The 2007 adoption of the United Nations Declaration on the Rights of Indigenous Peoples (UNDRIP) marked a critical juncture in the area of Indigenous rights. As a nonbinding agreement, its adoption is at the discretion of each state, resulting in significant state-level variation. Importantly, within-state variations remain underexplored. These differences are potentially significant in federal, decentralized countries such as Canada. This article examines why some provinces and territories lead in implementing the key principles embedded in UNDRIP, whereas others have dragged their feet. We collected 230 Canadian regulations introduced at the subnational level between 2007 and 2023, and assessed the impact of three key variables (i.e. political ideology, resource politics and issue voting). We found that none of these variables explained within-state variations on their own. To further explore the role of these variables, we subsequently compared two provinces at different stages of the UNDRIP implementation spectrum (Québec and British Columbia).
Antarctica was once considered biologically isolated, surrounded by oceanic barriers (Fraser et al.2018). However, floating materials such as kelp rafts (Fraser et al.2018, Avila et al.2020), wood (Lewis et al.2005) and plastics (Avila et al.2020) are now known to cross these barriers and reach Antarctic shores. Such incursions might enable non-native species (either rafting species themselves or associated hitchhikers) to colonize Antarctica as the climate warms (Avila et al.2020, Fraser et al.2020), but whether these species will be able to survive and reproduce in the Antarctic is not yet known. Sea ice is a defining characteristic of Antarctic coastlines, and modelled trajectories of kelp rafts (Fraser et al.2018) cross-referenced against sea-ice observations (Parkinson 2019) suggest that collisions between rafts and seasonal sea ice occur frequently (Fig. 1a); thus, rafts are expected to be entrained in, on or under sea ice and experience multiple freeze-and-thaw cycles on their journeys to Antarctica. In addition, kelp rafts that reach the Antarctic intertidal will experience temperatures well below 0°C if exposed to the air at low tide. Freezing can cause severe disruptive stress to seaweeds, and ice crystals growing in intercellular spaces can damage cell membranes and cause cell lysis (Eggert 2012). Such damage could affect the buoyancy of kelp tissue and decrease rafting ability. Although some non-native kelp rafts recovered from Antarctic shores appeared to still be reproductively viable, with mature gametes observed in reproductive tissue (Fraser et al.2018), tissue damage caused by freezing could have widespread effects on the health, function and establishment success of a non-native species traversing the Southern Ocean.
Background: ALS is a progressive neurodegenerative disease without a cure and limited treatment options. Edaravone, a free radical scavenger, was shown to slow disease progression in a select group of patients with ALS over 6 months; however, the effect on survival was not investigated in randomized trials. The objective of this study is to describe real-world survival effectiveness over a longer timeframe. Methods: This retrospective cohort study included patients with ALS across Canada with symptom onset up to three years. Those with a minimum 6-month edaravone exposure between 2017 and 2022 were enrolled in the interventional arm, and those without formed the control arm. The primary outcome of tracheostomy-free survival was compared between the two groups, accounting for age, sex, ALS-disease progression rate, disease duration, pulmonary vital capacity, bulbar ALS-onset, and presence of frontotemporal dementia or C9ORF72 mutation using inverse propensity treatment weights. Results: 182 patients with mean ± SD age 60±11 years were enrolled in the edaravone arm and 860 in the control arm (mean ± SD age 63±12 years). Mean ± SD time from onset to edaravone initiation was 18±10 months. Tracheostomy-free survival will be calculated. Conclusions: This study will provide evidence for edaravone effectiveness on tracheostomy-free survival in patients with ALS.
This systematic literature review aimed to provide an overview of the characteristics and methods used in studies applying the disability-adjusted life years (DALY) concept for infectious diseases within European Union (EU)/European Economic Area (EEA)/European Free Trade Association (EFTA) countries and the United Kingdom. Electronic databases and grey literature were searched for articles reporting the assessment of DALY and its components. We considered studies in which researchers performed DALY calculations using primary epidemiological data input sources. We screened 3053 studies of which 2948 were excluded and 105 studies met our inclusion criteria. Of these studies, 22 were multi-country and 83 were single-country studies, of which 46 were from the Netherlands. Food- and water-borne diseases were the most frequently studied infectious diseases. Between 2015 and 2022, the number of burden of infectious disease studies was 1.6 times higher compared to that published between 2000 and 2014. Almost all studies (97%) estimated DALYs based on the incidence- and pathogen-based approach and without social weighting functions; however, there was less methodological consensus with regards to the disability weights and life tables that were applied. The number of burden of infectious disease studies undertaken across Europe has increased over time. Development and use of guidelines will promote performing burden of infectious disease studies and facilitate comparability of the results.
Trichotillomania (TTM) and skin picking disorder (SPD) are common and often debilitating mental health conditions, grouped under the umbrella term of body-focused repetitive behaviors (BFRBs). Recent clinical subtyping found that there were three distinct subtypes of TTM and two of SPD. Whether these clinical subtypes map on to any unique neurobiological underpinnings, however, remains unknown.
Methods
Two hundred and fifty one adults [193 with a BFRB (85.5% [n = 165] female) and 58 healthy controls (77.6% [n = 45] female)] were recruited from the community for a multicenter between-group comparison using structural neuroimaging. Differences in whole brain structure were compared across the subtypes of BFRBs, controlling for age, sex, scanning site, and intracranial volume.
Results
When the subtypes of TTM were compared, low awareness hair pullers demonstrated increased cortical volume in the lateral occipital lobe relative to controls and sensory sensitive pullers. In addition, impulsive/perfectionist hair pullers showed relative decreased volume near the lingual gyrus of the inferior occipital–parietal lobe compared with controls.
Conclusions
These data indicate that the anatomical substrates of particular forms of BFRBs are dissociable, which may have implications for understanding clinical presentations and treatment response.
Dietary studies can offer insight into the effects of imperial rule on colonised populations. Inka expansion was associated with change in agricultural production and diet, including greater emphasis on maize. This article presents stable isotope analyses of ten individuals from two locations in Antofagasta de la Sierra, Argentina. AMS dating assigns one site to the start of the Inka period and one to the end. Despite diachronic changes in material culture, isotope analyses indicate that maize remained relatively unimportant in local diet. Given the symbolic value of maize in the Inka world, this lack of dietary change suggests limited imperial influence over local agricultural production and diet.
An examination of invasive procedure cancellations found that the lack of pre-procedural oral screening was a preventable cause, for children with congenital heart disease. The purpose of this study was to implement an oral screening tool within the paediatric cardiology clinic, with referral to paediatric dental providers for positive screens. The target population were children aged ≥6 months to <18 years old, being referred for cardiac procedures.
Methods:
The quality implementation framework method was used for this study design. The multi-modal intervention included education, audit and feedback, screening guidelines, environmental support, and interdisciplinary collaboration. Baseline rates for oral screenings were determined by retrospective chart audit from January 2018 to January 2019 (n = 211). Provider adherence to the oral screening tool was the outcome measure. Positive oral screens, resulting in referral to the paediatric dental clinic, were measured as a secondary outcome. Provider adherence rates were used as a process measure.
Results:
Data collected over 14 weeks showed a 29% increase in documentation of oral screenings prior to referral, as compared to the retrospective chart audit. During the study period, 13% of completed screenings were positive (n = 5). Provider compliance for the period was averaged at 70% adherence.
Conclusion:
A substantial increase in pre-procedural oral screenings by paediatric cardiologists was achieved using the quality implementation framework and targeted interventions.
Introduction: Emergency Department (ED) crowding is an international health system issue that is worsening. Further, ED crowding and “hallway medicine” has been identified as one of the most significant healthcare challenges currently facing Canadians. One contributor is preventable transfers from long-term care facilities (LTCFs) to Emergency Departments (EDs). In Canada, there were 63,752 LTCF patient transfers to the ED in 2014, with 24% (15,202) of them due to potentially preventable conditions. Each preventable transfer exposes patients to transport and hospital-related complications, and costs the healthcare system thousands of dollars. There have been many proposed and studied interventions aimed at alleviating the issue, but few attempts to assess and evaluate different interventions across institutions in a systematic manner. Methods: A scoping review of the literature using three electronic databases was conducted. A scoping review methodology was used due to the range of interventions and the heterogeneity in study design and outcome. Inclusion criteria included: studies on interventions designed to reduce transfers from LTCFs, studies that reported key outcomes such as number of ED transfers, and studies with a control or comparison group. Articles were screened by two independent reviewers (Cohen's k = 0.68), and study quality was assessed using the National Heart, Lung, Blood Institute quality assessment tools. Results: Findings were organized into five intervention types (telemedicine, outreach teams, interdisciplinary teams, integrated approaches, and other), and both a tabular and narrative synthesis was completed. Eleven studies had a good quality assessment rating, 13 studies had a fair rating, and two studies had a poor rating. Twenty out of the 26 studies reported statistically significant reductions in ED transfer rate, ranging from 10-70%. Interdisciplinary healthcare teams staffed within LTCFs were the most effective interventions. Conclusion: There are several promising interventions that have successfully reduced the number of preventable transfers from LTCFs to EDs, in a variety of health system settings. Further analysis of the relative resource requirements of each intervention, and practices that can enable successful implementation are needed to inform healthcare policy and administrative decision making. Widespread implementation of these interventions has the potential to considerably reduce ED crowding.
Hope is considered as an important factor in recovery from severe mental illness. So far it has been studied in patients with depression, anxiety disorders and post traumatic stress disorder, whereas empirical studies involving people with psychosis are scarce and their results are inconclusive.
Aim
We aimed at evaluating the relationship between
(i) hope and positive as well as negative psychotic symptoms and
(ii) hope and depression in people with psychosis.
Methods
In this cross-sectional study 148 patients with schizophrenia and schizo-affective disorder were interviewed by a psychologist who rated the positive and negative symptoms on the Positive and Negative Syndrome Scale (PANSS). Hope and depression were measured using the self-assessment scales Integrative Hope Scale (IHS) and the Center for Epidemiologic Studies Depression Scale (CES-D).
Results
No statistically significant correlation was found between hope and positive symptoms (r = .071, p = .414). Hope and negative symptoms, however, showed a statistically significant negative correlation (r = -.196, p = .023), as did hope and depression (r = -.255, p = .003). This latter relationship remained significant after controlling for negative symptoms in a partial correlation (r = -.216, p = .013).
Conclusion
While hope appears unrelated to positive symptoms, a significant correlation with negative symptoms and depression was found. These results emphasise the potential importance of hope as a target variable to support recovery in patients with psychosis. However, prospective studies are needed to clarify the causal relationships between hope and symptoms of psychotic disorders.
Some picritic lavas from near the base of the volcanic succession in the Lebombo region of the Karoo Province contain phenocrysts of olivine and skeletal microphenocrysts of ilmenite, the latter indicating rapid crystallization from the magma. These ilmenite grains contain up to 6% MgO. The near-liquidus crystallization and the high MgO content of the ilmenite are highly anomalous for ilmenite in tholeiitic rocks. These features reflect the atypical nature (high MgO and TiO2) of the parental magma and/or highly reduced conditions of crystallization.
This study was a randomised, double-blind, placebo-controlled cross-over trial examining the effects of β-hydroxy β-methylbutyrate free acid (HMB-FA) supplementation on muscle protein breakdown, cortisol, testosterone and resting energy expenditure (REE) during acute fasting. Conditions consisted of supplementation with 3 g/d HMB-FA or placebo during a 3-d meat-free diet followed by a 24-h fast. Urine was collected before and during the 24-h fast for analysis of 3-methylhistidine:creatinine ratio (3MH:CR). Salivary cortisol, testosterone, their ratio (T:C), and the cortisol awakening response were assessed. ANOVA was used to analyse all dependent variables, and linear mixed models were used to confirm the absence of carryover effects. Eleven participants (six females, five males) completed the study. Urinary HMB concentrations confirmed compliance with supplementation. 3MH:CR was unaffected by fasting and supplementation, but the cortisol awakening response differed between conditions. In both conditions, cortisol increased from awakening to 30 min post-awakening (P=0·01). Cortisol was reduced from 30 to 45 min post-awakening with HMB-FA (−32 %, d=−1·0, P=0·04), but not placebo (PL) (−6 %, d=−0·2, P=0·14). In males, T:C increased from 0 to 24 h of fasting with HMB-FA (+162 %, d=3·0, P=0·001), but not placebo (+13 %, d=0·4, P=0·60), due to reductions in cortisol. REE was higher at 24 h of fasting than 16 h of fasting independent of supplementation (+4·0 %, d=0·3, P=0·04). In conclusion, HMB-FA may affect cortisol responses, but not myofibrillar proteolysis, during acute 24-h fasting.
Whether monozygotic (MZ) and dizygotic (DZ) twins differ from each other in a variety of phenotypes is important for genetic twin modeling and for inferences made from twin studies in general. We analyzed whether there were differences in individual, maternal and paternal education between MZ and DZ twins in a large pooled dataset. Information was gathered on individual education for 218,362 adult twins from 27 twin cohorts (53% females; 39% MZ twins), and on maternal and paternal education for 147,315 and 143,056 twins respectively, from 28 twin cohorts (52% females; 38% MZ twins). Together, we had information on individual or parental education from 42 twin cohorts representing 19 countries. The original education classifications were transformed to education years and analyzed using linear regression models. Overall, MZ males had 0.26 (95% CI [0.21, 0.31]) years and MZ females 0.17 (95% CI [0.12, 0.21]) years longer education than DZ twins. The zygosity difference became smaller in more recent birth cohorts for both males and females. Parental education was somewhat longer for fathers of DZ twins in cohorts born in 1990–1999 (0.16 years, 95% CI [0.08, 0.25]) and 2000 or later (0.11 years, 95% CI [0.00, 0.22]), compared with fathers of MZ twins. The results show that the years of both individual and parental education are largely similar in MZ and DZ twins. We suggest that the socio-economic differences between MZ and DZ twins are so small that inferences based upon genetic modeling of twin data are not affected.
Obsessive-compulsive disorder (OCD) is associated with variable risk of suicide and prevalence of suicide attempt (SA). The present study aimed to assess the prevalence of SA and associated sociodemographic and clinical features in a large international sample of OCD patients.
Methods
A total of 425 OCD outpatients, recruited through the International College of Obsessive-Compulsive Spectrum Disorders (ICOCS) network, were assessed and categorized in groups with or without a history of SA, and their sociodemographic and clinical features compared through Pearson’s chi-squared and t tests. Logistic regression was performed to assess the impact of the collected data on the SA variable.
Results
14.6% of our sample reported at least one SA during their lifetime. Patients with an SA had significantly higher rates of comorbid psychiatric disorders (60 vs. 17%, p<0.001; particularly tic disorder), medical disorders (51 vs. 15%, p<0.001), and previous hospitalizations (62 vs. 11%, p<0.001) than patients with no history of SA. With respect to geographical differences, European and South African patients showed significantly higher rates of SA history (40 and 39%, respectively) compared to North American and Middle-Eastern individuals (13 and 8%, respectively) (χ2=11.4, p<0.001). The logistic regression did not show any statistically significant predictor of SA among selected independent variables.
Conclusions
Our international study found a history of SA prevalence of ~15% in OCD patients, with higher rates of psychiatric and medical comorbidities and previous hospitalizations in patients with a previous SA. Along with potential geographical influences, the presence of the abovementioned features should recommend additional caution in the assessment of suicide risk in OCD patients.
Trichotillomania is a relatively understudied psychiatric disorder. Even less is known about this disorder in the elderly. We describe an unusual case of an elderly woman presenting for the first time with trichotillomania at age 70 and highlight the treatment complexities we encountered.
Measurements are presented of the frequency of vortex shedding from two circular cylinders in a staggered arrangement. Seven different cylinders were used in turn as the upstream cylinder and the ratio of the diameter of the upstream cylinder to that of the downstream cylinder ranged from 0·00422 to 0·571. For a constant free-stream velocity changing the upstream cylinder changed the frequency of vortex shedding from the cylinder. It was found that the frequency of vortex shedding from the downstream cylinder N synchronised with that of the upstream cylinder n over a range of the latter frequency. When the frequencies were synchronised the ratio of the frequencies was given by n/N = 2. This behaviour is characteristic of a lock-in process.
Lock-in was found at each of the four upstream cylinder positions tried. However, it was only when the upstream cylinder position was such that the upstream cylinder only weakly affected the flow about the downstream cylinder that lock-in was accompanied by a decrease in the base pressure. The time-mean drag force then increased with lock-in, but by a small amount only. On the other hand the fluctuating lift force increased substantially.
This paper describes the results of an investigation of the flow over two cylinders in which the diameter of the upstream cylinder is half that of the downstream cylinder. Measurements were made of the surface pressure distribution around the downstream cylinder, and of the base pressures and Strouhal numbers of the two cylinders for separations between 1·32 D and 3·88 D, where D is the diameter of the downstream cylinder, and for angles of stagger between 0° and 45°.
With the cylinders in tandem two different flows were found, and it is suggested that there is a step change between the flows at a critical separation.
The behaviour of the measured quantities with a change of the angle of stagger is found, in general, to depend on whether the separation is less or greater than the critical separation. For each separation an angle of stagger is found at which the pressure distribution has a strong asymmetry over the rear of die cylinder. These pressure distributions are similar to each other, and they indicate the presence of an enhanced flow over the upper half of the cylinder. Also the lift force, for a given separation, then has its maximum value.
It is found that for separations greater than the critical separation the Strouhal numbers of the vortex shedding from the two cylinders are identical over a considerable range in the angle of stagger.
This paper describes a facility for the acquisition of simultaneous high quality particle image velocimetry (PIV) and unsteady surface pressures with high spatial and temporal resolution. The facility, developed through a long-standing collaboration between the Department of Aerospace Engineering at Glasgow University and the Fluid Loading and Instrumentation Centre at Heriot-Watt University, is currently being used in collaborative projects and has the potential to provide a level of detail previously unattainable for CFD validation.
Results from both measurement techniques will be presented to demonstrate the range and potential of the two systems. In addition, the value of combining the two techniques will be demonstrated by results from a blade-vortex interaction experiment in which both methods were applied. Finally, future applications of the combined system will be discussed.
