This study explored whether lifestyle therapy that promoted adherence to a Mediterranean-style diet as a treatment for depression led to environmental co-benefits. Participants (n 75 complete case) were Australian adults in the Curbing Anxiety and Depression using Lifestyle Medicine non-inferiority, randomised controlled trial, which showed that lifestyle therapy was non-inferior to psychotherapy in reducing depressive symptoms, when delivered in group format via video conferencing over an 8-week treatment period. In this secondary analysis, we hypothesised that the lifestyle arm would be superior to the psychotherapy arm in reducing the environmental impact of self-reported diet over time. Dietary intake derived from FFQ at baseline and 8 weeks was transformed into environmental impact scores by calculating global warming potential (GWP)*. GWP* was calculated for total dietary intake and distinct food groups (Australian Dietary Guidelines and NOVA classifications). Within-arm changes in GWP* over time were calculated using the median difference. Neither arm showed significant changes. Between-arm differences in percentage change in GWP* scores over time were analysed using generalised estimating equations models. No between-arm difference for total GWP* score was found (β = 11·06 (–7·04, 29·15)). When examining distinct food groups, results were mixed. These novel findings contribute to the sparse evidence base that has measured the environmental impact of diets in a clinical trial context. Whilst lifestyle therapy that reduced depressive symptoms did not have clear environmental benefits relative to psychotherapy, nutritional counselling that focuses on the environmental impact of food choices may drive more pronounced planetary co-benefits.
Maladaptive daydreaming is a distinct syndrome in which the main symptom is excessive vivid fantasising that causes clinically significant distress and functional impairment in academic, vocational and social domains. Unlike normal daydreaming, maladaptive daydreaming is persistent, compulsive and detrimental to one’s life. It involves detachment from reality in favour of intense emotional engagement with alternative realities and often includes specific features such as psychomotor stereotypies (e.g. pacing in circles, jumping or shaking one’s hands), mouthing dialogues, facial gestures or enacting fantasy events. Comorbidity is common, but existing disorders do not account for the phenomenology of the symptoms. Whereas non-specific therapy is ineffective, targeted treatment seems promising. Thus, we propose that maladaptive daydreaming be considered a formal syndrome in psychiatric taxonomies, positioned within the dissociative disorders category. Maladaptive daydreaming satisfactorily meets criteria for conceptualisation as a psychiatric syndrome, including reliable discrimination from other disorders and solid interrater agreement. It involves significant dissociative aspects, such as disconnection from perception, behaviour and sense of self, and has some commonalities with but is not subsumed under existing dissociative disorders. Formal recognition of maladaptive daydreaming as a dissociative disorder will encourage awareness of a growing problem and spur theoretical, research and clinical developments.
Knowledge of sex differences in risk factors for posttraumatic stress disorder (PTSD) can contribute to the development of refined preventive interventions. Therefore, the aim of this study was to examine if women and men differ in their vulnerability to risk factors for PTSD.
Methods
As part of the longitudinal AURORA study, 2924 patients seeking emergency department (ED) treatment in the acute aftermath of trauma provided self-report assessments of pre- peri- and post-traumatic risk factors, as well as 3-month PTSD severity. We systematically examined sex-dependent effects of 16 risk factors that have previously been hypothesized to show different associations with PTSD severity in women and men.
Results
Women reported higher PTSD severity at 3-months post-trauma. Z-score comparisons indicated that for five of the 16 examined risk factors the association with 3-month PTSD severity was stronger in men than in women. In multivariable models, interaction effects with sex were observed for pre-traumatic anxiety symptoms, and acute dissociative symptoms; both showed stronger associations with PTSD in men than in women. Subgroup analyses suggested trauma type-conditional effects.
Conclusions
Our findings indicate mechanisms to which men might be particularly vulnerable, demonstrating that known PTSD risk factors might behave differently in women and men. Analyses did not identify any risk factors to which women were more vulnerable than men, pointing toward further mechanisms to explain women's higher PTSD risk. Our study illustrates the need for a more systematic examination of sex differences in contributors to PTSD severity after trauma, which may inform refined preventive interventions.
The needs principle—that certain goods should be distributed according to need—has been central to much socialist and egalitarian thought. It is the principle which Marx famously takes to be that which is to govern the distribution of goods in the higher phase of communism. The principle is one that Marx himself took from the Blanquists. It had wider currency in the radical traditions of the nineteenth century. In the twentieth century it remained central to the mutualist form of socialism defended by Tawney and Titmuss. The principle underlay the development and justification of the modern welfare state—thus the National Health Service is still founded upon the idea that the distribution of medical resources should be determined by medical need, not by ability to pay. One source of the power of the needs principle lies in the fact that it appears to be both a principle of justice and a principle of community or social solidarity. As a principle of justice it is offered as a corrective to the particular forms of unequal distributions of goods that can result from market transactions, and as a principle of community or social solidarity as a corrective to the possessive individualism taken to be the corollary of a market order.
The management of unresponsive post-cardiac arrest patients has been an area of much controversy and research. Interventions during the arrest, particularly no-pause cardiopulmonary resuscitation and the advent of the automated external defibrillator have placed patients in a better position to respond to post-resuscitative measures. The initiation of post-arrest therapeutic hypothermia protocols, now referred to as targeted temperature management (TTM), has resulted in significant improvement in the number of neurologically intact survivors. Post-arrest care is centered on TTM, but includes a series of protocolized steps that define the measures to be taken to optimize outcome in these patients. Post-arrest care is the fifth and final link in the American Heart Association’s out-of-hospital chain of survival.

Paediatric renal tumours comprise a different spectrum of entities than occur in adults, reflecting the distinct chromatin state in the developing child. Nephroblastoma (Wilms tumour) with its histology of ‘nephrogenesis gone awry’ belongs to the primitive visceral cancers of childhood and is rare in adulthood, but makes up 90% of paediatric renal malignancies. Amongst paediatric renal neoplasms are some of the most aggressive and treatment-resistant cancers, including malignant rhabdoid tumour and renal medullary carcinoma. Recent molecular analysis has recognised that certain tumours arising within the kidney represent the visceral manifestation of a neoplasm that may alternatively arise within soft tissue (malignant rhabdoid tumour of kidney, congenital mesoblastic nephroma and peripheral primitive neuroectodermal tumour). Shared genetic driver mutations have been identified in what may appear to be very different tumours, but which have shared histology. This is true of the so-called BCOR family of tumours, whereby a BCOR rearrangement, or uncommonly instead the gene fusion YWHAE-NUTM2, occurs in clear cell sarcoma of kidney but also in high-grade endometrial stromal sarcoma and primitive mixed mesenchymal tumour of infancy. Dysregulation of chromatin is the oncogenic driver of some of these rare aggressive paediatric kidney cancers, including peripheral neuroectodermal tumour, clear cell sarcoma of kidney and malignant rhabdoid tumour.
To evaluate variables that affect risk of contamination for endoscopic retrograde cholangiopancreatography and endoscopic ultrasound endoscopes.
Design:
Observational, quality improvement study.
Setting:
University medical center with a gastrointestinal endoscopy service performing ∼1,000 endoscopic retrograde cholangiopancreatography and ∼1,000 endoscopic ultrasound endoscope procedures annually.
Methods:
Duodenoscope and linear echoendoscope sampling (from the elevator mechanism and instrument channel) was performed from June 2020 through September 2021. Operational changes during this period included standard reprocessing with high-level disinfection with ethylene oxide gas sterilization (HLD–ETO) was switched to double high-level disinfection (dHLD) (June 16, 2020–July 15, 2020), and duodenoscopes changed to disposable tip model (March 2021). The frequency of contamination for the co-primary outcomes were characterized by calculated risk ratios.
Results:
The overall pathogenic contamination rate was 4.72% (6 of 127). Compared to duodenoscopes, linear echoendoscopes had a contamination risk ratio of 3.64 (95% confidence interval [CI], 0.69–19.1). Reprocessing using HLD-ETO was associated with a contamination risk ratio of 0.29 (95% CI, 0.06–1.54). Linear echoendoscopes undergoing dHLD had the highest risk of contamination (2 of 18, 11.1%), and duodenoscopes undergoing HLD-ETO and the lowest risk of contamination (0 of 53, 0%). Duodenoscopes with a disposable tip had a 0% contamination rate (0 of 27).
Conclusions:
We did not detect a significant reduction in endoscope contamination using HLD-ETO versus dHLD reprocessing. Linear echoendoscopes have a risk of contamination similar to that of duodenoscopes. Disposable tips may reduce the risk of duodenoscope contamination.
On the dominant economic approach to environmental policy, environmental goods are conceptualised as forms of capital that provide services for human well-being. These services are assigned a monetary value to be weighed against the values of other goods and services. David Wiggins has offered a set of arguments against central assumptions about the nature of well-being, practical reason and ethical deliberation that underpin this dominant economic approach. In this paper I outline these arguments and consider their implications for understanding ethical demands across generations. The paper focuses, in particular, on their implications for understanding the nature and requirements of sustainability.
Trichotillomania (TTM) and skin picking disorder (SPD) are common and often debilitating mental health conditions, grouped under the umbrella term of body-focused repetitive behaviors (BFRBs). Recent clinical subtyping found that there were three distinct subtypes of TTM and two of SPD. Whether these clinical subtypes map on to any unique neurobiological underpinnings, however, remains unknown.
Methods
Two hundred and fifty one adults [193 with a BFRB (85.5% [n = 165] female) and 58 healthy controls (77.6% [n = 45] female)] were recruited from the community for a multicenter between-group comparison using structural neuroimaging. Differences in whole brain structure were compared across the subtypes of BFRBs, controlling for age, sex, scanning site, and intracranial volume.
Results
When the subtypes of TTM were compared, low awareness hair pullers demonstrated increased cortical volume in the lateral occipital lobe relative to controls and sensory sensitive pullers. In addition, impulsive/perfectionist hair pullers showed relative decreased volume near the lingual gyrus of the inferior occipital–parietal lobe compared with controls.
Conclusions
These data indicate that the anatomical substrates of particular forms of BFRBs are dissociable, which may have implications for understanding clinical presentations and treatment response.
No net loss approaches to environmental policy claim that policy should maintain aggregate levels of natural capital. Substitutability between natural assets allows losses in some assets to be compensated for by gains in others while maintaining overall levels of natural capital. This paper argues that significant goods that matter to people’s well-being will be lost through a policy of no net loss. The concepts of natural capital and ecosystem services that underpin the no net loss approach to environmental policy cannot capture important dimensions of value that are central to human well-being.
John O’Neill reads Montaigne’s Essays from their central principle of friendship as a communicative and pedagogical practice operative in society, literature and politics. The friendship between Montaigne and La Boétie was ruled neither by plenitude nor lack but by a capacity for recognition and transitivity. As an essayist Montaigne is an exemplary practitioner of a technique of difference and recognition that puts all certainties of history, philosophy and culture in the balance of weighted comparison. The essayist reveals how every absolute subjectivity or authority is shaken by its internal weakness once we move inside the contrastive structure of domination in politics, gender and race. O’Neill’s reading of the Essays strives to be faithful to the phenomenology of their embodied practices of reading-to-write-to re-read and re-write. From this standpoint he engages the principal critical readings of the Essays over the last century that have examined with great brilliance their history, structure and psychology. Whether the structure is evolutionary, structuralist, Marxist or psychoanalytical, O’Neill provides close readings of Montaigne’s literary critics. By bringing to bear the ethico-critical practice of ‘essaying’ to resist the subjection of the Essays to dominant criticism, O’Neill reminds readers that Montaigne’s appeal is in how he survived bloody cultural war with a balance of modesty and tolerance, invoking compromise where others practice violence.
Biomolecules have been traditionally immobilised onto surfaces using wet chemical techniques for various medical applications. Recent decades have seen plasma methods being used to prepare these surfaces through various forms of surface modification, but the direct exposure of biomolecules to plasma has been avoided due to fears that the molecules would be denatured by the energetic plasma species. Recent results are now demonstrating that direct plasma deposition of biomolecule coatings can be achieved. This creates the possibility to directly modify the surface of implants without any form of surface pre-treatment and this opens up the possibility to alter the healing processes. Materials such as collagen, chitosan, catalase and heparin can be effectively deposited onto surfaces with minimal impact on biological performance and without any chemical binders, linkers or impurities. The performance of these materials has been characterised using both in vitro and in vivo methodologies. In a further step, the results of a preclinical trial are presented which reveal that direct deposition of biomolecules onto open wounds can also be achieved and the impact of this on wound healing is measured in an immunocompromised animal model. A non-thermal plasma device was used to deliver collagen on to chronic wounds and the treatment was shown to promote wound closure in a rabbit wound healing model.
Field emission (FE) measurements are reported from carbon nanotube (CNT) fibers and laser-patterned free standing films fabricated by direct online condensation from a floating catalyst chemical vapor deposition reactor. Fiber and film cathodes showed stable emission in the 1–2 mA current (I) range at maximum cathode temperatures less than 1000 °C; film cathodes show localized heating at the triangular tips and higher maximum temperatures than the fibers. Fowler–Nordheim (FN) analysis indicated a change in the morphology of the emitters with increasing external electrical field (Eext). Fiber cathode I–Eext data are interpreted as FN emission from the fiber tip which is eventually limited by space-charge effects. At higher Eext, FN emission from the fiber sidewall occurs. The single fiber cathode stopped emitting abruptly when field induced self-heating effects became significant. For CNT films, self-heating effects can destroy a portion of the film, but FE can still occur from other areas.
The present study aimed to investigate socio-economic disparities in food and nutrient intakes among young Irish women. A total of 221 disadvantaged and seventy-four non-disadvantaged women aged 18–35 years were recruited. Diet was assessed using a diet history protocol. Of the total population, 153 disadvantaged and sixty-three non-disadvantaged women were classified as plausible dietary reporters. Food group intakes, nutrient intakes and dietary vitamin and mineral concentrations per MJ of energy consumed were compared between the disadvantaged and non-disadvantaged populations, as was compliance with dietary fibre, macronutrient and micronutrient intake guidelines. The disadvantaged women had lower intakes than the non-disadvantaged women of fruit, vegetables, fish, breakfast cereals, low-fat milk and wholemeal bread (all P< 0·001), yogurt (P= 0·001), low-fat spread (P= 0·002) and fresh meat (P= 0·003). They also had higher intakes of butter, processed red meats, white bread, sugar-sweetened beverages, fried potatoes and potato-based snacks (all P< 0·001) and full-fat milk (P= 0·014). Nutritionally, the disadvantaged women had higher fat, saturated fat and refined sugar intakes; lower dietary fibre, vitamin and mineral intakes; and lower dietary vitamin and mineral densities per MJ than their more advantaged peers. Non-achievement of carbohydrate (P= 0·017), fat (P< 0·001), saturated fat (P< 0·001), refined sugar (P< 0·001), folate (P= 0·050), vitamin C (P< 0·001), vitamin D (P= 0·047) and Ca (P= 0·019) recommendations was more prevalent among the disadvantaged women. Both groups showed poor compliance with Fe and Na guidelines. We conclude that the nutritional deficits present among these socially disadvantaged women are significant, but may be potentially ameliorated by targeted food-based interventions.
