Targeting the glutamatergic system is posited as a potentially novel therapeutic strategy for psychotic disorders. While studies in subjects indicate that antipsychotic medication reduces brain glutamatergic measures, they were unable to disambiguate clinical changes from drug effects.
Aims
To address this, we investigated the effects of a dopamine D2 receptor partial agonist (aripiprazole) and a dopamine D2 receptor antagonist (amisulpride) on glutamatergic metabolites in the anterior cingulate cortex (ACC), striatum and thalamus in healthy controls.
Method
A double-blind, within-subject, cross-over, placebo-controlled study design with two arms (n = 25 per arm) was conducted. Healthy volunteers received either aripiprazole (up to 10 mg/day) for 7 days or amisulpride (up to 400 mg/day) and a corresponding period of placebo treatment in a pseudo-randomised order. Magnetic resonance spectroscopy (1H-MRS) was used to measure glutamatergic metabolite levels and was carried out at three different time points: baseline, after 1 week of drug and after 1 week of placebo. Values were analysed as a combined measure across the ACC, striatum and thalamus.
Results
Aripiprazole significantly increased glutamate + glutamine (Glx) levels compared with placebo (β = 0.55, 95% CI [0.15, 0.95], P = 0.007). At baseline, the mean Glx level was 8.14 institutional units (s.d. = 2.15); following aripiprazole treatment, the mean Glx level was 8.16 institutional units (s.d. = 2.40) compared with 7.61 institutional units (s.d. = 2.36) for placebo. This effect remained significant after adjusting for plasma parent and active metabolite drug levels. There was an observed increase with amisulpride that did not reach statistical significance.
Conclusions
One week of aripiprazole administration in healthy participants altered brain Glx levels as compared with placebo administration. These findings provide novel insights into the relationship between antipsychotic treatment and brain metabolites in a healthy participant cohort.
Loneliness has become a major public health issue of the recent decades due to its severe impact on health and mortality. Little is known about the relation between loneliness and social anxiety. This study aimed (1) to explore levels of loneliness and social anxiety in the general population, and (2) to assess whether and how loneliness affects symptoms of social anxiety and vice versa over a period of five years.
Methods
The study combined data from the baseline assessment and the five-year follow-up of the population-based Gutenberg Health Study. Data of N = 15 010 participants at baseline (Mage = 55.01, s.d.age = 11.10) were analyzed. Multiple regression analyses with loneliness and symptoms of social anxiety at follow-up including sociodemographic, physical illnesses, and mental health indicators at baseline were used to test relevant covariates. Effects of loneliness on symptoms of social anxiety over five years and vice versa were analyzed by autoregressive cross-lagged structural equation models.
Results
At baseline, 1076 participants (7.41%) showed symptoms of social anxiety and 1537 (10.48%) participants reported feelings of loneliness. Controlling for relevant covariates, symptoms of social anxiety had a small significant effect on loneliness five years later (standardized estimate of 0.164, p < 0.001). Vice versa, there was no significant effect of loneliness on symptoms of social anxiety taking relevant covariates into account.
Conclusions
Findings provided evidence that symptoms of social anxiety are predictive for loneliness. Thus, prevention and intervention efforts for loneliness need to address symptoms of social anxiety.
Abrupt cessation of heavy cannabis use can cause a withdrawal syndrome characterised by irritability, anxiety, insomnia, reduced appetite and restlessness. Recent reports have also described people in whom cannabis withdrawal immediately preceded the acute onset of psychosis.
Aims
To identify cases of psychosis associated with cannabis withdrawal.
Method
We completed a systematic review of the literature, which comprised case reports, case series and other studies. We also searched a large electronic database of psychiatric healthcare records.
Results
The systematic review identified 44 individuals from 21 studies in whom cannabis withdrawal preceded the development of acute psychosis. In the health record study, we identified another 68 people, of whom 47 involved a first episode of psychosis and 21 represented further episodes of an existing psychotic disorder. Almost all people were daily cannabis users who had stopped using cannabis abruptly. Individuals who continued to use cannabis after the acute psychotic episode had a much higher risk of subsequent relapse than those who abstained (odds ratio 13.9 [95% CI: 4.1 to 56.9]; χ2 = 20.1, P < 0.00001).
Conclusions
Abrupt cannabis withdrawal may act as a trigger for the first episode of psychosis and a relapse of an existing psychosis. Acute psychotic symptoms can emerge after the cessation, as well as following the use, of cannabis.
Inscribed metrical texts are often marginalised in the study of archaic and classical poetics. This chapter integrates examples from c. 725 to c. 450 BCE with wider practices of intertextuality, where epigrams both share techniques with other kinds of literature and have some distinctive modes of signification. Early epigrams frequently use formulas to demonstrate the adherence of a dedication or funeral to traditional norms. The apparent decline in use of glaukopis on the Acropolis after 550 BCE suggests that these formulas were not just metrically convenient: semantics mattered. Epigrams also replicate formulaic phrases from both elegy and epic with their traditional connotations intact; such connotations enrich our understanding of Phrasicleia’s epitaph (CEG 24), and of ancient epigrammatists’ conceptualisation of the relationship between oral and inscribed poetry. Inscriptions, unlike other texts, also invited interpretation against monuments situated nearby. Early dialogues of this type tend to be cooperative rather than polemical. Examples discussed include Onatas’ signatures at Olympia, the tripods at the Theban Hismenion described by Herodotus, and the relationship of the Eion herms to CEG 2–3.
In response to the COVID-19 pandemic, we rapidly implemented a plasma coordination center, within two months, to support transfusion for two outpatient randomized controlled trials. The center design was based on an investigational drug services model and a Food and Drug Administration-compliant database to manage blood product inventory and trial safety.
Methods:
A core investigational team adapted a cloud-based platform to randomize patient assignments and track inventory distribution of control plasma and high-titer COVID-19 convalescent plasma of different blood groups from 29 donor collection centers directly to blood banks serving 26 transfusion sites.
Results:
We performed 1,351 transfusions in 16 months. The transparency of the digital inventory at each site was critical to facilitate qualification, randomization, and overnight shipments of blood group-compatible plasma for transfusions into trial participants. While inventory challenges were heightened with COVID-19 convalescent plasma, the cloud-based system, and the flexible approach of the plasma coordination center staff across the blood bank network enabled decentralized procurement and distribution of investigational products to maintain inventory thresholds and overcome local supply chain restraints at the sites.
Conclusion:
The rapid creation of a plasma coordination center for outpatient transfusions is infrequent in the academic setting. Distributing more than 3,100 plasma units to blood banks charged with managing investigational inventory across the U.S. in a decentralized manner posed operational and regulatory challenges while providing opportunities for the plasma coordination center to contribute to research of global importance. This program can serve as a template in subsequent public health emergencies.
This article examines the ways in which Brazil's African foreign policy during the Ernesto Geisel administration (1974–9) utilised notions of ‘racial democracy’ and the nation's Africanity in framing itself as an intrinsic partner to the continent across the Atlantic. It does this through an analysis of Brazil's involvement at the Second World Black and African Festival of Arts and Culture (FESTAC’77, 15 January–12 February 1977), hosted in Lagos, Nigeria. The international event celebrated past and present contributions of Black and African cultures to global civilisation. An assessment of the Brazilian government's delegation to FESTAC’77 shows how the Geisel administration attempted to depict Brazil as a harmoniously integrated society, where, through a historic process of mixing, the nation's racial identity was united into an equitable whole. In contrast, the propagation of these ideas at FESTAC’77 left the regime's racial ideology vulnerable to attack from international and domestic audiences.
Polynesia is a hotspot for marine biodiversity in the South Pacific Ocean, yet the distribution of many invertebrate taxa in this region is still often poorly assessed. Information on the diversity and phylogeography of sponges in particular remains limited in spite of their importance for coral reef ecosystems. Recent expeditions to the island group of Wallis and Futuna enabled the first larger-scale assessment of the Wallis Island sponge fauna, resulting in the molecular identification of 82 unique Molecular Operational Taxonomic Units (MOTUs) from 339 sponge samples based on 28S C-region rDNA and CO1 mtDNA data. Faunal comparisons with both adjacent archipelagos and more distant Indo-Pacific regions were predominantly based on the MOTUs obtained from Wallis Island ecoregions, and suggest high levels of endemism of sponges in Wallis and Futuna, corroborating previous data on the biodiversity of sponges and other marine phyla in the South Pacific. The results of this molecular taxonomic survey of the Wallis and Futuna sponge fauna aim to lay solid foundations for a sustainable ‘Blue Economy’ in Wallis and Futuna for the conservation of their local coral reefs.
The beach–foredune system at Bengello Beach has been monitored monthly to bimonthly at four profiles (P1–P4) since 1972 and documented the building of a foredune. This paper addresses the remarkable changes which occurred in 2022 as storm waves overtopped and trimmed this foredune at all profiles, then later removed this entire feature at two of the profiles (P3, P4) but not the others (P1, P2). Wave parameters for these storm events, measured by deepwater and nearshore wave buoys, enable a comparison of storm characteristics and resulting beach–foredune impact. During the storm event which destroyed the foredune, nearshore wave height exceeded deepwater wave height, in contrast with other storms that year. The beach–foredune lost 78 m3/m in 2022 and the notable 1974 storms that impacted this coastline resulted in 95 m3/m volume loss. During 2023, beach recovery has occurred, but not rebuilt the foredune. It had persisted for ~40 years enduring many other severe storm events, and the coastal protection afforded by the dune system has been compromised. This highlights the need to consider dune morphology in assessments of erosion hazard and inundation risk along similar coastlines.
We identify a set of essential recent advances in climate change research with high policy relevance, across natural and social sciences: (1) looming inevitability and implications of overshooting the 1.5°C warming limit, (2) urgent need for a rapid and managed fossil fuel phase-out, (3) challenges for scaling carbon dioxide removal, (4) uncertainties regarding the future contribution of natural carbon sinks, (5) intertwinedness of the crises of biodiversity loss and climate change, (6) compound events, (7) mountain glacier loss, (8) human immobility in the face of climate risks, (9) adaptation justice, and (10) just transitions in food systems.
Technical summary
The Intergovernmental Panel on Climate Change Assessment Reports provides the scientific foundation for international climate negotiations and constitutes an unmatched resource for researchers. However, the assessment cycles take multiple years. As a contribution to cross- and interdisciplinary understanding of climate change across diverse research communities, we have streamlined an annual process to identify and synthesize significant research advances. We collected input from experts on various fields using an online questionnaire and prioritized a set of 10 key research insights with high policy relevance. This year, we focus on: (1) the looming overshoot of the 1.5°C warming limit, (2) the urgency of fossil fuel phase-out, (3) challenges to scale-up carbon dioxide removal, (4) uncertainties regarding future natural carbon sinks, (5) the need for joint governance of biodiversity loss and climate change, (6) advances in understanding compound events, (7) accelerated mountain glacier loss, (8) human immobility amidst climate risks, (9) adaptation justice, and (10) just transitions in food systems. We present a succinct account of these insights, reflect on their policy implications, and offer an integrated set of policy-relevant messages. This science synthesis and science communication effort is also the basis for a policy report contributing to elevate climate science every year in time for the United Nations Climate Change Conference.

Schizotypy represents an index of psychosis-proneness in the general population, often associated with childhood trauma exposure. Both schizotypy and childhood trauma are linked to structural brain alterations, and it is possible that trauma exposure moderates the extent of brain morphological differences associated with schizotypy.
Methods
We addressed this question using data from a total of 1182 healthy adults (age range: 18–65 years old, 647 females/535 males), pooled from nine sites worldwide, contributing to the Enhancing NeuroImaging Genetics through Meta-Analysis (ENIGMA) Schizotypy working group. All participants completed both the Schizotypal Personality Questionnaire Brief version (SPQ-B), and the Childhood Trauma Questionnaire (CTQ), and underwent a 3D T1-weighted brain MRI scan from which regional indices of subcortical gray matter volume and cortical thickness were determined.
Results
A series of multiple linear regressions revealed that differences in cortical thickness in four regions-of-interest were significantly associated with interactions between schizotypy and trauma; subsequent moderation analyses indicated that increasing levels of schizotypy were associated with thicker left caudal anterior cingulate gyrus, right middle temporal gyrus and insula, and thinner left caudal middle frontal gyrus, in people exposed to higher (but not low or average) levels of childhood trauma. This was found in the context of morphological changes directly associated with increasing levels of schizotypy or increasing levels of childhood trauma exposure.
Conclusions
These results suggest that alterations in brain regions critical for higher cognitive and integrative processes that are associated with schizotypy may be enhanced in individuals exposed to high levels of trauma.
Within the last decade, online sustainability knowledge-action platforms have proliferated. We surveyed 198 sustainability-oriented sites and conducted a review of 41 knowledge-action platforms, which we define as digital tools that advance sustainability through organized activities and knowledge dissemination. We analyzed platform structure and functionality through a systematic coding process based on key issues identified in three bodies of literature: (a) the emergence of digital platforms, (b) the localization of the sustainable development goals (SDGs), and (c) the importance of multi-level governance to sustainability action. While online collaborative tools offer an array of resources, our analysis indicates that they struggle to provide context-sensitivity and higher-level analysis of the trade-offs and synergies between sustainability actions. SDG localization adds another layer of complexity where multi-level governance, actor, and institutional priorities may generate tensions as well as opportunities for intra- and cross-sectoral alignment. On the basis of our analysis, we advocate for the development of integrative open-source and dynamic global online data management tools that would enable the monitoring of progress and facilitate peer-to-peer exchange of ideas and experience among local government, community, and business stakeholders. We argue that by showcasing and exemplifying local actions, an integrative platform that leverages existing content from multiple extant platforms through effective data interoperability can provide additional functionality and significantly empower local actors to accelerate local to global actions, while also complex system change.
This study investigated sex differences in Fe status, and associations between Fe status and endurance and musculoskeletal outcomes, in military training. In total, 2277 British Army trainees (581 women) participated. Fe markers and endurance performance (2·4 km run) were measured at the start (week 1) and end (week 13) of training. Whole-body areal body mineral density (aBMD) and markers of bone metabolism were measured at week 1. Injuries during training were recorded. Training decreased Hb in men and women (mean change (–0·1 (95 % CI –0·2, –0·0) and –0·7 (95 % CI –0·9, –0·6) g/dl, both P < 0·001) but more so in women (P < 0·001). Ferritin decreased in men and women (–27 (95 % CI –28, –23) and –5 (95 % CI –8, –1) µg/l, both P ≤ 0·001) but more so in men (P < 0·001). Soluble transferrin receptor increased in men and women (2·9 (95 % CI 2·3, 3·6) and 3·8 (95 % CI 2·7, 4·9) nmol/l, both P < 0·001), with no difference between sexes (P = 0·872). Erythrocyte distribution width increased in men (0·3 (95 % CI 0·2, 0·4)%, P < 0·001) but not in women (0·1 (95 % CI –0·1, 0·2)%, P = 0·956). Mean corpuscular volume decreased in men (–1·5 (95 % CI –1·8, –1·1) fL, P < 0·001) but not in women (0·4 (95 % CI –0·4, 1·3) fL, P = 0·087). Lower ferritin was associated with slower 2·4 km run time (P = 0·018), sustaining a lower limb overuse injury (P = 0·048), lower aBMD (P = 0·021) and higher beta C-telopeptide cross-links of type 1 collagen and procollagen type 1 N-terminal propeptide (both P < 0·001) controlling for sex. Improving Fe stores before training may protect Hb in women and improve endurance and protect against injury.
Regression models with log-transformed dependent variables are widely used by social scientists to investigate nonlinear relationships between variables. Unfortunately, this transformation complicates the substantive interpretation of estimation results and often leads to incomplete and sometimes even misleading interpretations. We focus on one valuable but underused method, the presentation of quantities of interest such as expected values or first differences on the original scale of the dependent variable. The procedure to derive these quantities differs in seemingly minor but critical aspects from the well-known procedure based on standard linear models. To improve empirical practice, we explain the underlying problem and develop guidelines that help researchers to derive meaningful interpretations from regression results of models with log-transformed dependent variables.
Testing activities to gain specific design knowledge play an essential role in engineering design, when a structure needs to be developed, for which knowledge from analytical models or documentation is missing. As research into these testing activities is difficult, few insights into successful approaches exist.
In this contribution, we investigate testing activities to gain specific design knowledge through a laboratory task, where 10 engineering students optimize a system using a web-based process chain including rapid prototyping and testing. Design and testing data are acquired from 110 prototypes in 3 hours. A differentiation of performance is conducted and approaches of high- and low-performers are investigated to identify patterns.
Based on these patterns, hypotheses, and metrics indicating successful and non-successful approaches are derived as basis for development of metrics for testing to gain specific design knowledge. A successful approach was overstep the limit, where participants accept destruction of their system to identify boundaries. An unsuccessful approach was the change of many parameters in later tests. These hypotheses and their metrics can then be used in development and validation of support for testing.
An eighth-century Latin version of a Greek edition of Aratus preserves valuable ancient scholarship on the Phaenomena, including material not preserved in Greek. Examination of over thirteen thousand Latin–Greek correspondences enables one to interpret passages of the Latin that have so far resisted analysis, including information about an ancient edition equipped with critical signs and commentary, ancient discussion of the primary narratee in Aratus and Homer, and the alternative proem to Anclides (SH 84).
In the connected world, although societies are not directly involved in a military conflict, they are exposed to media reports of violence.
Aims
We assessed the effects of such exposures on mental health in Germany during the military conflict in Ukraine.
Method
We used the German population-based cohort for digital health research, DigiHero, launching a survey on the eighth day of the Russo-Ukrainian war. Of the 27 509 cohort participants from the general population, 19 444 (70.7%) responded within 17 days. We measured mental health and fear of the impact of war compared with other fears (natural disasters or health-related).
Results
In a subsample of 4441 participants assessed twice, anxiety in the population (measured by the Generalised Anxiety Disorder-7 screener) was higher in the first weeks of war than during the strongest COVID-19 restrictions. Anxiety was elevated across the whole age spectrum, and the mean was above the cut-off for mild anxiety. Over 95% of participants expressed various degrees of fear of the impact of war, whereas the percentage for other investigated fears was 0.47–0.82. A one-point difference in the fear of the impact of war was associated with a 2.5 point (95% CI 2.42–2.58) increase in anxiety (11.9% of the maximum anxiety score). For emotional distress, the increase was 0.67 points (0.66–0.68) (16.75% of the maximum score).
Conclusions
The population in Germany reacted to the Russo-Ukrainian war with substantial distress, exceeding reactions during the strongest restrictions in the COVID-19 pandemic. Fear of the impact of war was associated with worse mental health.
Androgen deprivation therapy (ADT) in men with prostate cancer (PCa) is associated with significant side effects. With the transition of PCa from a foudroyant course to a chronic disease, managing these side effects has become increasingly important. There is growing evidence that nutritional changes and physical activity are beneficial in these patients. Here we examine the impact of written patient information on the physical activity and dietary habits of PCa patients receiving ADT and behaviour changes between baseline and 1 year, in the open-label, non-interventional LEAN study. In total, 959 patients with advanced hormone-sensitive PCa requiring ADT with the Leuprorelin Sandoz® implant were included from January 2014 to July 2015 and followed for ≥ 12 months. At the start of the study, urologists received a questionnaire concerning the written information provided to patients regarding their disease, patient advocacy groups, diet and physical activity. Patients received a questionnaire on their dietary habits and physical activity at the start and end of the study. Urologists from 147 study centres and 540 patients responded to the questionnaires. While 69 % of these patients received disease-specific information, only 30 % and 17 % received information regarding nutrition and physical activity, respectively. The majority of urologists estimate that their patients rarely or never follow guidance on nutrition or physical activity, yet > 90 % of patients indicate they would make use of this information, if provided. Few patients showed behavioural changes between baseline and 1 year without evident differences between patients that received information and those that did not.
