We examined the association between bilingualism, executive function (EF), and brain volume in older monolinguals and bilinguals who spoke English, Spanish, or both, and were cognitively normal (CN) or diagnosed with Mild Cognitive Impairment (MCI) or dementia. Gray matter volume (GMV) was higher in language and EF brain regions among bilinguals, but no differences were found in memory regions. Neuropsychological performance did not vary across language groups over time; however, bilinguals exhibited reduced Stroop interference and lower scores on Digit Span Backwards and category fluency. Higher scores on Digit Span Backwards were associated with a younger age of English acquisition, and a greater degree of balanced bilingualism was associated with lower scores in category fluency. The initial age of cognitive decline did not differ between language groups. The influence of bilingualism appears to be reflected in increased GMV in language and EF regions, and to a lesser degree, in EF.
Jaswal & Akhtar provide several quotes ostensibly from people with autism but obtained via the discredited techniques of Facilitated Communication and the Rapid Prompting Method, and they do not acknowledge the use of these techniques. As a result, their argument is substantially less convincing than they assert, and the article lacks transparency.
Oral anticoagulation (OAC) reduces stroke risk in patients with atrial fibrillation (AF) or atrial flutter (AFL). However, OAC initiation rates in patients discharged directly from the emergency department (ED) are low. We aimed to address this care gap by implementing a quality improvement intervention.
Methods
The study was performed in four Canadian urban EDs between 2015 and 2016. Patients were included if they had an electrocardiogram (ECG) documenting AF/AFL in the ED, were directly discharged from the ED, and were alive after 90 days. Baseline rates of OAC initiation were determined prior to the intervention. Between June and December 2016, we implemented our intervention in two EDs (ED-intervention), with the remaining sites acting as controls (ED-control). The intervention included a reminder statement prompting OAC initiation according to guideline recommendations, manually added to ECGs with a preliminary interpretation of AF/AFL, along with a decision-support algorithm that included a referral sheet. The primary outcome was the rate of OAC initiation within 90 days of the ED visit.
Results
Prior to the intervention, 37.2% OAC-naïve patients with ECG-documented AF/AFL were initiated on OAC. Following implementation of the intervention, the rate of OAC initiation increased from 38.6% to 47.5% (absolute increase of 8.5%; 95% CI, 0.3% to 16.7%, p=0.04) among the ED-intervention sites, whereas the rate remained unchanged in ED-control sites (35.3% to 35.9%, p=0.9).
Conclusions
Implementation of a quality improvement intervention consisting of a reminder and decision-support tool increased initiation of OAC in high-risk patients. This support package can be readily implemented in other jurisdictions to improve OAC rates for AF/AFL.
The shallow subsurface of Groningen, the Netherlands, is heterogeneous due to its formation in a Holocene tidal coastal setting on a periglacially and glacially inherited landscape with strong lateral variation in subsurface architecture. Soft sediments with low, small-strain shear wave velocities (VS30 around 200 m s−1) are known to amplify earthquake motions. Knowledge of the architecture and properties of the subsurface and the combined effect on the propagation of earthquake waves is imperative for the prediction of geohazards of ground shaking and liquefaction at the surface. In order to provide information for the seismic hazard and risk analysis, two geological models were constructed. The first is the ‘Geological model for Site response in Groningen’ (GSG model) and is based on the detailed 3D GeoTOP voxel model containing lithostratigraphy and lithoclass attributes. The GeoTOP model was combined with information from boreholes, cone penetration tests, regional digital geological and geohydrological models to cover the full range from the surface down to the base of the North Sea Supergroup (base Paleogene) at ~800 m depth. The GSG model consists of a microzonation based on geology and a stack of soil stratigraphy for each of the 140,000 grid cells (100 m × 100 m) to which properties (VS and parameters relevant for nonlinear soil behaviour) were assigned. The GSG model serves as input to the site response calculations that feed into the Ground Motion Model. The second model is the ‘Geological model for Liquefaction sensitivity in Groningen’ (GLG). Generally, loosely packed sands might be susceptible to liquefaction upon earthquake shaking. In order to delineate zones of loosely packed sand in the first 40 m below the surface, GeoTOP was combined with relative densities inferred from a large cone penetration test database. The marine Naaldwijk and Eem Formations have the highest proportion of loosely packed sand (31% and 38%, respectively) and thus are considered to be the most vulnerable to liquefaction; other units contain 5–17% loosely packed sand. The GLG model serves as one of the inputs for further research on the liquefaction potential in Groningen, such as the development of region-specific magnitude scaling factors (MSF) and depth–stress reduction relationships (rd).
Recent results showed that the magnetic field of M-dwarf (dM) stars, currently the main targets in searches for terrestrial planets, is very different from the solar one, both in topology as well as in intensity. In particular, the magnetised environment surrounding a planet orbiting in the habitable zone (HZ) of dM stars can differ substantially to the one encountered around the Earth. These extreme magnetic fields can compress planetary magnetospheres to such an extent that a significant fraction of the planet's atmosphere may be exposed to erosion by the stellar wind. Using observed surface magnetic maps for a sample of 15 dM stars, we investigate the minimum degree of planetary magnetospheric compression caused by the intense stellar magnetic fields. We show that hypothetical Earth-like planets with similar terrestrial magnetisation (~1 G) orbiting at the inner (outer) edge of the HZ of these stars would present magnetospheres that extend at most up to 6.1 (11.7) planetary radii. To be able to sustain an Earth-sized magnetosphere, the terrestrial planet would either need to orbit significantly farther out than the traditional limits of the HZ; or else, if it were orbiting within the life-bearing region, it would require a minimum magnetic field ranging from a few G to up to a few thousand G.
The article defends the proposition that the distinctive diplomatic legacy of John Quincy Adams constitutes an important case study documenting the significance of moral reasoning in the statesman's obligation to uphold the national interest in a universal system of power politics. Recourse to political ethics afforded Adams a vantage point from which to evaluate the volatile problem of intervention—viewed in terms of the moral responsibilities and limits of American power—at a crucial turning point in the diplomatic history of the young republic. Important in this connection is how Adams attempted to reconcile the blessings of liberty with the methods and purposes of American diplomacy within and beyond the Western Hemisphere.An attempt is made to evaluate the sources and meaning of statesmanship for Adams who—as perhaps the nation's most experienced diplomat and accomplished Secretary of State—judged the limits and opportunities of American power from a perspective that considered moral precepts and international law as compatible with the prudent pursuit of American national interest in world affairs.
