To determine if limb lengths, as markers of early life environment, are associated with the risk of diabetes in China.
Design:
We performed a cohort analysis using data from the China Health and Retirement Longitudinal Study (CHARLS), and multivariable-adjusted Cox proportional hazard regression models were used to examine the associations between baseline limb lengths and subsequent risk of diabetes.
Setting:
The CHARLS, 2011–2018.
Participants:
The study confined the eligible subject to 10 711 adults aged over 45 years from the CHARLS.
Results:
During a mean follow-up period of 6·13 years, 1358 cases of incident diabetes were detected. When controlling for potential covariates, upper arm length was inversely related to diabetes (hazard ratio (HR) 0·95, 95 % CI (0·91, 0·99), P = 0·028), and for every 1-cm difference in knee height, the risk of diabetes decreased by about 4 % (HR 0·96, 95 % CI (0·93, 0·99), P = 0·023). The association between upper arm length and diabetes was only significant among females while the association between knee height and diabetes was only significant among males. In analyses stratified by BMI, significant associations between upper arm length/knee height and diabetes only existed among those who were underweight (HR 0·91, 95 % CI (0·83, 1·00), P = 0·049, HR 0·92, 95 % CI (0·86, 0·99), P = 0·031).
Conclusions:
Inverse associations were observed between upper arm length, knee height and the risk for diabetes development in a large Asian population, suggesting early life environment, especially infant nutritional status, may play an important role in the determination of future diabetes risk.
The findings regarding the associations between red meat, fish and poultry consumption, and the metabolic syndrome (Mets) have been inconclusive, and evidence from Chinese populations is scarce. A cross-sectional study was performed to investigate the associations between red meat, fish and poultry consumption, and the prevalence of the Mets and its components among the residents of Suzhou Industrial Park, Suzhou, China. A total of 4424 participants were eligible for the analysis. A logistic regression model was used to estimate the OR and 95 % CI for the prevalence of the Mets and its components according to red meat, fish and poultry consumption. In addition, the data of our cross-sectional study were meta-analysed under a random effects model along with those of published observational studies to generate the summary relative risks (RR) of the associations between the highest v. lowest categories of red meat, fish and poultry consumption and the Mets and its components. In the cross-sectional study, the multivariable-adjusted OR for the highest v. lowest quartiles of consumption was 1·23 (95 % CI 1·02, 1·48) for red meat, 0·83 (95 % CI 0·72, 0·97) for fish and 0·93 (95 % CI 0·74, 1·18) for poultry. In the meta-analysis, the pooled RR for the highest v. lowest categories of consumption was 1·20 (95 % CI 1·06, 1·35) for red meat, 0·88 (95 % CI 0·81, 0·96) for fish and 0·97 (95 % CI 0·85, 1·10) for poultry. The findings of both cross-sectional studies and meta-analyses indicated that the association between fish consumption and the Mets may be partly driven by the inverse association of fish consumption with elevated TAG and reduced HDL-cholesterol and, to a lesser extent, fasting plasma glucose. No clear pattern of associations was observed between red meat or poultry consumption and the components of the Mets. The current findings add weight to the evidence that the Mets may be positively associated with red meat consumption, inversely associated with fish consumption and neutrally associated with poultry consumption.
This study gives insights into the interfacial instability of water droplets through a combination of laboratory experiments, numerical simulations and analytical modelling. An experiment is conducted in a narrow gap between two plates to model the two-dimensional cylindrical geometry, and a pulsed laser beam is focused inside a water droplet to generate a cavitation bubble. Three distinct characteristics of droplet deformation can be observed: (i) splashing; (ii) ventilating; and (iii) a stable state. In addition, an analytical model considering the Rayleigh–Taylor instability and bubble oscillation is developed based on the assumption that fluid is inviscid and incompressible. The analytical model is solved to obtain a phase diagram describing three distinct phenomena. Two dimensionless parameters, $\hat {\eta }_{ins}$ and $\hat {\eta }_{sta}$, are used to determine the boundaries between different regimes. The parameter $\hat {\eta }_{ins}$ is defined by the ratio of the perturbation amplitude to the difference between the droplet and bubble radii, whereas the other parameter $\hat {\eta }_{sta}$ is defined by the ratio of the perturbation amplitude to the initial droplet radius. Interfacial instability is induced when the perturbed droplet surface penetrates the bubble boundary, namely, $\rvert \hat {\eta }_{ins} \rvert \geq 1$. Splashing and ventilating phenomena occur for $\hat {\eta }_{ins}\geq 1$ and $\hat {\eta }_{ins}\leq -1$, respectively. A stable state occurs when droplet fragmentation does not appear despite small-amplitude perturbations for $\hat {\eta }_{sta}\leq 0.1$. There is a transition zone between the ventilating and stable state, which is bounded by $\hat {\eta }_{ins} = -1$ and $\hat {\eta }_{sta} = 0.1$. Finally, the phase diagram is verified by the experimental and numerical results.
Early identification of patients with novel corona virus disease 2019 (COVID-19) who may be at high mortality risk is of great importance.
Methods:
In this retrospective study, we included all patients with COVID-19 at Huanggang Central Hospital from January 23 to March 5, 2020. Data on clinical characteristics and outcomes were compared between survivors and nonsurvivors. Univariable and multivariable logistic regression were used to explore risk factors associated with in-hospital death. A nomogram was established based on the risk factors selected by multivariable analysis.
Results:
A total of 150 patients were enrolled, including 31 nonsurvivors and 119 survivors. The multivariable logistic analysis indicated that increasing the odds of in-hospital death associated with higher Sequential Organ Failure Assessment score (odds ratio [OR], 3.077; 95% confidence interval [CI]: 1.848-5.122; P < 0.001), diabetes (OR, 10.474; 95% CI: 1.554-70.617; P = 0.016), and lactate dehydrogenase greater than 245 U/L (OR, 13.169; 95% CI: 2.934-59.105; P = 0.001) on admission. A nomogram was established based on the results of the multivariable analysis. The AUC of the nomogram was 0.970 (95% CI: 0.947-0.992), showing good accuracy in predicting the risk of in-hospital death.
Conclusions:
This finding would facilitate the early identification of patients with COVID-19 who have a high-risk for fatal outcome.
For a wide range of insect species, the microbiota has potential roles in determining host developmental programme, immunity and reproductive biology. The tea geometrid moths Ectropis obliqua and E. grisescens are two closely related species that mainly feed on tea leaves. Although they can mate, infertile hybrids are produced. Therefore, these species provide a pair of model species for studying the molecular mechanisms of microbiotal involvement in host reproductive biology. In this study, we first identified and compared the compositions of microbiota between these sibling species, revealing higher microbiotal diversity for E. grisescens. The microbiota of E. obliqua mainly comprised the phyla Firmicutes, Proteobacteria and Cyanobacteria, whereas that of E. grisescens was dominated by Proteobacteria, Actinobacteria and Firmicutes. At the genus level, the dominant microbiota of E. grisescens included Wolbachia, Enterobacter and Pseudomonas and that of E. obliqua included Melissococcus, Staphylococcus and Enterobacter. Furthermore, we verified the rate of Wolbachia to infect 80 samples from eight different geographical populations, and the results supported that only E. grisescens harboured Wolbachia. Taken together, our findings indicate significantly different microbiotal compositions for E. obliqua and E. grisescens, with Wolbachia possibly being a curial factor influencing the reproductive isolation of these species. This study provides new insight into the mechanisms by which endosymbiotic bacteria, particularly Wolbachia, interact with sibling species.
The present study investigated the effect of Bacillus subtilis DSM 29784 (Ba) and enzymes (xylanase and β-glucanases; Enz), alone or in combination (BE) as antibiotic replacements, on the growth performance, digestive enzyme activity, immune response and the intestinal barrier of broiler chickens. In total, 1200 1-d-old broilers were randomly assigned to five dietary treatments, each with six replicate pens of forty birds for 63 d as follows: (a) basal diet (control), supplemented with (b) 1 × 109 colony-forming units (cfu)/kg Ba, (c) 300 mg/kg Enz, (d) 1 × 109 cfu/kg Ba and 300 mg/kg Enz and (e) 250 mg/kg enramycin (ER). Ba, Enz and BE, similar to ER, decreased the feed conversion rate, maintained intestinal integrity with a higher villus height:crypt depth ratio and increased the numbers of goblet cells. The BE group exhibited higher expression of claudin-1 and mucin 2 than the other four groups. BE supplementation significantly increased the α-diversity and β-diversity of the intestinal microbiota and markedly enhanced lipase activity in the duodenal mucosa. Serum endotoxin was significantly decreased in the BE group. Compared with those in the control group, increased superoxide dismutase and glutathione peroxidase activities were observed in the jejunal mucosa of the Ba and BE groups, respectively. In conclusion, the results suggested that dietary treatment with Ba, Enz or BE has beneficial effects on growth performance and anti-oxidative capacity, and BE had better effects than Ba or Enz alone on digestive enzyme activity and the intestinal microbiota. Ba or Enz could be used as an alternative to antibiotics for broiler chickens.
In vivo and in vitro trials were conducted to assess the effects of tributyrin (TB) supplementation on short-chain fatty acid (SFCA) concentrations, fibrolytic enzyme activity, nutrient digestibility and methanogenesis in adult sheep. Nine 12-month-old ruminally cannulated Small Tail ewes (initial body weight 55 ± 5.0 kg) without pregnancy were used for the in vitro trial. In vitro substrate made to offer TB at 0, 2, 4, 6 and 8 g/kg on a dry matter (DM) basis was incubated by ruminal microbes for 72 h at 39°C. Forty-five adult Small Tail ewes used for the in vivo trial were randomly assigned to five treatments with nine animals each for an 18-d period according to body weight (55 ± 5.0 kg). Total mixed ration fed to ewes was also used to offer TB at 0, 2, 4, 6 and 8 g/kg on a DM basis. The in vitro trial showed that TB supplementation linearly increased apparent digestibility of DM, crude protein, neutral detergent fibre and acid detergent fibre, and enhanced gas production and methane emissions. The in vivo trial showed that TB supplementation decreased DM intake, but enhanced ruminal fermentation efficiency. Both in vitro and in vivo trials showed that TB supplementation enhanced total SFCA concentrations and carboxymethyl cellulase activity. The results indicate that TB supplementation might exert advantage effects on rumen microbial metabolism, despite having an enhancing effect on methanogenesis.
Brief cognitive–behavioural therapy (CBT) is an emerging treatment for schizophrenia in community settings; however, further trials are needed, especially in non-Western countries.
Aims
To test the effects of brief CBT for Chinese patients with schizophrenia in the community (trial registration: ChiCTR-TRC-13003709).
Method
A total of 220 patients with schizophrenia from four districts of Beijing were randomly assigned to either brief CBT plus treatment as usual (TAU) or TAU alone. Patients were assessed at baseline, post-treatment and at 6- and 12-month follow-ups by raters masked to group allocation.
Results
At the post-treatment assessment and the 12-month follow-up, patients who received brief CBT showed greater improvement in overall symptoms, general psychopathology, insight and social functioning. In total, 37.3% of those in the brief CBT plus TAU group experienced a clinically significant response, compared with only 19.1% of those in the TAU alone group (P = 0.003).
Conclusions
Brief CBT has a positive effect on Chinese patients with schizophrenia in the community.
AMS 14C ages of post-glacial core sediments from the subaqueous Yangtze delta, along with sedimentary structures and distributions of grain size, pollen spores, and dinoflagellate cysts, show an estuarine depositional system from 13 to 8.4 cal ka BP and a deltaic system from 5.9 cal ka BP to the present. The estuarine system consists of intertidal to subtidal flat, estuarine, and estuarine-front facies, characterized by sand"mud couplets and a high sedimentation rate. The deltaic system includes nearshore shelf and prodelta mud featured by lower sedimentation rate, markedly fewer coastal wetland herbaceous pollens, and more dinoflagellate cysts. We explain the extremely high sedimentation rate during 9.2–8.4 cal ka BP at the study site as a result of rapid sea-level rise, high sediment load due to the unstable monsoonal climate, and subaqueous decrease of elevation from inner to outer estuary. A depositional hiatus occurred during 8.2–5.9 cal ka BP, the transition from estuarine to deltaic system, caused possibly by a shortage of sediment supply resulting from delta initiation in paleo-incised Yangtze valley and strong tidal or storm-related reworking in offshore areas. The subsequent development of deltaic system at the study site indicates accelerated progradation of Yangtze delta post-5.9 cal ka BP.
In the near future, the inactivated enterovirus 71 (EV71) vaccine is expected to become available on the market in China. Since EV71 is a major cause of hand, foot and mouth disease (HFMD), the vaccine is expected to significantly reduce the number of cases, as well as the detrimental economic effect of the disease. However, for a national vaccination strategy to be developed, policy-makers need more information on the socioeconomic burden of EV71 HFMD infection. Based on the 2011 population data, we estimated the clinical and economic effect of EV71 HFMD infection in children aged 0–9 years in Shanghai, China. The annual cost related to HFMD is >US$7.66 million for a population of 1·42 million children aged 0–9 years with an average cost of US$208.2/case. The extrapolated cost for EV71 HFMD infection was US$3.53 million, comprising 46·1% of the overall cost associated with HFMD. Around 97% of all of the HFMD-related expenses were paid for by the families creating a considerable economic burden. Our findings could provide the necessary recommendations on the most effective national EV71 vaccine implementation, as well as a baseline data for assessing the cost-effectiveness of the vaccine in China.
The goal of machining scheme selection (MSS) is to select the most appropriate machining scheme for a previously designed part, for which the decision maker must take several aspects into consideration. Because many of these aspects may be conflicting, such as time, cost, quality, profit, resource utilization, and so on, the problem is rendered as a multiobjective one. Consequently, we consider a multiobjective optimization problem of MSS in this study, where production profit and machining quality are to be maximized while production cost and production time must be minimized, simultaneously. This paper presents a new discrete method for particle swarm optimization, which can be widely applied in MSS to find out the set of Pareto-optimal solutions for multiobjective optimization. To deal with multiple objectives and enable the decision maker to make decisions according to different demands on each evaluation index, an analytic hierarchy process is implemented to determine the weight value of evaluation indices. Case study is included to demonstrate the feasibility and robustness of the hybrid algorithm. It is shown from the case study that the multiobjective optimization model can simply, effectively, and objectively select the optimal machining scheme according to the different demands on evaluation indices.
The Chinese National Twin Registry (CNTR), established in 2001, is the first and largest population-based twin registry in China. Based on the CNTR, a new twin cohort was recruited from 2011 to study the relationship between environmental risk factors and chronic diseases. So far, 33,874 twin pairs from nine provinces have been recruited, in which hundreds of disease-discordant twin pairs and even thousands of exposure-discordant twin pairs were found in this cohort. The updates of the CNTR will be introduced in detail in this article.
Background: Though multiple policies have been implemented, the cigarette control in China is still facing a great challenge. At the same time, alcohol drinking has increasingly become a public health problem. Considering cigarette smoking and alcohol drinking often co-occur, a few studies tested the covariance of these phenotypes. However, the genetic and environmental correlation between them among Chinese population has not been determined. The main aim of this study is to fill this gap. Methods: From the Chinese National Twin Registry, we obtained the data on cigarette smoking and alcohol drinking behaviors. The ordinal bivariate genetic analysis was performed to fit the categorical variables. After identifying the best decomposition among the Cholesky, common, and independent pathway model, we established the most parsimonious submodel. Results: The correlation between current tobacco and alcohol use could be explained by Cholesky model. The shared environmental variances for both phenotypes were dropped to construct the most parsimonious submodel. Furthermore, the most parsimonious submodel showed a moderate correlation (0.32, 95%CI = 0.17 – 0.46) between the genetic components and a negligible non-shared environmental correlation. Conclusion: As the first bivariate genetic analysis on current tobacco smoking and current alcohol drinking in China, this study suggested a common genetic vulnerability to tobacco and alcohol use in male twins. Further studies should be carried out to track the pertinent genes that are related to the comorbidity of smoking and drinking in Chinese population. Another urgent need is to recognize the behavior-specific environmental risk factors.
Twins, due to their unique genetic and environmental relationships, have provided crucial insight in our understanding of genetic contributions to numerous etiologically complex disorders in developed countries. As the leading cause of death and adult disability, cardio- and cerebrovascular diseases are common in China, followed by cancer. Obesity and psychological disorders are increasing. The overall goal of this program is to develop a resource for genetic epidemiologic studies of these and other common and complex diseases in China. Our initial focus is to delineate the genetic and environmental determinants of vascular diseases in general, coronary artery disease (CAD) and stroke in particular. To date, we have over 4500 twin pairs registered and about 700 twin pairs studied for various metabolic traits (e.g., lipids, glucose, insulin, etc.). The long-term plan of this program is to (1) establish a population-based twin registry from several selected regions in China for future studies of specific common complex diseases; (2) conduct detailed phenotyping for clinical and intermediate traits related to cardiovascular diseases; (3) expand studies of twins to twin families by including their parents, siblings, and offspring for genetic linkage and association studies; and (4) follow up twins in the registry longitudinally. The goals of the program are health education and promotion of healthy behavior, early identification of cases to provide timely medical attention, and the evaluation of long-term effects of identified risk factors. We want to develop collaborations with investigators who have expertise in cancer, psychological disorders, and other disease areas.
This study reports on the determination of zygosity in Chinese adult twins by simple questionnaire and physical features comparison. The subjects were 511 twin pairs from two cities and their town areas, consisting of 371 monozygotic (MZ) and 140 same-sex dizygotic (DZ) pairs, identified by ABO blood group and multiplex polymerase chain reaction of several polymorphic short tandem repeat markers. The twins themselves responded to 8 questionnaire items, 4 items on twin similarity, and 4 items on the frequency of mistaking one twin for another by parents, relatives, teachers and strangers when they were 6 to 13 years old. Research assistants responded to 20 items regarding twins' physical features at the moment of interview. A parsimonious model established using stepwise logistic regression analysis of the 28 items showed that the total accuracy of zygosity diagnosis was 90.1%. The accuracy was 89.2% when using only the items dealing with the confusion of twins and 85.4% using only similarity. In the questionnaire, ‘facial appearance’, ‘mistaken by teachers’ and ‘mistaken by strangers’ had stronger discriminating power between MZ and DZ twins. Two physical features — ‘eyelid’ and ‘middigital hair’ — were informative to some extent. There was no statistically significant sex and area difference in the validity of such questionnaire and physical features comparison-based classification. In conclusion, questionnaire-based zygosity assessment in this Chinese adult twin sample could still be regarded as a valid and valuable classification method. Physical features comparison, however, could only provide limited information for zygosity determination.
This study reports on the inter- and intrarater reliability of physical features observation. Study subjects were 176 Chinese adult persons, consisting of 89 males and 87 females. Three trained research assistants responded simultaneously and respectively to 12 items regarding the subject's physical features including ‘hair’, ‘Mongoloid folds’, left and right ‘ear lobes’, ‘earwax’, ‘nostril shape’, ‘tongue rolling’, left and right ‘hitchhiker's thumb’, ‘mid-digital hair’ and left and right ‘simian crease’ at the moment of interview. And 14 days later, these subjects received the same observation once again. The results showed that the inter- and intra-observer agreements of ‘hair’, ‘earwax’, ‘tongue rolling’, ‘mid-digital hair’ and ‘simian crease’ were almost perfect with most kappa (κ) coefficients ≥ .80, while ‘Mongoloid fold’ and ‘nostril shape’ showed poor inter-observer agreement and ‘nostril shape’ showed poor intra-observer agreement (κ < .40). Two other physical features, ‘hitchhiker's thumb’ and ‘ear lobes’ showed moderate inter-observer agreement and three features, ‘hitchhiker's thumb’, ‘ear lobes’ and ‘Mongoloid fold’, showed moderate intra-observer agreement (.40≤ κ <.80). In conclusion, this study suggests that as far as reliability is concerned, the five features which were ‘hair’, ‘earwax’, ‘tongue rolling’, ‘mid-digital hair’ and ‘simian crease’ could be considered in zygosity determination of Chinese adult twins, while the two features, ‘Mongoloid fold’ and ‘nostril shape’, should be abandoned.
A method of absolutely unfolding the atomic spectrum by a pinhole transmission grating spectrometer (TGS) is discussed in detail. The numerical process considers both the factors of higher-dispersive-order overlapping of TGS and source-size widening effects. Real soft-X-ray spectra of aluminum, copper, and gold plasmas are presented of experiments performed on the Six-Beam Laser Facility at the Shanghai Institute of Optics and Fine Mechanics.
