The European shores of the Mediterranean are characterised by well-known sociocultural and economic dynamics during the Bronze and Early Iron Ages (2200–550 BC), but our understanding of the African shores is comparatively vague. Here, the authors present results from excavations at Kach Kouch, Morocco, revealing an occupation phase from 2200–2000 cal BC, followed by a stable settlement from c. 1300–600 BC characterised by wattle and daub architecture, a farming economy, distinctive cultural practices and extensive connections. Kach Kouch underscores the agency of local communities, challenging the notion of north-western Africa as terra nullius prior to Phoenician arrival.
One of the most relevant risk factors for suicide is the presence of previous attempts. The symptomatic profile of people who reattempt suicide deserves attention. Network analysis is a promising tool to study this field.
Objective
To analyze the symptomatic network of patients who have attempted suicide recently and compare networks of people with several attempts and people with just one at baseline.
Methods
1043 adult participants from the Spanish cohort “SURVIVE” were part of this study. Participants were classified into two groups: single attempt group (n = 390) and reattempt group (n = 653). Different network analyses were carried out to study the relationships between suicidal ideation, behavior, psychiatric symptoms, diagnoses, childhood trauma, and impulsivity. A general network and one for each subgroup were estimated.
Results
People with several suicide attempts at baseline scored significantly higher across all clinical scales. The symptomatic networks were equivalent in both groups of patients (p > .05). Although there were no overall differences between the networks, some nodes were more relevant according to group belonging.
Conclusions
People with a history of previous attempts have greater psychiatric symptom severity but the relationships between risk factors show the same structure when compared with the single attempt group. All risk factors deserve attention regardless of the number of attempts, but assessments can be adjusted to better monitor the occurrence of reattempts.
To summarize insights generated during the preceding four conventions of the European Access Academy (EAA) regarding the interface of patient organizations and medical societies with the evolving European Union (EU) health technology assessment (HTA) process.
Methods
In 2022 and 2023 four EAA conventions were held on the EU HTA regulation, focusing on: (i) its relevance for beating cancer; (ii) stakeholder involvement; (iii) recommended preparatory steps to ensure its successful implementation; and (iv) the role of hematology and oncology as a pacemaker for the EU HTA process. Here we summarize insights generated at the four EAA conventions about the integration of patient and clinician insights in the evolving EU HTA process, including joint scientific consultations (JSC) and joint clinical assessments (JCA).
Results
Throughout the conventions it became clear that the interface of patient associations and clinical societies with the EU HTA process is key for successful implementation of the regulation. All involved stakeholders rely on the principles of evidence-based medicine (EBM), including best internal and external evidence, patient values and expectations, and clinical experience. It was agreed that patient and clinician perspectives on the assessments are needed to balance the technical analysis of best external evidence. While patient input is rather well defined, when and how input from clinical societies is best incorporated during the process remains unclear.
Conclusions
As stipulated by the EBM triad, systematic involvement of patients and clinicians throughout both JSC and JCA is key to ensuring best outcomes for patients and society as a whole, in line with the objectives of the EU HTA regulation.
Public procurement is a fundamental aspect of public administration. Its vast size makes its oversight and control very challenging, especially in countries where resources for these activities are limited. To support decisions and operations at public procurement oversight agencies, we developed and delivered VigIA, a data-based tool with two main components: (i) machine learning models to detect inefficiencies measured as cost overruns and delivery delays, and (ii) risk indices to detect irregularities in the procurement process. These two components cover complementary aspects of the procurement process, considering both active and passive waste, and help the oversight agencies to prioritize investigations and allocate resources. We show how the models developed shed light on specific features of the contracts to be considered and how their values signal red flags. We also highlight how these values change when the analysis focuses on specific contract types or on information available for early detection. Moreover, the models and indices developed only make use of open data and target variables generated by the procurement processes themselves, making them ideal to support continuous decisions at overseeing agencies.
This study compared the efficacy and tolerability of three enteral formulas in critically ill patients with COVID-19 who were ventilated and in the prone position: (a) immunomodulatory (IMM), (b) ω3 and (c) maltodextrins (MD). Primary outcome was the percentage of patients who received both 80 % of their protein and calorie targets at 3 d after enrolment. Secondary, mechanical ventilation-free time, ICU mortality and markers of nutritional status. Tolerance of enteral nutrition was evaluated by diarrhoea and gastroparesis rate. A total of 231 patients were included, primary outcome achieved was in ω3 group (76·5 % v. 59·7 and 35·2 %, P < 0·001) v. IMM and MD groups. Mechanical ventilation-free time was longer in ω3 and MD groups: 23·11 (sd 34·2) h and 22·59 (sd 42·2) h v. 7·9 (sd 22·6) h (P < 0·01) in IMM group. Prealbumin final was 0·203 ± 0·108 g/L and 0·203 ± 0·095 g/L in IMM and ω3 groups v 0·164 ± 0·070 g/L (p < 0·01) MD group. Transferrin were 1·515 ± 0·536 g/L and 1·521 ± 0·500 g/L in IMM and ω3 groups v 1·337 ± 0·483 g/L (p < 0·05) MD group. Increase of lymphocytes was greater in ω3 group: 1056·7 (sd 660·8) cells/mm3v. 853·3 (sd 435·9) cells/mm3 and 942·7 (sd 675·4) cells/mm3 (P < 0·001) in IMM and MD groups. Diarrhoea and gastroparesis occurred in 5·1 and 3·4 %, respectively. The findings of this study indicate that enteral nutrition is a safe and well-tolerated intervention. The ω3 formula compared with IMM and MD did improve protein and calorie targets.
Suicidal behavior constitutes a multi-cause phenomenon that may also be present in people without a mental disorder. This study aims to analyze suicidal behavior outcomes in a sample of attempters, from a symptom-based approach.
Methods
The sample comprised 673 patients (72% female; M = 40.9 years) who attended a hospital emergency department due to a suicide attempt. A wide range of clinical factors (e.g., psychopathology symptoms, psychiatric diagnoses, impulsivity, acquired capability), was administered within 15 days after the index attempt. Nine psychopathology domains were explored to identify the profile of symptoms, using latent profile analysis. The relationship between the profile membership and suicide outcome (i.e., intensity of suicidal ideation, number of suicide behaviors, and medical injury derived from index attempt) was also studied, using linear and logistic regression.
Results
Three psychopathology profiles were identified: high-symptom profile (45.02% of participants), moderate-symptom profile (42.50%), and low-symptom profile (12.48%). High-symptom profile members were more likely to show higher risk of non-suicidal self-injury, acquired capability for suicide, and more severe suicide behavior and ideation. On the other hand, a more severe physical injury was associated with low-symptom profile membership in comparison to membership from the other profiles (OR < 0.45, p < .05).
Conclusions
A symptom-based approach may be useful to monitor patients and determine the risk of attempt repetition in the future and potential medical injury, and to optimize prevention and intervention strategies.
This study aims to develop a framework for establishing priorities in the regional health service of Murcia, Spain, to facilitate the creation of a comprehensive multiple criteria decision analysis (MCDA) framework. This framework will aid in decision-making processes related to the assessment, reimbursement, and utilization of high-impact health technologies.
Method
Based on the results of a review of existing frameworks for MCDA of health technologies, a set of criteria was proposed to be used in the context of evaluating high-impact health technologies. Key stakeholders within regional healthcare services, including clinical leaders and management personnel, participated in a focus group (n = 11) to discuss the proposed criteria and select the final fifteen. To elicit the weights of the criteria, two surveys were administered, one to a small sample of healthcare professionals (n = 35) and another to a larger representative sample of the general population (n = 494).
Results
The responses obtained from health professionals in the weighting procedure exhibited greater consistency compared to those provided by the general public. The criteria more highly weighted were “Need for intervention” and “Intervention outcomes.” The weights finally assigned to each item in the multicriteria framework were derived as the equal-weighted sum of the mean weights from the two samples.
Conclusions
A multi-attribute function capable of generating a composite measure (multicriteria) to assess the value of high-impact health interventions has been developed. Furthermore, it is recommended to pilot this procedure in a specific decision context to evaluate the efficacy, feasibility, usefulness, and reliability of the proposed tool.
We report the first high-repetition-rate generation and simultaneous characterization of nanosecond-scale return currents of kA-magnitude issued by the polarization of a target irradiated with a PW-class high-repetition-rate titanium:sapphire laser system at relativistic intensities. We present experimental results obtained with the VEGA-3 laser at intensities from $5\times {10}^{18}$ to $1.3\times {10}^{20}$ W cm${}^{-2}$. A non-invasive inductive return-current monitor is adopted to measure the derivative of return currents of the order of kA ns${}^{-1}$ and analysis methodology is developed to derive return currents. We compare the current for copper, aluminium and Kapton targets at different laser energies. The data show the stable production of current peaks and clear prospects for the tailoring of the pulse shape, which is promising for future applications in high-energy-density science, for example, electromagnetic interference stress tests, high-voltage pulse response measurements and charged particle beam lensing. We compare the target discharge of the order of hundreds of nC with theoretical predictions and a good agreement is found.
Functional response and mutual interference are important attributes of natural enemies that should be analysed in species with the potential to be used as biological control agents in order to increase the predictive power of the possible benefits and/or consequences of their release in the field. Our main objective was to determine the functional response and mutual interference of Coptera haywardi (Oglobin), a pupal parasitoid of economically important fruit flies (Diptera: Tephritidae). The functional response of C. haywardi on A. ludens pupae corresponded to a type II model, with an attack rate of 0.0134 host pupa/h and a handling time of 1.843 h, which reveals a meticulous selection process of pupal hosts. The effect of mutual interference among foraging females was negatively correlated with increased parasitoid density in the experimental arena, showing a gradual decline in attack rate per individual female. The increase in the number of foraging females also had an impact on the number of oviposition scars per pupa and the number of immature parasitoids per dissected pupa, but not on the percentage of adult emergence or the sex ratio. Our results suggest that C. haywardi could act as a complementary parasitoid in the control of fruit fly pupae, since the random distribution of these pupae in the soil would decrease the possibility of aggregation and mutual interference between foraging females.
This study examined the incorporation of benzalkonium chloride into palygorskite and montmorillonite, assessing their potential as drug carriers. The aim was to evaluate the use of both clay minerals as viable options for antibacterial drug delivery. Various amounts (0.5, 1.0 and 2.0 times the cation-exchange capacity) of benzalkonium chloride were incorporated into both clay minerals, and the resulting materials were characterized using Fourier-transform infrared spectroscopy, thermogravimetric analysis, X-ray diffraction and elemental analysis using both CHNS-O elemental analysis and energy-dispersive X-ray spectroscopy. The Fourier-transform infrared spectroscopy and elemental analysis results indicate that benzalkonium chloride was incorporated successfully into the clay minerals. The X-ray diffraction traces of organo-montmorillonite indicate that the d-value increased as benzalkonium chloride content increased, confirming the intercalation of benzalkonium chloride within the montmorillonite interlayer space. By contrast, this behaviour was not observed for palygorskite. For the benzalkonium chloride-release studies, an initial burst release was found within the first 5 h, followed by a sustained release of benzalkonium chloride during the remaining testing time (24 h). Drug-release profiles were similar for modified palygorskite or montmorillonite during the testing time (24 h). Both clay minerals modified with benzalkonium chloride are promising materials for use as antibacterial fillers for several applications, including in the dental care industry.
The use of prostaglandin E1 is well documented in ductus arteriosus-dependent CHD or in neonatal pulmonary pathologies that cause severe pulmonary hypertension. The intravenous infusion is well established in loading infusion and maintenance with an onset of action of 30 minutes until 2 hours or even more. Our aim is to report three patients with pulmonary atresia that presented hypercyanotic spell due to a ductal spasm during cardiac catheterisation in whom the administration of a bolus of alprostadil reversed the spasm and increased pulmonary flow, immediately stabilising the condition of the patients allowing subsequent successful stent placement with no serious complications or sequelae after the administration of the bolus. More studies are needed to make a recommendation regarding the use of alprostadil in bolus in cases where the ductal spasm might jeopardise the life of the patient.
This paper tests whether logically equivalent risk formats can lead to different health state utilities elicited by means of the traditional standard gamble (SG) method and a modified version of the method that we call “double lottery.” We compare utilities for health states elicited when probabilities are framed in terms of frequencies with respect to 100 people in the population (i.e., X out of 100 who follow a medical treatment will die) with SG utilities elicited for frequencies with respect to 1,000 people in the population (i.e., Y out of 1,000 who follow a medical treatment will die). We found that people accepted a lower risk of death when success and failure probabilities were framed as frequencies type “Y deaths out of 1,000” rather than as frequencies type “X deaths out of 100” and hence the utilities for health outcomes were higher when the denominator was 1000 than when it was 100. This framing effect, known as Ratio Bias, may have important consequences in resource allocation decisions.
Poppy (also common poppy or corn poppy; Papaver rhoeas L., PAPRH) is one of the most harmful weeds in winter cereals. Knowing the precise and accurate location of weeds is essential for developing effective site-specific weed management (SSWM) for optimized herbicide use. Among the available tools for weed mapping, deep learning (DL) is used for its accuracy and ability to work in complex scenarios. Crops represent intricate situations for weed detection, as crop residues, occlusion of weeds, or spectral similarities between crop and weed seedlings are frequent. Timely discrimination of weeds is needed, because postemergence herbicides are used just when weeds and crops are at an early growth stage. This study addressed P. rhoeas early detection in wheat (Triticum spp.) by comparing the performance of six DL-based object-detection models focused on the “You Only Look Once” (YOLO) architecture (v3 to v5) using proximal RGB images to train the models. The models were assessed using open-source software, and evaluation offered a range of results for quality of recognition of P. rhoeas as well as computational capacity during the inference process. Of all the models, YOLOv5s performed best in the testing phase (75.3%, 76.2%, and 77% for F1-score, mean average precision, and accuracy, respectively). These results indicated that under real field conditions, DL-based object-detection strategies can identify P. rhoeas at an early stage, providing accurate information for developing SSWM.
In this contribution we describe the jet transport techniques that we used in Pérez-Hernández and Benet (2022) for the estimation of the Yarkovsky transversal acceleration for (99942) Apophis, which included optical and radar astrometry observations obtained during 2021 Apophis’ fly-by. Our numerical approach exploits automatic differentiation techniques which improve the orbital determination problem. We obtain a non-zero Yarkovsky parameter A2 = (−2.899±0.025) × 10−14 au d−2 which is consistent with other recent determinations of this parameter. Our results allow to constrain the collision probabilities for the close approaches in 2029, 2036 and 2068.
The extensive post-1973 literature analyzing and comparing national energy policies across countries has generally excluded Cuba, purportedly because of the unavailability of appropriate data. As a result, very little serious work has been carried out assessing Cuba's current energy balances and the efficacy of its policies in adjusting to the new global energy situation. While it is incontrovertible that available official Cuban energy data are weak, it can be argued that, when supplemented with data from other sources and with reasonable estimates, they can serve as the basis for tentative analysis of energy policies. This note attempts to lay the groundwork for such future analyses by bringing together and evaluating energy supply and consumption data covering the first two decades of revolutionary government. While the emphasis is on the period 1959–78, pre-1959 data are introduced when appropriate in an effort to put recent trends in historical perspective. The first section focuses on primary energy production and considers the contribution of commercial and noncommercial sources to domestic energy supply. In the second section, imports of primary energy products are considered and their role in total energy supply evaluated. The last section examines tentatively some aspects of Cuban energy consumption and attempts to relate consumption patterns to policies that were in effect during the period.
Two differing views of the economy of contemporary Cuba continue to coexist. One portrays it as dynamic, largely unaffected by inflation, providing full employment for its citizens, diversified, and the unquestioned leader within Latin America in real economic growth in the 1970s and 1980s. The logical conclusion to be drawn from this view is that the Cuban economic model and its implementation in the island have been a success and that other developing countries in Latin America and elsewhere would do well to emulate the Cuban paradigm.
OBJECTIVES/GOALS: The clinical management of differences of sex development (DSDs) aims to guarantee best practices in medical care while addressing concerns related to non-reversible surgeries. Rhetorical analysis was conducted to study the balance between science policy and patient advocacy related to DSD surgeries as depicted in medical education materials METHODS/STUDY POPULATION: Unrestricted transcripts of two educational videos and text from all chapters of a handbook addressed to medical learners and faculty by the Association of American Medical Colleges (AAMC) were submitted to automated word cloud analysis (NVivo, QSR International®). Words with a weighted percentage > 0.19% from total words of a given source were defined as words of frequent use and were selected for further analysis after exclusion of words as conjunctions, prepositions, pronouns, or conversational fillers. Words sharing noun, adjective and adverb forms were coded and weighed as a single word following the Oxford dictionary. Discrepancies on word selection, exclusion or coding were resolved between four raters. The rhetorical context of most frequent words was identified. RESULTS/ANTICIPATED RESULTS: The word cloud analysis of the video resource intended for medical learners (n=104 words of frequent use) and the video intended for medical faculty (n= 94 words of frequent use) depicts a patient-centered approach (word people’) that is based on expert opinion (word [I] think’). The handbook (n= 998 words of frequent use) makes reiterated reference to patients’; lgbt’; gender’; health’; and caring’ while underscoring health concerns that are unrelated to genital variance (health’; caring’ and medical’). The noun surgery’ did not figure among the most frequent words in spoken language nor in written text even when summing its adjective and adverb forms. DISCUSSION/SIGNIFICANCE: Educational materials by the AAMC on DSDs accentuate patient-centered care within a medical humanism framework. However, the lack of discussion of DSD surgeries is an educational gap that should be addressed by key science policy and patient advocacy stakeholders.
