Healthcare resource use and costs for people with type 2 diabetes mellitus with and without severe mental illness in England: longitudinal matched-cohort study using the Clinical Practice Research Datalink

Background
Approximately 60 000 people in England have coexisting type 2 diabetes mellitus (T2DM) and severe mental illness (SMI). They are more likely to have poorer health outcomes and require more complex care pathways compared with those with T2DM alone. Despite increasing prevalence, little is known about the healthcare resource use and costs for people with both conditions.
AimsTo assess the impact of SMI on healthcare resource use and service costs for adults with T2DM, and explore the predictors of healthcare costs and lifetime costs for people with both conditions.
MethodThis was a matched-cohort study using data from the Clinical Practice Research Datalink linked to Hospital Episode Statistics for 1620 people with comorbid SMI and T2DM and 4763 people with T2DM alone. Generalised linear models and the Bang and Tsiatis method were used to explore cost predictors and mean lifetime costs respectively.
ResultsThere were higher average annual costs for people with T2DM and SMI (£1930 higher) than people with T2DM alone, driven primarily by mental health and non-mental health-related hospital admissions. Key predictors of higher total costs were older age, comorbid hypertension, use of antidepressants, use of first-generation antipsychotics, and increased duration of living with both conditions. Expected lifetime costs were approximately £35 000 per person with both SMI and T2DM. Extrapolating nationally, this would generate total annual costs to the National Health Service of around £250 m per year.
ConclusionsOur estimates of resource use and costs for people with both T2DM and SMI will aid policymakers and commissioners in service planning and resource allocation.
Tea Flavonoids and Risk of Cardiovascular and All-Cause Mortality: A Systematic Review and Meta-Analysis

There is increasing evidence that both black and green tea are beneficial for prevention of cardiovascular disease (CVD). We conducted a systematic review and meta-analysis evaluating the effects of tea flavonoids on cardiovascular (CVD) and all-cause mortality outcomes.Searches across five databases including PubMed and Embase were conducted through November 2018 to identify randomized controlled trials (RCTs) and prospective cohort studies reporting cardiovascular and all-cause mortality outcomes. Two investigators independently conducted abstract and full-text screenings, data extractions, and risk of bias (ROB) assessments using the Nutrition Evidence Library Bias Assessment Tool (NEL BAT). Mixed-effects dose-response meta-regression and standard random-effects meta-analyses for outcomes with ≥ 4 studies were performed. 0 RCTs and 38 prospective cohort studies were included in the systematic review. NEL BAT scores ranged from 0–15 (0 being the lowest risk). Our linear meta-regression model showed that each cup increase in daily tea consumption (about 280 mg and 338 mg of total flavonoids for black and green tea, respectively) was associated with 3–4% lower risk of CVD mortality (predicted adjusted RR = 0.96; CI 0.93–0.99 for green tea and RR = 0.97; CI 0.94–0.99 for black tea). Furthermore, eachcup increase in daily tea consumption was associated a 2% lower risk of all-cause mortality (predicted adjusted relative risk (RR) = 0.98; 95% CI 0.97–0.99 for black tea and RR = 0.98; CI 0.96–0.99 for green tea, respectively). Two studies reported multivariable Cox regression analysis results for the relationship between black tea intake and risks of all-cause mortality outcomes. The results from these two studies were combined with our linear meta-regression result in a random-effects model meta-analysis and showed that each cup increase in daily black tea consumption was associated with an average of 3% lower risk of all-cause mortality (pooled adjusted RR = 0.97; 95% CI 0.87- 1.00) with large heterogeneity (I2 = 81.4%; p = 0.005). Current evidence indicates that increased tea consumption may reduce cardiovascular and all-cause mortality in a dose-response manner. This systematic review was registered on PROSPERO.

In 2015, Guglielmi and Badia discussed optimal strategies in a particular type of service system with two strategic servers. In their setup, each server can be either active or inactive and an active server can be requested to transmit a sequence of packets. The servers have varying probabilities of successfully transmitting when they are active, and both servers receive a unit reward if the sequence of packets is transmitted successfully. Guglielmi and Badia provided an analysis of optimal strategies in four scenarios: where each server does not know the other’s successful transmission probability; one of the two servers is always inactive; each server knows the other’s successful transmission probability and they are willing to cooperate.
Unfortunately, the analysis by Guglielmi and Badia contained some errors. In this paper we correct these errors. We discuss three cases where both servers (I) communicate and cooperate; (II) neither communicate nor cooperate; (III) communicate but do not cooperate. In particular, we obtain the unique Nash equilibrium strategy in Case II through a Bayesian game formulation, and demonstrate that there is a region in the parameter space where there are multiple Nash equilibria in Case III. We also quantify the value of communication or cooperation by comparing the social welfare in the three cases, and propose possible regulations to make the Nash equilibrium strategy the socially optimal strategy for both Cases II and III.

We have observed the G23 field of the Galaxy AndMass Assembly (GAMA) survey using the Australian Square Kilometre Array Pathfinder (ASKAP) in its commissioning phase to validate the performance of the telescope and to characterise the detected galaxy populations. This observation covers ~48 deg2 with synthesised beam of 32.7 arcsec by 17.8 arcsec at 936MHz, and ~39 deg2 with synthesised beam of 15.8 arcsec by 12.0 arcsec at 1320MHz. At both frequencies, the root-mean-square (r.m.s.) noise is ~0.1 mJy/beam. We combine these radio observations with the GAMA galaxy data, which includes spectroscopy of galaxies that are i-band selected with a magnitude limit of 19.2. Wide-field Infrared Survey Explorer (WISE) infrared (IR) photometry is used to determine which galaxies host an active galactic nucleus (AGN). In properties including source counts, mass distributions, and IR versus radio luminosity relation, the ASKAP-detected radio sources behave as expected. Radio galaxies have higher stellar mass and luminosity in IR, optical, and UV than other galaxies. We apply optical and IR AGN diagnostics and find that they disagree for ~30% of the galaxies in our sample. We suggest possible causes for the disagreement. Some cases can be explained by optical extinction of the AGN, but for more than half of the cases we do not find a clear explanation. Radio sources aremore likely (~6%) to have an AGN than radio quiet galaxies (~1%), but the majority of AGN are not detected in radio at this sensitivity.

The physical mechanism governing the centring of a hollow liquid shell in capillary oscillations, which has been observed in experiments, is investigated theoretically. First, the shell is assumed to be inviscid and to have a thickness that is much less than its spherical radius. A system of one-dimensional nonlinear equations of motion is derived using a thin-sheet model. From a numerical study the nonlinear effects of the wave are found to cause the core to oscillate slowly relative to the shell while the centre of mass of the whole system remains stationary. The effects of small viscosity are then considered in an approximation. Finally the strength of the centring mechanism is compared with that of the decentring effect due to buoyancy. The findings are consistent with the limited experimental information available.

This paper details the experimental results for the influence of transport currents on the ac magnetisation of high-Tc superconductors. It has been found that the variation of the third harmonic amplitude V3 with an ac field can still be described by square and cubic laws for low and high ac fields, respectively, the same relationships as observed in the absence of transport currents. The only significant difference is that the magnitude of V3 in the low field region increases linearly with transport current. Consequently, the crossover point (from square law to cubic law) moves to higher ac fields and the variation of V3 with transport current is less pronounced in the high field region. The results are explained in terms of an effective increase in Hg, which is a parameter in Kim critical state model, with increase of transport current.

Space Processing is a fledgling and somewhat controversial subject. The scientific community as well as NASA have not yet quite made up their minds as to what to do with Space Processing. The scientific society views Space Processing with some suspicion, and rightfully so. They're not sure whether this is something real or just a Class “A” con job. NASA has made a lot of promises to this program and has delivered very little. Space Processing has a good chance of becoming a self-fulfilled prophecy. Therefore, responsibility is laid on the shoulders of those of us who believe in this program to convince the scientific community that there is something real there and lobby NASA that they should either “put up or shut up.” This is one of the reasons why I took up the challenge to fly in the Space Shuttle and to live and train with these “Space jocks” for a total of about three years.

High quality metallic and metallic-glass microballoons (MMB and MGMB) are of considerable interest for fusion target applications on account of the intrinsic properties of these materials such as high density, high strength and high atomic number. We report the first successfully formed submillimeter and millimeter spherical shells of tin and of a gold-lead-antimony alloy by means of the hollow-jet instability technique developed by one of us (JMK). Examination of tin specimens by means of SEM has revealed that surface quality varied from poor to excellent. Whereas this metal has been employed only as a convenient and inexpensive material, the gold alloy is important because it is hard, has high atomic number, and may be solidified into the amorphous state through the provision of a modest cooling rate. We have produced AuPbSb spherules up to 1.5 mm in diameter using LN2 or chilled methanol as a coolant, and have found that these amorphous samples possess a superb surface smoothness compatible with fusion target requirements. Hollow spheres currently made of this alloy have an average O.D. of 2000 μm.

During the past few years we have been studying several of the physical processes relevant to the production of spherical shells for inertial confinement fusion targets, both in a microgravity environment and in a containerless environment. The work has led to the development of several experimental facilities. Those which are most unique are described here, and fall into three categories as follows: 1. Ones which provide an induced low- or microgravity containerless environment, such as a vertical drag-free wind tunnel, two differing low-pressure and/or high-temperature drop towers for processing metallic or metallic-glass specimens, and a neutral buoyancy tank, 2. Ones providing containerless processing capability, such as a focusing radiator and an electrostatic levitator and 3. Ones providing extended microgravity and containerless capabilities, such as the KC-135 aircraft and the Space Processing Application Rockets. The physical processes which we have been studying include, but are not limited to, those which establish the shell sphericity, concentricity, surface topology, material properties, coatings, heating and cooling requirements and the effects of gravity on fusion pellet fabrication processes.
Polymer Coating of Glass Microballoons Levitated in a Focused Acoustic Field

Inertial confinement fusion (ICF) glass microballoons (GMBs) levitated in a focusing radiator acoustic device can be coated with liquid materials by deploying the liquid into the levitation field with a stepped-horn atomizer. The GMB can be forced to the center of the coating liquid with a strong acoustically generated centering force. Water solutions of organic polymers, uv-curable liquid organic monomers, and paraffin waxes have been used to prepare solid coatings on the surface of GMBs using this technique.
