Patients with posttraumatic stress disorder (PTSD) exhibit smaller regional brain volumes in commonly reported regions including the amygdala and hippocampus, regions associated with fear and memory processing. In the current study, we have conducted a voxel-based morphometry (VBM) meta-analysis using whole-brain statistical maps with neuroimaging data from the ENIGMA-PGC PTSD working group.
Methods
T1-weighted structural neuroimaging scans from 36 cohorts (PTSD n = 1309; controls n = 2198) were processed using a standardized VBM pipeline (ENIGMA-VBM tool). We meta-analyzed the resulting statistical maps for voxel-wise differences in gray matter (GM) and white matter (WM) volumes between PTSD patients and controls, performed subgroup analyses considering the trauma exposure of the controls, and examined associations between regional brain volumes and clinical variables including PTSD (CAPS-4/5, PCL-5) and depression severity (BDI-II, PHQ-9).
Results
PTSD patients exhibited smaller GM volumes across the frontal and temporal lobes, and cerebellum, with the most significant effect in the left cerebellum (Hedges’ g = 0.22, pcorrected = .001), and smaller cerebellar WM volume (peak Hedges’ g = 0.14, pcorrected = .008). We observed similar regional differences when comparing patients to trauma-exposed controls, suggesting these structural abnormalities may be specific to PTSD. Regression analyses revealed PTSD severity was negatively associated with GM volumes within the cerebellum (pcorrected = .003), while depression severity was negatively associated with GM volumes within the cerebellum and superior frontal gyrus in patients (pcorrected = .001).
Conclusions
PTSD patients exhibited widespread, regional differences in brain volumes where greater regional deficits appeared to reflect more severe symptoms. Our findings add to the growing literature implicating the cerebellum in PTSD psychopathology.
It remains unclear which individuals with subthreshold depression benefit most from psychological intervention, and what long-term effects this has on symptom deterioration, response and remission.
Aims
To synthesise psychological intervention benefits in adults with subthreshold depression up to 2 years, and explore participant-level effect-modifiers.
Method
Randomised trials comparing psychological intervention with inactive control were identified via systematic search. Authors were contacted to obtain individual participant data (IPD), analysed using Bayesian one-stage meta-analysis. Treatment–covariate interactions were added to examine moderators. Hierarchical-additive models were used to explore treatment benefits conditional on baseline Patient Health Questionnaire 9 (PHQ-9) values.
Results
IPD of 10 671 individuals (50 studies) could be included. We found significant effects on depressive symptom severity up to 12 months (standardised mean-difference [s.m.d.] = −0.48 to −0.27). Effects could not be ascertained up to 24 months (s.m.d. = −0.18). Similar findings emerged for 50% symptom reduction (relative risk = 1.27–2.79), reliable improvement (relative risk = 1.38–3.17), deterioration (relative risk = 0.67–0.54) and close-to-symptom-free status (relative risk = 1.41–2.80). Among participant-level moderators, only initial depression and anxiety severity were highly credible (P > 0.99). Predicted treatment benefits decreased with lower symptom severity but remained minimally important even for very mild symptoms (s.m.d. = −0.33 for PHQ-9 = 5).
Conclusions
Psychological intervention reduces the symptom burden in individuals with subthreshold depression up to 1 year, and protects against symptom deterioration. Benefits up to 2 years are less certain. We find strong support for intervention in subthreshold depression, particularly with PHQ-9 scores ≥ 10. For very mild symptoms, scalable treatments could be an attractive option.
Florpyrauxifen-benzyl is a postemergence rice herbicide that has reduced rice yield in some situations, and producers are concerned that the impact could be even greater with low rice seeding densities. Therefore, research was conducted in Stoneville, MS, from 2019 to 2021, to evaluate the effect of florpyrauxifen-benzyl on rice yield when a hybrid was seeded at reduced densities. Rice cultivar FullPage RT 7521 FP was seeded at 10, 17, 24, 30, and 37 kg ha−1. At the 4-leaf to 1-tiller growth stage, florpyrauxifen-benzyl was applied at 0 or 58 g ai ha−1. Rice injury following application of florpyrauxifen-benzyl was ≤8% across all seeding rates and evaluation intervals. Application of florpyrauxifen-benzyl reduced plant heights by 14% to all seeding rates but did not result in delayed rice maturity. When florpyrauxifen-benzyl was not applied to rice that was seeded at 10 and 17 kg ha−1 seeding rates, rice matured slower than when it was seeded at 24, 30, and 37 kg ha−1. When florpyrauxifen-benzyl was applied, rough rice grain yields were reduced by at the 17 and 37 kg ha−1 seeding rates, but not at any other seeding rate. In conclusion, application of florpyrauxifen-benzyl at a 2× rate can cause a loss of yield resulting from variation in rice densities.
Accurate diagnosis of bipolar disorder (BPD) is difficult in clinical practice, with an average delay between symptom onset and diagnosis of about 7 years. A depressive episode often precedes the first manic episode, making it difficult to distinguish BPD from unipolar major depressive disorder (MDD).
Aims
We use genome-wide association analyses (GWAS) to identify differential genetic factors and to develop predictors based on polygenic risk scores (PRS) that may aid early differential diagnosis.
Method
Based on individual genotypes from case–control cohorts of BPD and MDD shared through the Psychiatric Genomics Consortium, we compile case–case–control cohorts, applying a careful quality control procedure. In a resulting cohort of 51 149 individuals (15 532 BPD patients, 12 920 MDD patients and 22 697 controls), we perform a variety of GWAS and PRS analyses.
Results
Although our GWAS is not well powered to identify genome-wide significant loci, we find significant chip heritability and demonstrate the ability of the resulting PRS to distinguish BPD from MDD, including BPD cases with depressive onset (BPD-D). We replicate our PRS findings in an independent Danish cohort (iPSYCH 2015, N = 25 966). We observe strong genetic correlation between our case–case GWAS and that of case–control BPD.
Conclusions
We find that MDD and BPD, including BPD-D are genetically distinct. Our findings support that controls, MDD and BPD patients primarily lie on a continuum of genetic risk. Future studies with larger and richer samples will likely yield a better understanding of these findings and enable the development of better genetic predictors distinguishing BPD and, importantly, BPD-D from MDD.
To understand healthcare workers’ (HCWs) beliefs and practices toward blood culture (BCx) use.
Design:
Cross-sectional electronic survey and semi-structured interviews.
Setting:
Academic hospitals in the United States.
Participants:
HCWs involved in BCx ordering and collection in adult intensive care units (ICU) and wards.
Methods:
We administered an anonymous electronic survey to HCWs and conducted semi-structured interviews with unit staff and quality improvement (QI) leaders in these institutions to understand their perspectives regarding BCx stewardship between February and November 2023.
Results:
Of 314 HCWs who responded to the survey, most (67.4%) were physicians and were involved in BCx ordering (82.3%). Most survey respondents reported that clinicians had a low threshold to culture patients for fever (84.4%) and agreed they could safely reduce the number of BCx obtained in their units (65%). However, only half of them believed BCx was overused. Although most made BCx decisions as a team (74.1%), a minority reported these team discussions occurred daily (42.4%). A third of respondents reported not usually collecting the correct volume per BCx bottle, half were unaware of the improved sensitivity of 2 BCx sets, and most were unsure of the nationally recommended BCx contamination threshold (87.5%). Knowledge regarding the utility of BCx for common infections was limited.
Conclusions:
HCWs’ understanding of best collection practices and yield of BCx was limited.
Rice producers battle herbicide-resistant weeds worldwide while producing rice for ≥50% of the world’s population. Oxyfluorfen can provide rice producers with an alternative site of action for barnyardgrass control, as there are no documented cases of grass weeds being resistant to the herbicide in the mid-southern United States. Oxyfluorfen is anticipated to be labeled in the Roxy Rice Production System and may be sold as a clomazone/oxyfluorfen premixture; hence, experiments were conducted in 2021 and 2022 to evaluate preemergence-applied clomazone/oxyfluorfen ratios compared to clomazone alone on silt loam and clay soils. All ratios of the herbicides caused less than 7% injury to rice in two of four site-years on silt loam soils, whereas, in the two other site-years, the mixtures caused 10% to 40% rice injury at all observation timings. All combinations of the two herbicides provided at least 73% barnyardgrass control 5 wk after rice emergence (WAE) in three of the four site-years on silt loam soils. In at least two of four site-years at 1 and 3 WAE, barnyardgrass control was improved when oxyfluorfen was added to clomazone compared to clomazone alone. On clay soil, barnyardgrass control in both site-years was ≥77% at 5 WAE for all clomazone and oxyfluorfen ratios. Injury to rice ranged from 13% to 30% for all treatments containing clomazone and oxyfluorfen in one of two site-years on clay soil at all observation timings. At 7 WAE, contrasts indicated that the 1:3 ratio of clomazone to oxyfluorfen provided greater barnyardgrass control than the 1:1.5 and 1:2 ratios in one of two site-years. Based on these findings, oxyfluorfen would improve the consistency of barnyardgrass control over clomazone alone in some instances. However, there is an increased risk of injury to rice with the addition of oxyfluorfen.
Oxyfluorfen is a herbicide that inhibits protoporphyrinogen IX oxidase and has shown significant potential in its ability to control barnyardgrass. Oxyfluorfen is categorized as a Group 14 herbicide by the Herbicide Resistance Action Committee (HRAC)/Weed Science Society of America (WSSA). Despite its current lack of labeling for use on rice in the mid-southern United States due to its potential to cause crop injury, the introduction of a trait in rice that confers resistance to oxyfluorfen could provide producers with an effective alternative site of action for weed control. Field experiments were conducted during the 2021 and 2022 growing seasons near Stuttgart, AR, and near Lonoke, AR, to determine the optimum rates of clomazone (280 or 336 g ha−1) and oxyfluorfen (673 or 840 g ha−1) to use in sequential preemergence (PRE) and postemergence (POST) applications on a silt loam soil and to assess the efficacy of oxyfluorfen when combined with clomazone and quinclorac applied PRE, followed by oxyfluorfen applied POST. No differences in barnyardgrass control were observed among treatments 14 d after emergence in 3 site years, as all control was ≥90%. By 35 d after the POST application, barnyardgrass control was ≥94% for all herbicide treatments in all site years. All herbicide treatments resulted in lower barnyardgrass seed production than a nontreated control in 2021. Contrasts revealed that oxyfluorfen applied PRE on a silt loam soil resulted in barnyardgrass control that was similar to that of clomazone or quinclorac applied alone at 14 d after emergence. Although oxyfluorfen combined with clomazone or quinclorac did not increase barnyardgrass control, an additional site of action for control of this weed could help reduce the evolution of resistance. Mixing oxyfluorfen with clomazone in a dry-seeded rice production system in the mid-southern United States would effectively control barnyardgrass and reduce the risk for resistance to both herbicides, further highlighting the potential of oxyfluorfen in rice production.
Emerging evidence suggests that routine physical activity may improve exercise capacity, long-term outcomes, and quality of life in individuals with Fontan circulation. Despite this, it is unclear how active these individuals are and what guidance they receive from medical providers regarding physical activity. The aim of this study was to survey Fontan patients on personal physical activity behaviours and their cardiologist-directed physical activity recommendations to set a baseline for future targeted efforts to improve this.
Methods:
An electronic survey assessing physical activity habits and cardiologist-directed guidance was developed in concert with content experts and patients/parents and shared via a social media campaign with Fontan patients and their families.
Results:
A total of 168 individuals completed the survey. The median age of respondents was 10 years, 51% identifying as male. Overall, 21% of respondents spend > 5 hours per week engaged in low-exertion activity and only 7% spend > 5 hours per week engaged in high-exertion activity. In all domains questioned, pre-adolescents reported higher participation rates than adolescents. Nearly half (43%) of respondents reported that they do not discuss activity recommendations with their cardiologist.
Conclusions:
Despite increasing evidence over the last two decades demonstrating the benefit of exercise for individuals living with Fontan circulation, only a minority of patients report engaging in significant amounts of physical activity or discussing activity goals with their cardiologist. Specific, individualized, and actionable education needs to be provided to patients, families, and providers to promote and support regular physical activity in this patient population.
Early nutritional and growth experiences can impact development, metabolic function, and reproductive outcomes in adulthood, influencing health trajectories in the next generation. The insulin-like growth factor (IGF) axis regulates growth, metabolism, and energetic investment, but whether it plays a role in the pathway linking maternal experience with offspring prenatal development is unclear. To test this, we investigated patterns of maternal developmental weight gain (a proxy of early nutrition), young adult energy stores, age, and parity as predictors of biomarkers of the pregnancy IGF axis (n = 36) using data from the Cebu Longitudinal Health and Nutrition Survey in Metro Cebu, Philippines. We analyzed maternal conditional weight measures at 2, 8, and 22 years of age and leptin at age 22 (a marker of body fat/energy stores) in relation to free IGF-1 and IGFBP-3 in mid/late pregnancy (mean age = 27). Maternal IGF axis measures were also assessed as predictors of offspring fetal growth. Maternal age, parity, and age 22 leptin were associated with pregnancy free IGF-1, offspring birth weight, and offspring skinfold thickness. We find that free IGF-1 levels in pregnancy are more closely related to nutritional status in early adulthood than to preadult developmental nutrition and demonstrate significant effects of young adult leptin on offspring fetal fat mass deposition. We suggest that the previously documented finding that maternal developmental nutrition predicts offspring birth size likely operates through pathways other than the maternal IGF axis, which reflects more recent energy status.
The diffusion of water in Li-montmorillonite was studied by incoherent quasielastic neutron scattering. Experiments were carried out on sedimented samples equilibrated at relative humidities of 32%, 58%, and 98%, corresponding approximately to 1, 2, and 3 molecular layers of water in the clay. At all three humidities, although the mobility of the water molecules is less than in bulk water, all water molecules in the system undergo translational diffusion, at least over short distances (>5 Å), with correlation times shorter than 5 × 10−11 sec.
Various models of molecular motion have been used to account for the exact shape of the scattering. The only completely successful model is one where a water molecule undergoes jump-translational diffusion and rotational diffusion. The mean square jump length is 10–15 Å2 with a residence time between jumps of 4–2 × 10−11 sec. The translational diffusion coefficient increases with humidity, having values of 4, 7, and 10 × 10−10 m2/sec for the three humidities. These values can be combined with values previously obtained by tracer measurements to give an estimate of 0.75–0.8 for the tortuosity factor. Although the samples are anisotropic, there is no clear evidence that the diffusion of water over distances 5–20 Å is anisotropic. An upper limit of 3 can be deduced for the rate of diffusion parallel to the direction perpendicular to the platelets.
The use of neutron diffraction to determine some of the structural properties of montmorillonite-water systems at low water concentrations is described. The samples were prepared by compression or suction to give clay samples with between one and three molecular layers of water between the plates.
About 10% of the platelets in the clay are randomly oriented. The remainder are partially oriented in the plane of the sample, with an angular spread of 40° about the mean orientation. It is suggested that these oriented domains are formed from the larger platelets present in the system. The Bragg diffraction pattern is better explained by a disordered lattice model rather than by a mixture model with small particles having a well-defined lattice spacing. We have fitted both the intensities of (00l) reflections and the shape of the (001) reflection quantitatively to a model which allows for a Gaussian spread of platelet spacing about a mean value. The half width of the spread is about 10% of the lattice spacing.
No significant structural differences are found between Li, Na, K, and Cs montmorillonites. The method of preparation has no effect on the structural properties of the large platelet particles but does affect the randomly oriented fraction. The lattice spacing of the latter appears to be better defined for samples prepared by compression.
Experiments on the variation of lattice spacing with humidity indicate that the structural model we have used is adequate except at humidities where the system is changing over from one to two, or two to three water layers.
A reliable method, utilizing a fluoride ion-selective electrode, is described for the determination of fluoride in clays and shales. Interference by aluminum and iron is minimal. The reproducibility of the method is about ±5% at different levels of fluoride concentration.
Data are presented for various clay minerals and for the <2-µm fractions of marine and nonmarine clays and shales. Fluoride values range from 44 ppm (0.0044%) for nontronite from Colfax, WA, to 51,800 ppm (5.18%) for hectorite from Hector, CA. In general, clays formed under hydrothermal conditions are relatively high in fluoride content, provided the hydrothermal waters are high in fluoride content. Besides hectorite, dickite from Ouray, CO, was found to contain more than 50 times as much fluoride (6700 ppm) as highly crystalline geode kaolinite (125 ppm). The clay stratum immediately overlying a fluorite mineralized zone in southern Illinois was found to have a higher fluoride content than the same stratum in a nonmineralized zone approximately 1 mile away. Nonmarine shales in contact with Australian coals were found to be lower in fluoride content than were marine shales in contact with Illinois coals.
It is believed that, in certain instances, peak shifts on DTA curves of similar clay minerals are the result of significant differences in their fluoride content.
Understanding characteristics of healthcare personnel (HCP) with SARS-CoV-2 infection supports the development and prioritization of interventions to protect this important workforce. We report detailed characteristics of HCP who tested positive for SARS-CoV-2 from April 20, 2020 through December 31, 2021.
Methods:
CDC collaborated with Emerging Infections Program sites in 10 states to interview HCP with SARS-CoV-2 infection (case-HCP) about their demographics, underlying medical conditions, healthcare roles, exposures, personal protective equipment (PPE) use, and COVID-19 vaccination status. We grouped case-HCP by healthcare role. To describe residential social vulnerability, we merged geocoded HCP residential addresses with CDC/ATSDR Social Vulnerability Index (SVI) values at the census tract level. We defined highest and lowest SVI quartiles as high and low social vulnerability, respectively.
Results:
Our analysis included 7,531 case-HCP. Most case-HCP with roles as certified nursing assistant (CNA) (444, 61.3%), medical assistant (252, 65.3%), or home healthcare worker (HHW) (225, 59.5%) reported their race and ethnicity as either non-Hispanic Black or Hispanic. More than one third of HHWs (166, 45.2%), CNAs (283, 41.7%), and medical assistants (138, 37.9%) reported a residential address in the high social vulnerability category. The proportion of case-HCP who reported using recommended PPE at all times when caring for patients with COVID-19 was lowest among HHWs compared with other roles.
Conclusions:
To mitigate SARS-CoV-2 infection risk in healthcare settings, infection prevention, and control interventions should be specific to HCP roles and educational backgrounds. Additional interventions are needed to address high social vulnerability among HHWs, CNAs, and medical assistants.
Radiation dermatitis (RD) is a frequent toxicity during radiotherapy (RT) for head and neck cancer (HNC). We report the first use of KeraStat® Cream (KC), a topical, keratin-based wound dressing, in patients with HNC receiving RT.
Methods:
This pilot study randomized HNC patients treated with definitive or postoperative RT (≥60 Gy) to KC or standard of care (SOC), applied at least twice daily during and for 1-month after RT. Outcomes of interest included adherence to the assigned regimen (at least 10 applications per week of treatment), clinician- and patient-reported RD, and skin-related quality of life.
Results:
24 patients were randomized and completed the study. Most patients had stage III-IV disease and oropharynx cancer. Median RT dose was 68 Gy; the bilateral neck was treated in 19 patients, and 18 patients received concurrent chemotherapy. Complete adherence was observed in 7/12 (SOC) vs. 10/12 (KC, p = 0.65). Adherence by patient-week was 61/68 versus 64/67, respectively (p = 0.20). No differences in RD were observed between groups.
Conclusion:
A randomized trial of KC versus SOC in HNC patients treated with RT is feasible with good adherence to study agent. An adequately powered randomized study is warranted to test the efficacy of KC in reducing RD.
OBJECTIVES/GOALS: Contingency management (CM) procedures yield measurable reductions in cocaine use. This poster describes a trial aimed at using CM as a vehicle to show the biopsychosocial health benefits of reduced use, rather than total abstinence, the currently accepted metric for treatment efficacy. METHODS/STUDY POPULATION: In this 12-week, randomized controlled trial, CM was used to reduce cocaine use and evaluate associated improvements in cardiovascular, immune, and psychosocial well-being. Adults aged 18 and older who sought treatment for cocaine use (N=127) were randomized into three groups in a 1:1:1 ratio: High Value ($55) or Low Value ($13) CM incentives for cocaine-negative urine samples or a non-contingent control group. They completed outpatient sessions three days per week across the 12-week intervention period, totaling 36 clinic visits and four post-treatment follow-up visits. During each visit, participants provided observed urine samples and completed several assays of biopsychosocial health. RESULTS/ANTICIPATED RESULTS: Preliminary findings from generalized linear mixed effect modeling demonstrate the feasibility of the CM platform. Abstinence rates from cocaine use were significantly greater in the High Value group (47% negative; OR = 2.80; p = 0.01) relative to the Low Value (23% negative) and Control groups (24% negative;). In the planned primary analysis, the level of cocaine use reduction based on cocaine-negative urine samples will serve as the primary predictor of cardiovascular (e.g., endothelin-1 levels), immune (e.g., IL-10 levels) and psychosocial (e.g., Addiction Severity Index) outcomes using results from the fitted models. DISCUSSION/SIGNIFICANCE: This research will advance the field by prospectively and comprehensively demonstrating the beneficial effects of reduced cocaine use. These outcomes can, in turn, support the adoption of reduced cocaine use as a viable alternative endpoint in cocaine treatment trials.
Herbicide-resistant barnyardgrass and weedy rice control, without crop injury, is a challenge for rice producers in the United States. Herbicides not initially labeled for rice, such as oxyfluorfen, are now being evaluated as new tools for weed control. The ROXY® trait allows for the use of oxyfluorfen in rice for weed control preemergence and postemergence. Experiments were initiated in 2021 and 2022 to evaluate (1) the effectiveness of preemergence- and postemergence-applied oxyfluorfen on barnyardgrass and weedy rice, (2) the sensitivity of oxyfluorfen-resistant rice to oxyfluorfen as a function of application timing, and (3) the influence of soil moisture on oxyfluorfen-resistant rice sensitivity to oxyfluorfen. In the field, a rate response was observed for oxyfluorfen applied to weedy rice when averaged over application timings of 1-leaf, 2-leaf, 3-leaf, and tillering, with oxyfluorfen at 1,680 g ai ha−1 resulting in 81% and 72% control 7 d after application (DAA) in 2021 and 2022, respectively. Under greenhouse conditions, barnyardgrass and weedy rice control averaged by the rate of oxyfluorfen was ≥85 and ≥70%, respectfully, 7 DAA for the 1-, 2-, and 3-leaf rice growth stage timings. Preemergence applications of oxyfluorfen under 100% soil saturation resulted in 75% injury to oxyfluorfen-resistant rice, greater than all other soil moisture at 7 DAA. All postemergence applications of oxyfluorfen resulted in 63% to 70% injury to oxyfluorfen-resistant rice at 7 DAA, regardless of soil moisture. Barnyardgrass and weedy rice control with oxyfluorfen is achieved with timely applications; however, injury to oxyfluorfen-resistant rice is likely.
n-3 fatty acid consumption during pregnancy is recommended for optimal pregnancy outcomes and offspring health. We examined characteristics associated with self-reported fish or n-3 supplement intake.
Design:
Pooled pregnancy cohort studies.
Setting:
Cohorts participating in the Environmental influences on Child Health Outcomes (ECHO) consortium with births from 1999 to 2020.
Participants:
A total of 10 800 pregnant women in twenty-three cohorts with food frequency data on fish consumption; 12 646 from thirty-five cohorts with information on supplement use.
Results:
Overall, 24·6 % reported consuming fish never or less than once per month, 40·1 % less than once a week, 22·1 % 1–2 times per week and 13·2 % more than twice per week. The relative risk (RR) of ever (v. never) consuming fish was higher in participants who were older (1·14, 95 % CI 1·10, 1·18 for 35–40 v. <29 years), were other than non-Hispanic White (1·13, 95 % CI 1·08, 1·18 for non-Hispanic Black; 1·05, 95 % CI 1·01, 1·10 for non-Hispanic Asian; 1·06, 95 % CI 1·02, 1·10 for Hispanic) or used tobacco (1·04, 95 % CI 1·01, 1·08). The RR was lower in those with overweight v. healthy weight (0·97, 95 % CI 0·95, 1·0). Only 16·2 % reported n-3 supplement use, which was more common among individuals with a higher age and education, a lower BMI, and fish consumption (RR 1·5, 95 % CI 1·23, 1·82 for twice-weekly v. never).
Conclusions:
One-quarter of participants in this large nationwide dataset rarely or never consumed fish during pregnancy, and n-3 supplement use was uncommon, even among those who did not consume fish.
This study describes the local Emergency Medical Services (EMS) response and patient encounters corresponding to the civil unrest occurring over a four-day period in Spring 2020 in Indianapolis, Indiana (USA).
Methods:
This study describes the non-conventional EMS response to civil unrest. The study included patients encountered by EMS in the area of the civil unrest occurring in Indianapolis, Indiana from May 29 through June 1, 2020. The area of civil unrest defined by Indianapolis Metropolitan Police Department covered 15 blocks by 12 blocks (roughly 4.0 square miles) and included central Indianapolis. The study analyzed records and collected demographics, scene times, interventions, dispositions, EMS clinician narratives, transport destinations, and hospital course with outcomes from receiving hospitals for patients extracted from the area of civil unrest by EMS.
Results:
Twenty-nine patients were included with ages ranging from two to sixty-eight years. In total, EMS transported 72.4% (21 of 29) of the patients, with the remainder declining transport. Ballistic injuries from gun violence accounted for 10.3% (3 of 29) of injuries. Two additional fatalities from penetrating trauma occurred among patients without EMS contact within and during the civil unrest. Conditions not involving trauma occurred in 37.9% (11 of 29). Among transported patients, 33.3% (7 of 21) were admitted to the hospital and there was one fatality.
Conclusions:
While most EMS transports did not result in hospitalization, it is important to note that the majority of EMS calls did result in a transport. There was a substantial amount of non-traumatic patient encounters. Trauma in many of the encounters was relatively severe, and the findings imply the need for rapid extraction methods from dangerous areas to facilitate timely in-hospital stabilization.
The oral cavity contains numerous microorganisms, including antimicrobial-resistant bacteria. These microorganisms can be transmitted via respiratory particles from patients to healthcare providers and vice versa during dental care. We evaluated the spread of Staphylococcus aureus during standardized dental procedures using different scaling devices and rinsing solutions.
Methods:
During systematic therapy for dental biofilm removal (guided biofilm therapy), using an airflow or ultrasound device to a model simulation head. Staphylococcus aureus suspension was injected into the mouth of the model to mimic saliva. Different suction devices (conventional saliva ejector or a prototype) and rising solutions (water or chlorhexidine) were used. To assess contamination with S. aureus, an air-sampling device was placed near the oral cavity and samples of surface areas were collected.
Results:
S. aureus was only detected by air sampling when the conventional saliva ejector with airflow was used. No growth was observed during treatments with the ultrasonic piezo instrument or the prototype suction device. Notably, a rinsing solution of chlorhexidine digluconate decreased the bacterial load compared to water. Surface contamination was rarely detected (1 of 120 samples).
Conclusions:
Although our findings indicate potential airborne bacterial transmission during routine prophylactic procedures, specific treatment options during biofilm removal appear to reduce air contamination. These options include ultrasonic piezo devices or the prototype suction device. The use of chlorhexidine reduced the CFU counts of S. aureus detected by air sampling. Surface contamination during dental procedures was a rare occurrence.
