Elevated maternal interleukin 6 (IL-6) during pregnancy has been associated with adverse fetal brain development and neurodevelopmental disorders, which often involve executive functioning (EF) impairments. However, the association between maternal IL-6 levels during pregnancy and EF remains largely unexplored.
Methods
The COPSYCH study is based on the prospective COPSAC2010 birth cohort of 700 mother-child pairs, recruited during pregnancy. The children’s executive functioning was assessed at age 10 using: (i) the Behavior Rating Inventory of Executive Function, Second Edition (BRIEF-2) parental questionnaire, and (ii) a comprehensive neuropsychological test battery. Maternal blood levels of IL-6 and hs-CRP were measured at gestational week 24. Associations between IL-6 (main analysis) and hs-CRP (secondary analysis) and EF in children at age 10 were investigated with regression models with extensive confounder adjustment.
Results
Six hundred and four children (86% of the cohort) completed the 10-year follow-up. Higher maternal IL-6 levels were significantly associated with less efficient parental-rated executive functioning in the children: BRIEF-2 Global Executive Composite score (p = 0.003), Behavior Regulation Index (p = 0.005), Emotion Regulation Index (p=0.04), and Cognitive Regulation Index (p=0.007). Interaction analysis with sex was significant (p-value=0.01) and exploratory analyses showed that IL-6 associations to BRIEF-2 were solely driven by boys. Associations between IL-6 and neuropsychological tests, as well as associations between hs-CRP and EF outcomes, were non-significant.
Conclusion
IL-6 during pregnancy was associated with less efficient everyday EF in children at age 10. If replicated, preventive strategies targeting inflammation in pregnancy may ameliorate adverse cognitive outcomes in offspring.
We present generalized additive latent and mixed models (GALAMMs) for analysis of clustered data with responses and latent variables depending smoothly on observed variables. A scalable maximum likelihood estimation algorithm is proposed, utilizing the Laplace approximation, sparse matrix computation, and automatic differentiation. Mixed response types, heteroscedasticity, and crossed random effects are naturally incorporated into the framework. The models developed were motivated by applications in cognitive neuroscience, and two case studies are presented. First, we show how GALAMMs can jointly model the complex lifespan trajectories of episodic memory, working memory, and speed/executive function, measured by the California Verbal Learning Test (CVLT), digit span tests, and Stroop tests, respectively. Next, we study the effect of socioeconomic status on brain structure, using data on education and income together with hippocampal volumes estimated by magnetic resonance imaging. By combining semiparametric estimation with latent variable modeling, GALAMMs allow a more realistic representation of how brain and cognition vary across the lifespan, while simultaneously estimating latent traits from measured items. Simulation experiments suggest that model estimates are accurate even with moderate sample sizes.
Product architecture decisions are made early in the product development process and have far-reaching effects. Unless anticipated through experience or intuition, many of these effects may not be apparent until much later in the development process, making changes to the architecture costly in time, effort and resources. Many researchers through the years have studied various elements of product architecture and their effects. By using a repeatable process for aggregating statements on the effects of architecture strategies from a selection of the literature on the topic and storing them in a systematic database, this information can then be recalled and presented in the form of a Product Architecture Strategy and Effect (PASE) matrix. PASE matrices allow for the identification, comparison, evaluation, and then selection of the most desirable product architecture strategies before expending resources along a specific development path. This paper introduces the PASE Database and matrix and describes their construction and use in guiding design decisions. This paper also provides metrics for understanding the robustness of this database.
Environmental plastic pollution comprises partially degraded particles representing a continuum of sizes, shapes, polymer types and chemical compositions. Owing to their potential for biological uptake, small microplastic particles (sMP; <100 μm) and nanoplastics (NPs) are considered to be a potential risk to organisms. Understanding how sMPs and NPs behave in the environment, and how environmental matrices affect their detection, is fundamental to quantifying exposures, assessing hazards and understanding these risks. For this purpose, high-quality, well-characterised and environmentally relevant test and reference materials are crucial. The current lack of environmentally relevant sMP and NP reference materials has resulted in many studies applying commercially available spherical, homogenous and monodisperse particles, typically produced for specific purposes and without environmental relevance. There is a need for sMP and NP test/reference materials for fate and effects assessments and analytical protocol validation that more accurately represent the sMP and NP present in the environment. To date, feasible methods for producing relevant sMP and NP test materials in sufficient quantities for environmental fate and effects studies remain lacking. The current review provides an overview and comparison of the available methods, highlighting those that show the most promise for producing environmentally relevant sMP and NP with further development and optimisation.
The aim of the article is to reframe speculation from being seen as synonymous with unacademic conjecture, or as a means for questioning consensus and established narratives, to becoming a productive practical engagement with the archaeological and responding to its intrinsic uncertainties. In the first part of the article, we offer a review of speculation in the history of archaeological reasoning. In the second part, we proceed to discussing ways of embracing the speculative mandate, referring back to our engagements with the art/archaeology project Ineligible and reflections on how to work with the unknowns and uncertainties of archaeology. In the third and last part, we conclude by making the case for fertilizing the archaeological potential nested in the empirical encounter, creating more inceptions than conclusions, fostering ambiguities, contradictions and new spaces of experiential inquiry. This leads us to suggest that—when working with the archaeological—speculation should be seen not only as a privilege, but also as an obligation, due to the inherent and inescapable uncertainties of the discipline. In other words, archaeology has been given a mandate for speculation through its material engagements.
Anxiety disorders are the most prevalent mental health conditions among children and adolescents. However, it is estimated that less than 25% of all children and adolescents with an anxiety disorder receive professional help. Thus, it is of utmost importance to develop novel interventions that aim to increase treatment accessibility.
Objectives
The aim of this study is threefold, to determine the effectiveness of CoolMinds, an iCBT intervention for adolescents with anxiety disorders. In addition to investigate predictive factors and the networks between symptoms, severity and change from pre- to post- treatment.
Methods
The study is designed as a three-armed randomized controlled trial comparing iCBT with planned feedback, iCBT with on-demand help and a waitlist control, with 56 patients in each group. The participants in the two treatment conditions will receive 12 weeks of iCBT, while participants in the waitlist control wait for 12 weeks, before receiving iCBT with planned feedback. The participants in the two iCBT conditions will be randomized to get a booster session or not, 12 weeks after finishing treatment. The participants are adolescents between the age of 12 and 17 years and their parents. The families must live in the Region of Southern Denmark, and the adolescents must have a principal anxiety diagnosis according to DSM-5 criteria. The primary outcome measure are the Youth Online Diagnostic Assessment - child and parent versions. Outcomes will be evaluated at baseline, post-treatment and at the 3-, 6- and 12-month follow-ups. Symptoms of anxiety and depression are also measured between each session with PHQ-9 and S-SCAS.
Results
The results from this study will be submitted to high-status international and peer-reviewed journals, as well as be presented at national and international conferences.
Conclusions
This study will allow us to determine the efficacy of iCBT in adolescents with anxiety, where parent involvement is emphasized as part of the treatment. The results from this study intends to enhance accessibility of evidence-based treatment for adolescents with anxiety.
Insomnia in depression is common and difficult to resolve. Music is commonly used as a sleep aid, and clinical trials pointing to positive effects of music as a sleep aid are increasing adding to the evidence base. There is little knowledge on the effectiveness of music for depression related insomnia.
Objectives
A recent RCT study conducted in psychiatry at Aalborg University Hospital examined effects of a music intervention for insomnia in depression. The intervention group listened to music at bedtime for four weeks, controls were offered music intervention post-test. Primary outcome measure was Pittsburgh Sleep Quality Index (PSQI). Secondary outcomes included Actigraphy, The Hamilton depression Rating Scale (HAMD-17) and World Health Organisation well-being questionnaires (WHO-5, WHOQOL-BREF).
Methods
A two-armed randomized controlled trial (n=112) and a qualitative interview study (n=4)
Results
The RCT study showed signficant improvements for the music intervention group in sleep quality and quality of life at four weeks according to global PSQI scores (effect size= -2.1, 95%CI -3.3; -0.9) and WHO-5 scores (effect size 8.4, 95%CI 2.7; 14.0). Actigraphy measures showed no changes and changes in depression symptoms (HAMD-17) were not detected.
The interview study unfolded examples of the influences of music on sleep and relaxation. Music distracted, affected mood and arousal positively and supported formation of sleep habits.
Results from the trial are discussed and merged with findings from the interview study. The results from the trial suggested moderate effects of music listening for the population while findings from the interview study showed examples of individual and highly varying outcomes.
Conclusions
Music is suggested as a low-cost, side-effect free and safe intervention in supplement to existing treatments improving sleep in depression.
Antipsychotics (AP) are used as the primary pharmaceutical treatment for schizophrenia. Randomised clinical trials (RCT) show that the initiation of AP treatment often induces side effects such as substantial weight gain and metabolic disturbances, including an increased risk of type-2 diabetes (DM). However, the limitations of RCTs are often small cohorts that only represent a minority of patients with schizophrenia seen in everyday clinical settings leading to selection bias. Many RCTs are also limited by a short follow-up time, as evaluation of metabolic disturbances requires months to years of observation.
Objectives
Within a large cohort of real-world patients with long-term follow-up, we aim to study the differential metabolic side effects of specific antipsychotic drugs.
Methods
We performed a retrospective cohort study using the electronic patient record system “MidtEPJ”, which contains data from blood samples and medication usage from all patients registered with a schizophrenia diagnosis (ICD-10 code DF20) in the central region of Denmark from 2016-2022. Patients were followed from September 2016 (for patients with a schizophrenia diagnosis before this date) or their first schizophrenia diagnosis. The exposure is treatment with AP medication. Outcomes of interest are the development of DM, defined as a diagnosis of DM or usage of anti-diabetic medication, and changes in HbA1c, glucose, and cholesterol levels (high-density lipoprotein [HDL], low-density lipoprotein [LDL], total cholesterol and triglycerides). We performed cox regression analyses to study the associations between specific AP compounds with the differential risk for developing DM and changes in metabolic markers.
Results
We identified 4909 individual patients with a schizophrenia diagnosis from October 1st, 2016, to September 30th, 2022. AP was subscribed to 4609 of these patients. The results will be presented at the 2023 EPA Congress.
To illustrate that different approaches lead to different costs a cost calculation on four different welfare assessment systems for four different animal species has been carried out; an integrated pig herd (450 sows), a dairy cattle herd with automatic milking (90 cows), an organic egg production system (3000 layers) and a mink farm (1000 mink). We calculated the cost to be: €375 per annum for the mink farm and €2205, €2430 and €2435 for the egg production system, the AMS dairy herd and the integrated pig farm, respectively. The costs can be reduced by: reducing the number of indicators and/or the recording frequency, reducing sample sizes, more intensive use of existing data and by exchanging external for internal recordings.
A welfare indicator protocol integrating a total of 38 measures from 4 information sources: housing system, management, animal behaviour and clinical health has been developed for decision support in Automatic Milking System (AMS) herds. Two expert opinion studies focusing on the relevance of the welfare indicator protocol as basis for on-farm welfare assessment have been carried out; one study focused on the opinion of 21 AMS researchers; the other on that of 14 AMS production advisors and 15 veterinary practitioners. The researchers were asked to score the individual welfare relevance of the 38 measures. Furthermore, both the panels were asked to prioritise the 10 most important measures. The 21 researchers generally appreciated the listed protocol measures as highly relevant. The researcher and advisor panels agreed on prioritised measures from all 4 information sources among the 10 most important welfare indicators. In summary, researchers as well as production advisors and veterinarians supported the suggested measures for inclusion in a welfare assessment system. This has demonstrated that integration of different information is considered decisive for operational welfare assessment at herd level.
This study aimed to identify current weak points in animal welfare in Danish dairy production at herd level using the Welfare Quality® (WQ) protocol, and at national level using the Danish Animal Welfare Index (DAWIN) protocol. The DAWIN was developed as a monitoring tool for the welfare of the Danish dairy cow population, derived from the aggregation of DAWIN assessments at herd level. The DAWIN dairy cow protocol covers 29 measures (13 resource- and 16 animal-based measures) that were weighted and aggregated into a final overall population welfare score. A total of 3,591 cows from 60 dairy herds were assessed throughout 2015. Results from both the WQ and DAWIN were presented at six criteria levels in order to identify specific areas of concern relating to animal welfare at herd versus population level. Both protocols indicated a good general level of welfare across study herds, but also identified insufficient water supply as the main area of concern. In addition, resting comfort (ie time needed to lie down, collisions with barn equipment, cleanliness of rear body parts, animals lying outside of the designated lying area) and disease (in terms of the proportion of cows with chronically elevated somatic cell counts) were identified as problematic areas. The two assessment protocols both identified behavioural deficits, but in the WQ it was due to zero-grazing systems in contrast to the insufficient numbers of cow brushes in the DAWIN protocol. Despite differences in the aggregation, similar areas of concern were identified at criteria level.
People prefer their own initials to other letters, influencing preferences in many domains. The “name letter effect” (Nuttin, 1987) may not apply to negatively valenced targets if people are motivated to downplay or distance themselves from negative targets associated with the self, as previous research has shown (e.g., Finch & Cialdini, 1989). In the current research we examine the relationship between same initial preferences and negatively valenced stimuli. Specifically, we examined donations to disaster relief after seven major hurricanes to test the influence of the name letter effect with negatively valenced targets. Individuals who shared an initial with the hurricane name were overrepresented among hurricane relief donors relative to the baseline distribution of initials in the donor population. This finding suggests that people may seek to ameliorate the negative effects of a disaster when there are shared characteristics between the disaster and the self.
Smoking is probably the one single factor with the highest impact on reducing the life expectancies of patients with mental illness. In Denmark, 38.8% of patients with persistent mental health problem are smoking. Patients may have problem in participating in ordinary smoking cession programs offered in the community, but they are concerned about the impact of tobacco use on their health and finances and are motivated to stop smoking.Videoconferencing addressing smoking cessation might be an alternative to ordinary consultation at the clinic because the patients can access the treatment at home.
Objectives
Compare rates of smoking cessation in two interventions.
Methods
Patients diagnosed with schizophrenia, bipolar disorders or depression in 3 outpatient clinics are eligible for inclusion.Primary outcome is changes in number of cigarettes smoked pr. patients per day in at 6-month follow-up. Secondary outcome is abstinence from smoking at 6-month follow-up.This is a two-arm randomized controlled trial. 1. Daily video consultants at the start of smoking cessation and the months after. 2. Treatment as usual consistent of smoking cessation treatment in the community by weekly consultants.
Results
By September 2021, we have included 64 patients. Among patients, receiving video 26% has stopped and 15% has stopped from treatment as usual.Many patients has reduced their smoking considerably. The patients in general express that they are satisfied with both interventions.
Conclusions
Smoking cessation delivered by daily short video consultants seems to be the best and most effective way to help patients with serious mental illness to stop smoking.
This study aimed to investigate the effects of automated metrics-based summative feedback on performance, retention and cognitive load in distributed virtual reality simulation training of mastoidectomy.
Method
Twenty-four medical students were randomised in two groups and performed 15 mastoidectomies on a distributed virtual reality simulator as practice. The intervention group received additional summative metrics-based feedback; the control group followed standard instructions. Two to three months after training, participants performed a retention test without learning supports.
Results
The intervention group had a better final-product score (mean difference = 1.0 points; p = 0.001) and metrics-based score (mean difference = 12.7; p < 0.001). At retention, the metrics-based score for the intervention group remained superior (mean difference = 6.9 per cent; p = 0.02). Also at the retention, cognitive load was higher in the intervention group (mean difference = 10.0 per cent; p < 0.001).
Conclusion
Summative metrics-based feedback improved performance and lead to a safer and faster performance compared with standard instructions and seems a valuable educational tool in the early acquisition of temporal bone skills.
Recently video gaming, have attracted considerable attention for its possible beneficial therapeutic effects, the possibility for testing behavior in safe artificial environments and as a tool for professionals and patients to build specific competencies for the everyday life. Also, a substantial amount of research suggests that videogaming might improve the participants social and cognitive skills and emotional regulation. There is little or no evidence that videogaming increases long term aggression or leads to physical aggression. At a medium secure forensic psychiatric in-patient ward, the patients and staff engage in weekly E – Sport sessions (primarily counterstrike) to further the recovery process.
Objectives
To provide a standardized description of how E-sport is organized and used in the recovery process among forensic psychiatric patients.
Methods
The Template for Intervention Description and Replication (TIDieR) checklist and guide is widely used to in health research to describe interventions in clinical trials and other health research contexts. By use of TIDieR we describe a newly developed E-sport intervention, in which staff members and patients in a medium secure forensic psychiatric ward engage in weekly E-Sport sessions (primarily counterstrike) to improve patient–staff relationship.
Results
The E-sport intervention is detailed by use of the 12 TIDieR items and practical experiences and insights will be described.
Conclusions
This standardized and detailed description of how is used in a recovery-oriented process in forensic psychiatry can be used for future studies that wishes to implement the intervention or for research studies replicating the treatment.
To analyze the evacuation preparedness of hospitals within the European Union (EU).
Method:
This study consisted of 2 steps. In the first step, a systematic review of the subject matter, according to the PRISMA flow diagram, was performed. Using Scopus (Elsevier, Amsterdam, Netherlands), PubMed (National Library of Medicine, Bethesda, MD), and Gothenburg University´s search engine, 11 questions were extracted from the review and were sent to representatives from 15 European Union (EU)- and non-EU countries.
Results:
The findings indicate that there is neither a full preparedness nor a standard guideline for evacuation within the EU or other non-EU countries in this study. A major shortcoming revealed by this study is the lack of awareness of the untoward consequences of medical decision-making during an evacuation. Some countries did not respond to the questions due to the lack of relevant guidelines, instructions, or time.
Conclusion:
Hospitals are exposed to internal and external incidents and require an adequate evacuation plan. Despite many publications, reports, and conclusions on successful and unsuccessful evacuation, there is still no common guide for evacuation, and many hospitals lack the proper preparedness. There is a need for a multinational collaboration, specifically within the EU, to establish such an evacuation planning or guideline to be used mutually within the union and the international community.
Iodine intake affects the occurrence of thyroid disorders. However, the association of iodine intake with longevity remains to be described. This led us to perform a 20 years’ follow-up on participants from the Randers–Skagen (RaSk) study. Residents in Randers born in 1920 (n 210) and Skagen born in 1918–1923 (n 218) were included in a clinical study in 1997–1998. Mean iodine content in drinking water was 2 µg/l in Randers and 139 µg/l in Skagen. We collected baseline data through questionnaires, performed physical examinations and measured iodine concentrations in spot urine samples. Income data were retrieved from Danish registries. We performed follow-up on mortality until 31 December 2017 using Danish registries. Complete follow-up data were available on 428 out of 430 of participants (99·5 %). At baseline, the median urinary iodine concentration was 55 µg/l in Randers and 160 µg/l in Skagen residents. Participants were long-term residents with 72·8 and 92·7 % residing for more than 25 years in Randers and Skagen, respectively. Cox regression showed that living in Skagen compared with Randers was associated with a lower hazard ratio (HR) of death in both age- and sex-adjusted analyses (HR 0·60, 95 % CI 0·41, 0·87, P = 0·006), but also after adjustment for age, sex, number of drugs, Charlson co-morbidity index, smoking, alcohol and income (HR 0·60, 95 % CI 0·41, 0·87, P = 0·008). Residing in iodine-replete Skagen was associated with increased longevity. This indicates that long-term residency in an iodine-replete environment may be associated with increased longevity compared with residency in an iodine-deficient environment.
Use of antimicrobials for food-producing animals is a major public concern due to the risk of antimicrobial resistance. Although dairy production has a relatively low usage of antimicrobials, the potential for further reduction should be explored. The objective of the study was to estimate the current differences in antimicrobial use in Danish organic and conventional dairy herds and to describe the differences between them. Based on data from three different sources, 2604 herds (306 organic and 2298 conventional) were identified for the study. These herds had been either organic or conventional for the entire period from 2015 to 2018. Antimicrobial use was calculated as the treatment incidence in Animal Daily Doses (ADDs)/100 animals/day for three age groups: adult cattle, young stock and calves. For adult cattle, the ratio of median treatment incidence between conventional and organic production ranged from 2.8 : 1 to 3.4 : 1, depending on the specific year. For cows, 25% of the organic herds had a higher treatment incidence than the 25% of conventional herds with the lowest treatment incidence. Antimicrobial use for young stock was low and at a similar level in both the organic and conventional production systems. For calves, the median treatment incidence was 1.2 times higher in conventional herds and 1.6 times higher for the 75th percentile. Analyses of treatment incidence in adult cattle showed an overall decrease from 2015 to 2018 in both organic and conventional herds. The decrease was greater for the conventional herds (0.12 ADD/100 animals/day) compared to the organic herds (0.04 ADD/100 animals/day) over the 4-year period. In addition, herd size was an important risk factor for treatment incidence in conventional herds, increasing by 0.07 ADD/100 animals/day per 100 cows, whereas herd size had a minor influence on the treatment incidence in organic herds. The results of this study demonstrate the large variation in antimicrobial use within both organic and conventional herds, suggesting that further reduction is possible. Furthermore, herd size appears to be a risk factor in conventional herds but not in organic herds – an aspect that should be studied in more detail.
