Background
Ménière's disease is a debilitating chronic peripheral vestibular disorder associated with psychiatric co-morbidities, notably depression.
MethodsDatabase searches were performed to identify studies that assessed depression in Ménière's disease. Metrics used to diagnose depression were extracted, along with the prevalence of depression in each study.
ResultsFifteen studies from 8 different countries reported on 6587 patients. The weighted average age was 55.3 years (range, 21–88 years). Depression was measured by eight different scales, with Zung's Self-Rating Depression Scale used most often. A weighted proportion of 45.9 per cent of patients (confidence interval = 28.9–63.3) were depressed. Weighted averages (± standard deviations) of Beck's Depression Inventory and the Illness Behavior Questionnaire – Dysphoria were 8.5 ± 7.9 and 2.4 ± 1.7, respectively.
ConclusionThe prevalence of depression in patients with Ménière's disease is nearly 50 per cent. Treating otolaryngologists should have a low threshold to screen and refer appropriately. Identifying and treating depression should allow for improvement of overall quality of life in patients with Ménière's disease.

Background
Item 9 of the Patient Health Questionnaire-9 (PHQ-9) queries about thoughts of death and self-harm, but not suicidality. Although it is sometimes used to assess suicide risk, most positive responses are not associated with suicidality. The PHQ-8, which omits Item 9, is thus increasingly used in research. We assessed equivalency of total score correlations and the diagnostic accuracy to detect major depression of the PHQ-8 and PHQ-9.
MethodsWe conducted an individual patient data meta-analysis. We fit bivariate random-effects models to assess diagnostic accuracy.
Results16 742 participants (2097 major depression cases) from 54 studies were included. The correlation between PHQ-8 and PHQ-9 scores was 0.996 (95% confidence interval 0.996 to 0.996). The standard cutoff score of 10 for the PHQ-9 maximized sensitivity + specificity for the PHQ-8 among studies that used a semi-structured diagnostic interview reference standard (N = 27). At cutoff 10, the PHQ-8 was less sensitive by 0.02 (−0.06 to 0.00) and more specific by 0.01 (0.00 to 0.01) among those studies (N = 27), with similar results for studies that used other types of interviews (N = 27). For all 54 primary studies combined, across all cutoffs, the PHQ-8 was less sensitive than the PHQ-9 by 0.00 to 0.05 (0.03 at cutoff 10), and specificity was within 0.01 for all cutoffs (0.00 to 0.01).
ConclusionsPHQ-8 and PHQ-9 total scores were similar. Sensitivity may be minimally reduced with the PHQ-8, but specificity is similar.

Background
Different diagnostic interviews are used as reference standards for major depression classification in research. Semi-structured interviews involve clinical judgement, whereas fully structured interviews are completely scripted. The Mini International Neuropsychiatric Interview (MINI), a brief fully structured interview, is also sometimes used. It is not known whether interview method is associated with probability of major depression classification.
AimsTo evaluate the association between interview method and odds of major depression classification, controlling for depressive symptom scores and participant characteristics.
MethodData collected for an individual participant data meta-analysis of Patient Health Questionnaire-9 (PHQ-9) diagnostic accuracy were analysed and binomial generalised linear mixed models were fit.
ResultsA total of 17 158 participants (2287 with major depression) from 57 primary studies were analysed. Among fully structured interviews, odds of major depression were higher for the MINI compared with the Composite International Diagnostic Interview (CIDI) (odds ratio (OR) = 2.10; 95% CI = 1.15–3.87). Compared with semi-structured interviews, fully structured interviews (MINI excluded) were non-significantly more likely to classify participants with low-level depressive symptoms (PHQ-9 scores ≤6) as having major depression (OR = 3.13; 95% CI = 0.98–10.00), similarly likely for moderate-level symptoms (PHQ-9 scores 7–15) (OR = 0.96; 95% CI = 0.56–1.66) and significantly less likely for high-level symptoms (PHQ-9 scores ≥16) (OR = 0.50; 95% CI = 0.26–0.97).
ConclusionsThe MINI may identify more people as depressed than the CIDI, and semi-structured and fully structured interviews may not be interchangeable methods, but these results should be replicated.

On Perspectives. Mathematical logic arose from a concern with the nature and the limits of rational or mathematical thought, and from a desire to systematise the modes of its expression. The pioneering investigations were diverse and largely autonomous. As time passed, and more particularly in the last two decades, interconnections between different lines of research and links with other branches of mathematics proliferated. The subject is now both rich and varied. It is the aim of the series to provide, as it were, maps or guides to this complex terrain. We shall not aim at encyclopaedic coverage', nor do we wish to prescribe, like Euclid, a definitive version of the elements of the subject. We are not committed to any particular philosophical programme. Nevertheless we have tried by critical discussion to ensure that each book represents a coherent line of thought', and that, by developing certain themes, it will be of greater interest than a mere assemblage of results and techniques.
The books in the series differ in level: some are introductory, some highly specialised. They also differ in scope: some offer a wide view of an area, others present a single line of thought. Each book is, at its own level, reasonably self-contained. Although no book depends on another as prerequisite, we have encouraged authors to fit their book in with other planned volumes, sometimes deliberately seeking coverage of the same material from different points of view. We have tried to attain a reasonable degree of uniformity of notation and arrangement. However, the books in the series are written by individual authors, not by the group. Plans for books are discussed and argued about at length. Later, encouragement is given and revisions suggested. But it is the authors who do the work', if, as we hope, the series proves of value, the credit will be theirs.
History of the Q-Group. During 1968 the idea of an integrated series of monographs on mathematical logic was first mooted. Various discussions led to a meeting at Oberwolfach in the spring of 1969. Here the founding members of the group (R. O. Gandy, A. Levy, G. H. Mu'ller, G. E. Sacks, D. S. Scott) discussed the project in earnest and decided to go ahead with it. Professor F. K. Schmidt and Professor Hans Hermes gave us encouragement and support.

Callianassid fossils, preserved within their burrows, collected from Mount Discovery, East Antarctica, provide the first such occurrence in Antarctica as well as evidence for deposition in a shallow marine environment distal to a deltaic system. The age of the specimens, based upon associated dinoflagellate cysts, is late early to middle Eocene.

Reports of bloodstream infections caused by methicillin-resistant Staphylococcus aureus among chronic hemodialysis patients to 2 Centers for Disease Control and Prevention surveillance systems (National Healthcare Safety Network Dialysis Event and Emerging Infections Program) were compared to evaluate completeness of reporting. Many methicillin-resistant S. aureus bloodstream infections identified in hospitals were not reported to National Healthcare Safety Network Dialysis Event.

We assessed the impact of Haemophilus influenzae type b (Hib) vaccination, introduced in France in early 1993, on the incidence of invasive Haemophilus influenzae (Hi) disease up to 2008. The incidence of Hi meningitis fell from 0·9/100 000 in 1991–1992 to 0·09/100 000 in 1996–2008, with a marked decline (96%) in children aged <5 years, including infants aged <3 months, from 12 to 0·4/100 000. The incidence of invasive Hi disease also decreased in children aged <15 years from 6 to 0·7/100 000, remained stable in the 15–64 years age group at about 0·5/100 000, and increased slightly from 2·0 to 2·4/100 000 in persons aged >64 years. No emergence of non-encapsulated or encapsulated non-vaccine serotypes was observed. These findings confirm the major direct impact of Hib vaccination on the incidence of Hi invasive disease in children and the indirect benefit of vaccination for infants too young to be vaccinated.
