In this address I shall discuss head trauma from an angle which may be unusual for neuroscientists. Our preoccupations are diagnostic challenges and management problems, but that which we experience at the bedside is only a narrow segment of a continuum which started with trauma somewhere in a war, on the road, at home, on the football field, in the boxing ring, and in many other distinct locations. When our role is over, there are only three places where head trauma victims can be found; in cemeteries, where every year, 5,000 new graves are made to accommodate fatal head injuries in Canada; in chronic hospitals, which are already overloaded with victims of various insults to the brain, and, of course, within society, which accepts the fully recovered or tolerates the subtle and not so subtle consequences of so-called ‘minor’ head injuries.

All those related to the medical profession are familiar with the term “resuscitation” which usually means restoration of normal cardio-respira-tory activity in someone who has suffered from the effects of severely impaired functions of these two organs.
The concept of “brain resuscitation” has emerged during the past few years and to the best of our knowledge the term was probably first coined by SAFAR (1977). This term is used more freely by anesthetists. Neuro-surgeons, when they relieve otherwise fatal herniations of the brain, are not in the habit of calling the effort resuscitation. Since, in our opinion, the term brain resuscitation focuses attention on a set of measures in an acute catastrophic situation, we would like to support its use.

Canadians are suing their doctors for malpractice, at three times the rate they were 15 years ago, and during that period awards to injured patients have quadrupled in size. Since the middle 1970's when the first medical malpractice “crisis” occurred in the United States, Canadian clinical neuroscientists have felt increasingly threatened by the prospect of a lawsuit for negligence. Since medicine is intrinsically a risk taking business, adverse outcomes are inevitable. Nevertheless, accusations of negligence and carelessness set out in a Statement of Claim causes considerable stress for the defendant physician, who frequently reacts with self-doubt, depression and aggressive behaviour. A “tort” is defined as a wrongful act, and the 4 elements comprising the tort of medical malpractice are: a breech of standard care, the breech was the proximate cause of the injury, the injury produced measurable damage, and it was foreseeable that the injury would have been less injurious to the patient if the caregiver had conducted a different course. In order to put these issues into current perspective, this symposium was convened as part of the XXIV Canadian Congress of Neurological Sciences, held in Ottawa June 17, 1989.

A case of a primary intracranial embryonal carcinoma, the first with ultrastructural study, is reported. The tumor was associated with precocious puberty in a 6½-year-old female. Characteristic embryoid bodies were present. At the ultrastructural level three cell types were noted: undifferentiated, differentiated, and intermediate types. The undifferentiated showed scanty cytoplasmic organelles and numerous free polysomes, while the differentiated cells contained well-developed mitochondria, Golgi apparatus, rough endoplasmic reticulum, and some contained secretory granules. The intermediate cells possessed dilated and irregularly-shaped mitochondria but still retained large numbers of free polysomes. The authors suggest that intracranial germ cell tumors be named in conformity with germ cell tumors in other sites, and that terms such as “ectopic pinealoma” and “atypical teratoma of the pineal” be used no longer.
