Availability and affordability of healthy and less healthy food in Nova Scotia: where you shop may affect the availability and price of healthy food

Objective:
The present study aimed to examine the availability and price of healthier compared with less healthy foods by geography, store category and store type for convenience stores, and by store size for grocery stores in Nova Scotia.
Design:A cross-sectional study that examined differences in the overall availability and price of healthier compared to less healthy foods in grocery and convenience stores in Nova Scotia. The Nova Scotia Consumer Food Environment project was part of a larger initiative of the Nova Scotia government (Department of Health and Wellness) to assess the food and beverage environment in Nova Scotia in 2015/16.
Setting:Four geographic zones (Nova Scotia Health Authority Management Zones) in Nova Scotia, Canada.
Participants:A sample of forty-seven grocery stores and fifty-nine convenience stores were selected from a list of 210 grocery stores and 758 convenience stores in Nova Scotia to ensure geographic and store type representation in our sample.
Results:Findings indicate that rurality had a significant effect on food availability as measured by the Nutrition Environment Measures Surveys (NEMS) score (P < 0·01); there was a higher availability of healthy foods in rural compared to urban areas for convenience stores but not grocery stores. Healthier foods were also more available in chain stores compared to independent stores (P < 0·01) and in large stores compared to small and medium stores (P < 0·001 and P < 0·01, respectively).
Conclusions:The availability of and accessibility to less healthy foods in Nova Scotia food environment suggests that there is a need for government policy action to support a food environment that contributes to healthier diets.

Studies examining associations between fetal serotonin reuptake inhibitor (SRI) exposure and child autism spectrum disorder (ASD) diagnoses or delayed language remain mixed and rarely prospectively follow children or employ gold-standard assessments. We prospectively followed a cohort of mother–child dyads from pregnancy through early elementary school (N = 178), and obtained maternal and alternate–caregiver ratings of behaviors related to ASD (N = 137), as well as direct, gold-standard assessments of child ASD symptoms and pragmatic language among dyads who experienced prenatal depression and either took SRIs or were medication free during pregnancy (N = 44). Prenatal SRI exposure was related to maternal ratings of ASD-related behaviors (β = 0.24 95% confidence interval; CI [0.07, 0.48]), and, among boys, alternative caregiver ratings (males-only β = 0.28 95% CI [0.02, 0.55], females-only β = −0.21 95% CI [–0.63, 0.08]). However, results of our direct assessments suggest an association between SRI exposure and reduced pragmatic language scores (β = –0.27, 95% CI [–0.53, –0.01], but not ASD (Autism Diagnostic Observation Schedule β = 0.14 95% CI [–0.15, 0.41]; Social Responsiveness Scale β = 0.08 95% CI [–0.25, 0.40]). These discrepancies point to issues regarding how ASD is assessed, and the possibility that SRIs may be more strongly associated with language or other broader behaviors that coincide with ASD. Larger prospective studies that incorporate thorough, gold-standard assessments of ASD, language, and other ASD-related behaviors are needed.

Objectives
The Flint Community Resilience Group (Flint, Michigan USA) and the Centers for Disease Control and Prevention (CDC; Atlanta, Georgia USA) assessed behavioral health concerns among community members to determine the impact of lead contamination of the Flint, Michigan water supply.
MethodsA Community Assessment for Public Health Emergency Response (CASPER) was conducted from May 17 through May 19, 2016 using a multi-stage cluster sampling design to select households and individuals to interview.
ResultsOne-half of households felt overlooked by decision makers. The majority of households self-reported that at least one member experienced more behavioral health concerns than usual. The prevalence of negative quality of life indicators and financial concerns in Flint was higher than previously reported in the Michigan 2012 and 2014 Behavioral Risk Factor Surveillance System (BRFSS) survey.
ConclusionsThe following can be considered to guide recovery efforts in Flint: identifying additional resources for behavioral health interventions and conducting follow-up behavioral health assessments to evaluate changes in behavioral health concerns over time; considering the impact of household economic factors when implementing behavioral health interventions; and ensuring community involvement and engagement in recovery efforts to ease community stress and anxiety.

Summary
In her unfinished autobiography, Jean Rhys (1890–1979) describes the birth of her writing career as a quasi-memorial for herself as a person: buying some black exercise books and the red, blue, green and yellow quill pens to ‘cheer up’ her table and banish its bareness, Rhys represents her writing out of her unhappy first love affair as a compulsive purgation of the experience, but one that left her bereft in the recognition that something in her had died. ‘I filled three exercise books and half another, then I wrote: “Oh God, I'm only twenty and I'll have to go on living and living and living.” I knew then that it was finished and that there was no more to say.’ From this genesis of art in inner death, Rhys went on to write five novels and numerous short stories that present a strikingly consistent world view and aesthetic, both of which hearken back to the affective stance she assumes in her earliest writing and then reiterates throughout her work up to and including her posthumously published autobiography. From her despairing diaries written at the age of twenty to her death nearly seventy years later, Rhys examined in meticulously polished prose a set of themes including the workings of the mind and emotion, gender and race relations, colonial history and, as we argue in this introduction, a certain spectrality of existence. Jean Rhys: Twenty- First-Century Approaches examines this cogency of style and world view from the major approaches that have defined Rhys criticism, and brings them together with emerging theoretical work to set the course for Rhys studies in the twenty-first century.
Much has been written about how Rhys's work defies periodisation and transcends categories, in the same way that her characters wander nomadically and live on the edges of what Sasha in Good Morning, Midnight thinks of as ‘you, who represent Society’. She is viewed as a modernist, postcolonial, Caribbean, British and Creole writer – yet to parse these identities is to fall short of the complexity of her work and the ways in which it troubles the very categories through which she is read. Social stress and the oxytocin receptor gene interact to predict antisocial behavior in an at-risk cohort

Polymorphisms in the oxytocin receptor gene are commonly associated with prosocial behaviors in the extant literature, yet their role in antisocial behaviors has rarely been explored, particularly during the transition from adolescence to early adulthood. We examined a prospective cohort (N = 404), collecting youth, mother, and clinician reports of conduct-disordered and antisocial behavior at ages 15 and 20. The oxytocin receptor gene rs53576 polymorphism was hypothesized to interact with social stress to predict antisocial outcomes. Structural equation modeling results revealed a significant main effect at age 15 (p = .025); those with the G allele exhibited higher levels of conduct problems. Structural equation modeling revealed a significant Gene × Environment interaction at age 20 (p = .029); those with the G allele who experienced high social stress exhibited higher levels of antisocial behavior. Heterozygous (AG) grouping models were compared, and parameter estimations supported G dominant groupings. These novel findings suggest that rs53576 polymorphisms may influence social salience and contribute to risk for antisocial outcomes, particularly under conditions of high social stress.
Contextual effects on the conversations of mothers and their children with language impairment

Twenty-four mothers and their preschool children with language impairment participated in two 12-min sessions of toy play and book reading that were transcribed to yield maternal mean length of utterance in morphemes (MLU-m), type token ratio (TTR), and maternal interaction style (directive vs. responsive). Maternal MLU-m was significantly longer during book reading than during toy play, whereas TTR was similar across contexts. In contrast, children's MLU-m was similar across contexts, whereas TTR was higher during book reading. Mothers used an eliciting style characterized by more commands and questions during toy play than during book reading. Only maternal MLU-m predicted children's expressive language skills (i.e., a composite score of two standardized language tests). The implications include sampling both book reading and play interactions because they provide differential opportunities for conversation and language productivity.

Objective.
To determine the prevalence of Acinetobacter baumannii, an important healthcare-associated pathogen, among mechanically ventilated patients in Maryland.
Design.The Maryland MDRO Prevention Collaborative performed a statewide cross-sectional active surveillance survey of mechanically ventilated patients residing in acute care and long-term care (LTC) facilities. Surveillance cultures (sputum and perianal) were obtained from all mechanically ventilated inpatients at participating facilities during a 2-week period.
Setting.All healthcare facilities in Maryland that provide care for mechanically ventilated patients were invited to participate.
Patients.Mechanically ventilated patients, known to be at high risk for colonization and infection with A. baumannii, were included.
Results.Seventy percent (40/57) of all eligible healthcare facilities participated in the survey, representing both acute care (n = 30) and LTC (n = 10) facilities in all geographic regions of Maryland. Surveillance cultures were obtained from 92% (358/390) of eligible Patients. A. baumannii was identified in 34% of all mechanically ventilated patients in Maryland; multidrug-resistant A. baumannii was found in 27% of all Patients. A. baumannii was detected in at least 1 patient in 49% of participating facilities; 100% of LTC facilities had at least 1 patient with A. baumannii, compared with 31% of acute care facilities. A. baumannii was identified from all facilities in which 10 or more patients were sampled.
Conclusions.A. baumannii is common among mechanically ventilated patients in both acute care and LTC facilities throughout Maryland, with a high proportion of isolates demonstrating multidrug resistance.
Characterization of two non-native invasive bark beetles, Scolytus schevyrewi and Scolytus multistriatus (Coleoptera: Curculionidae: Scolytinae)

Scolytus schevyrewi Semenov, the banded elm bark beetle, and S. multistriatus Marsham, the smaller European elm bark beetle, are morphologically similar. Reliance on adult external morphological characters for identification can be problematic because of wide within-species variability and the need for good-quality specimens. The inability to identify developmental stages can also hamper early-detection programs. Using two character identification systems, genitalic (aedeagus) morphology, and DNA markers (random amplified polymorphic DNA polymerase chain reaction (RAPD-PCR)) to distinguish S. schevyrewi from S. multistriatus, we examined specimens from geographically distinct populations of both species collected from infested host trees or semiochemical-baited funnel traps. We found that aedeagus morphology can be used to identify the two species. The use of two oligonucleotide primers in the RAPD-PCR analysis yielded distinct DNA banding patterns for the two species. Species identification using RAPD-PCR analysis was validated by a blind test and used to make species identifications of larval specimens. These tools improve the ability to differentiate between S. schevyrewi and S. multistriatus at immature and adult stages, and could be developed and used for other scolytines as well.

The effects of infant suckling patterns on the post-partum resumption of ovulation and on birth-spacing are investigated among the Gainj of highland New Guinea. Based on hormonal evidence, the median duration of lactational anovulation is 20·4 months, accounting for about 75% of the median interval between live birth and next successful conception (i.e. resulting in live birth). Throughout lactation, suckling episodes are short and frequent, the interval changing slowly over time, from 24 minutes in newborns to 80 minutes in 3-year olds. Maternal serum prolactin concentrations decline in parallel with the changes in suckling patterns, approaching the level observed in non-nursing women by about 24 months post-partum. A path analysis indicates that the interval between suckling episodes is the principal determinant of maternal prolactin concentration, with time since parturition affecting prolactin secretion only in so far as it affects suckling frequency. The extremely prolonged contraceptive effect of breast-feeding in this population thus appears to be due to (i) a slow decline in suckling frequency with time since parturition and (ii) absence of a decline over time in hypothalamic–pituitary responsiveness to the suckling stimulus.

Can Canadian Seniors on Public Pensions Afford a Nutritious Diet?*

This study examined whether Canada's public pensions (Old Age Security and Canada Pension Plan) provided adequate income for seniors living in Nova Scotia in 2005 to afford a basic nutritious diet. Monthly incomes were compared to essential monthly expenses for four household scenarios: (a) married couple, 80 years and 78 years, in urban Nova Scotia; (b) single male, 77 years in rural Nova Scotia; (c) a couple, 70 years and 65 years, in rural Nova Scotia; (d) widowed female, 85 years, in urban Nova Scotia. The monthly food costs for the four households were CAN$313.32, $193.83, $316.71, and $150.89, respectively. Results showed that both single-member households lacked the necessary funds for a nutritious diet, while living with a partner seemed to protect against inadequate financial resources. These findings illustrate the need to improve Canada's retirement systems to ensure all seniors have adequate financial resources to meet their basic needs—including nutritious food—prevent chronic disease, and ultimately improve quality of life.

The Gainj of highland Papua New Guinea do not use contraception but have a total fertility rate of only 4·3 live births per woman, one of the lowest ever recorded in a natural fertility setting. From an analysis of cross-sectional demographic and endocrinological data, the causes of low reproductive output have been identified in women of this population as: late menarche and marriage, a long interval between marriage and first birth, a high probability of widowhood at later reproductive ages, low effective fecundability and prolonged lactational amenorrhoea. These are combined with near-universal marriage, a low prevalence of primary sterility and a pattern of onset of secondary sterility similar to that found in other populations. Of all the factors limiting fertility, by far the most important are those involved in birth spacing, especially lactational amenorrhoea.
