Reducing the Use of Rapid Tranquilisation in Over 65s in a General Hospital

Aims
A previous audit of use of rapid tranquillisation in older adults conducted in 2019 identified high rates of use of sedation, and poor adherence to local guidelines. Following this audit, a number of quality improvement (QI) initiatives were undertaken in order to try to improve practice, including multiple teaching sessions to a variety of staff. This re-audit was conducted to study whether initiatives had been effective in line with the Plan Do Study Act cycle of Quality Improvement.
MethodsUsing the same audit tool developed in 2019, six wards (2 geriatric, 3 medical and 1 surgical) were audited. Patients over 65 given oral or intramuscular sedating medications had their drug charts and notes reviewed. Data were collected on type of sedation, route prescribed, whether it was prescribed regularly or PRN, whether an indication was documented, underlying diagnosis and what monitoring took place post sedation.
Results297 drug charts were reviewed, and 13 patients were prescribed rapid tranquilisation (RT). The maximum daily dose was included in 63% of prescriptions similar to that of the first audit (58%). The most common route of administration was intramuscular, unlike the previous audit which was oral/intramuscular.
50% of prescriptions documented an indication, of which 25% were illegible. Whilst in the first audit the figure was 33%.
Of all the patients prescribed RT, 77% had a diagnosis of delirium, 77% had a diagnosis of dementia and about 53.8% had both. In both audits 100% of patients had a diagnosis of dementia or delirium. Most prescriptions were for lorazepam (75%). There was no evidence of observations being taken in line with post RT monitoring in the trust policy in both audits
ConclusionFurther work needs to be done to improve practice. Interventions to date have not been effective. Further plans for QI work include updating the RT policy to be more specific and useful for the acute trust, to fit in with a recently introduced electronic records system (ERS) and to include a clear section on older adults with signposting to the delirium and dementia policies. As well as adding prompts and protocols to the ERS to support safe prescribing and dispensing of RT. Teaching will be repeated and a poster has been developed and promoted on all the wards. The project group are planning to join the trust's ‘medication safety huddle’ regularly to include pharmacists in teaching and work. The audit will be repeated in three months time.
Household food insecurity is prevalent in a cohort of postpartum women who registered in the Canada Prenatal Nutrition Program in Toronto

Objective:
To assess the prevalence, severity and socio-demographic predictors of household food insecurity among vulnerable women accessing the Canada Prenatal Nutrition Program (CPNP) and to examine associations between household food insecurity and breastfeeding practices to 6 months.
Design:Cohort investigation pooling data from two studies which administered the 18-item Household Food Security Survey Module at 6 months postpartum and collected prospective infant feeding data at 2 weeks and 2, 4 and 6 months. Household food insecurity was classified as none, marginal, moderate or severe. Logistic regression analyses were performed to assess predictors of household food insecurity and associations between household food security (any and severity) and continued and exclusive breastfeeding.
Setting:Three Toronto sites of the CPNP, a federal initiative targeting socially and/or economically vulnerable women.
Participants:316 birth mothers registered prenatally in the CPNP from 2017 to 2020.
Results:Household food insecurity at 6 months postpartum was highly prevalent (44 %), including 11 % in the severe category. Risk of household food insecurity varied by CPNP site (P < 0·001) and was higher among multiparous participants (OR 2·08; 95 % CI 1·28, 3·39). There was no association between the prevalence or severity of food insecurity and continued or exclusive breastfeeding to 6 months postpartum in the adjusted analyses.
Conclusions:Household food insecurity affected nearly half of this cohort of women accessing the CPNP. Further research is needed on household food insecurity across the national CPNP and other similar programmes, with consideration of the implications for programme design, service delivery and policy responses.

Aims
The risk of antipsychotic-associated cardiovascular and metabolic events may differ among countries, and limited real-world evidence has been available comparing the corresponding risks among children and young adults. We, therefore, evaluated the risks of cardiovascular and metabolic events in children and young adults receiving antipsychotics.
MethodsWe conducted a multinational self-controlled case series (SCCS) study and included patients aged 6–30 years old who had both exposure to antipsychotics and study outcomes from four nationwide databases of Taiwan (2004–2012), Korea (2010–2016), Hong Kong (2001–2014) and the UK (1997–2016) that covers a total of approximately 100 million individuals. We investigated three antipsychotics exposure windows (i.e., 90 days pre-exposure, 1–30 days, 30–90 days and 90 + days of exposure). The outcomes were cardiovascular events (stroke, ischaemic heart disease and acute myocardial infarction), or metabolic events (hypertension, type 2 diabetes mellitus and dyslipidaemia).
ResultsWe included a total of 48 515 individuals in the SCCS analysis. We found an increased risk of metabolic events only in the risk window with more than 90-day exposure, with a pooled IRR of 1.29 (95% CI 1.20–1.38). The pooled IRR was 0.98 (0.90–1.06) for 1–30 days and 0.88 (0.76–1.02) for 31–90 days. We found no association in any exposure window for cardiovascular events. The pooled IRR was 1.86 (0.74–4.64) for 1–30 days, 1.35 (0.74–2.47) for 31–90 days and 1.29 (0.98–1.70) for 90 + days.
ConclusionsLong-term exposure to antipsychotics was associated with an increased risk of metabolic events but did not trigger cardiovascular events in children and young adults.
Beyond morbidity and mortality in reintroduction programmes: changing health parameters in reintroduced eastern bettongs Bettongia gaimardi

The eastern bettong Bettongia gaimardi, a potoroid marsupial, has been extinct on the Australian mainland since the 1920s. Sixty adult bettongs were reintroduced from the island of Tasmania to two predator-free fenced reserves on mainland Australia. We examined baseline health parameters (body weight, haematology and biochemistry, parasites and infectious disease exposure) in a subset of 30 (13 male, 17 female) individuals at translocation and again at 12–24 months post-reintroduction. The mean body weight increased significantly post-reintroduction but there were no significant differences in body weight between the two reintroduction sites or between the sexes in response to reintroduction. Differences were evident in multiple haematological and biochemical variables post-reintroduction but there were few differences between the two reintroduced populations or between the sexes in response to reintroduction. Ectoparasite assemblages differed, with five of 13 species failing to persist, and an additional four species were identified post-reintroduction. None of the bettongs had detectable antibodies to the alphaherpesviruses Macropodid herpesvirus 1 and 2 post-reintroduction, including one individual that was seropositive at translocation. Similarly, the novel gammaherpesvirus potoroid herpesvirus 1 was not detected by polymerase chain reaction (PCR) in any of the bettongs post-reintroduction, including one individual that was PCR-positive at translocation. None of the bettongs had detectable antibodies to Toxoplasma gondii either at translocation or post-reintroduction. Our data demonstrate changing baseline health parameters in eastern bettongs following reintroduction to the Australian mainland are suggestive of improved health in the reintroduced populations, and provide additional metrics for assessing the response of macropodoids to reintroduction.
Adventures with a Flipped Classroom and a Materials Science and Engineering MOOC : “Fools Go Where Angels Fear to Tread”

In the fall of 2012 the Stanford University materials science course Solar Cells, Fuel Cells and Batteries: Materials for the Energy Solution was offered as a flipped class and a massively open online course (MOOC). To the best of our knowledge, this was the first materials science MOOC. Here we describe how the course was implemented, and present results on performance, demographics and other observations that were made. Finally, we provide some perspectives for the future of the implementation of these engineering MOOCs.

Summary
Of styles and genres
The vast and far-reaching changes in musical style that have occurred in the past hundred years make it a richly rewarding period for singers. Faced with such a chaotic and variegated tapestry of stylistic strands, ranging from the truly bizarre to the comfortably familiar, the contemporary singer can be forgiven for proceeding with extreme caution. Yet, for those committed to keeping in touch with the new, it is a privilege to be caught up in the aftermath of the early twentieth century's exhilarating maelstrom of contrasting artistic influences, inspirations, experiments and occasional shockwaves, all of which reverberate to the present day.
At the beginning of the twentieth century, many European ‘late Romantic’ composers were working within the basic vocal traditions of the nineteenth century, while others had chosen to strike out in bracing new directions. In opera, composers such as Puccini (1858–1924) and Richard Strauss (1864–1949) maintained the tradition of predominantly lyrical writing, with a growing diversity of vocal characterisation, and the less-known figures of Franz Schreker (1878–1934) and Erich Korngold (1897–1957) enjoyed runaway successes with Der ferne Klang (1912) and Die tote Stadt (1920). The emergence across Eastern Europe of a new kind of pungently declamatory vocal writing, pioneered in the previous century by Musorgsky (1839–81), found its fulfilment in the Janáček operas, and those of Prokofiev (1891–1953) and Shostakovich (1906–75).

The re-discovery and recent commercial release, on Regis/Forum, of one of my earliest performances of Schoenberg's Pierrot Lunaire, cannot help but evoke a mood of piquant personal reminiscence. The recording was made in 1967 at Olympic Studios in London, roughly two years after I first became acquainted with this riveting, endlessly compelling and many-layered masterpiece. The mid-1960s were marked by optimism, eager creative responses to the challenge of the new, and a spirit of idealism that is perhaps difficult to recapture in today's more commercially-driven world.
