The essayist Okabe Itsuko (1923-2008) readied herself for death throughout her life and, when it came in the early morning hours of April 29, with her died an independent woman's voice of conscience for postwar Japan. The translation which follows is of an address she gave at the annual service in memory of “all the war dead” held by Higashi Honganji temple in Kyoto, shortly after the U.S. invasion of Iraq five years ago and a year before she announced she was laying down her pen after a half-century career of essay writing and a total of 134 books. The sight of the diminutive and frail 80-year old speaking before the great Kyoto hall of True Pure Land Buddhism, naming herself “a woman aggressor” responsible for the deaths of her loved ones in the war six decades earlier, may have been jarring but her touchstone story of her fiancé Kimura Kunio and her core value of respect for all people, including the weak and marginal, were well-known to a Japanese public longing for a voice grounded in the beauty of daily life and Japanese culture but unyielding in its call for peace and dignity and respect for all men and women.
In this study, we show that the characteristic radius rs, mass Ms, and the X-ray temperature, TX, of galaxy clusters form a thin plane in the space of (log rs, log Ms, log TX). This tight correlation indicates that the cluster structure including the temperature is affected by the formation time of individual clusters. Numerical simulations show that clusters move along the fundamental plane as they evolve. The plane and the cluster evolution within the plane can be explained by a similarity solution of structure formation. The angle of the plane shows that clusters have not achieved “virial equilibrium”. The details of this study are written in Fujita et al. (2018a,b).
Nivolumab has become an effective treatment option for cancer in various sites; however, this drug may cause immune-related adverse effects due to its mechanism of action. Furthermore, little has been reported on thiamine deficiency (TD) in patients receiving nivolumab treatment.
Method
From a series of cancer patients, we reported a patient with recurrent renal cell carcinoma who developed TD after the start of nivolumab treatment.
Results
A 74-year-old man with recurrent renal cell carcinoma was referred to the psycho-oncology department as he had lost about 4 kg and displayed a loss of energy after four cycles of nivolumab treatment. Psychiatric interviews revealed a decrease in energy. Neurological examination did not reveal any impairment in consciousness, ataxia, or ocular symptoms. He did not develop appetite loss. The malabsorption or overconsumption of some nutrients is thought to occur due to the rapid loss of weight; thus, a reduction in vitamin B1, which has a short storage period in the body and is often deficient in cancer patients, was suspected. The diagnosis of TD was supported by the patient's abnormally low serum thiamine level.
Significance of results
In patients treated with nivolumab, it is necessary to pay careful attention to TD when proceeding with the treatment. It is hoped that future research may reveal the link between nivolumab administration and TD.
Procyanidins have been reported to possess potential for the prevention of hyperglycaemia. However, there are very few data for procyanidins about the difference the degree of polymerisation (DP) has on anti-hyperglycaemic effects. Moreover, the underlying molecular mechanisms by which procyanidins suppress hyperglycaemia are not yet fully understood. In the present study, we prepared procyanidin fractions with different DP, namely low-DP (DP≤3) and high-DP (DP≥4) fractions, from a cacao liquor procyanidin-rich extract (CLPr). These fractions were administered orally to Institute of Cancer Research (ICR) mice and their anti-hyperglycaemic effects were examined. We found that CLPr and its fractions prevent postprandial hyperglycaemia accompanied by an increase in the plasma glucagon-like peptide-1 (GLP-1) level with or without glucose load. In the absence of glucose load, both fractions increased the plasma insulin level and activated its downstream signalling pathway in skeletal muscle, resulting in promotion of the translocation of GLUT4. Phosphorylation of AMP-activated protein kinase (AMPK) was also involved in the promotion of GLUT4 translocation. High- and low-DP fractions showed a similar activation of insulin and AMPK pathways. In conclusion, cacao liquor procyanidins prevent hyperglycaemia by promoting GLUT4 translocation in skeletal muscle, and both the GLP-1-activated insulin pathway and the AMPK pathway are involved in the underlying molecular mechanism.
Polycondensation reactions between amino acids and phenols are one of the pathways for the formation of humic substances, and clay minerals are able to catalyse these reactions. To investigate the catalytic power of allophane, an allophane fraction (ALF) was separated from weathered pumice (WP) that contained imogolite as an impurity by taking advantage of differences in sedimentation velocity. The iron content in the separated ALF was increased by up to 3.0% compared to that in the starting WP (1.3%), and the ALF was further treated with sodium dithionate and citric acid (ALF-DC) to remove the iron. The catalytic powers of WP, ALF and ALF-DC were evaluated, based on the degree of darkening of reaction mixtures from polycondensation reactions between catechol and tryptophan, model compounds for precursors of humic substances. The catalytic power for ALF was significantly higher than the corresponding values for WP and ALF-DC. This can be attributed to the high iron content of the ALF, which serves as a Lewis acid that can enhance nucleophilic reactions which occur during the polycondensation reactions.
An outbreak of enterohaemorrhagic Escherichia coli O157 occurred in multiple prefectures of Japan in November 2009. We conducted two case–control studies with trace-back and trace-forward investigations to determine the source. The case definition was met by 21 individuals; 14 (66.7%) were hospitalised, but no haemolytic uraemic syndrome, acute encephalopathy or deaths occurred. Median age was 23 (range 12–48) years and 14 cases were male (66.7%). No significant associations with food were found in a case–control study by local public health centres, but our matched case–control study using Internet surveys found that beef hanging tender (or hanger steak), derived from the diaphragm of the cattle, was significantly associated with illness (odds ratio = 15.77; 95% confidence interval, 2.00–124.11). Pulsed-field gel electrophoresis analysis of isolates from patients and the suspected food showed five different patterns: two in faecal and food samples, and another three in patient faecal samples only, although there were epidemiological links to the meat consumed at the restaurants. Trace-back investigation implicated a common food processing company from outside Japan. Examination of the logistics of the meat processing company suggested that contamination did not occur in Japan. We concluded that the source of the outbreak was imported hanging tender. This investigation revealed that Internet surveys could be useful for outbreak investigations.
Thirty angiosperm species in 20 families, collected in Nam Kading National Protected Area, are recorded in Laos for the first time. Because the flora of Nam Kading is rich in endemic species, the area represents one of the core sites of plant biodiversity in Laos. To elucidate the plant diversity accurately, further intensive floristic surveys are required.
Effective conservation of global species diversity requires a clear understanding of spatial scales that support overall diversity across broad scales. Abandonment of semi-natural grasslands has increased their fragmentation and decreased their areal extent. We quantified diversity patterns of plant communities in Japan across hierarchical scales to facilitate the development of an effective nationwide strategy for conserving species diversity in remnant semi-natural grasslands. We applied additive partitioning of plant species diversity, using a nested hierarchical design at three spatial scales (quadrat, grassland, and western and eastern regions of Japan) for three groups of plant species (all species, grassland species and national Red Listed species). We consistently found lower proportions of among-quadrats diversity, and higher proportions of among-grasslands diversity and between-regions diversity in the overall diversity of the entire species complement than would be expected by chance. The high contribution of among-grasslands diversity to overall diversity suggests that each grassland had a unique species content. The second-ranking contributor to overall diversity differed between grassland species and Red Listed species: the second-ranking contributor for grassland species was diversity at the among-quadrats scale but the second-ranking contributor for all species and for Red Listed species was diversity at the between-regions scale. Thus, effective conservation of diversity of the entire species complement in remnant semi-natural grasslands requires preservation of beta diversity in individual grasslands. Our findings highlight the importance of strengthening local preservation and restoration activities within each grassland, and of nationwide strategies for conserving Red Listed species in remnant semi-natural grassland communities.
Shiga-toxin-producing Escherichia coli (STEC) infections usually cause haemolytic uraemic syndrome (HUS) equally in male and female children. This study investigated the localization of globotriaosylceramide (Gb3) in human brain and kidney tissues removed from forensic autopsy cases in Japan. A fatal case was used as a positive control in an outbreak of diarrhoeal disease caused by STEC O157:H7 in a kindergarten in Urawa in 1990. Positive immunodetection of Gb3 was significantly more frequent in female than in male distal and collecting renal tubules. To correlate this finding with a clinical outcome, a retrospective analysis of the predictors of renal failure in the 162 patients of two outbreaks in Japan was performed: one in Tochigi in 2002 and the other in Kagawa Prefecture in 2005. This study concludes renal failure, including HUS, was significantly associated with female sex, and the odds ratio was 4·06 compared to male patients in the two outbreaks. From 2006 to 2009 in Japan, the risk factor of HUS associated with STEC infection was analysed. The number of males and females and the proportion of females who developed HUS were calculated by age and year from 2006 to 2009. In 2006, 2007 and 2009 in adults aged >20 years, adult women were significantly more at risk of developing HUS in Japan.
Herpes simplex virus in the trigeminal ganglia of humans was studied in Philadelphia, Pennsylvania, United States of A merica, and in Kyoto, Japan. The prevalence of recurrent herpes labialis and of clinically latent herpes simplex virus within trigeminal ganglia was determined in inhabitants of the two cities. In addition, a comparison was made of the prevalence of mononuclear cell infiltration in the trigeminal ganglia of Americans and Japanese. Recurrent herpes labialis was found to be significantly less common in the Japanese city than in the American city. Herpes simplex virus was rescued less commonly from the trigeminal ganglia of cadavers in Japan than in America. The difference was significant. The frequency of mononuclear cell infiltration in the trigeminal ganglia of Americans and Japanese is not significantly different. These observations, as well as previously reported serological studies, suggest that despite the ubiquitous nature of herpes simplex virus in A merica and Japan, the Japanese have less clinically overt desease caused by this virus.
A large outbreak of enterohaemorrhagic Escherichia coli (EHEC) O111 and O157 occurred in Japan in April 2011. We conducted an unmatched case-control study and trace-back investigation to determine the source of EHEC O111 infection and risk factors for severe complications. Pulsed-field gel electrophoresis was performed to help define cases. A total of 86 individuals met the case definition. Of these, 40% experienced haemolytic uraemic syndrome (HUS), 24% acute encephalopathy, and 6% died. Illness was significantly associated with eating the raw beef dish yukhoe (odds ratio 19·64, 95% confidence interval 7·03–54·83), the likely food vehicle. EHEC O111 and its closely related stx-negative variants were found in the beef. HUS occurred most frequently in individuals aged 5–9 years, and this age group was significantly associated with acute encephalopathy. The prevalence of HUS and acute encephalopathy was higher than in previous non-O157-related outbreaks, indicating a high risk of severe complications.
A case of salvage supracricoid laryngectomy with cricohyoidoepiglottopexy after failed radiation therapy and vertical partial laryngectomy had successful oncological and functional outcomes. This is the first reported application of salvage supracricoid laryngectomy with cricohyoidoepiglottopexy after the failure of two major treatments.
Case report:
A 65-year-old man was referred for salvage supracricoid laryngectomy with cricohyoidoepiglottopexy. The right recurrent hemilarynx was successfully resected. After pexis, the right lobe of the thyroid gland was repositioned to overlap and reinforce the pexis gap and fill the devoid portion of the strap muscular closure. Multiple scattered foci (recurrent tumour–node–metastasis stage T2) were identified around the arytenoid cartilage and beneath the musculocutaneous flap. Four years after supracricoid laryngectomy with cricohyoidoepiglottopexy, the patient's recovery was following a favourable course and he had satisfactory laryngeal function.
Conclusion:
Appropriate case selection and proficient surgical skills were essential for a successful outcome. Head and neck surgeons should not be afraid to adopt functional preservation open surgical procedures in well-selected and well-motivated patients. A requirement for more challenging surgical procedures and meticulous rehabilitation processes should not exclude appropriate treatments from a surgeon's repertoire.
Streptococcal toxic shock syndrome (STSS) is a severe invasive infection characterized by the sudden onset of shock, multi-organ failure, and high mortality. In Japan, appropriate notification measures based on the Infectious Disease Control law are mandatory for cases of STSS caused by β-haemolytic streptococcus. STSS is mainly caused by group A streptococcus (GAS). Although an average of 60–70 cases of GAS-induced STSS are reported annually, 143 cases were recorded in 2011. To determine the reason behind this marked increase, we characterized the emm genotype of 249 GAS isolates from STSS patients in Japan from 2010 to 2012 and performed antimicrobial susceptibility testing. The predominant genotype was found to be emm1, followed by emm89, emm12, emm28, emm3, and emm90. These six genotypes constituted more than 90% of the STSS isolates. The number of emm1, emm89, emm12, and emm28 isolates increased concomitantly with the increase in the total number of STSS cases. In particular, the number of mefA-positive emm1 isolates has escalated since 2011. Thus, the increase in the incidence of STSS can be attributed to an increase in the number of cases associated with specific genotypes.
In May 2011, an outbreak of Shiga toxin-producing Escherichia coli (STEC) O157 was reported from Yamagata Prefecture, Japan. Investigations, including a case-control study, revealed that the outbreak was linked to two varieties of rice cakes produced by a local manufacturer between 2 and 7 May. Active and passive surveillance identified 136 suspected cases, 142 confirmed cases, 26 asymptomatic cases, and 25 secondary cases. While no environmental samples taken from the manufacturing premises tested positive for STEC, other than a stool sample taken from one employee, on-site and epidemiological investigations indicated that STEC was introduced during the manufacturing process of rice cakes rather than through contamination of raw materials. This was the first reported outbreak of STEC associated with cakes and confectionery in Japan, which indicates that contamination and outbreaks of STEC can occur in any food unless proper precautions are taken.
Hyperglycaemia and insulin resistance are associated with the increased risk of themetabolic syndrome and other severe health problems. The insulin-sensitive GLUT4 regulatesglucose homoeostasis in skeletal muscle and adipose tissue. In this study, we investigatedwhether cacao liquor procyanidin (CLPr) extract, which contains epicatechin, catechin andother procyanidins, improves glucose tolerance by promoting GLUT4 translocation andenhances glucose uptake in muscle cells. Our results demonstrated that CLPr increasedglucose uptake in a dose-dependent manner and promoted GLUT4 translocation to the plasmamembrane of L6 myotubes. Oral administration of a single dose of CLPr suppressed thehyperglycaemic response after carbohydrate ingestion, which was accompanied by enhancedGLUT4 translocation in ICR mice. These effects of CLPr were independent ofα-glucosidase inhibition in the small intestine. CLPr also promoted GLUT4translocation in skeletal muscle of C57BL/6 mice fed a CLPr-supplemented diet for7 d. These results indicate that CLPr is a beneficial food material for improvementof glucose tolerance by promoting GLUT4 translocation to the plasma membrane of skeletalmuscle.
We examined host abundance of limoniid flies between Agaricales (gill mushrooms) and Aphyllophorales (non-gilled fungi) and the importance of these flies as phoronts of mites inhabiting these fungal orders. Fungal sporophores were collected around transects established in five different-aged (13 to more than 100 years) forest stands from spring to fall in temperate forests of Japan. Limoniid flies and mites were collected from sporophores during transect sampling and reared from sporophores in the laboratory. All 11 limoniid species reared from sporophores used Aphyllophorales and six of these limoniid species also used Agaricales as their hosts. The total number of limoniid flies reared from Aphyllophorales was approximately seven times that reared from Agaricales. Eight of 15 Aphyllophorales species yielded multiple limoniid species. We conclude that Aphyllophorales are more important as hosts for fungivorous limoniid flies than Agaricales. Even a single fungal species can support more than one limoniid or mite species. One or two limoniid species are phoronts for one to three mite species inhabiting sporophores of Agaricales and Aphyllophorales. These flies have an important role as phoronts for some mite genera inhabiting fungal sporophores.
Kyushu University has installed the hard X-ray beamline, BL06, at bending magnet source of SAGA Light Source in Japan. It is designed for X-ray absorption fine structure (XAFS) spectroscopy and small-angle X-ray scattering (SAXS) experiments. The X-ray energy covered by the beamline ranges from 2.1 to 23 keV using a Si(111) flat double-crystal monochromator. Two bent cylindrical-type focusing mirrors are used, each with a different focal point. They are set in the optics section and can be used independently. One of the mirrors has a focal distance of 6 m, which corresponds to the sample stage for XAFS experiments, while the second mirror focuses at 10 m, corresponding to the detector for the SAXS experiments. The photon flux measured by the ion chamber at mirror focal point is 1010 phs/s. The experimental hutch houses the detection equipment for both XAFS and SAXS. For XAFS studies, a set of ion chambers for transmission mode and a Lytle detector and a Si drift detector for fluorescence yield mode are available. For SAXS studies, the camera pass length can be chosen from 0.5 to 2.5 m with 0.5 m increments in between, and an image plate is operated as signal detector. The beamline will be opened for user operation around summer 2010.
In December 2006, an outbreak of gastroenteritis occurred involving 372 guests and 72 employees at a hotel after a guest vomited in corridors on the third (F3) and 25th (F25) floors. Norovirus with identical genotype was confirmed by real-time reverse transcription–polymerase chain reaction in faecal samples from guest cases and employees. Spread of the outbreak on F25 was compared with that on F3. The attack rate in the guests who visited F25 alone (15·0%, 106/708 guests) was significantly higher than in those who visited F3 alone (3·5%, 163/4710 guests) (relative risk 4·3, 95% confidence interval 3·4–5·5, P<0·001). The outbreak on F3 ended within 2 days, while that on F25 extended over 7 days. The environmental ratios of F3 to F25 were 7·4 for volume, 6·9 for floor area and 7·6 for ventilation rate. This outbreak suggests that environmental differences can affect the propagation and persistence of a norovirus outbreak following environmental contamination.
