psychiatric interviews with caregivers and adolescents, observer ratings of child behavior, child self-report, cognitive testing, neuroendocrine measures, medical records, dermatoglyphics, genotyping, and neuroimaging. Across the various studies, testing occasions occurred between 3 months and 18 years of age. Data collection for some aspects of the research program has concluded and, for other aspects, longitudinal follow-ups are in progress.

Imagine, if you will, that you live in the richest, most influential nation on earth. This is a country whose political system has been consciously adopted by neighboring heads of state seeking to duplicate the same stability and prosperity in their own societies. It is a country that has certainly experienced unprecedented growth and cultural achievement in its past two hundred years and thus contains some of the most magnificent and intellectually exciting urban centers in the world. You would be justifiably proud to live in such a place.

Objective
With the upsurge in online dietary modification programmes, online dietary assessment tools are needed to capture food intake. Although the DASH (Dietary Approaches to Stop Hypertension) diet is recommended by the US Department of Agriculture, there are no online instruments that capture DASH food servings. Our objective was to assess the validity of a new, short, online dietary questionnaire developed to capture intake of DASH food servings. The DASH Online Questionnaire (OLQ) was validated against the well-known Block Food Frequency Questionnaire (FFQ).
DesignThis was a cross-sectional validation of the DASH OLQ, which contained eleven food groups (breakfast cereals; dairy; drinks; fats and oils; fruits; grains and snacks; meat, fish and poultry; mixed dishes; sweets; vegetables; and nuts, seeds and legumes). Each subject completed a DASH OLQ once weekly for four weeks and one 98.2 Block FFQ (110 questions) between weeks 2 and 4. DASH OLQ were averaged and then compared with the Block FFQ for nutrient intakes as well as intakes of DASH food groups.

ResultsThere were significant positive correlations between the Block FFQ and the DASH OLQ for all food groups ranging from r = 0·8 for the nuts/seeds/legumes category to r = 0·3 for vegetables and mixed dishes. A comparison of nutrient intakes found strong positive correlations in all nutrient categories. Of particular interest in the DASH diet and the web-based nutrition and physical activity programme were total fat (r = 0·62), total carbohydrate (r = 0·67), total K (r = 0·68), total Ca (r = 0·69), total vitamin C (r = 0·60) and total energy intake (r = 0·68).
ConclusionsThe DASH OLQ captures food and nutrient intake well in relation to the more established Block FFQ.

Summary
This chapter contains the primary report of findings from the comparison of the Community Reinforcement (CRA) and traditional approaches. Study participants completed a comprehensive assessment at intake that included measurement of numerous demographic characteristics, motivation for change, psychological functioning, drinking history, and current drinking practices. The taking and monitoring of disulfiram were important distinguishing aspects of the treatment groups. The a priori treatment contrasts were made at proximal and distal follow-up points using three primary dependent measures. The three outcome measures were total standard drinks consumed during the assessment period, number of drinking days per week, and estimated peak blood alcohol concentration (BAC) for the assessment period. Traditional and CRA groups also had similar outcomes among the disulfiram-ineligible clients. The chapter concludes with a confessional litany of some errors the authors made along the way, in the hope of saving colleagues from similar pitfalls.

Two of the patient's rights in a Uniform Act for the Terminally Ill should he a) the right to be treated if there is reasonable doubt of the patient's wishes until it becomes clear that the treatment is not beneficial and that the patient is indeed terminally ill, and b) the right not to he subjected to treatment with no medical benefit. A physician with appropriate supporting independent opinions would he the logical individual to whom the responsibility of protecting those rights should be given. This is particularly true when the patient is incapable of his or her own medical decisions and has left no instructions regarding his or her personal goals in medical care in the event of severe illness. Two cases and an analysis of power in medical decision-making illustrate these points.
