In >100,000 observations across Swiss acute-care hospitals, hand hygiene (HH) adherence significantly increased during the first coronavirus disease 2019 (COVID-19) wave. However, despite persisting COVID-19 activity, HH adherence returned to prepandemic levels over a 2-year observation period. These results indicate that training and support remains challenging.
The dating of pollen grains is emerging as the method of choice for lacustrine climate archives that contain few datable macrofossils. Due to the need for high-purity pollen concentrates, new methods are constantly being developed to precisely separate pollen grains. Flow cytometry represents a promising alternative to conventional approaches, enabling the identification of pollen grains through fluorescence and rapid separation for radiocarbon analysis using accelerator mass spectrometry, which has so far been limited to sediments with a high proportion of conifer pollen. We present a revised method for processing large sediment samples, resulting in high-purity pollen and spore concentrates. Using this approach small- to medium-sized pollen and bryophyte spores were isolated from Lake Van sediment samples (Eastern Anatolia, Turkey) in sufficient purity for radiocarbon dating. However, a systematic age discrepancy between pollen and bryophyte spore concentrates was noted. By adapting the chemical and cytometric methods, pure pollen concentrates can be created for sediments with low organic content enabling age determination of climate archives with a low proportion of large pollen or low pollen concentration.
National concerns over food insecurity and obesity have prompted legislation seeking to further restrict Supplemental Nutrition Assistance Program (SNAP) purchases. The objective of this study is to provide insight on the potential impact of proposed purchase restrictions by comparing SNAP participant and income-eligible non-participants’ expenditures on current SNAP-restricted foods, that is, hot foods, prepared foods, alcohol, vitamins and meal supplements.
Design:
Cross-sectional study. Bivariate analysis and multivariable regression analysis with an instrumental variables approach were employed to compare the probability of purchasing and expenditures on current SNAP-restricted foods among SNAP participants and income-eligible non-participants.
Setting:
National Household Food Acquisition and Purchase Survey, 2012–2013.
Participants:
2513 households, of which 1316 were SNAP participants and 1197 were income-eligible non-participants.
Results:
Both the share of households purchasing and household expenditures on current SNAP-restricted foods were similar among SNAP participants and income-eligible non-participants.
Conclusions:
Results provide further empirical evidence that proposed SNAP purchase restrictions on sugar-sweetened beverages, snack foods and luxury foods are unlikely to have a meaningful effect on SNAP household food purchases.
The ongoing retreat of mountain glaciers necessitates the development of future scenarios of glacier runoff. These scenarios are not only governed by future climate scenarios influencing glacier mass balance but also by the glacier volumes, which are subject to melt. Ground-penetrating radar (GPR) is a valuable tool for measuring the thickness of mountain glaciers, although ground-based measurements are labour-intensive, so not all glaciers can be surveyed. This study presents the results of GPR surveys on 64 Alpine glaciers, carried out between 1995 and 2010. The glacier areas range from 0.001 to 18.4 km2, and their ice thickness was surveyed with an average density of 36 points km-2. The point measurements were extrapolated manually to derive volume maps. The mean ice thickness varies between 10 and 92 m; the maximum ice thickness is about three times the mean thickness. According to the glacier state recorded in the second glacier inventory, the 64 glaciers cover an area of 223.3 ± 3.6 km2, with a mean thickness of 50 ± 3 m and a glacier volume of 11.9 ± 1.1 km3. The mean maximum ice thickness is 119 ± 5m.
Surface mineralogical compositions and their association to modern processes are well known from the east Atlantic and south-west Indian sectors of the Southern Ocean, but data from the interface of these areas - the Prydz Bay–Kerguelen region - is still missing. The objective of our study was to provide mineralogical data of reference samples from this region and to relate these mineralogical assemblages to hinterland geology, weathering, transport and depositional processes. Clay mineral assemblages were analysed by means of X-ray diffraction technique. Heavy mineral assemblages were determined by counting of gravity-separated grains under a polarizing microscope. Results show that by use of clay mineral assemblages four mineralogical provinces can be subdivided: i) continental shelf, ii) continental slope, iii) deep sea, iv) Kerguelen Plateau. Heavy mineral assemblages in the fine sand fraction are relatively uniform except for samples taken from the East Antarctic shelf. Our findings show that mineralogical studies on sediment cores from the study area have the potential to provide insights into past shifts in ice-supported transport and activity and provenance of different water masses (e.g. Antarctic slope current and deep western boundary current) in the Prydz Bay–Kerguelen region.

risk for psychosis but already suffer from mental and functional
disturbances

To evaluate the acute effects of a combined supportive and antipsychotic
treatment on prodromal symptoms
Method
Putatively prodromal individuals were randomly assigned to a
needs-focused intervention without (n=59) or with
amisulpride (n=65). Outcome measures at 12-weeks effects
were prodromal symptoms, global functioning and extrapyramidal
side-effects
Results
Amisulpride plus the needs-focused intervention produced superior effects
on attenuated and full-blown psychotic symptoms, basic, depressive and
negative symptoms, and global functioning. Main side-effects were
prolactin associated
Conclusions
Coadministration of amisulpride yielded a marked symptomatic benefit.
Effects require confirmation by a placebo-controlled study
Rabies virus was inadvertently transmitted to a lung transplant recipient through donor lungs. The patient was given ventilatory assistance and cared for postoperatively for 6 weeks before a diagnosis of rabies virus infection was made. Postexposure prophylaxis (PEP) was offered to potentially exposed healthcare workers (HCWs).
Methods.
Only HCWs classified as belonging to possible and/or proven contact groups (according to a standardized interview) received PEP. The risk of individual HCWs being exposed to rabies virus was reassessed on the basis of viral concentrations measured in the patient's excretions and body fluids. HCWs who were vaccinated as part of PEP were followed up prospectively according to a standardized procedure.
Results.
Of 179 HCWs and other patient contacts, 132 met the eligibility criteria for PEP (118 [89.4%] with possible contact and 14 [10.6%] with proven contact with the patient's excretions and/or body fluids). One hundred thirty-one individuals started PEP, and 126 met the inclusion criteria for analysis. Of these, 48 (38%) developed at least 1 adverse effect (8 [6.3%] had fever, 37 [29.4%] had headache, 3 [2.4%] had lymphadenopathy, 17 [13.5%] had dizziness, and 6 [4.8%] had paresthesia). No HCW or other patient contact developed rabies or serious PEP-related adverse effects. Reassessment of the individual's risk of infection as a function of the viral concentration in the patient's excretions and/or body fluids (up to 5.12 × 107 copies/mL) revealed that 103 HCWs (78.0%) had contact with high-risk substances (89 [67.40%] had possible contact and 14 [10.7%] had proven contact).
Conclusion.
HCWs can be exposed to significant viral concentrations in excretions and/or body fluids from rabies virus-infected lung transplant recipients. Because widespread use of PEP entails the possibility of significant health problems for HCWs considered to be at risk of contracting rabies, applying a rational indication for PEP is crucial.
The Early Detection and Intervention Programme of the German Research Network on Schizophrenia (GRNS) investigates the initial prodromal phase of psychosis in a multidimensional approach. Two intervention strategies are being studied by two large-scale multicentre projects.
Aims
To present the concept of the intervention studies, and to provide an interim report of the recruitment procedure.
Method
Comprehensive cognitive-behavioural therapy has been developed for patients in the ‘early initial prodromal state’. For patients in the ‘late initial prodromal state’ the atypical neuroleptic amisulpride is explored. Both interventions are evaluated in randomised controlled trials using clinical management as the control condition.
Results
Between January 2001 and March 2003, 1212 individuals seeking help for mental health problems were screened for putative prodromal symptoms at four university centres. More than 388 individuals fulfilled criteria for both interventions and 188 (48. 5%) gave informed consent to participate in the trials.
Conclusions
The screening procedure appears to be feasible and trial participation seems to be acceptable to a relevant proportion of people at increased risk of developing psychosis.
Silicon is the dominating material for the fabrication of MEMS devices,especially in high volume production. However, metals with their typicalproperties are used to enhance or enable the functionality of MEMS. Incontrast to microelectronic technologies, not only the electrical but alsothe mechanical and optical behavior of metals could be helpful. Newrequirements in MEMS technologies demand optimized processes inmetallization for the fabrication of microstructures.
This paper presents some metallization applications and related technologydevelopment in the field of MEMS.
Five fungal strains that are able to grow on toluene were isolated from enrichment cultures. Three different techniques were used:
solid state-like batches, air biofilters and liquid cultures. Fungal growth in the latter systems was favoured by combining low pH and
low water activity. Soil and groundwater samples from gasoline-polluted environments were used as inocula. The isolates were
identified as deuteromycetes belonging to the genera Cladophialophora, Exophiala and Leptodontium and the ascomycete Pseudeurotium
zonatum. The previously isolated toluene-degrading fungus Cladosporium sphaerospermum was included in the present study. Results
showed that fungi grew on toluene with doubling times of about 2 to 3 days. Some of the strains also grew on ethylbenzene and
styrene. The apparent half-saturation constant (Km) for toluene oxidation ranged from 5 to 22 μM. Degradation activity was inhibited
by 50% at toluene concentrations ranging from 2.4 to 4.7 mM. These kinetic parameters are comparable to analogous data reported
for toluene-degrading bacteria. The ability of fungi to grow at low water activities and low pH suggest that they may be used for
the purification of gas streams containing aromatic hydrocarbons in air biofilters.
