There are two kinds of time bias: near bias and future bias. While philosophers typically hold that near bias is rationally impermissible, many hold that future bias is rationally permissible. Call this normative hybridism. According to arbitrariness objections, certain patterns of preference are rationally impermissible because they are arbitrary. While arbitrariness objections have been leveled against both near bias and future bias, the kind of arbitrariness in question has been different. In this article we investigate whether there are forms of arbitrariness that are common to both kinds of preferences and, hence, whether there are versions of the arbitrariness objection that are objections to both near bias and future bias. If there are, then this might go some way toward undermining normative hybridism and to defending thoroughgoing time-neutralism.
Face-to-face administration is the “gold standard” for both research and clinical cognitive assessments. However, many factors may impede or prevent face-to-face assessments, including distance to clinic, limited mobility, eyesight, or transportation. The COVID19 pandemic further widened gaps in access to care and clinical research participation. Alternatives to face-to-face assessments may provide an opportunity to alleviate the burden caused by both the COVID-19 pandemic and longer standing social inequities. The objectives of this study were to develop and assess the feasibility of a telephone- and video-administered version of the Uniform Data Set (UDS) v3 cognitive batteries for use by NIH-funded Alzheimer’s Disease Research Centers (ADRCs) and other research programs.
Participants and Methods:
Ninety-three individuals (M age: 72.8 years; education: 15.6 years; 72% female; 84% White) enrolled in our ADRC were included. Their most recent adjudicated cognitive status was normal cognition (N=44), MCI (N=35), mild dementia (N=11) or other (N=3). They completed portions of the UDSv3 cognitive battery, plus the RAVLT, either by telephone or video-format within approximately 6 months (M:151 days) of their annual in-person visit, where they completed the same in-person cognitive assessments. Some measures were substituted (Oral Trails for TMT; Blind MoCA for MoCA) to allow for phone administration. Participants also answered questions about the pleasantness, difficulty level, and preference for administration mode. Cognitive testers provided ratings of perceived validity of the assessment. Participants’ cognitive status was adjudicated by a group of cognitive experts blinded to most recent inperson cognitive status.
Results:
When results from video and phone modalities were combined, the remote assessments were rated as pleasant as the inperson assessment by 74% of participants. 75% rated the level of difficulty completing the remote cognitive assessment the same as the in-person testing. Overall perceived validity of the testing session, determined by cognitive assessors (video = 92%; phone = 87.5%), was good. There was generally good concordance between test scores obtained remotely and in-person (r = .3 -.8; p < .05), regardless of whether they were administered by phone or video, though individual test correlations differed slightly by mode. Substituted measures also generally correlated well, with the exception of TMT-A and OTMT-A (p > .05). Agreement between adjudicated cognitive status obtained remotely and cognitive status based on in-person data was generally high (78%), with slightly better concordance between video/in-person (82%) vs phone/in-person (76%).
Conclusions:
This pilot study provided support for the use of telephone- and video-administered cognitive assessments using the UDSv3 among individuals with normal cognitive function and some degree of cognitive impairment. Participants found the experience similarly pleasant and no more difficult than inperson assessment. Test scores obtained remotely correlated well with those obtained in person, with some variability across individual tests. Adjudication of cognitive status did not differ significantly whether it was based on data obtained remotely or in-person. The study was limited by its’ small sample size, large test-retest window, and lack of randomization to test-modality order. Health and social care workers (HSCWs) are at risk of experiencing adverse mental health outcomes (e.g. higher levels of anxiety and depression) because of the COVID-19 pandemic. This can have a detrimental effect on quality of care, the national response to the pandemic and its aftermath.
Aims
A longitudinal design provided follow-up evidence on the mental health (changes in prevalence of disease over time) of NHS staff working at a remote health board in Scotland during the COVID-19 pandemic, and investigated the determinants of mental health outcomes over time.
Method
A two-wave longitudinal study was conducted from July to September 2020. Participants self-reported levels of depression (Patient Health Questionnaire-9), anxiety (Generalised Anxiety Disorder-7) and mental well-being (Warwick-Edinburgh Mental Well-being Scale) at baseline and 1.5 months later.
Results
The analytic sample of 169 participants, working in community (43%) and hospital (44%) settings, reported substantial levels of depression and anxiety, and low mental well-being at baseline (depression, 30.8%; anxiety, 20.1%; well-being, 31.9%). Although mental health remained mostly constant over time, the proportion of participants meeting the threshold for anxiety increased to 27.2% at follow-up. Multivariable modelling indicated that working with, and disruption because of, COVID-19 were associated with adverse mental health changes over time.
Conclusions
HSCWs working in a remote area with low COVID-19 prevalence reported substantial levels of anxiety and depression, similar to those working in areas with high COVID-19 prevalence. Efforts to support HSCW mental health must remain a priority, and should minimise the adverse effects of working with, and disruption caused by, the COVID-19 pandemic.
This chapter provides an overview of the congenital cardiac condition known as d-transposition of the great arteries and its most modern form of repair, the arterial switch operation. Using a case-based format, the chapter aims to describe the neonatal physiology prior to repair, the surgery itself, and what anesthetic and other healthcare providers can expect as major postoperative sequelae long term. The chapter discusses how these salient details can affect pre-, intra-, and postoperative management of these patients undergoing noncardiac surgery.
The new Sentencing Council Guideline on sentencing offenders with mental disorders, effective from 1 October 2020, is essential reading for all psychiatrists who give evidence in the criminal courts, revealing something of required judicial thinking, our common ground on public safety concerns but differences in focus on culpability and punishment.
Hoerl & McCormack (H&M) posit two systems – the temporal updating system and the temporal reasoning system – and suggest that they explain an inherent contradiction in people's naïve theory of time. We suggest there is no contradiction. Something does, however, require explanation: the tension between certain sophisticated beliefs about time, and certain phenomenological states or beliefs about those phenomenological states. The temporal updating mechanism posited by H&M may contribute to this tension.
An electrodynamic containment system has been used to study the freeezing behaviour of supercooled water drops, of radius range 25 to 90 μm. The drops were freozen at temperatures between 0 and — 29°C in an environment whose relative humidity was approximately 90% with respect to ice. Freezing events were observed visually and photographically, and measurements were mager of the accompanying freactional mass loss Δm/m.
The most common moger of freeezing (70% of the drops studied) resulted in an apparently spherical ice particle. However, 18% exhibited spikes or other protuberances and the freeezing of 3% was accompanied by the ejection of numerous ice particles. In each of these situations values of Δm/m ranged freom about 5 to 15%. A further 9% of the drops exhibited one or more secondary mass-loss events, occurring several seconds after the freeezing process was complete; these were thus indicative of the ejection of ice particles.
Almost all of the values of Δm/m were significantly in excess of those predicted on the basis of evaporation during freeezing, suggesting that an additional mechanism of mass loss was also present. The measured freeezing times were consigerrably shorter than the classical values—at least, for the larger drops freeezing at warmer temperatures. Some visual observations were consistent with the “supersaturation wave” around a freeezing drop, which has been predicted by Nix and Fukuta (1974).
Measurements were made of the vertical electric field Strength around snow cornices on Bridger Ridge (2 590 m a.s.l.) in the Bridger Range, South-western Montana. The fields were considerably enhanced, owing to the exposed position of the cornices, but were nevertheless appreciably lower than those shown by Latham and Saunders (1970[b]) to be necessary in order to provide significant additional bonding when ice crystals collide with an ice surface. However, measurements made on Bridger Ridge and neighbouring Bangtail Ridge showed that the charges carried on snow crystals saltating over the surface of cornices were close to their limiting values.
Rough calculations indicated that pressure melting is unlikely to be of importance in the development of snow cornices formed from granular crystals, that frictional melting is probably significant only at fairly low temperatures and moderately high wind velocities, and that strong electrostatic forces between highly charged snow crystals saltating over the surface of a cornice may be sufficient to provide bonding where the crystal velocities are comparatively low.
The force F required to separate two ice spheres was measured as a function of environmental temperature T, relative humidity H and the strength E of the external electric field in which the spheres were situated. It was found that over the entire attainable range of T and H, F increased rapidly with increasing E. The increased adhesion was not accompanied by an increase in the rate of growth of the ice bridge between the two spheres and is explicable in terms of Davis’s (1964) calculations of the purely electrostatic forces between two spheres situated in an electric field. The experiments indicate that the rate of growth of snowflakes in a cloud by means of ice crystal aggregation will be markedly enhanced if the cloud is highly electrified.
The five centuries of the 'Abbasid period (eighth to thirteenth centuries AD) were the golden age of Arabic literature. They saw the appearance not only of poetry and belles-lettres (which are covered in a previous volume), but also of an extensive body of writings concerned with subjects ranging from theology and law to history and the natural sciences. This volume of The Cambridge History of Arabic Literature surveys the most important of these writings, including the literature of Sunnism and Shi'ism, Arabic philosophy, Sufism, Islamic law, grammar, lexicography, administration, historiography, mathematics, astronomy, astrology, geography, alchemy and medicine. It contains separate chapters on six of the greatest scholars of the Middle Ages, as well as on the Arabic literature of the Christians and Jews who lived under the rule of the 'Abbasid caliphate, and includes a study of one of the great cultural movements of the period, the translations from Greek into Arabic.
The study aimed to determine the geographical diversity in seasonality of major diarrhoeal pathogens among 21 138 patients enrolled between 2010 and 2012 in two urban and two rural sites in Bangladesh under the surveillance system of the International Centre for Diarrhoeal Disease Research, Bangladesh (icddr,b). Distinct patterns in seasonality were found for rotavirus diarrhoea which peaked in winter across the sites (December and January) and dipped during the rainy season (May) in urban Dhaka, August in Mirpur and July in Matlab, equated by time-series analysis using quasi-Poisson regression model. Significant seasonality for shigellosis was observed in Dhaka and rural Mirzapur. Cholera had robust seasonality in Dhaka and Matlab in the hot and rainy seasons. For enterotoxogenic Escherichia coli (ETEC) diarrhoea, clearly defined seasonality was observed in Dhaka (summer). Understanding the seasonality of such pathogens can improve case management with appropriate therapy, allowing policy-makers to identify periods of high disease burden.
The study identified the common aetiological agents and prominent clinical features of dysentery cases in children aged <5 years and compared this to non-dysentery diarrhoeal cases from the same population. From January 2010 to December 2011, 2324 children aged <5 years received treatment at Kumudini Hospital, of which 682 (29%) presented with dysentery. Of the dysenteric children, aetiology could not be determined for over half (61%). Shigella spp. accounted for 32% of dysentery cases. Significant associations were found between presence of blood in stool and: child age (24–59 months) [odds ratio (OR) 2·21, 95% confidence interval (CI) 1·49–3·27], no treatment of drinking water at home (OR 2·00, 95% CI 1·09–3·67), vomiting (OR 0·19, 95% CI 0·14–0·25), abdominal pain (OR 4·68, 95% CI 3·24–6·77), straining (OR 16·45, 95% CI 11·92–22·69), wasting (OR 1·66, 95% CI 1·15–2·41), and presence of Shigella in stool (OR 6·25, 95% CI 4·20–9·29) after controlling for confounders. This study makes it clear that appropriate public health strategies are needed to reduce the burden of dysentery in Bangladesh.
Sentinel species are increasingly used by disease managers to detect and monitor the prevalence of zoonotic diseases in wildlife populations. Characterizing home-range movements of sentinel hosts is thus important for developing improved disease surveillance methods, especially in systems where multiple host species co-exist. We studied ranging activity of major hosts of bovine tuberculosis (TB) in an upland habitat of New Zealand: we compared home-range coverage by ferrets (Mustela furo), wild deer (Cervus elaphus), feral pigs (Sus scrofa), brushtail possums (Trichosurus vulpecula) and free-ranging farmed cattle (Bos taurus). We also report in detail the proportional utilization of a seasonal (4-monthly) range area for the latter four species. Possums covered the smallest home range (<30 ha), ferrets covered ∼100 ha, pigs ∼4 km2, deer and cattle both >30 km2. For any given weekly period, cattle, deer and pigs were shown to utilize 37–45% of their estimated 4-month range, while possums utilized 62% during any weekly period and 85% during any monthly period of their estimated 4-month range. We suggest that present means for estimating TB detection kernels, based on long-term range size estimates for possums and sentinel species, probably overstate the true local surveillance coverage per individual.
This volume of The Cambridge History of Arabic Literature covers artistic prose and poetry produced in the heartland and provinces of the 'Abbasid empire during the second great period of Arabic literature, from the mid-eighth to the thirteenth centuries AD. 'Abbasid literature was characterised by the emergence of many new genres and of a scholarly and sophisticated critical consciousness. This volume deals chronologically with the main genres and provides extended studies of major poets, prose-writers and literary theorists. It concludes with a comprehensive survey of the relatively unknown literature of the Yemen to appear in a European language since the manuscript discoveries of recent years. To make the material accessible to non-specialist readers, 'Abbasid authors are quoted in English translation wherever possible, and clear explanations of their literary techniques and conventions are provided. With chapters by leading specialists from the Middle East, Europe and America, the volume represents a wide cross-section of current academic opinion.
