Background: Our prior six-year review (n=2165) revealed 24% of patients undergoing posterior decompression surgeries (laminectomy or discectomy) sought emergency department (ED) care within three months post-surgery. We established an integrated Spine Assessment Clinic (SAC) to enhance patient outcomes and minimize unnecessary ED visits through pre-operative education, targeted QI interventions, and early post-operative follow-up. Methods: We reviewed 13 months of posterior decompression data (n=205) following SAC implementation. These patients received individualized, comprehensive pre-operative education and follow-up phone calls within 7 days post-surgery. ED visits within 90 days post-surgery were tracked using provincial databases and compared to our pre-SAC implementation data. Results: Out of 205 patients, 24 (11.6%) accounted for 34 ED visits within 90 days post-op, showing a significant reduction in ED visits from 24% to 11.6%, and decreased overall ED utilization from 42.1% to 16.6% (when accounting for multiple visits by the same patient). Early interventions including wound monitoring, outpatient bloodwork, and prescription adjustments for pain management, helped mitigate ED visits. Patient satisfaction surveys (n=62) indicated 92% were “highly satisfied” and 100% would recommend the SAC. Conclusions: The SAC reduced ED visits after posterior decompression surgery by over 50%, with pre-operative education, focused QI initiatives, and its individualized, proactive approach.
Hunting, habitat loss and fragmentation have caused a rapid decline in the distribution and abundance of the clouded leopard Neofelis nebulosa across its range, and in several areas, including Taiwan, the species is now extirpated. Taiwan, a former stronghold for the species, is a candidate for its reintroduction, based on increasing prey abundance and high forest coverage. Such future reintroduction efforts, however, are hampered by a lack of analysis of potential clouded leopard habitat on the island. To address this, we explore habitat suitability for the species in Taiwan. We used a multi-scale, multivariate habitat selection model based on clouded leopard presence–absence data from extensive camera-trap surveys across its current range to predict suitable habitat in Taiwan. Our findings indicate that 38% of Taiwanese territory is potentially suitable habitat for the clouded leopard, of which 46% is under protection. This demonstrates the high potential of Taiwan's habitat for clouded leopard reintroduction.
Folate and choline are methyl donor nutrients that may play a role in fetal brain development. Animal studies have reported that prenatal folate and choline supplementation are associated with better cognitive outcomes in offspring and that these nutrients may interact and affect brain development. Human studies that have investigated associations between maternal prenatal folate or choline levels and neurodevelopmental outcomes have reported contradictory findings and no human studies have examined the potential interactive effect of folate and choline on children’s neurodevelopment. During the second trimester of pregnancy, maternal red blood cell folate was measured from blood samples and choline intake was estimated using a 24-h dietary recall in 309 women in the APrON cohort. At 3–5 years of age, their children’s neurodevelopment was assessed using the Wechsler Preschool and Primary Scales of Intelligence – Fourth EditionCND, NEPSY-II language and memory subtests, four behavioral executive function tasks, and the Movement Assessment Battery for Children – Second Edition. Adjusted regressions revealed no associations between maternal folate and choline levels during pregnancy and most of the child outcomes. On the Dimensional Change Card Sort, an executive function task, there was an interaction effect; at high levels of choline intake (i.e., 1 SD above the mean; 223.03 mg/day), higher maternal folate status was associated with decreased odds of receiving a passing score (β = −0.44; 95%CI −0.81, −0.06). In conclusion, maternal folate status and choline intake during the second trimester of pregnancy were not associated with children’s intelligence, language, memory, or motor outcomes at 3–4 years of age; however, their interaction may have an influence children’s executive functions.
Conservation scientists are increasingly recognizing the need to evaluate the effectiveness of interventions to improve human–wildlife coexistence across different contexts. Here we assessed the long-term efficacy of the Long Shields Community Guardians programme in Zimbabwe. This community-based programme seeks to protect livestock and prevent depredation by lions Panthera leo through non-lethal means, with the ultimate aim of promoting human–lion coexistence. Using a quasi-experimental approach, we measured temporal trends in livestock depredation by lions and the prevalence of retaliatory killing of lions by farmers and wildlife managers. Farmers that were part of the Long Shields programme experienced a significant reduction in livestock loss to lions, and the annual number of lions subject to retaliatory killing by farmers dropped by 41% since the start of the programme in 2013, compared to 2008–2012, before the programme was initiated. Our findings demonstrate the Long Shields programme can be a potential model for limiting livestock depredation by lions. More broadly, our study demonstrates the effectiveness of community-based interventions to engage community members, improve livestock protection and ameliorate levels of retaliatory killing, thereby reducing human–lion conflict.
Negative interactions with humans resulting from livestock predation is a major factor influencing the decline of African lion Panthera leo populations across Africa. Here we investigate lion depredation within two Maasai communities in southern Kenya where people and lions coexist in the absence of any formal protected areas. We explore the factors that increase the frequency and severity of lion attacks on pastoralists and their livestock and assess the effectiveness of livestock guarding to reduce damage. Finally, we examine in which circumstances lion depredation triggers retaliation by people. Over a period of 26 months, lions attacked livestock 29 times, resulting in 41 livestock deaths and 19 injuries. There were also two attacks on people. Lions preferred cattle over the more numerous sheep and goats. Attacks on livestock occurred mostly during the dry season and were not affected by changes in prey density or variation in pastoral settlement that brought livestock into closer proximity with lions. Livestock were guarded during 48.2% of lion attacks. Active guarding at pasture disrupted the majority of lion attacks, resulting in lower mortality rates. Passive guarding in corrals at night also disrupted attacks but did not lead to lower livestock mortality.
Schizophrenia is a disorder characterized by pervasive deficits in cognitive functioning. However, few well-powered studies have examined the degree to which cognitive performance is impaired even among individuals with schizophrenia not currently on antipsychotic medications using a wide range of cognitive and reinforcement learning measures derived from cognitive neuroscience. Such research is particularly needed in the domain of reinforcement learning, given the central role of dopamine in reinforcement learning, and the potential impact of antipsychotic medications on dopamine function.
Methods
The present study sought to fill this gap by examining healthy controls (N = 75), unmedicated (N = 48) and medicated (N = 148) individuals with schizophrenia. Participants were recruited across five sites as part of the CNTRaCS Consortium to complete tasks assessing processing speed, cognitive control, working memory, verbal learning, relational encoding and retrieval, visual integration and reinforcement learning.
Results
Individuals with schizophrenia who were not taking antipsychotic medications, as well as those taking antipsychotic medications, showed pervasive deficits across cognitive domains including reinforcement learning, processing speed, cognitive control, working memory, verbal learning and relational encoding and retrieval. Further, we found that chlorpromazine equivalency rates were significantly related to processing speed and working memory, while there were no significant relationships between anticholinergic load and performance on other tasks.
Conclusions
These findings add to a body of literature suggesting that cognitive deficits are an enduring aspect of schizophrenia, present in those off antipsychotic medications as well as those taking antipsychotic medications.
Passive acoustic monitoring is rapidly gaining recognition as a practical, affordable and robust tool for measuring gun hunting levels within protected areas, and consequently for its potential to evaluate anti-poaching patrols’ effectiveness based on outcome (i.e., change in hunting pressure) rather than effort (e.g., kilometres patrolled) or output (e.g., arrests). However, there has been no report to date of a protected area successfully using an acoustic grid to explore baseline levels of gun hunting activity, adapting its patrols in response to the evidence extracted from the acoustic data and then evaluating the effectiveness of the new patrol strategy. We report here such a case in Cameroon’s Korup National Park, where anti-poaching patrol effort was markedly increased in the 2015–2016 Christmas/New Year holiday season to curb the annual peak in gunshots recorded by a 12-sensor acoustic grid in the same period during the previous 2 years. Despite a three- to five-fold increase in patrol days, distance and area covered, the desired outcome – lower gun hunting activity – was not achieved under the new patrol scheme. The findings emphasize the need for adaptive wildlife law enforcement and how passive acoustic monitoring can help attain this goal, and they warn about the risks of using effort-based metrics of anti-poaching strategies as a surrogate for desired outcomes. We propose ways of increasing protected areas’ capacity to adopt acoustic grids as a law enforcement monitoring tool.
Evidence from previous small trials has suggested the effectiveness of early social communication interventions for autism.
Objectives
The Preschool Autism Communication Trial (PACT) investigated the efficacy of such an intervention in the largest psychosocial autism trial to date.
Aims
To provide a stringent test of a pre-school communication intervention for autism.
Methods
152 children with core autism aged 2 years - 4 years 11 months in a 3 site 2 arm single (assessor) blinded randomised controlled trial of the parent-mediated communication-focused intervention added to treatment as usual (TAU) against TAU alone. Primary outcome; severity of autism symptoms (modified social communication algorithm from Autism Diagnostic Observation Schedule-Generic, ADOS-G). Secondary outcomes; blinded measures of parent-child interaction, child language, and adaptation in school.
Results
At 13 month endpoint the treatment resulted in strong improvement in parental synchronous response to child (adjusted between-group effect size 1.22 (95% CI 0.85, 1.59) and child initiations with parent (ES 0.41 (0.08, 0.74) but small effect on autism symptomatology (ADOS-G, ES -0.24 (95% CI -0.59, 0.11) ns). Parents (not blind to allocation) reported strong treatment effects on child language and social adaptation but effects on blinded research assessed language and school adaptation were small.
Conclusions
Addition of the PACT intervention showed clear benefit in improving parent-child dyadic social communication but no substantive benefit over TAU in modifying objectively rated autism symptoms. This attenuation on generalisation from ‘proximal’ intervention effects to wider symptom change in other contexts remains a significant challenge for autism treatment and measurement methodology.
Fueled by rapid methodological and analytic advances, functional magnetic resonance imaging (fMRI) has become the dominant method to characterize the relationship between brain function, the environment, and symptoms of psychiatric illness. The widespread adoption of this in vivo imaging approach has allowed for the study of brain systems that underlie symptom expression, treatment response, and risk for illness onset. Yet a host of approaches exist for the collection and analysis of fMRI data, and researchers often struggle to select appropriate study designs and analytic methods. Here we take a critical look at how recent advances in fMRI methods can inform our understanding of brain functions in mental illness. The benefits and limitations of different experimental approaches, from task-evoked fMRI to resting-state designs are described, and how these data provide complementary perspectives on the neurobiological basis of psychiatric illness. Established and cutting-edge analytic techniques for fMRI data are covered. Finally, some of the constraints and limitations on the interpretation of fMRI analyses are reviewed, highlighting common pitfalls to avoid, including issues pertaining to assumptions of mechanistic specificity, causality, diagnostic and symptom specificity, as well as controversial inferential strategies utilized by much of the field.
Coexistence of people and large carnivores depends on a complex combination of factors that vary geographically. Both the number and range of the Asiatic lion Panthera leo leo in the Greater Gir landscape, India, has increased since the 1990s. The challenge has been managing the success of conservation, with a particular focus on the spillover population ranging extensively in human-dominated landscapes. To understand the factors conducive to lion survival in this landscape, we undertook an interview-based survey. Overall, people expressed positive, tolerant attitudes towards lions. There was a distinct contrast between people's liking for lions (76.9% of respondents) compared to leopards (27.7%) in spite of greater depredation of livestock by lions (82.6%) than by leopards (17.4%). Younger people and respondents having greater awareness regarding lions expressed positive attitudes. Although community discussions on lions had a positive effect, there was no evidence that land-holding, management interventions, personal encounters with lions, or association of lions with religion affected attitudes. Respondents who had experienced livestock depredation tended to express negative attitudes. Respondents with positive attitudes towards lions favoured non-interventionist strategies for managing lions in the village areas. We advocate consideration of varied factors influencing tolerance of wildlife in conservation planning. We emphasize that site-specific human–wildlife conflict issues such as crop-foraging by wild ungulates and variation in attitudes towards different species should also be considered. Specifically, improved livestock management, motivation of local youth and their participation in awareness campaigns could all further strengthen the prevalent positive attitudes towards lions.
Anthropogenic habitat alteration and invasive species are threatening carnivores globally. Understanding the impact of these factors is critical for creating localized, effective conservation programmes. Madagascar's Eupleridae have been described as the least studied and most threatened group of carnivores. We investigated the effects of habitat degradation and the presence of people and exotic species on the modelled occupancy of the endemic fosa Cryptoprocta ferox, conducting camera-trap surveys in two western deciduous forests, Ankarafantsika National Park and Andranomena Special Reserve. Our results indicated no clear patterns between habitat degradation and fosa occupancy but a strong negative association between cats Felis sp. and fosas. Cat occupancy was negatively associated with birds and positively associated with contiguous forest and narrow trails. In contrast, dog Canis lupus familiaris occupancy was best predicted by wide trails, degraded forest and exotic civets. Our results suggest fosas are capable of traversing degraded landscapes and, in the short term, are resilient to contiguous forest disturbance. However, high occupancy of cats and dogs in the landscape leads to resource competition through prey exploitation and interference, increasing the risk of transmission of potentially fatal diseases. Management strategies for exotic carnivores should be considered, to reduce the widespread predation of endemic species and the transmission of disease.
Electron microprobe analyses are presented of chevkinite-group minerals occurring as microphenocrysts in peralkaline rhyolites of the Greater Olkaria Volcanic Complex (Kenya) and as a groundmass phase in a peralkaline quartz trachyte lava from the Tarosero volcano (Tanzania), both in the East African Rift Valley. Their compositions conform closely to the formula: (REE, Ca, Th)4 Fe2+ (Fe2+, Al, Ti, Zr, Nb)2 Ti2 (Si4O22). Compared to published analyses of chevkinite-group minerals, the Olkaria phases are relatively enriched in Nb and the LREE; the Tarosero phase is more calcic and relatively Zr- and Nb-rich. The main substitution in the A site at Olkaria is Ca ⇌ Ce. The overall charge-balancing substitution seems to be (McDowell, 1979):
Phenocryst/glass ratios are presented for Nb, REE, Sr, Th, U and Y in two, and Ba, Zr and Hf in one, Kenyan samples. Partition coefficients are lower in the more peralkaline rock, with the exception of Sr, which is higher. The lower values are consistent with a lower degree of polymerization of more peralkaline melts. The higher Sr value may be a function of Sr partitioning into phenocryst phases coexisting with chevkinite.
Major and trace element and Sr, Nd and O isotopic data are presented for ferrobasalts, icelandites, rhyolites, mixed pumices and silicic xenoliths of the 1875 eruption of Askja. Trace element modelling and Sr and Nd data largely confirm previous major element calculations that fractional crystallization was dominant in the generation of the basalt-ferrobasalt-icelandite-rhyolite suite. Relative enrichment in Rb (and Th and U?), depletion in Cs, and low values of δ18O/16O, in the rhyolites are not explained by this mechanism alone. The silicic magmas were selectively contaminated by diffusion from partially molten granitic wall rocks, now found as xenoliths in the eruptive products, the process being particularly marked by lower δ18O and Cs/Rb ratios in the rhyolites than in the associated basalts. This is the first record of a combined fractional crystallization-selective contamination process in an Icelandic silicic complex.
The Palaeogene Eskdalemuir dyke, part of the Mull dyke swarm in the Southern Uplands of Scotland, is ~60 km long and up to 40 m thick. Its southern tip is 230 km from the inferred source on Mull. The dyke is composite, with tholeiitic basaltic margins and a vitreous central facies ranging from basaltic andesite to andesite in composition. Plagioclase and pyroxene phenocrysts and matrix crystals in the central facies show unusually large compositional ranges and complex textural relationships. Wholerock major and trace-element abundances show linear variations against MgO content, consistent with the rocks in the central facies having formed by mixing of basalt and rhyolite magmas. The rhyolite can be closely matched by rocks from the Mull centre. The mafic and silicic magmas were intruded from a compositionally zoned chamber beneath Mull, perhaps during collapse of the Centre 1 caldera. The lower-viscosity basaltic magma was emplaced before, but lubricated the lateral propagation of, the silicic magma, which mixed with the partially solidified basalt, the proportion of rhyolite increasing towards the dyke centre. The Eskdalemuir dyke represents an unusual, perhaps unique, example of a rhyolite magma being emplaced >200 km from its inferred source. The supposed correlative of the Eskdalemuir dyke north of the Southern Uplands Fault, the Dalraith-Linburn dyke, is not comagmatic with it.
Mafic lamprophyre dykes in northern England were emplaced soon after the final closure of the Iapetus Ocean at the end of the Caledonian orogeny. High Mg/(Mg+Fe) ratios and Cr and Ni contents are consistent with equilibrium with mantle peridotite. Incompatible trace element abundances suggest that the mantle sources were metasomatized prior to the melting events. Three components are recognized in the lamprophyre chemistry: (i) a depleted mantle source, taken to be that overlying the subducting lithosphere; (ii) a H2O-rich subduction zone component, related to the dehydration of the subducting oceanic crust; and (iii) a CO2-rich phase thought to result from the degassing of the mantle after ocean closure. This multi-component origin may be applicable to other lamprophyres of calc-alkaline affinity.
This sensitivity study applies the offline Canadian Land Surface Scheme (CLASS) version 3.6 to simulate snowpack evolution in idealized topography using observations at Likely, British Columbia, Canada over 1 July 2008 to 30 June 2009. A strategy for a subgrid-scale snow (SSS) parameterization is developed to incorporate two key features: ten elevation bands at 100 m intervals to capture air temperature lapse rates, and five slope angles on four aspects to resolve solar radiation impacts on the evolution of snow depth and SWE. Simulations reveal strong elevational dependencies of snow depth and SWE when adjusting temperatures using a moist adiabatic lapse rate with elevation, with 26% peak SWE differences between that at the average elevation versus the mean of the remainder of the elevation bands. Differences in peak SWE on north- and south-facing slopes increase from 3.0 mm at 10° slope to 17.9 mm at 50° slope. When applied to elevation, slope and aspect combinations derived from a high-resolution digital elevation model, elevation dominates the control of peak SWE values. Inclusion of the range of SSS effects into a regional climate model will improve snowpack and hydrological simulations of western North America's snow-dominated, mountainous watersheds.
Extensive areas of tropical forests have been, and continue to be, disturbed as a result of selective timber extraction. Although such anthropogenic disturbance typically results in the loss of biodiversity, many species persist, and their conservation in production landscapes could be enhanced by a greater understanding of how biodiversity responds to forest management practices. We conducted intensive camera-trap surveys of eight protected forest areas in Sabah, Malaysian Borneo, and developed estimates of Sunda clouded leopard Neofelis diardi population density from spatially explicit capture–recapture analyses of detection data to investigate how the species’ abundance varies across the landscape and in response to anthropogenic disturbance. Estimates of population density from six forest areas were 1.39–3.10 individuals per 100 km2. Our study provides the first evidence that the population density of the Sunda clouded leopard is negatively affected by hunting pressure and forest fragmentation, and that among selectively logged forests, time since logging is positively associated with abundance. We argue that these negative anthropogenic impacts could be mitigated with improved logging practices, such as reducing the access of poachers by effective gating and destruction of road access points, and by the deployment of anti-poaching patrols. By calculating a weighted mean population density estimate from estimates developed here and from the literature, and by extrapolating this value to an estimate of current available habitat, we estimate there are 754 (95% posterior interval 325–1,337) Sunda clouded leopards in Sabah.
The rate of movement of the Ross Ice Shelf has been determined at its terminal face to the south-east of Scott Base. An average rate of movement of 23 cm./day in a direction 270° True was determined for the period March 1957 to October 1958. The terminating face of the ice shelf in McMurdo Sound is only about 3 metres thick compared with a thickness of several hundred metres in the Ross Sea. Altimeter heights are used to demonstrate a thinning of the shelf which is attributed to melting from below.
Blackberry is a troublesome species across much of the southeastern United States. Control of blackberry with the pyridine herbicides is often variable among different locations. Experiments were conducted to determine whether application timing, either spring or fall, affected efficacy of the pyridine herbicides triclopyr, fluroxypyr and picloram, and metsulfuron. The pyridine herbicides provided greater control when applied in the fall. At 12 mo after treatment, fluroxypyr plus picloram and fluroxypyr plus triclopyr provided 83% control when applied in the fall and 65% when applied in the spring. Conversely, metsulfuron provided 85% control, and application timing was not significant. Although metsulfuron effectively controls blackberry, it is also highly injurious to bahiagrass. Therefore, chlorosulfuron was tested to determine whether it would provide blackberry control while not injuring bahiagrass. Blackberry control with chlorosulfuron was similar to metsulfuron. These data indicated blackberry control in bahiagrass pastures can be effectively accomplished with chlorosulfuron.
With the loss of methyl bromide (MeBr) and high prices of alternatives, tomato growers are applying lower fumigant rates or adopting a reduced system. Without the broad-spectrum control provided by the complete fumigant system, a fallow weed program might be needed to avoid an increase in pest pressure with consecutive years of application of the reduced-fumigant system. Nutsedges are among the pests of interest due to their fast reproduction by underground structures and ability to spread and quickly infest a field. Field trials were conducted between February and December of 2011 in Balm, FL, to evaluate the impacts of fallow treatments, fumigants, and halosulfuron on nutsedge control. The trial design was a split–split plot with main, sub-, and subsubplots being fallow, fumigant, and herbicide treatment, respectively. Fallow treatments were spaced evenly throughout the fallow season and consisted of sequential combinations of cultivation (C) and/or glyphosate (G) applied at 2.24 kg ae ha−1; including: C, CC, G, GG, CG, GC, GCG, and NO (nontreated control). Fumigant treatments included a reduced-fumigant system of 1,3-dichloropropene plus chloropicrin 40:60 (1,3-D + pic) at 281 kg ha−1, a complete fumigant system of dimethyl disulfide plus chloropicrin 79:21 (DMDS + pic) at 545 kg ha−1, and no fumigant (NoFum). Herbicide treatments were either two POST applications of halosulfuron at 39 g ai ha−1 (Hal) or no halosulfuron (NoHal). In general, the fallow weed treatment GCG was the most effective in reducing nutsedge shoot emergence through the plastic mulch. When the reduced-fumigant system 1,3-D + pic was combined with GCG fallow treatment and halosulfuron (GCG:1,3-D + pic:Hal), no differences were found between this combination and the complete fumigant system DMDS + pic with halosulfuron and combined with CC, G, GG, CG, GC or GCG. This study shows the importance of adding a fallow weed program and halosulfuron to either a reduced or complete fumigation system to minimize the reproduction and growth of nutsedges.
