Approaches to improving mental health care for autistic children and young people: a systematic review and meta-analysis

Autistic children and young people (CYP) experience mental health difficulties but face many barriers to accessing and benefiting from mental health care. There is a need to explore strategies in mental health care for autistic CYP to guide clinical practice and future research and support their mental health needs. Our aim was to identify strategies used to improve mental health care for autistic CYP and examine evidence on their acceptability, feasibility, and effectiveness. A systematic review and meta-analysis were carried out. All study designs reporting acceptability/feasibility outcomes and empirical quantitative studies reporting effectiveness outcomes for strategies tested within mental health care were eligible. We conducted a narrative synthesis and separate meta-analyses by informant (self, parent, and clinician). Fifty-seven papers were included, with most investigating cognitive behavioral therapy (CBT)-based interventions for anxiety and several exploring service-level strategies, such as autism screening tools, clinician training, and adaptations regarding organization of services. Most papers described caregiver involvement in therapy and reported adaptations to communication and intervention content; a few reported environmental adjustments. In the meta-analyses, parent- and clinician-reported outcomes, but not self-reported outcomes, showed with moderate certainty that CBT for anxiety was an effective treatment compared to any comparison condition in reducing anxiety symptoms in autistic individuals. The certainty of evidence for effectiveness, synthesized narratively, ranged from low to moderate. Evidence for feasibility and acceptability tended to be positive. Many identified strategies are simple, reasonable adjustments that can be implemented in services to enhance mental health care for autistic individuals. Notable research gaps persist, however.

The Rapid ASKAP Continuum Survey (RACS) is the first large-area survey to be conducted with the full 36-antenna Australian Square Kilometre Array Pathfinder (ASKAP) telescope. RACS will provide a shallow model of the ASKAP sky that will aid the calibration of future deep ASKAP surveys. RACS will cover the whole sky visible from the ASKAP site in Western Australia and will cover the full ASKAP band of 700–1800 MHz. The RACS images are generally deeper than the existing NRAO VLA Sky Survey and Sydney University Molonglo Sky Survey radio surveys and have better spatial resolution. All RACS survey products will be public, including radio images (with $\sim$ 15 arcsec resolution) and catalogues of about three million source components with spectral index and polarisation information. In this paper, we present a description of the RACS survey and the first data release of 903 images covering the sky south of declination $+41^\circ$ made over a 288-MHz band centred at 887.5 MHz.

With 30 threatened species (14 categorized as Critically Endangered and 16 as Endangered, sensu IUCN), Coccothrinax (c. 54 species) is the flagship palm genus for conservation in the Caribbean Island Biodiversity Hotspot. Coccothrinax has its centre of taxonomic diversity in these islands, with c. 51 endemic species. We present a conservation framework for the 14 Critically Endangered species, found in Cuba, Haiti or the Dominican Republic. Only two species (C. jimenezii, C. montana) occur in more than one country (Haiti and the Dominican Republic). Immediate threats include oil drilling and nickel mining, intrusion of saline water into soil, urban and agricultural development, low population recruitment, uncontrolled fires, interspecific hybridization, and unsustainable ethnobotanical practices. Coccothrinax bermudezii, C. borhidiana, C. crinita ssp. crinita, C. leonis and C. spissa are not conserved in protected areas. Coccothrinax bermudezii, C. jimenezii, C. leonis and C. nipensis are not part of ex situ collections. Based on results from a conservation project targeting C. jimenezii, we recommend international cooperation between the three range states to implement integrative conservation management plans, plant exploration initiatives, taxonomic revisions, outreach, and fundraising. The ultimate aim of this review is to provide baseline information that will develop conservation synergy among relevant parties working on Coccothrinax conservation in Cuba, Haiti and the Dominican Republic. Such collaborations could also benefit through partnerships with botanists working in other countries.

The Parkes-MIT-NRAO (PMN) 4.85 GHz continuum radio survey of the southern sky was undertaken in 1990 June and November. This survey was performed on the Parkes 64 m telescope using the NRAO seven-beam receiver. A point-source catalogue of 36,640 radio sources has been produced for the Southern Survey zone −87.5° ≤δ ≤ −37° and for the Tropical Survey zone −29° ≤δ ≤ −9.5°. The flux limit of this survey varies with declination and is typically about 30 mJy.
We have begun to cross-correlate the PMN data with sources contained in catalogues compiled at radio, and other, wavelengths. We have found associations for 96% of the PKSCAT90 2700 MHz database sources, and 95% of the Molonglo 408 MHz Catalogue sources within in the PMN Southern and Tropical Survey zones.
A program to identify the optical counterparts of PMN Southern Survey point sources, S4.85 GHz ≥ 70 mJy, using the COSMOS database, is under way. To facilitate this programme we are improving the positional accuracy of PMN sources with observations made at the Australia Telescope National Facility compact array. We have developed a new “snapshot” mode of observing to process the large number of sources (~ 8000) in our sample. It is possible to obtain accurate positions from three snapshots efficiently with a total integration of < 3 minutes.

Functional transcranial Doppler (fTCD) sonography provides a high temporal resolution measure of blood flow and has over the years proved to be a valuable tool in the clinical evaluation of patients with cerebrovascular disorders. More recently, due to advances in physics and computing, it has become possible to derive indices of cerebrovascular autoregulation (CA) as well as cerebrovascular pressure reactivity (CR), using non-invasive techniques. These indices provide a dynamic representation of the brain's regulatory blood flow mechanisms not only in pathological states but also in health. However, whilst the temporal resolution of these regulatory indices is very good, spatially, the localization of brain regions remains very poor, thus limiting its brain mapping capacity. Functional MRI, on the contrary, is a brain-imaging technique that operates on similar blood flow principles; however, unlike fTCD, it provides high spatial resolution. Because both fTCD and fMRI determine blood flow-dependant imaging parameters, the coupling of fTCD with fMRI may provide greater insight into brain function by virtue of the combined enhanced temporal and spatial resolution that each technique affords. This review summarizes the fTCD technique with particular emphasis on the CA and CR indices and their relationship in traumatic brain injury as well as in health.

A conservation assessment for the three cycad species native to the Bahamas Islands is presented. Results are based on field surveys on all islands where these species occur. Zamia angustifolia is native to Eleuthera, Zamia integrifolia is native to Abaco, Andros, Eleuthera, Grand Bahama and New Providence, and Zamia lucayana is endemic to Long Island. Z. angustifolia is of the highest conservation concern because of the small number of adult plants, its restricted distribution and the extensive development occurring within its habitat. Z. integrifolia also has a restricted distribution on Eleuthera and Grand Bahama and, although threatened by urban development in New Providence, it is relatively common on Abaco and Andros. Z. lucayana comprises three populations within a narrow strip of land of c. 1 km2; we propose a reassignment of its current conservation status from Endangered to Critically Endangered. We assessed the genetic structure of Z. lucayana based on 15 polymorphic microsatellite DNA loci; this indicated that the three known populations should be considered a single management unit. However, the high number of private alleles suggests that genetic drift, indicative of recent fragmentation, is progressing. We propose in situ conservation strategies, and we also collected germplasm from a total of 24 populations of these three cycad species, for ex situ conservation.

Antimicrobial drug shortages continue to increase, with few new therapeutic options available. Nationally, proposals have been offered to alleviate drug shortages; however, these recommendations are unlikely to effect change in the near future. Thus, antimicrobial stewardship leaders in acute care hospitals must develop a prospective management strategy to lessen the impact of these shortages on patient care. Herein, we describe several resources available to aid professionals in antimicrobial stewardship and healthcare epidemiology to manage drug shortages. An effective approach should include prospectively tracking shortages and maximizing inventory by appropriately managing usage. Several tenets should underpin this management. Alternative agents should be rationally chosen before the inventory of the primary agent has reached zero, ethical considerations should be taken into account, and timely notification and communication with key stakeholders should occur throughout the prescribing and dispensing process.

The science of extra-solar planets is one of the most rapidly changing areas of astrophysics and since 1995 the number of planets known has increased by almost two orders of magnitude. A combination of ground-based surveys and dedicated space missions has resulted in 560-plus planets being detected, and over 1200 that await confirmation. NASA's Kepler mission has opened up the possibility of discovering Earth-like planets in the habitable zone around some of the 100,000 stars it is surveying during its 3 to 4-year lifetime. The new ESA's Gaia mission is expected to discover thousands of new planets around stars within 200 parsecs of the Sun. The key challenge now is moving on from discovery, important though that remains, to characterisation: what are these planets actually like, and why are they as they are?
In the past ten years, we have learned how to obtain the first spectra of exoplanets using transit transmission and emission spectroscopy. With the high stability of Spitzer, Hubble, and large ground-based telescopes the spectra of bright close-in massive planets can be obtained and species like water vapour, methane, carbon monoxide and dioxide have been detected. With transit science came the first tangible remote sensing of these planetary bodies and so one can start to extrapolate from what has been learnt from Solar System probes to what one might plan to learn about their faraway siblings. As we learn more about the atmospheres, surfaces and near-surfaces of these remote bodies, we will begin to build up a clearer picture of their construction, history and suitability for life.
The Exoplanet Characterisation Observatory, EChO, will be the first dedicated mission to investigate the physics and chemistry of Exoplanetary Atmospheres. By characterising spectroscopically more bodies in different environments we will take detailed planetology out of the Solar System and into the Galaxy as a whole.
EChO has now been selected by the European Space Agency to be assessed as one of four M3 mission candidates.

During the course of a petrological investigation by one of us (J. C. G.) of the glacial deposits in South-West Wales, samples from a sandy boulder clay at Ludchurch in South-East Pembrokeshire were found to contain abundant grains of a platy colourless mineral with a specific gravity greater than 2·9, a perfect pinacoidal cleavage and high double refraction. This was ultimately diagnosed as diaspore, with which were associated various iron ores, zircon, rutile, chlorite, various tourmalines (brown, green, and pink being the commonest), pyroxenes, amphiboles, staurolite, kyanite, brookite, anatase, topaz, garnet, apatite, muscovite, and andalusite, an assemblage typical of the Irish Sea Drift of this district. A search through the literature for a description of detrital diaspore showed that there did not appear to have been published any detailed description with figures of the detrital forms.

Summary
Any reader who approaches Herodotus' great book with the conventional assumptions of what a modern, Western, post-Thucydidean narrative history is or ought to be - that is, expecting a generally austere concentration on political and military affairs, perhaps citing the texts of treaties, adducing inscriptional evidence, and so on - is likely to be disconcerted, if also delighted, by the way the text unfolds itself. One does not immediately see what is coming. The prospectus-paragraph sets out the project clearly enough. Two parallel clauses, the second of which reinforces, varies and amplifies the first, declare the author's aim as being 'to prevent the memory of human actions being obliterated by the passage of time, and to ensure that great and wonderful achievements, whether carried out by Greeks or by foreigners, are not denied their proper celebration'. It goes on immediately to define a more precise focus: 'to investigate why they (the Greeks and foreigners) went to war with each other' (praef.).
And indeed by the beginning of chapter 6 (say, in our terms, a couple of pages), Herodotus fingers the man who he is 'personally convinced' set the long series of hostilities in motion: Croesus the king of Lydia. Croesus was the first ruler to levy tribute from the Greek settlements on the west coast of Asia Minor; he was also, we go on to discover in the course of the first book, the man who made the fateful mistake of attempting an eastward expansion of his kingdom and suffered a disastrous defeat at the hands of the Persians, thus bringing Greeks and Persians face to face for the first time and setting the scene for the wars that would follow.
