Methicillin-resistant Staphylococcus aureus (MRSA) spa type t4549 is increasingly prevalent in Denmark, yet its epidemiological sources remain unclear. This study aimed to generate hypotheses about possible risk factors that may be associated with MRSA t4549 infections. We conducted a nationwide case – case questionnaire study comparing MRSA t4549 cases to other MRSA types (t002, t008, t127, t304, and t223) reported between January 2022 and November 2023. The analysis, which included descriptive statistics and logistic regression, found that 75% of MRSA t4549 cases were male. Infections were significantly more frequent in the foot (28%) and toe (54%) compared to other MRSA types. Key risk factors identified were contact with pheasants (OR = 8.70; 95%CI 1.25–174.29), participation in indoor team sports (OR = 7.54, 95%CI: 1.58–54.82) and swimming (OR = 4.15, 95%CI: 1.97–9.03). Although the limited number of cases warrants cautious interpretation, it is crucial to emphasize the need for preventive measures at both the individual and sports facility levels. Further environmental studies are needed to clarify the role of the environment and wildlife in MRSA t4549 transmission. The increasing prevalence of this spa type in Denmark underlines the importance of implementing effective public health strategies to reduce the risk of MRSA transmission.
Clozapine is the most effective antipsychotic for treatment-resistant psychosis. However, clozapine is underutilised in part because of potential agranulocytosis. Accumulating evidence indicates that below-threshold haematological readings in isolation are not diagnostic of life-threatening clozapine-induced agranulocytosis (CIA).
Aims
To examine the prevalence and timing of CIA using different diagnostic criteria and to explore demographic differences of CIA in patients registered on the UK Central Non-Rechallenge Database (CNRD).
Method
We analysed data of all patients registered on the UK Clozaril® Patient Monitoring Service Central Non-Rechallenge Database (at least one absolute neutrophil count (ANC) < 1.5 × 109/L and/or white blood cell count < 3.0 × 109/L) between May 2000 and February 2021. We calculated prevalence rates of agranulocytosis using threshold-based and pattern-based criteria, stratified by demographic factors (gender, age and ethnicity). Differences in epidemiology based on rechallenge status and clozapine indication were explored. The proportion of patients who recorded agranulocytosis from a normal ANC was explored.
Results
Of the 3029 patients registered on the CNRD with 283 726 blood measurements, 593 (19.6%) were determined to have threshold-based agranulocytosis and 348 (11.4%) pattern-based agranulocytosis. In the total sample (75 533), the prevalence of threshold-based agranulocytosis and pattern-based agranulocytosis was 0.8% and 0.5%, respectively. The median time to threshold-based agranulocytosis was 32 weeks (IQR 184) and 15 (IQR 170) weeks for pattern-based agranulocytosis. Among age groups, the prevalence of pattern-based agranulocytosis and threshold-based agranulocytosis was highest in the >48 age group. Prevalence rates were greatest for White (18%) and male individuals (13%), and lowest for Black individuals (0.1%). The proportion of people who were determined to have pattern-based agranulocytosis without passing through neutropenia was 70%.
Conclusions
Threshold-based definition of agranulocytosis may over-diagnose CIA. Monitoring schemes should take into consideration neutrophil patterns to correctly identify clinically relevant CIA. In marked contrast to previous studies, CIA occurred least in Black individuals and most in White individuals.
This Element explores multi-faceted linkages between feeding and relationship formation based on ethnographic case studies in Morocco, Madagascar, Sri Lanka, Taiwan, and Costa Rica. Research demonstrates that there are many culturally valued ways of feeding children, contradicting the idea of a single universally optimal feeding standard. It demonstrates further that in many parts of the world, feeding plays a central role in bonding and relationship formation, something largely overlooked in current developmental theories. Analysis shows that feeding contributes to relationship formation through what we call proximal, transactional, and distal dimensions. This Element argues that feeding practices can lead to qualitatively distinct forms of relationships. It has important theoretical and practical implications, calling for the expansion of attachment theory to include feeding and body-centered caregiving and significant changes to global interventions currently based on 'responsive feeding.' The test-negative design (TND) has become a standard approach for vaccine effectiveness (VE) studies. However, previous studies suggested that it may be more vulnerable than other designs to misclassification of disease outcome caused by imperfect diagnostic tests. This could be a particular limitation in VE studies where simple tests (e.g. rapid influenza diagnostic tests) are used for logistical convenience. To address this issue, we derived a mathematical representation of the TND with imperfect tests, then developed a bias correction framework for possible misclassification. TND studies usually include multiple covariates other than vaccine history to adjust for potential confounders; our methods can also address multivariate analyses and be easily coupled with existing estimation tools. We validated the performance of these methods using simulations of common scenarios for vaccine efficacy and were able to obtain unbiased estimates in a variety of parameter settings.
Recognising the significant extent of poor-quality care and human rights issues in mental health, the World Health Organization launched the QualityRights initiative in 2013 as a practical tool for implementing human rights standards including the United Nations Convention on Rights of Persons with Disabilities (CRPD) at the ground level.
Aims
To describe the first large-scale implementation and evaluation of QualityRights as a scalable human rights-based approach in public mental health services in Gujarat, India.
Method
This is a pragmatic trial involving implementation of QualityRights at six public mental health services chosen by the Government of Gujarat. For comparison, we identified three other public mental health services in Gujarat that did not receive the QualityRights intervention.
Results
Over a 12-month period, the quality of services provided by those services receiving the QualityRights intervention improved significantly. Staff in these services showed substantially improved attitudes towards service users (effect sizes 0.50–0.17), and service users reported feeling significantly more empowered (effect size 0.07) and satisfied with the services offered (effect size 0.09). Caregivers at the intervention services also reported a moderately reduced burden of care (effect size 0.15).
Conclusions
To date, some countries are hesitant to reforming mental health services in line with the CRPD, which is partially attributable to a lack of knowledge and understanding about how this can be achieved. This evaluation shows that QualityRights can be effectively implemented even in resource-constrained settings and has a significant impact on the quality of mental health services.
Delays in rotavirus vaccine schedule could improve performance in low- and middle-income countries (LMICs). However, delaying the first dose could be detrimental if infants experience severe rotavirus gastroenteritis (RVGE) early in life. Our objective was to describe the timing and predictors of severe RVGE in unvaccinated children in LMICs. We analysed the placebo arms from two clinical trials (cohort 1: NCT00241644; cohort 2: NCT00362648). We estimated the rate, cumulative incidence (per 1000 infants) and age distribution of severe RVGE episodes. Cox proportional hazards models were used to estimate hazard ratios and 95% confidence intervals (CI) for the association between baseline factors and severe RVGE. Cumulative incidence at 6 months of age was 23/1000 (95% CI 15–30) in cohort 1 and 6/1000 (95% CI 3–8) in cohort 2. Early antibiotic use (compared with no use) was associated with 2.03 (95% CI 1.18–3.48) and 1.41 (95% CI 0.80–2.51) times the rate of severe RVGE in cohorts 1 and 2, respectively. The cumulative incidence of severe RVGE was low at 6 months of age, suggesting that a 4-week delay in the vaccination schedule may not result in a large number of severe RVGE episodes prior to vaccine receipt.
The catastrophic lahars triggered by the 13 November 1985 eruption of the ice-clad Nevado del Ruiz volcano, Colombia, demonstrate that the interaction of hot pyroclasts with snow and ice can release 30–50 millionm3 of meltwater in 30–90 minutes. The 1985 eruption caused a 16% loss in area and a 9% loss in volume of snow, firn and ice. Turbulent pyroclastic density currents mechanically mixed with snow and produced meltwater at a rate of 0.5–1.6mms–1. Laboratory experiments suggest that turbulent, fluidized pyroclastic density currents exert mechanical and thermal scour, thereby efficiently transferring heat from hot pyroclasts to snow. Ice cap loss at Nevado del Ruiz continued between 1985 and 2000, representing a ∽52% decline in area and a ∽30% fall in volume. Ice 60–190m thick caps the east and southeast summit plateau, whereas an ice field < 30m thick and devoid of snow is retreating on the north, northeast and west edges. This asymmetrical distribution of ice reflects combined long-term effects of the 1985 eruption and of the post-1985 ice cap retreat. Should volcanic activity resume, steep-sided glaciers can fail and pyroclastic flows and surges can sweep the snowpack and generate mixed avalanches and lahars. Although the potential source of meltwater has decreased since 1985, extensive debris at the ice cap margins can be incorporated to future lahars.
Glaciological experiments have been carried out at Dôme du Goûter (4300 m a.s.l.), Mont Blanc, in order to understand the flow of firn/ice in this high-altitude Alpine glacierized area. Accumulation measurements from stakes show a very strong spatial variability and an unusual feature of mass-balance fluctuations for the Alps, i.e. the snow accumulation does not show any seasonal patterns. Measured vertical velocities which should match with long-term mean mass balance are consistent with observed accumulations. Therefore, the measurement of vertical velocities seems a good way of quickly obtaining reliable mean accumulation values for several decades in such a region.
A simple flow model can be used to determine the main flowlines of the glacier and to propose snow/ice age of core samples from the two boreholes drilled down tο the bedrock in June 1994. These results coincide with radioactivity measurements made to identify the well-known radioactive snow layers of 1963 and 1986. We can hope to obtain ice samples 55–60 years old about 20 or 30 m above the bedrock (110 m deep). Below, the deformation of the ice layers is loo great to be dated accurately.
Assessing risks from potential glacier hazards in relation to safety considerations for settlements and other fixed installations in high mountain areas requires the application of experience gained from previous events, combined with simple rules derived from basic glaciological theory. The general characteristics of steep, and usually unmeasured, glaciers can be estimated on the basis of a rough parameterization scheme. Variations in glacier length, ice avalanches, and glacier floods then have to be considered for time periods ranging from a few years up to a few decades. As a result of such systematic assessments, maps of potentially dangerous zones can be prepared. Although the inhabitants of many Alpine villages have always lived with the risk of glacier hazards, it now appears that modern construction work, especially that connected with the development of tourism, has started to infiltrate previously avoided high-risk zones more and more. In order to plan reasonable safety measures, risks from glacier hazards have to be compared with those from other natural hazards in mountain areas, such as snow avalanches, landslides, rock falls! or storm-induced floods. Decisions about the acceptable level of risk are difficult and subjective; they are also often influenced by political and economical considerations rather than by scientific reasoning.
Almost nothing is known about the potential negative effects of Internet-based psychological treatments for depression. This study aims at investigating deterioration and its moderators within randomized trials on Internet-based guided self-help for adult depression, using an individual patient data meta-analyses (IPDMA) approach.
Method
Studies were identified through systematic searches (PubMed, PsycINFO, EMBASE, Cochrane Library). Deterioration in participants was defined as a significant symptom increase according to the reliable change index (i.e. 7.68 points in the CES-D; 7.63 points in the BDI). Two-step IPDMA procedures, with a random-effects model were used to pool data.
Results
A total of 18 studies (21 comparisons, 2079 participants) contributed data to the analysis. The risk for a reliable deterioration from baseline to post-treatment was significantly lower in the intervention v. control conditions (3.36 v. 7.60; relative risk 0.47, 95% confidence interval 0.29–0.75). Education moderated effects on deterioration, with patients with low education displaying a higher risk for deterioration than patients with higher education. Deterioration rates for patients with low education did not differ statistically significantly between intervention and control groups. The benefit–risk ratio for patients with low education indicated that 9.38 patients achieve a treatment response for each patient experiencing a symptom deterioration.
Conclusions
Internet-based guided self-help is associated with a mean reduced risk for a symptom deterioration compared to controls. Treatment and symptom progress of patients with low education should be closely monitored, as some patients might face an increased risk for symptom deterioration. Future studies should examine predictors of deterioration in patients with low education.
Infection with Shiga toxin-producing Escherichia coli (STEC) by serotypes other than O157 (non-O157) have been increasingly reported in the United States. This increase in reporting is primarily due to the improvements in diagnostic tests. We analysed 1497 STEC cases reported in Michigan from 2001 to 2012. A significant increase in the number of non-O157 STEC cases was observed over time, and similar incidence rates were observed for O157 and non-O157 STEC cases in certain time periods. The odds of hospitalization was two times higher in O157 STEC cases relative to non-O157 STEC cases when adjusted for age and gender, suggesting that O157 STEC causes more severe clinical outcomes in all age groups. The use of population-based surveillance to better define trends and associations with disease severity are critical to enhance our understanding of STEC infections and improve upon current prevention and control efforts.
A significant fraction (~ 30%) of the gamma-ray sources detected by the Fermi Gamma-ray Space Telescope is still of unknown origin, being not yet associated with counterparts at lower energies. Many unidentified gamma-ray sources (UGSs) could be blazars, the largest identified population of extragalactic gamma-ray sources and the rarest class of active galactic nuclei. In particular, it has been found that blazars occupy a defined region in WISE three dimensional color space, well separated from that occupied by other sources in which thermal emission prevails. For farther sources with weaker IR emission, additional informations can be obtained combining WISE data with X-ray or radio emission. Alternatively, the low-frequency radio emission can be used for identifying potential gamma-ray candidate blazars. However, optical spectroscopic observations represent the tell-tale tool to confirm the exact nature of these sources. To this end, an extensive observational campaign has been performed with several optical telescopes, aimed at pinpointing the exact nature of gamma-ray candidate blazars selected with the different selection methods mentioned above. The results of this campaign lead to the discovery of 60 new gamma-ray blazars, thus confirming the effectiveness of these selection criteria.
This study presents results of selective laser melting (SLM), powder metallurgy (PM), and casting technologies applied for producing Ti–TiB composites from Ti–TiB2 powder. Diffraction patterns and microstructural investigations reveal that chemical reaction occurred between Ti and TiB2 during all the three processes, leading to the formation of Ti–TiB composites. The ultimate compressive strength of SLM-processed and cast samples are 1421 and 1434 MPa, respectively, whereas the ultimate compressive strengths of PM-processed 25%, 29%, and 36% porous samples are 510, 414, and 310 MPa, respectively. The Young's moduli of porous composite samples are 70, 45, and 23 GPa for 25%, 29%, and 36% porosity levels, respectively, and are lower than those of SLM-processed (145 GPa) and cast (142 GPa) samples. Fracture analysis of the SLM-processed and cast samples shows shear fracture and microcracks across the samples, whereas failure of porous samples occurs due to porosities and weak bonds among particles.
This descriptive longitudinal study was conducted to investigate the faecal shedding of Shiga toxin-producing E. coli (STEC) in finishing swine and to characterize the swine STEC isolates that were recovered. Three cohorts of finishing swine (n = 50/cohort; total 150 pigs) were included in the longitudinal study. Individual faecal samples were collected every 2 weeks (8 collections/pig) from the beginning (pig age 10 weeks) to the end (pig age 24 weeks) of the finishing period. STEC isolates were recovered in at least one sample from 65·3% (98/150) of the pigs, and the frequency distribution of first-time STEC detection during the finishing period resembled a point-source outbreak curve. Nineteen O:H serotypes were identified among the STEC isolates. Most STEC isolates (n = 148) belonged to serotype O59:H21 and carried the stx2e gene. One O49:H21 STEC isolate carried the stx2e and eae genes. High prevalence rates of STEC during the finishing period were observed, and STEC isolates in various non-O157 serogroups were recovered. These data enhance understanding of swine STEC epidemiology, and future research is needed to confirm whether or not swine STEC are of public health concern.
A 3-year longitudinal study was conducted on a multi-site farrow-to-finish production system. For each of 18 cohorts at three finishing sites, 50 pigs were randomly selected. Faecal samples were collected every 2 weeks for 16 weeks. Salmonella was cultured from 453 (6·6%) of 6836 faecal samples. The pig-level incidence of Salmonella was 20·8% (187/899 pigs). Salmonella prevalence varied between cohorts and within pigs. The adjusted Salmonella prevalence decreased over the finishing period from 6·4% to 0·8%. Intermittent detection of Salmonella was found in more than 50% of pigs that were positive at more than one collection. The finding that the majority of pigs shed intermittently has implications for surveillance and research study design when determining Salmonella status. The variability in shedding over time, as well as between and within cohorts and pigs suggests that there may be time-variant risk factors for Salmonella shedding in swine.
Homework assignments have been a key component of cognitive behavioural therapy (CBT) for decades, but very few studies examined the effect of compliance on outcome in regard to smoking behaviour. To fill this gap in the existing literature, the present report evaluated the role of homework compliance on short-term outcome in the context of evidence-based care for smoking cessation. Compliance with out-of-session assignments was assessed in a four-session CBT-nicotine replacement therapy protocol for smoking cessation among adult daily smokers (n= 94). Consistent with expectation, homework compliance was associated with better short-term outcomes, including amount of cigarettes consumed as well as abstinence. Importantly, these effects remained after controlling for a wide range of potentially confounding variables (e.g., motivation to quit, nicotine dependence at intake, attendance problems, and in-session participation). Estimates of quantity versus quality of homework compliance did not differentially predict outcomes. Out-of-session homework assignments incrementally predict short-term smoking abstinence in the context of evidence-based care for smoking cessation.
A numerical model to compute the dynamics of glaciers is presented. Ice damage due tocracks or crevasses can be taken into account whenever needed. This model allowssimulations of the past and future retreat of glaciers, the calving process or thebreak-off of hanging glaciers. All these phenomena are strongly affected by climatechange.
Ice is assumed to behave as an incompressible fluid with nonlinear viscosity, so that thevelocity and pressure in the ice domain satisfy a nonlinear Stokes problem. The shape ofthe ice domain is defined using the volume fraction of ice, that is one in the ice regionand zero elsewhere. The volume fraction of ice satisfies a transport equation with asource term on the upper ice-air free surface accounting for ice accumulation or melting.If local effects due to ice damage must be taken into account, the damage functionD is introduced, ranging between zero if no damage occurs and one.Then, the ice viscosity μ in the momentum equation must be replaced by(1 − D)μ. The damage function Dsatisfies a transport equation with nonlinear source terms to model cracks formation orhealing.
A splitting scheme allows transport and diffusion phenomena to be decoupled. Two fixedgrids are used. The transport equations are solved on an unstructured grid of small cubiccells, thus allowing numerical diffusion of the volume fraction of ice to be reduced asmuch as possible. The nonlinear Stokes problem is solved on an unstructured mesh oftetrahedrons, larger than the cells, using stabilized finite elements.
Two computations are presented at different time scales. First, the dynamics ofRhonegletscher, Swiss Alps, are investigated in 3D from 2007 to 2100 using severalclimatic scenarios and without considering ice damage. Second, ice damage is taken intoaccount in order to reproduce the calving process of a 2D glacier tongue submerged bywater.
