People living with HIV/AIDS (PLWH) often experience co-morbid/co-occurring mental health conditions, e.g., depression, anxiety, and post-traumatic stress disorder (PTSD). In resource-limited settings, where provider shortages are common, task shifting and task sharing (i.e., service delivery by non-professionals) are recommended strategies to promote access to and utilization of mental health and psychosocial support (MHPSS) services among PLWH. We conducted a global scoping review of the literature on MHPSS task shifting and sharing intervention studies for PLWH. Data extracted and summarized included study characteristics, intervention components, whether trauma informed study design, how lay health workers (LHWs) were identified and trained to deliver MHPSS services, and findings related to mental health outcomes. Results indicated that from 2013 through 2022, published intervention research concerning task shifting and sharing approaches was much more prolific in low- and middle-income countries than in high-income countries. MHPSS interventions delivered by a variety of LHWs yielded promising associations on an array of mental health outcomes, including PTSD/trauma and suicidality, though understudied. Underreported details regarding LHW recruitment/selection, compensation, supervision and assessment made it difficult to identify common or best practices. Further research is needed to facilitate the adoption and implementation of MHPSS task shifting and sharing interventions.
Preliminary evidence suggests that a ketogenic diet may be effective for bipolar disorder.
Aims
To assess the impact of a ketogenic diet in bipolar disorder on clinical, metabolic and magnetic resonance spectroscopy outcomes.
Method
Euthymic individuals with bipolar disorder (N = 27) were recruited to a 6- to 8-week single-arm open pilot study of a modified ketogenic diet. Clinical, metabolic and MRS measures were assessed before and after the intervention.
Results
Of 27 recruited participants, 26 began and 20 completed the ketogenic diet. For participants completing the intervention, mean body weight fell by 4.2 kg (P < 0.001), mean body mass index fell by 1.5 kg/m2 (P < 0.001) and mean systolic blood pressure fell by 7.4 mmHg (P < 0.041). The euthymic participants had average baseline and follow-up assessments consistent with them being in the euthymic range with no statistically significant changes in Affective Lability Scale-18, Beck Depression Inventory and Young Mania Rating Scale. In participants providing reliable daily ecological momentary assessment data (n = 14), there was a positive correlation between daily ketone levels and self-rated mood (r = 0.21, P < 0.001) and energy (r = 0.19 P < 0.001), and an inverse correlation between ketone levels and both impulsivity (r = −0.30, P < 0.001) and anxiety (r = −0.19, P < 0.001). From the MRS measurements, brain glutamate plus glutamine concentration decreased by 11.6% in the anterior cingulate cortex (P = 0.025) and fell by 13.6% in the posterior cingulate cortex (P = <0.001).
Conclusions
These findings suggest that a ketogenic diet may be clinically useful in bipolar disorder, for both mental health and metabolic outcomes. Replication and randomised controlled trials are now warranted.
We report the case of a 16-year-old female with previously diagnosed bilateral sub-segmental pulmonary emboli who presented in cardiogenic shock from depressed biventricular function with cardiac MRI demonstrating concern for microvascular coronary injury. She was ultimately diagnosed with catastrophic antiphospholipid antibody syndrome-induced ischaemic cardiomyopathy, potentially associated with an underlying autoimmune connective tissue disease.
In this article, I offer an account of the “linguistic landscapes” associated with the commodification and purchase of internet services in southern Peru. Through an account of public signage deployed by internet lounges in this area as well as the ideologies that make sense of it, the analysis reveals one of the ways in which moral panic over digital gaming gets semiotically mediated. To do this, the article develops a theoretical machinery that makes sense of the ways in which public signs—relative to their infrastructural contexts—come to project channels for their uptake. When these relationships are considered relative to the ideologies that target them, what gets revealed are the moral contours of Southern Peruvian public life. The analysis ultimately shows how the concept “linguistic landscape” can be useful for the semiotic study of infrastructure as well as the forms of the public life that it helps to mediate.
A new model of the steady boundary layer flow around a rotating sphere is developed that includes the widely observed collision and subsequent eruption of boundary layers at the equator. This is derived following the Segalini & Garrett (J. Fluid Mech., vol. 818, 2017, pp. 288–318) asymptotic approach for large Reynolds numbers but replacing the Smith & Duck (Q. J. Mech. Appl. Maths, vol. 30, issue 2, 1977, pp. 143–156) correction with a higher-order version of the Stewartson (Grenzschichtforschung/Boundary Layer Research, 1958, pp. 59–71. Springer) model of the equatorial flow. The Stewartson model is then numerically solved, for the first time, via a geometric multigrid method that solves the steady planar Navier–Stokes equations in streamfunction-vorticity form on large rectangular domains in a quick and efficient manner. The results are then compared with a direct numerical simulation of the full unsteady problem using the Semtex software package where it is found that there is broad qualitative agreement, namely the separation and reattachment of the boundary layer at the equator. However, the presence of unobserved behaviour such as a large area of reverse flow seen at lower Reynolds numbers than those observed in other studies, and that the absolute error increases with Reynolds number suggest the model needs improvement to better capture the physical dynamics.
Individuals with mood disorders are predisposed to metabolic dysfunction, while those with metabolic dysregulation such as diabetes and obesity experience more severe depressive symptoms. Both metabolic dysfunction and mood disorders are independently associated with cognitive deficits. Therefore, given their close association, this study aimed to explore the association between metabolic dysfunction in individuals with mood disorders in relation to cognitive outcomes. A comprehensive search comprised of these three domains was carried out; a random-effects meta-analysis pooling mean cognitive outcomes was conducted (PROSPERO ID: CRD42022295765). Sixty-three studies were included in this review; 26 were synthesized in a quantitative meta-analysis. Comorbid metabolic dysregulation was associated with significantly lower global cognition among individuals with mood disorders. These trends were significant within each mood disorder subgroup, including major depressive disorder, bipolar disorder, and self-report depression/depressive symptoms. Type 2 diabetes was associated with the lowest cognitive performance in individuals with mood disorders, followed by peripheral insulin resistance, body mass index ⩾25 kg/m2, and metabolic syndrome. Significant reduction in scores was also observed among individual cognitive domains (in descending order) of working memory, attention, executive function, processing speed, verbal memory, and visual memory. These findings demonstrate the detrimental effects of comorbid metabolic dysfunction in individuals with mood disorders. Further research is required to understand the underlying mechanisms connecting mood disorders, metabolism, and cognition.
This study investigates the impact of primary care utilisation of a symptom-based head and neck cancer risk calculator (Head and Neck Cancer Risk Calculator version 2) in the post-coronavirus disease 2019 period on the number of primary care referrals and cancer diagnoses.
Methods
The number of referrals from April 2019 to August 2019 and from April 2020 to July 2020 (pre-calculator) was compared with the number from the period January 2021 to August 2022 (post-calculator) using the chi-square test. The patients’ characteristics, referral urgency, triage outcome, Head and Neck Cancer Risk Calculator version 2 score and cancer diagnosis were recorded.
Results
In total, 1110 referrals from the pre-calculator period were compared with 1559 from the post-calculator period. Patient characteristics were comparable for both cohorts. More patients were referred on the cancer pathway in the post-calculator cohort (pre-calculator patients 51.1 per cent vs post-calculator 64.0 per cent). The cancer diagnosis rate increased from 2.7 per cent in the pre-calculator cohort to 3.3 per cent in the post-calculator cohort. A lower rate of cancer diagnosis in the non-cancer pathway occurred in the cohort managed using the Head and Neck Cancer Risk Calculator version 2 (10 per cent vs 23 per cent, p = 0.10).
Conclusion
Head and Neck Cancer Risk Calculator version 2 demonstrated high sensitivity in cancer diagnosis. Further studies are required to improve the predictive strength of the calculator.
Functional connectivity of the default mode network (DMN) during rest has been shown to be different among adults with Mild Cognitive Impairment (MCI) relative to aged-matched individuals without MCI and is predictive of transition to dementia. Post-traumatic stress disorder (PTSD) is also associated with aberrant connectivity of the DMN. Prior work from this group has demonstrated a higher rate of MCI and PTSD among World Trade Center (WTC) responders relative to the general population. The current study sought to investigate the main and interactive effects of MCI and PTSD on DMN functioning. Based on prior work, we hypothesized that MCI, but not PTSD, would predict aberrant connectivity in the DMN.
Participants and Methods:
99 WTC responders aged 44–65 stratified by MCI status (yes/no) and PTSD status (yes/no) and matched for age in years, sex (male vs. female), race (white, black, and other), and educational attainment (high school or less, some college / technical school, and university degree), and occupation on September 11, 2001 (law enforcement vs. other) underwent fMRI using a 3T Siemens Biograph MR scanner. A single 10-minute continuous functional MR sequence was acquired while participants were at rest with their eyes open. Group-level analyses were conducted using SPM-12, with correction for multiple comparisons using AFNI's 3dClustSim. Based on this threshold, the number of comparisons in our imaging volume, and the smoothness of our imaging data as measured by 3dFWHMx-acf, a minimum cluster size of 1134 voxels was required to have a corrected p . .05 with 2-sided thresholding. Spherical 3 mm seeds were placed in the dorsal (4, -50, 26) and ventral (4, -60, 46) posterior cingulate cortex (PCC).
Results:
Individuals with PTSD demonstrated significantly less connectivity of the dorsal posterior cingulate cortex (PCC) with medial insula (T = 5.21), subthalamic nucleus (T = 4.66), and postcentral gyrus (T = 3.81). There was no difference found in this study for connectivity between groups stratified by MCI status. There were no significant results for the ventral PCC seed.
Conclusions:
Contrary to hypotheses that were driven by a study of cortical thickness in WTC responders, the impact of PTSD appears to outweigh the impact of MCI on dorsal DMN connectivity among WTC responders stratified by PTSD and MCI status. This study is limited by several issues, including low number of female and minority participants, relatively small group cell sizes (n = 23–27 per cell), a brief resting state sequence (10 minutes), and lack of a non-WTC control group. Importantly, responders are a unique population so generalizability to other populations may be limited. Individuals in the current study are now being followed longitudinally to relate baseline resting state functional connectivity with cognitive changes and changes in connectivity over a four-year period.
We present detailed characterization of laser-driven fusion and neutron production ($\sim {10}^5$/second) using 8 mJ, 40 fs laser pulses on a thin (<1 μm) D${}_2$O liquid sheet employing a measurement suite. At relativistic intensity ($\sim 5\times {10}^{18}$ W/cm${}^2$) and high repetition rate (1 kHz), the system produces deuterium–deuterium (D-D) fusion, allowing for consistent neutron generation. Evidence of D-D fusion neutron production is verified by a measurement suite with three independent detection systems: an EJ-309 organic scintillator with pulse-shape discrimination, a ${}^3\mathrm{He}$ proportional counter and a set of 36 bubble detectors. Time-of-flight analysis of the scintillator data shows the energy of the produced neutrons to be consistent with 2.45 MeV. Particle-in-cell simulations using the WarpX code support significant neutron production from D-D fusion events in the laser–target interaction region. This high-repetition-rate laser-driven neutron source could provide a low-cost, on-demand test bed for radiation hardening and imaging applications.
Recent evidence from case reports suggests that a ketogenic diet may be effective for bipolar disorder. However, no clinical trials have been conducted to date.
Aims
To assess the recruitment and feasibility of a ketogenic diet intervention in bipolar disorder.
Method
Euthymic individuals with bipolar disorder were recruited to a 6–8 week trial of a modified ketogenic diet, and a range of clinical, economic and functional outcome measures were assessed. Study registration number: ISRCTN61613198.
Results
Of 27 recruited participants, 26 commenced and 20 completed the modified ketogenic diet for 6–8 weeks. The outcomes data-set was 95% complete for daily ketone measures, 95% complete for daily glucose measures and 95% complete for daily ecological momentary assessment of symptoms during the intervention period. Mean daily blood ketone readings were 1.3 mmol/L (s.d. = 0.77, median = 1.1) during the intervention period, and 91% of all readings indicated ketosis, suggesting a high degree of adherence to the diet. Over 91% of daily blood glucose readings were within normal range, with 9% indicating mild hypoglycaemia. Eleven minor adverse events were recorded, including fatigue, constipation, drowsiness and hunger. One serious adverse event was reported (euglycemic ketoacidosis in a participant taking SGLT2-inhibitor medication).
Conclusions
The recruitment and retention of euthymic individuals with bipolar disorder to a 6–8 week ketogenic diet intervention was feasible, with high completion rates for outcome measures. The majority of participants reached and maintained ketosis, and adverse events were generally mild and modifiable. A future randomised controlled trial is now warranted.
Rifts are full-thickness fractures that propagate laterally across an ice shelf. They cause ice-shelf weakening and calving of tabular icebergs, and control the initial size of calved icebergs. Here, we present a joint inverse and forward computational modeling framework to capture rifting by combining the vertically integrated momentum balance and anisotropic continuum damage mechanics formulations. We incorporate rift–flank boundary processes to investigate how the rift path is influenced by the pressure on rift–flank walls from seawater, contact between flanks, and ice mélange that may also transmit stress between flanks. To illustrate the viability of the framework, we simulate the final 2 years of rift propagation associated with the calving of tabular iceberg A68 in 2017. We find that the rift path can change with varying ice mélange conditions and the extent of contact between rift flanks. Combinations of parameters associated with slower rift widening rates yield simulated rift paths that best match observations. Our modeling framework lays the foundation for robust simulation of rifting and tabular calving processes, which can enable future studies on ice-sheet–climate interactions, and the effects of ice-shelf buttressing on land ice flow.
This collection profiles understudied figures in the book and print trades of the seventeenth century. With an equal balance between women and men, it intervenes in the history of the trades, emphasising the broad range of material, cultural, and ideological work these people undertook. It offers a biographical introduction to each figure, placing them in their social, professional, and institutional settings. The collection considers varied print trade roles including that of the printer, publisher, paper-maker, and bookseller, as well as several specific trade networks and numerous textual forms. The biographies draw on extensive new archival research, with details of key sources for further study on each figure. Chronologically organised, this Element offers a primer both on numerous individual figures, and on the tribulations and innovations of the print trade in the century of revolution.
Thermal x-rays from neutron stars are mainly radiated by accretion discs originating in the flux of material from a companion star. The companions are white dwarf stars with a range of masses, and some black holes. X-ray bursts are attributed to catastrophic nuclear events on the neutron star surface following accretion from the companion. Structure in the rotating accretion disc is observed as quasi-periodic oscillations (QPOs).
Most of our understanding of the location and nature of the beamed emission comes from the pulse profiles, which are available over the whole electromagnetic spectrum. The radio profiles are the most detailed, with observations of polarisation, width and components.
Finding the population of pulsars in the Milky Way galaxy requires a knowledge of the parameters and limitations of the various surveys made with different instruments and in different regions of the sky. We list the available survey data and show how models of the galactic population can be compared with the observational data, allowing estimates of pulsar birthrate and lifetime. Determination of accurate positions of individual pulsars require a Solar System ephemeris and a complex geometrical computation. Binary pulsar orbits display reletivistic effects which can be measured with remarkable precision to yield parameters of orbits and checks on relativistic theory.
The characteristic steps in the rotation rates of pulsars are known as glitches and arise in the irregular transfer of angular momentum from the interior to the crust as a neutron star spins down. They are related to the structure and the fluid dynamics of some superfluid components. The angular momentum is quantised in vortices, which may be pinned to the crystal structure of the crust. Glitches may be related to catastrophic unpinning events and to cracking of the crust itself. Timing noise is quasi-random variation in rotation rate. In many pulsars, the spin-down rate is seen to switch abruptly as the emission changes, indicating that changes in magnetospheric particle flows are responsible for both spin-down and radiation.
Pulsar distances are obtained from their frequency dispersion, geometrically from annual parallax, and from optical identifiction with supernova remnants, globular clusters and binary companions. For most pulsars, distances are only available from observation of effects of propagation in the interstellar medium, particularly neutral hydrogen absorption and frequency dispersion. Interpretation of the dispersion measure requires a model of the electron distribution through the Galaxy.
