On-Field Signs Predict Future Acute Symptoms After Sport-Related Concussion: A Structural Equation Modeling Study

Objectives: This study investigated the relationship between on-field, objective signs immediately following sport-related concussion and self-reported symptom endorsement within 1 day post injury. Methods: A retrospective case series of 237 concussed high school athletes was performed. On-field signs were evaluated immediately post injury. Self-reported symptoms (2 clusters) were collected within 1 day post injury. A two-step structural equation model and follow-up bivariate regression analyses of significant on-field signs and symptom clusters were performed. Results: Signs of immediate memory, β=0.20, p=.04, and postural instability, β=0.19, p < .01, significantly predicted a greater likelihood of endorsing the cognitive-migraine-fatigue symptom cluster within 1 day post injury. Regarding signs correlated with specific symptoms, immediate memory was associated with symptoms of trouble remembering, χ 2 =37.92, p < .001, odds ratio (OR)=3.89 (95% confidence interval (CI) [2.47, 6.13]), and concentration difficulties, χ 2 =10.84, p=.001, OR=2.13 (95% CI [1.37, 3.30]). Postural instability was associated with symptom endorsement of trouble remembering, χ 2 =12.08, p < .001, OR=1.76 (95% CI [1.29, 2.40]). Conclusions: Certain post-concussion on-field signs exhibited after injury were associated with specific symptom endorsement within 1 day post injury. Based on these associations, individualized education-based interventions and academic accommodations may help reduce unanticipated worry from parents, students, and teachers following a student-athlete’s sport-related concussion, especially in cases of delayed onset symptoms. An outbreak of respiratory diphtheria occurred in two health districts in the province of KwaZulu-Natal in South Africa in 2015. A multidisciplinary outbreak response team was involved in the investigation and management of the outbreak. Fifteen cases of diphtheria were identified, with ages ranging from 4 to 41 years. Of the 12 cases that were under the age of 18 years, 9 (75%) were not fully immunized for diphtheria. The case fatality was 27%. Ninety-three household contacts, 981 school or work contacts and 595 healthcare worker contacts were identified and given prophylaxis against Corynebacterium diphtheriae infection. A targeted vaccination campaign for children aged 6–15 years was carried out at schools in the two districts. The outbreak highlighted the need to improve diphtheria vaccination coverage in the province and to investigate the feasibility of offering diphtheria vaccines to healthcare workers.

This paper examines choices of earth-working tools made by Neolithic Chinese populations. In the Hemudu Culture (7000–5000 B.P.), bone (scapula) digging tools were used from the earliest times, whereas peoples in surrounding areas used stone spades. A range of experiments on manufacturing costs, durability, and use efficiency under realistic conditions show that bone and stone spades are functionally equivalent when soils are soft, but that stone implements provide significant and easily perceived advantages when working harder soils. The persistence of scapular spades in the Hemudu Culture would have constrained decisions about undertaking large construction projects under normal soil conditions. Our results show that, in addition to generalized labor for construction, labor demands for producing earth-working implements for large-scale prehistoric earthworks could have also been substantial. These findings not only help explain the processes of intensifying rice-agriculture and sedentary settlements in the Lower Yangzi Basin, but also create a solid foundation for further investigation of how the recruitment of both generalized and specialized laborers, the organization of craft production, and the relevant logistics for large-scale earthworks may have paralleled concentrations of political power in prehistory.

An optimum method is proposed to prepare thin foil transmission electron microscopy (TEM) lamellae of multiphase porous functional ceramics: prefilling the pore space of these materials with an epoxy resin prior to focused ion beam milling. Several advantages of epoxy impregnation are demonstrated by successful preparation of TEM specimens that maintain the structural integrity of the entire lamella. Feasibility of the TEM alignment procedure is demonstrated, and ideal TEM analyses are illustrated on solid oxide fuel cell and solid oxide electrolysis cell materials. Some potential drawbacks of the TEM specimen preparation method are listed for other samples.

Objective.
To determine relative rates of blood culture contamination for 3 skin antisepsis interventions—10% povidone iodine aqueous solution (PI), 2% iodine tincture (IT), and 2% chlorhexidine gluconate in 70% isopropyl alcohol (CHG)—when used by dedicated phlebotomy teams to obtain peripheral blood cultures.
Design.Randomized crossover trial with hospital floor as the unit of randomization.
Setting.Teaching hospital with 885 beds.
Patients.All adult patients undergoing peripheral blood culture collection on 3 medical-surgical floors from May 2009 through September 2009.
Intervention.Each antisepsis intervention was used for 5 months on each study floor, with random crossover after a 1-month washout period. Phlebotomy teams collected all peripheral blood cultures. Each positive blood culture was adjudicated by physicians blinded to the intervention and scored as a true positive or contaminated blood culture. The primary outcome was the rate of blood culture contamination for each antisepsis agent.
Results.In total, 12,904 peripheral blood culture sets were evaluated, of which 735 (5.7%) were positive. There were 98 contaminated cultures, representing 13.3% of all positive cultures. The overall blood culture contamination rate for the study population was 0.76%. Intent-to-treat rates of contaminated blood cultures were not significantly different among the 3 antiseptics (P = .18), yielding 0.58% with PI (95% confidence interval [CI], 0.38%-0.86%), 0.76% with IT (95% CI, 0.52%-1.07%), and 0.93% with CHG (95% CI, 0.67%-1.27%).
Conclusion.Choice of antiseptic agent does not impact contamination rates when blood cultures are obtained by a phlebotomy team and should, therefore, be based on costs or preference.

Study of the microscopic structure of bone dates back to the earliest use of the optical microscope. Understanding the complexities of the structure of this material has roughly paralleled advances in microscopic technique. Relatively recently a new powerful microscopic technique, atomic force microscopy (AFM), has been invented. So it should not be surprising to expect a new step in our understanding of bone. There have been several recent publications applying AFM to bone and its constituents. None of these studies have examined the shape of naturally occurring mineralites in bone. The issue we addsgss in this work is the elucidation of the three dimensional structure of the apatite mineralites in bone. Chiefly, two methods have been used previously to ascertain this structure: transmission electron microscopy (TEM) and X-ray diffraction (XRD,). The major difficulty with TEM is that it is a projection technique. Each image only yields information about geometry in a single plane. Landis et al. showed how to circumvent this problem using tomographic TEM. AFM provides a corroborative and perhaps more gentle method of directly measuring the structure of very small bone mineralites. It is possible that the interaction energy of the electron beam with very small mineralites is sufficient to vaporize them. The problem with XRD is that the line broadening used can have contributions from sources not directly related to mineralite size. In particular, if a single mineralite contains more than one crystalline domain, then the line broadening yields a measure of the domain size, not the overall mineralite size.

The corrosion of iron overpack materials for nuclear waste forms in salt is highly dependent upon the species produced by the radiolysis of brines which may contact the metal. A computer code has been developed to predict the concentrations of the radiolytically produced species as a function of time using published values for radiation yields of species, chemical reaction rate constants, and parameters derived from experimental evidence.

Design studies for high-level nuclear waste packages for salt repositories have identified low-carbon steel as a candidate material for containers. Among the requirements are strength, corrosion resistance, and fabricability. The studies of the corrosion resistance and structural stability of iron-base materials (particularly lowcarbon steel) are treated in this paper. The materials have been exposed in brines that are characteristic of the potential sites for salt repositories. The effects of temperature, radiation level, oxygen level and other parameters are under investigation. The initial development of corrosion models for these environments is presented with discussion of the key mechanisms under consideration.
