The series editors look forward to sharing with you the story of epilepsy genetics through a series of Elements. They will bring together many voices, by text as well as video, to illustrate the history of epilepsy genetics, the many on-going efforts in the field, and how they hope to address the still unanswered questions that command the attention of all of us and our colleagues across the globe.
The Patient Registry for Adolescents and Adults with Stable Fontan Circulation aims to describe a contemporary cohort of Fontan patients who could be eligible for a clinical trial investigating macitentan, an endothelin receptor antagonist. This international, non-interventional, multicentre, cross-sectional, observational registry enrolled patients with “stable” Fontan circulation ≥10 years following extra-cardiac conduit or lateral tunnel procedure. Main exclusion criteria were NYHA functional class IV, reoperation of Fontan circulation, or signs of disease worsening. Patient characteristics at enrolment are described; available data were collected during a single registration visit. Of the 266 screened patients, 254 were included in this analysis. At enrolment, median (interquartile range) age was 24 (20;30) years, 37%/63% of patients were from the USA/Europe, 54% were male, 54%/47% had undergone extra-cardiac conduit/lateral tunnel procedures, and 95% were in NYHA functional class I or II. History of arrhythmia was more common in older patients and patients with lateral tunnel; overall prevalence was 19%. Most laboratory values were within the normal range but mean creatinine clearance was abnormally low (87.7 ml/min). Angiotensin-converting enzyme inhibitors were used by 48% of patients and their use was associated with creatinine clearance <90 ml/min (p = 0.007), as was Fontan completion at an older age (p = 0.007). 53.4% of patients had clinical characteristics that could potentially meet an endothelin receptor antagonist trial’s eligibility criteria. The PREpArE-Fontan registry describes a cohort of patients who could potentially participate in an endothelin receptor antagonist trial and identified early subtle signs of Fontan failure, even in “stable” patients.
We study the damping of collisionless Alfvénic turbulence in a strongly magnetised plasma by two mechanisms: stochastic heating (whose efficiency depends on the local turbulence amplitude $\unicode[STIX]{x1D6FF}z_{\unicode[STIX]{x1D706}}$) and linear Landau damping (whose efficiency is independent of $\unicode[STIX]{x1D6FF}z_{\unicode[STIX]{x1D706}}$), describing in detail how they affect and are affected by intermittency. The overall efficiency of linear Landau damping is not affected by intermittency in critically balanced turbulence, while stochastic heating is much more efficient in the presence of intermittent turbulence. Moreover, stochastic heating leads to a drop in the scale-dependent kurtosis over a narrow range of scales around the ion gyroscale.
Stochastic heating refers to an increase in the average magnetic moment of charged particles interacting with electromagnetic fluctuations whose frequencies are smaller than the particles’ cyclotron frequencies. This type of heating arises when the amplitude of the gyroscale fluctuations exceeds a certain threshold, causing particle orbits in the plane perpendicular to the magnetic field to become stochastic rather than nearly periodic. We consider the stochastic heating of protons by Alfvén-wave (AW) and kinetic-Alfvén-wave (KAW) turbulence, which may make an important contribution to the heating of the solar wind. Using phenomenological arguments, we derive the stochastic-proton-heating rate in plasmas in which $\unicode[STIX]{x1D6FD}_{\text{p}}\sim 1$–30, where $\unicode[STIX]{x1D6FD}_{\text{p}}$ is the ratio of the proton pressure to the magnetic pressure. (We do not consider the $\unicode[STIX]{x1D6FD}_{\text{p}}\gtrsim 30$ regime, in which KAWs at the proton gyroscale become non-propagating.) We test our formula for the stochastic-heating rate by numerically tracking test-particle protons interacting with a spectrum of randomly phased AWs and KAWs. Previous studies have demonstrated that at $\unicode[STIX]{x1D6FD}_{\text{p}}\lesssim 1$, particles are energized primarily by time variations in the electrostatic potential and thermal-proton gyro-orbits are stochasticized primarily by gyroscale fluctuations in the electrostatic potential. In contrast, at $\unicode[STIX]{x1D6FD}_{\text{p}}\gtrsim 1$, particles are energized primarily by the solenoidal component of the electric field and thermal-proton gyro-orbits are stochasticized primarily by gyroscale fluctuations in the magnetic field.
The El Niño event in 2015/2016 was one of the strongest since at least 1950. Through surveys and interviews with key informants, we found businesses in the capital cities of Zambia, Botswana and Kenya experienced major disruption to their activities from El Niño related hydroelectric load shedding, water supply disruption and flooding, respectively. Yet, during the 2015/2016 El Niño, fluctuations in precipitation were not extreme considering the strength of the El Niño event. Results therefore highlight that even fairly moderate precipitation anomalies can contribute to major disruption to economic activity. Addressing the risk of disruption – and supporting the private sector to adapt – is a development priority.
We evaluated the utility of vancomycin-resistant Enterococcus (VRE) surveillance by varying 2 parameters: admission versus weekly surveillance and perirectal swabbing versus stool sampling.
Design
Prospective, patient-level surveillance program of incident VRE colonization.
Setting
Liver transplant surgical intensive care unit (SICU) of a tertiary-care referral medical center with a high prevalence of VRE.
Patients
All patients admitted to the SICU from June to August 2015.
Methods
We conducted a point-prevalence estimate followed by admission and weekly surveillance by perirectal swabbing and/or stool sampling. Incident colonization was defined as a negative screen followed by positive surveillance. VRE was detected by culture on Remel Spectra VRE chromogenic agar. Microbiologically-confirmed VRE bloodstream infections (BSIs) were tracked for 2 months. Statistical analyses were calculated using the McNemar test, the Fisher exact test, the t test, and the χ2 test.
Results
In total, 91 patients underwent VRE surveillance testing. The point prevalence of VRE colonization was 60.9%; VRE prevalence on admission was 30.1%. Weekly surveillance identified an additional 7 of 28 patients (25.0%) with incident colonization. VRE BSIs were more common in VRE-colonized patients than in noncolonized patients (8 of 43 vs 2 of 48; P=.028). In a direct comparison, perirectal swabs were more sensitive than stool samples in detecting VRE (64 of 67 vs 56 of 67; P=.023). Compliance with perirectal swabbing was 89% (201 of 226) compared to 56% (127 of 226) for stool collection (P≤0.001).
Conclusions
We recommend weekly VRE surveillance over admission-only screening in high-burden units such as liver transplant SICUs. Perirectal swabs had greater collection compliance and sensitivity than stool samples, making them the preferred methodology. Further work may have implications for antimicrobial stewardship and infection control.
The scholarly study of fifteenth-century English verse is very much a late twentieth-century phenomenon. A number of the writings associated with the fifteenth-century authors covered in this collection of essays were not accessible in usable editions until some point in the twentieth century, and the critical tendency to overlook fifteenth-century poetry was in part an inevitable result of its simple unavailability. But the early decades of the twentieth century saw significant changes in the landscape of fifteenth-century verse, attributable largely to the efforts of dedicated individuals working in isolation. Henry Bergen, most significantly, produced in the first two decades of the twentieth century notable editions of the two longest poetic works of the fifteenth century, John Lydgate's Troy Book and Fall of Princes, each over 30,000 lines (Bergen 1906–35 and 1924-27). The work of Eleanor Hammond on fifteenth-century manuscript and textual culture in England generated partial editions and an important survey of fifteenth-century poetry in the form of English Verse between Chaucer and Surrey (1927). And Walter Schirmer's study of John Lydgate, published originally in German in 1952 and translated into English in 1961, offered a Kulturbild, a historical and cultural analysis of the most prolific poet of the century that has still not been superseded. These figures stand apart from a general tendency to see the verse of the period between Chaucer and the early sixteenth century as largely unrewarding.
In the fervent, contentious, and sometimes ostentatious religious culture of fifteenth-century England, one writer stands out as a particularly prolific and versatile author of devotional texts: the monk of Bury St Edmunds, John Lydgate (c.1370–1449). Lydgate wrote thousands of lines of religious poetry for a wide range of patrons, both individual and institutional, and his poetry provides a comprehensive picture of orthodox fifteenth-century English religious life and its concerns: highly sacramental, habitually influenced by meditative spirituality and imitatio Christi, defiantly anti-Lollard, and profoundly invested in the cults of the saints and of the Virgin. Perhaps more surprisingly, however, Lydgate's religious poetry is, like his more overtly political poems, often highly topical. Lydgate was long viewed as a repetitive monk, cloistered in self-indulgent rhetoric (Mortimer 2005: 2–20, summarises the relevant unflattering assessments and Lydgate's changing critical fortunes). Now, Lydgate is increasingly seen as a poet who innovated and experimented: he negotiated the vernacular translation of religious material, the incorporation of Chaucerian diction and themes, and the use of aureate language and humanist ideas all within the parameters of orthodoxy. Lydgate's poetry also inaugurates several new European traditions into English devotional culture: subjects such as the Dance of Death and visual-material forms such as mural poetry and the pietà or image of pity find early English expressions in Lydgate's poetry. In short, Lydgate provides us with a ready conspectus of religious literary forms, from short prayers to epic narratives, poised between cloistered monasticism and a vigorous patronage culture of ‘pray and display’.
