Cable-driven exosuits have the potential to support individuals with motor disabilities across the continuum of care. When supporting a limb with a cable, force sensors are often used to measure tension. However, force sensors add cost, complexity, and distal components. This paper presents a design and control approach to remove the force sensor from an upper limb cable-driven exosuit. A mechanical design for the exosuit was developed to maximize passive transparency. Then, a data-driven friction identification was conducted on a mannequin test bench to design a model-based tension controller. Seventeen healthy participants raised and lowered their right arms to evaluate tension tracking, movement quality, and muscular effort. Questionnaires on discomfort, physical exertion, and fatigue were collected. The proposed strategy allowed tracking the desired assistive torque with a root mean square error of 0.71 Nm (18%) at 50% gravity support. During the raising phase, the electromyography signals of the anterior deltoid, trapezius, and pectoralis major were reduced on average compared to the no-suit condition by 30, 38, and 38%, respectively. The posterior deltoid activity was increased by 32% during lowering. Position tracking was not significantly altered, whereas movement smoothness significantly decreased. This work demonstrates the feasibility and effectiveness of removing the force sensor from a cable-driven exosuit. A significant increase in discomfort in the lower neck and right shoulder indicated that the ergonomics of the suit could be improved. Overall this work paves the way toward simpler and more affordable exosuits.
Radiation dermatitis (RD) occurs in 95% of patients receiving radiation therapy (RT) for cancer treatment, affecting 800 million patients annually. We aimed to demonstrate the feasibility of developing a historical RD cohort, Radiation Induced Skin Reactions (RISREAC) cohort.
Methods:
This retrospective study evaluated RD-related clinical documentation for 245 breast cancer patients who received RT at the University of Rochester Medical Center, to understand the RD progression, scoring, and management. All statistical analyses were performed at 0.05 level of significance.
Results:
Clinician-documented RD severity was observed for 169 (69%) patients with a mean severity of 1.57 [1.46, 1.68]. The mean descriptor-based severity score of 2.31 [2.18, 2.45] moderately correlated (r = 0.532, P < 0.0001) with documented RD grade. Most patients (91.8%) received skin care treatment during RT, with 66.7% receiving more than 2 modalities.
Conclusions:
The RISREAC cohort is the first retrospective cohort established from clinical documentation of radiation-induced skin changes for the study of RD and cutaneous radiation injury (CRI). RD symptom descriptors were more reliably documented and suitable for all skin types compared to Radiation Therapy Oncology Group (RTOG) or Common Toxicity Criteria for Adverse Events (CTCAE) grades. A new descriptor-based scoring tool would be useful for RD and CRI.
The contemporary story of the wolves of Yellowstone National Park (YNP) has become part of the fabric of the American west. The short film, How Wolves Change Rivers, which combines footage of Yellowstone with narration by journalist George Monbiot (Sustainable Human, 2014) has been viewed over 43 million times (including adoption by school curricula globally), and has contributed to popularising the concept that reintroducing wolves has caused ‘trophic cascades’ in YNP. This is an irresistible story, one which appeals to conservationists on many levels. Wolves are charismatic carnivores, and the film's narrative provides a simple explanation of trophic cascades and their potential. Monbiot's narration speaks of ‘bare valley sides’ becoming ‘forests of aspen and willow and cottonwood’, with wolves causing a ‘radical change’ in ungulate behaviour.
This is known as the ‘ecology of fear’ effect, where the presence of a predator causes a behavioural change in another species. In Yellowstone the introduction of wolves leads ungulates to avoid areas where they are most vulnerable to predation. The decrease in valley herbivory has resulted not only in an increase in species diversity as available habitat increased, but also the increased vegetation cover led to a reduction in soil erosion. It is argued that the ‘instrumentally valuable’ wolves have also transformed the physical geography of YNP – or so the story goes. However, there is still considerable scientific debate regarding the ecological impact of the wolf reintroduction to YNP and the mechanisms by which impacts are mediated, as will be discussed later in the chapter.
Emma Marris (2014, 2017), amongst others, has questioned the evidence that the re-introduction of wolves triggered the purported trophic cascades in Yellowstone. In a 2014 Nature paper she cites the work of Marshall et al (2013), who suggest that it is beavers, rather than wolves, that play a key role in helping willows thrive, adding that the removal of wolves in the 1920s allowed elk to eat so much willow that there was none left for the beavers, causing their decline. Marris's point is that the absence of a scientific consensus is not reflected in the popular press, with a simplistic version of the scientific story being reported far more often than the more complex, but more accurate, reality (Marris 2017).
Homeless shelter residents and staff may be at higher risk of SARS-CoV-2 infection. However, SARS-CoV-2 infection estimates in this population have been reliant on cross-sectional or outbreak investigation data. We conducted routine surveillance and outbreak testing in 23 homeless shelters in King County, Washington, to estimate the occurrence of laboratory-confirmed SARS-CoV-2 infection and risk factors during 1 January 2020–31 May 2021. Symptom surveys and nasal swabs were collected for SARS-CoV-2 testing by RT-PCR for residents aged ≥3 months and staff. We collected 12,915 specimens from 2,930 unique participants. We identified 4.74 (95% CI 4.00–5.58) SARS-CoV-2 infections per 100 individuals (residents: 4.96, 95% CI 4.12–5.91; staff: 3.86, 95% CI 2.43–5.79). Most infections were asymptomatic at the time of detection (74%) and detected during routine surveillance (73%). Outbreak testing yielded higher test positivity than routine surveillance (2.7% versus 0.9%). Among those infected, residents were less likely to report symptoms than staff. Participants who were vaccinated against seasonal influenza and were current smokers had lower odds of having an infection detected. Active surveillance that includes SARS-CoV-2 testing of all persons is essential in ascertaining the true burden of SARS-CoV-2 infections among residents and staff of congregate settings.
Assistive forces transmitted from wearable robots to the robot’s users are often defined by controllers that rely on the accurate estimation of the human posture. The compliant nature of the human–robot interface can negatively affect the robot’s ability to estimate the posture. In this article, we present a novel algorithm that uses machine learning to correct these errors in posture estimation. For that, we recorded motion capture data and robot performance data from a group of participants (n = 8; 4 females) who walked on a treadmill while wearing a wearable robot, the Myosuit. Participants walked on level ground at various gait speeds and levels of support from the Myosuit. We used optical motion capture data to measure the relative displacement between the person and the Myosuit. We then combined this data with data derived from the robot to train a model, using a grading boosting algorithm (XGBoost), that corrected for the mechanical compliance errors in posture estimation. For the Myosuit controller, we were particularly interested in the angle of the thigh segment. Using our algorithm, the estimated thigh segment’s angle RMS error was reduced from 6.3° (2.3°) to 2.5° (1.0°), mean (standard deviation). The average maximum error was reduced from 13.1° (4.9°) to 5.9° (2.1°). These improvements in posture estimation were observed for all of the considered assistance force levels and walking speeds. This suggests that ML-based algorithms provide a promising opportunity to be used in combination with wearable-robot sensors for an accurate user posture estimation.
Growth hormone (GH) deficiency is associated with decreased fertility. Since the placenta starts to produce a biologically active GH variant in the first gestational weeks, GH replacement therapy is usually stopped upon confirmation of conception or in the first trimester. Also acromegaly is associated with decreased fertility caused either by size effects of the pituitary adenoma that might lead to gonadotroph insufficiency or co-secretion of prolactin. Women with acromegaly should be treated in centers with adequate experience. In women with macroadenomas, transsphenoidal removal or size reduction of the adenoma prior to conception should be considered. The size of GH producing adenomas or residual tumors usually does not increase during pregnancy and symptoms of acromegaly might even improve due to hepatic GH resistance caused by high estrogen concentrations. In case of symptomatic tumor growth during pregnancy pharmacologic therapy with the somatostatin analogs octreotide and lanreotide might be considered before surgery. Comorbidities of acromegaly such as impaired glucose tolerance, diabetes, and hypertension deserve special attention. Rebound disease activity after delivery is frequent.
Over the past 20 years, collaboration has become an essential aspect of archaeological practice in North America. In paying increased attention to the voices of descendant and local communities, archaeologists have become aware of the persistent injustices these often marginalized groups face. Building on growing calls for a responsive and engaged cultural heritage praxis, this forum article brings together a group of Native and non-Native scholars working at the nexus of history, ethnography, archaeology, and law in order to grapple with the role of archaeology in advancing social justice. Contributors to this article touch on a diverse range of critical issues facing Indigenous communities in the United States, including heritage law, decolonization, foodways, community-based participatory research, and pedagogy. Uniting these commentaries is a shared emphasis on research practices that promote Indigenous sovereignty and self-determination. In drawing these case studies together, we articulate a sovereignty-based model of social justice that facilitates Indigenous control over cultural heritage in ways that address their contemporary needs and goals.
Scrub typhus is a common bacterial infection in Asia caused by Orientia tsutsugamushi. This serological cohort study estimated the incidence of infection in a rural population in South India. Participants were enrolled through systematic sampling in 46 villages at baseline, and revisited the following year. Blood samples were tested for IgG antibodies using ELISA, followed by indirect immunofluorescence assays (IFA) in those positive for ELISA at both rounds. A case was defined as sero-conversion (ELISA), or at least a 4-fold titre increase (IFA), between the two time points. In addition to crude incidence rate estimates, we used piecewise linear rates across calendar months, with rates proportional to the monthly incidence of local hospital cases to address seasonality and unequal follow-up times. Of 402 participants, 61.7% were female. The mean age was 46.7 years, (range 13–88). 21 participants showed evidence for serological infection. The estimated incidence was 4.4 per 100 person-years (95% CI 2.8–6.7). The piecewise linear rates approach resulted in a similar estimate of 4.6 per 100 person years (95% CI 2.9–6.9). Considering previous estimates of symptomatic scrub typhus incidence in the same study population, only about 2–5% of infections may result in clinically relevant disease.
Experience gained from responding to major outbreaks may have influenced the early coronavirus disease-2019 (COVID-19) pandemic response in several countries across Africa. We retrospectively assessed whether Guinea, Liberia and Sierra Leone, the three West African countries at the epicentre of the 2014–2016 Ebola virus disease outbreak, leveraged the lessons learned in responding to COVID-19 following the World Health Organization's (WHO) declaration of a public health emergency of international concern (PHEIC). We found relatively lower incidence rates across the three countries compared to many parts of the globe. Time to case reporting and laboratory confirmation also varied, with Guinea and Liberia reporting significant delays compared to Sierra Leone. Most of the selected readiness measures were instituted before confirmation of the first case and response measures were initiated rapidly after the outbreak confirmation. We conclude that the rapid readiness and response measures instituted by the three countries can be attributed to their lessons learned from the devastating Ebola outbreak, although persistent health systems weaknesses and the unique nature of COVID-19 continue to challenge control efforts.
Disadvantages of intravenous therapeutic unfractionated heparin, the first-line anti-coagulant agent in children with complex congenital heart disease, include unpredictable pharmacokinetics requiring frequent phlebotomies and the need for continuous intravenous access.
Objective:
To compare efficacy and safety of low-molecular-weight heparin administered by a subcutaneous indwelling catheter with intravenous unfractionated heparin.
Materials and methods:
Clinical data from 31 inpatients prospectively enrolled to receive subcutaneous low-molecular-weight heparin were compared with those from a historical group of 44 inpatients receiving intravenous unfractionated heparin. Investigation of parents’ satisfaction by telephone survey.
Results:
The percentage of anti-factor Xa levels outside therapeutic range was lower in the subcutaneous low-molecular-weight heparin group compared with the percentage of activated partial thromboplastin times outside therapeutic range in the intravenous unfractionated heparin group (40% versus 90%, p < 0.001). Neither group had a major complication. Transient local reactions occurred in 19% of patients of the subcutaneous low-molecular-weight heparin group. The number of needle punctures and that of placement of indwelling catheters were significantly lower in the subcutaneous low-molecular-weight heparin compared with the intravenous unfractionated heparin group (p < 0.001). In total, 84.2% of parents in the subcutaneous low-molecular-weight heparin group reported a positive experience when asked about comparison with prior intravenous unfractionated heparin treatment.
Conclusion:
Subcutaneous low-molecular-weight heparin offers a safe anti-coagulation regimen for children with complex congenital heart disease providing more efficient therapeutic anti-coagulation and a reduction in needle punctures, thus causing less pain and anxiety in this children.
Cloth face covering has been recommended by the Centers for Disease Control and Prevention to decrease community viral transmission. This study aims to determine the filtration efficiency and airflow resistance of common household materials available for homemade mask production by comparing numbers of fabrics, various layers, and manipulation.
Methods:
Common household woven, knitted, and nonwoven fabrics were tested for filtration efficiency using a fit testing setup and airflow resistance with pressure gauge setup. Three different levels of layering (1, 2, and 4) were tested. Some fabric material was further tested after washing and drying. Filtration performance, the area under the fitted curve comparing airflow resistance and filtration efficiency, was calculated for each fabric material and compared.
Results:
Layering increased filtration efficiency and airflow resistance (P < 0.0001 and P < 0.01, respectively). Polyester felt demonstrated the highest filtration performance index (P < 0.0001), higher than all tested 100% cotton materials (all P < 0.05) as well as surgical masks (P < 0.05). Washing plus drying did not alter filtration performance significantly (P > 0.05).
Conclusions:
A filtration performance of common household fabrics were compared. Homemade mask designers and producers will have improved data to better balance effectiveness, availability, and comfort with the goal of decreasing community viral transmission.
The Antarctic Impulsive Transient Antenna (ANITA) balloon experiment was designed to detect radio signals initiated by high-energy neutrinos and cosmic ray (CR) air showers. These signals are typically discriminated by the polarization and phase inversions of the radio signal. The reflected signal from CRs suffer phase inversion compared to a direct ‘tau neutrino’ event. In this paper, we study subsurface reflection, which can occur without phase inversion, in the context of the two anomalous up-going events reported by ANITA. It is found that subsurface layers and firn density inversions may plausibly account for the events, while ice fabric layers and wind ablation crusts could also play a role. This hypothesis can be tested with radar surveying of the Antarctic region in the vicinity of the anomalous ANITA events. Future experiments should not use phase inversion as a sole criterion to discriminate between down-going and up-going events, unless the subsurface reflection properties are well understood.
This research communication addresses the hypothesis that Southeast dairy producers' self-reported bulk tank somatic cell count (BTSCC) was associated with producers' response to three statements (1) ‘a troublesome thing about mastitis is the worries it causes me,’ (2) ‘a troublesome thing about mastitis is that cows suffer,’ and (3) ‘my broad goals include taking good care of my cows and heifers.’ Surveys were mailed to producers in Georgia, Kentucky, Mississippi, North Carolina, South Carolina, Tennessee, and Virginia (29% response rate, N = 596; final analysis N = 574), as part of a larger survey to assess Southeastern dairy producers' opinions related to BTSCC. Surveys contained 34 binomial (n = 9), Likert scale (n = 7), and descriptive (n = 18) statements targeted at producer self-assessment of herd records, management practices, and BTSCC. Statements 1 and 2 were assessed on a 5-point Likert scale from ‘strongly disagree’ to ‘strongly agree.’ Statement 3 was assessed on a 5-point Likert scale from ‘very unimportant’ to ‘very important.’ Reported mean BTSCC for all participants was 254 500 cells/ml. Separate univariable logistic regressions using generalized linear mixed models (SAS 9.4, Cary, NC, USA) with a random effect of farm, were performed to determine if BTSCC was associated with probability for a producer's response to statements. If BTSCC was significant, forward manual addition was performed until no additional variables were significant (P ≤ 0.05), but included BTSCC, regardless of significance. Bulk tank somatic cell count was associated with ‘a troublesome thing about mastitis is the worries it causes me,’ but not with Statements 2 or 3. This demonstrates that >75% of Southeastern dairy producers are concerned with animal care and cow suffering, regardless of BTSCC. Understanding Southeast producers' emphasis on cow care is necessary to create targeted management tools for herds with elevated BTSCC.
Depression and obesity are highly prevalent, and major impacts on public health frequently co-occur. Recently, we reported that having depression moderates the effect of the FTO gene, suggesting its implication in the association between depression and obesity.
Aims
To confirm these findings by investigating the FTO polymorphism rs9939609 in new cohorts, and subsequently in a meta-analysis.
Method
The sample consists of 6902 individuals with depression and 6799 controls from three replication cohorts and two original discovery cohorts. Linear regression models were performed to test for association between rs9939609 and body mass index (BMI), and for the interaction between rs9939609 and depression status for an effect on BMI. Fixed and random effects meta-analyses were performed using METASOFT.
Results
In the replication cohorts, we observed a significant interaction between FTO, BMI and depression with fixed effects meta-analysis (β=0.12, P = 2.7 × 10−4) and with the Han/Eskin random effects method (P = 1.4 × 10−7) but not with traditional random effects (β = 0.1, P = 0.35). When combined with the discovery cohorts, random effects meta-analysis also supports the interaction (β = 0.12, P = 0.027) being highly significant based on the Han/Eskin model (P = 6.9 × 10−8). On average, carriers of the risk allele who have depression have a 2.2% higher BMI for each risk allele, over and above the main effect of FTO.
Conclusions
This meta-analysis provides additional support for a significant interaction between FTO, depression and BMI, indicating that depression increases the effect of FTO on BMI. The findings provide a useful starting point in understanding the biological mechanism involved in the association between obesity and depression.
Improving children's learning and development in conflict-affected countries is critically important for breaking the intergenerational transmission of violence and poverty. Yet there is currently a stunning lack of rigorous evidence as to whether and how programs to improve learning and development in conflict-affected countries actually work to bolster children's academic learning and socioemotional development. This study tests a theory of change derived from the fields of developmental psychopathology and social ecology about how a school-based universal socioemotional learning program, the International Rescue Committee's Learning to Read in a Healing Classroom (LRHC), impacts children's learning and development. The study was implemented in three conflict-affected provinces of the Democratic Republic of the Congo and employed a cluster-randomized waitlist control design to estimate impact. Using multilevel structural equation modeling techniques, we found support for the central pathways in the LRHC theory of change. Specifically, we found that LRHC differentially impacted dimensions of the quality of the school and classroom environment at the end of the first year of the intervention, and that in turn these dimensions of quality were differentially associated with child academic and socioemotional outcomes. Future implications and directions are discussed.
Mass-loss in cool supergiants remains poorly understood, but is one of the key elements in their evolution towards exploding as supernovae. Some show evidence of asymmetric mass loss, discrete mass-ejections and outbursts, with seemingly little to distinguish them from more quiescent cases. To explore the prevalence of discrete ejections and companions we have conducted a high-constrast survey using near-infrared imaging and optical polarimetric imaging of nearby southern and equatorial red supergiants, using the extreme adaptive optics instrument SPHERE, which was designed to image planets around nearby stars. We present the initial results of this survey, including the detection of large (500 nm) dust grains in the ejecta of VY CMa and a candidate dusty torus aligned with the maser ring of VX Sgr. We briefly speculate on the consequences for our understanding of mass loss in these extreme stars.
Fictional accounts of epilepsy are of interest because they may convey information on images and public views of epilepsy which are not contained in medical texts. Thus, medical and nonmedical traditions together form the cultural history of epilepsy. Of the numerous possible aspects of epilepsy in fiction, this paper looks especially at the writers'background of knowledge about epilepsy; epilepsy as a handicap and a reason for social rejection, with special reference to epilepsy under the Nazi rule; threats to patients'lives; the motive of the child with epilepsy as a divine child; and epilepsy as a fate, and a reason for distinction. Literary writers may help their readers understand that a person's suffering and fighting a condition like epilepsy very much deserves our attention and sympathy. Without being exclusive, the paper pays special attention to epilepsy in the writings of Canadian authors.
We report the oldest fossil occurrence of freshwater potamolepid sponges (Demospongiae, Spongillina, Potamolepidae) to date, originating from middle Eocene lake sediments accumulated in the Giraffe kimberlite maar, northern Canada. Sponges are represented by strongyle spicules that are gemmuloscleres. These are described herein as belonging to a new species, Potamophloios canadensis. Because the most similar extant potamolepid sponges inhabit subtropical to tropical water bodies, these observations provide further evidence of biogeographic reorganizations in response to warm high-latitude Eocene paleoclimates.
