This study analyses the evolution of body mass index (BMI) as income increases across the population, controlling for age, sex, educational level, marital status and some lifestyle habits. To this end, a quantile regression, an econometric technique that readjusts the weights of the variables in each quantile to minimize deviations, has been carried out, where the variable that orders the sample is income. We use 316,777 observations from the European Health Interview Survey (EHIS) for these regressions. This approach allows us to separate the regression analyses for low-, middle-, and high-income groups, evidence that as individual income increases, BMI tends to rise. Consequently, individuals with higher incomes exhibit higher BMI levels. Additionally, the estimated parameters for explanatory variables increase with income, signifying that wealthier individuals not only have a greater likelihood of increased BMI but also that socio-economic factors influencing BMI – whether positively or negatively – evince a stronger impact as income levels rise.
This article analyses the self-perceived health of the population surveyed by the European Health Interview Survey (EHIS), with 316,277 observations. The main novelties of this research are: first, we use the econometric technique of quantile regression, which will allow us to distinguish the respondent’s income level. Second, the personal, social, lifestyle and macroeconomic context dimensions of the respondents are considered simultaneously as determinants of self-perceived health. In this way, we will be able to see what the determinants of health are, and whether they vary with income. Finally, it is evident that there are indeed different responses to the same stimulus depending on the level of income, especially in the elasticity of the response, seeing how the higher the income, the more or less the same stimulus influences a person. In addition, it is established that age, nationality and employment status are the most influential variables in self-perceived health.
Considered the epidemic of the 21st century by the WHO, obesity is a global problem that is on the rise and will continue to increase in the coming years. Spain and Andalusia, in particular, are no exception to this pathology, which has tripled since the 1970s, representing a public health challenge. The aim of this study is to analyse the socioeconomic determinants of this pathology, with special emphasis on answering the question of what has a greater influence on overweight, education level, or income. For this purpose, we have used the European Survey of Health in Spain (ESHS-2020), a microdata base, with a total of 22,072 valid individual observations (of which 2,820 belong to the Andalusian population). Results we obtain in our estimations of qualitative response models reveal that, although both income and educational attainment could be effective in the fight against overweight, the social gradient of this health problem is greater with respect to educational attainment. Additionally, there are many other variables and other factors related to the individual’s overweight (mental health, subjective state of health, oral health, among others) which are much less explored and which must be considered in health policies to combat this disease.
We use three-dimensional (3-D) fully kinetic particle-in-cell simulations to study the occurrence of magnetic reconnection in a simulation of decaying turbulence created by anisotropic counter-propagating low-frequency Alfvén waves consistent with critical-balance theory. We observe the formation of small-scale current-density structures such as current filaments and current sheets as well as the formation of magnetic flux ropes as part of the turbulent cascade. The large magnetic structures present in the simulation domain retain the initial anisotropy while the small-scale structures produced by the turbulent cascade are less anisotropic. To quantify the occurrence of reconnection in our simulation domain, we develop a new set of indicators based on intensity thresholds to identify reconnection events in which both ions and electrons are heated and accelerated in 3-D particle-in-cell simulations. According to the application of these indicators, we identify the occurrence of reconnection events in the simulation domain and analyse one of these events in detail. The event is related to the reconnection of two flux ropes, and the associated ion and electron exhausts exhibit a complex 3-D structure. We study the profiles of plasma and magnetic-field fluctuations recorded along artificial-spacecraft trajectories passing near and through the reconnection region. Our results suggest the presence of particle heating and acceleration related to small-scale reconnection events within magnetic flux ropes produced by the anisotropic Alfvénic turbulent cascade in the solar wind. These events are related to current structures of the order of a few ion inertial lengths in size.
The objective of this study was to evaluate the reduction in nematode faecal egg count (FEC) in Pelibuey lambs segregated as resistant (RES), susceptible (SUS) and intermediate (INT) to gastrointestinal nematodes. Twenty-nine weaned Pelibuey lambs, aged five months old, free of nematode infection, were used. Nine lambs were RES, six were SUS and 14 were INT lambs. The study consisted of two phases: in Phase 1 the lambs were infected experimentally with Haemonchus contortus. In Phase 2, the lambs were naturally infected by grazing. Faecal and blood samples were taken every week. The packed cell volume and total protein were quantified. The FEC value (FECmax) per lamb was recorded together with a natural reduction in FEC in the two phases. The data were analysed with a model of measures repeated over time. During Phase 1, the RES lambs showed the lowest FEC (1061 ± 1053) compared to the other groups (INT: 2385 ± 1794 eggs per gram of faeces (EPG); and SUS: 3958 ± 3037 EPG). However, in Phase 2 no significant differences (p > 0.05) were observed between the groups of lambs (RES: 275 ± 498 EPG; SUS: 504 ± 1036 EPG; and INT: 603 ± 1061 EPG). At the end of Phase 1, the FEC of RES lambs was naturally reduced by 75.5% in respect to FECmax (p < 0.05), and at the end of Phase 2 the reduction in FEC was 90% in respect to FECmax (p > 0.05); the same behaviour was observed in RES and SUS lambs. It is concluded that the artificial infection in the lambs induced a more rapid immune response in RES than SUS lambs, and all lambs developed high acquired resistance by continuous infection.
To determine the incidence of traumatic events in Ecuadorian women exposed to domestic violence and other complex social situations and their relationship with PTSD.
Methods
We applied a transversal descriptive study accomplished by the INEC (National Institute of Statistics and Census). The INEC recruited Ecuadorian women from 15 years old and more, the surveys were focused on this population. In total, 18,800 rural and urban housings were selected all over the country, 24 provinces. Date of the survey: November 16–December 15 of 2011. A, D and G were taken as references for guidelines following the criteria diagnosis of DSM V (Diagnostic and Statistical Manual of Mental Disorders) to determine a Traumatic Event.
Results
The average age of the sample was 28 years old. The standard deviation was 21, ages: 15–25 years old: 14.265 (21.6%), 25–35 years old: 9.324 (14.1%), 35–45 years old: 8.132 (12.3%), 45–55 years old: 6.283 (9.5%), 55–65 years old: 4.302 (6.5%), > 65 years old: 23.745 (35.9%). Prevalence of the traumatic event (DSM-V) 4.6%. Women experienced any kind of violence 60.6%: 61.4% urban, 58.% rural. Types of abuse: psychological: 53.9%, physical: 38.0; sexual, patrimonial: 35.3%. Domestic violence 76.0% y other types of violence 24.0%.
Conclusion
Domestic violence rate is high, also, in this study, we determined that women face an important index of violence during their daily activities. Psychological abuse is the highest abuse, higher in the urban areas. These results based on acute traumatic events may predispose women to develop PTSD. The prevalence of traumatic events must be an alert to the Mental Health Organizations, not only in Ecuador but also in Latin American.

The Bay of Málaga is located in a high biodiversity and productivity area that harbours a wide variety of commercial species exploited by different fishing fleets. Benthic and demersal fauna from circalittoral soft bottoms have been studied using a benthic dredge (BD) (8 sampling stations) and an otter trawl (OT) (8 sampling stations on a seasonal basis). Some sediment and water column variables, as well as the trawling activity, have also been studied and used for analysing their linkage with the fauna. A total of 287 species have been found in these bottoms and fish, molluscs and crustaceans represented the most diverse and abundant faunistic groups. A new record of the decapod Hippolyte leptometrae for Spanish waters is also included in this study. Some multivariate analyses using BD samples indicated the presence of three assemblages, but these seem to represent different facies of a single benthic community due to the absence of acute sediment changes and significant differences in the fauna. OT samples only displayed differences related to seasons but not to sediment types or depth. These seasonal differences seem to be linked to biological and ecological features of both dominant and/or commercial species. Mud and organic matter contents (%OM) in sediment, as well as the temperature, were the main variables linked to the spatial distribution of the benthic community identified with BD, whereas medium and coarse sand as well as gravel contents were the main variables linked to the changes of the epibenthic and demersal assemblage resulting from OT samples. The information of this study is of importance for improving the knowledge on the biodiversity of circalittoral soft bottoms of the Mediterranean and Alboran Sea as well as for the potential creation of a Marine Fisheries Reserve in the Bay of Málaga.
How do parties protect themselves from electoral manipulation? To answer this question, we study the drivers of polling station party representatives’ presence and their impact on electoral outcomes in an environment where electoral irregularities are common. Using election data from the Mexican Chamber of Deputies, we find a robust positive correlation between the presence of party representatives and that party’s vote share. The evidence suggests that this correlation can be attributed to party representatives influencing the electoral results. We also formulate a game theoretic model of the levels of representation chosen by parties in a given precinct and structurally estimate its parameters. We find that parties send their representatives where they expect their opponents to send their own. The finding suggests representatives play a primarily protective role, even when they are often involved in irregularities themselves.
Mud volcanoes are singular seafloor structures classified as ‘sensitive habitats’. Here we report on the sponge fauna from a field of eight mud volcanoes located in the Spanish margin of the northern Gulf of Cadiz (North-eastern Atlantic), at depths ranging from 380 to 1146 m. Thirty-eight beam-trawl samplings were conducted (covering over 61,000 m2) from 2010 to 2012, in the frame of a EC-LIFE + INDEMARES grant. A total of 1659 specimens were retrieved, belonging to 82 species, from which 79 were in the Class Demospongiae and three in Hexactinellida. Two species were new to science (Jaspis sinuoxea sp. nov.; Myrmekioderma indemaresi sp. nov.) and three others recorded for the first time in the Atlantic Ocean (Geodia anceps, Coelosphaera cryosi and Petrosia raphida). Five additional species were ‘Atlantic oddities’, since this study provides their second record in the Atlantic Ocean (Lanuginella cf. pupa, Geodia cf. spherastrella, Cladocroce spathiformis, Cladocroce fibrosa and Haliclona pedunculata). Basic numerical analyses indicated a significant linear relationship between the species richness per m2 and the number of sponge individuals per m2, meaning that in most volcanoes many species occur in equivalent, moderate abundance. Likewise, sponge species richness increased with depth, while the abundance of hard substrata resulting from carbonate precipitation and the fishing activities around the volcanoes had no detectable effect on the sponge fauna. However, in the latter case, a negative trend – lacking statistical support – underlaid the analyses, suggesting that a more extensive sampling would be necessary to derive more definitive conclusions in this regard.
Early, conforming antibiotic treatment in elderly patients hospitalised for community-acquired pneumonia (CAP) is a key factor in the prognosis and mortality. The objective was to examine whether empirical antibiotic treatment was conforming according to the Spanish Society of Pulmonology and Thoracic Surgery guidelines in these patients. Multicentre study in patients aged ⩾65 years hospitalised due to CAP in the 2013–14 and 2014–15 influenza seasons. We collected socio-demographic information, comorbidities, influenza/pneumococcal vaccination history and antibiotics administered using a questionnaire and medical records. Bivariate analyses and multilevel logistic regression were made. In total, 1857 hospitalised patients were included, 82 of whom required intensive care unit (ICU) admission. Treatment was conforming in 51.4% (95% confidence interval (CI) 49.1–53.8%) of patients without ICU admission and was associated with absence of renal failure without haemodialysis (odds ratio (OR) 1.49, 95% CI 1.15–1.95) and no cognitive dysfunction (OR 1.71, 95% CI 1.25–2.35), when the effect of the autonomous community was controlled for. In patients with ICU admission, treatment was conforming in 45.1% (95% CI 34.1–56.1%) of patients and was associated with the hospital visits in the last year (<3 vs. ⩾3, OR 2.70, 95% CI 1.03–7.12) and there was some evidence that this was associated with season. Although the reference guidelines are national, wide variability between autonomous communities was found. In patients hospitalised due to CAP, health services should guarantee the administration of antibiotics in a consensual manner that is conforming according to clinical practice guidelines.
Both maternal Fe deficiency (ID) and being overweight or obese (Ow/Ob, BMI≥25 kg/m2) may negatively affect offspring brain development. However, the two risk factors correlate and their independent effects on infant neurodevelopment are unclear. PREOBE is a prospective observational study that included 331 pregnant Spanish women, of whom 166 had pre-gestational Ow/Ob. Fe status was analysed at 34 weeks and at delivery, and babies were assessed using Bayley III scales of neurodevelopment at 18 months. In confounder-adjusted analyses, maternal ID at 34 weeks was associated with lower composite motor scores at 18 months (mean 113·3 (sd 9·9) v. 117·1 (sd 9·2), P=0·039). Further, the offspring of mothers with ID at delivery had lower cognitive scores (114·0 (sd 9·7) v. 121·5 (sd 10·9), P=0·039) and lower receptive, expressive and composite (99·5 (sd 8·6) v. 107·6 (sd 8·3), P=0·004) language scores. The negative associations between maternal ID at delivery and Bayley scores remained even when adjusting for maternal Ow/Ob and gestational diabetes. Similarly, maternal Ow/Ob correlated with lower gross motor scores in the offspring (12·3 (sd 2·0) v. 13·0 (sd 2·1), P=0·037), a correlation that remained when adjusting for maternal ID. In conclusion, maternal ID and pre-gestational Ow/Ob are both negatively associated with Bayley scores at 18 months, but independently and on different subscales. These results should be taken into account when considering Fe supplementation for pregnant women.
In the present investigation, nanostructured ceramic HfN coatings were deposited onto silicon (100) wafer by magnetron sputtering DC method, from a metallic Hf target. The deposition process followed by a similar pattern as the multilayer film deposition, using cycles with the nitrogen gas turned on for 90 s and turned off for 15 s; four sets of samples were obtained using 5, 10, 15 and 20 cycles. The X ray diffraction (XRD) identified the presence of two different cubic crystalline phases of HfN, corroborated by Rietveld analysis. The Vickers hardness test showed that the hardness values increases with more cycles, due to a higher compressive stress evaluated by Stoney formula. All samples were investigated with no visible fracture until 10 grf for the 5 cycles sample; however, no fractures were visible at all for the 15 and 20 cycle samples for that given load, instead fractures started to appear at 25grf for the 10 and 15 cycles coating. Eventually it is distinguished that, the thickness and morphology of the coatings were measured by field emission scanning electron microscopy FE-SEM. As well as, the thickness increased from 0.4 µm to almost 1.33 µm as the number of cycles also increased, where we can observe the formation of columnar growth, moreover it is possible to distinguish the formation of two different clusters which might be related to different phases.
The composition and structure of sublittoral faunal assemblages inhabiting soft bottoms (15–72 m depth) within the Marine Protected Area ‘Acantilados y Fondos Marinos de Calahonda-Castell de Ferro’ in southern Spain (North Alboran Sea, Mediterranean) have been studied in relation to sediment and water column variables. Three assemblages were identified and corresponded to mixed bottom, unstable bottom and coastal detritic bottom assemblages, based on Pérès & Picard's (1964) benthic classification. A total of 14,318 individuals were collected and 218 species identified, molluscs being the best represented group (141 species). Species richness displayed significant differences with depth and transect, with the highest values observed in the medium to very fine sand and muddy bottoms with bioclasts located at the shallowest sampling stations. The presence of some rare and poorly known invertebrates that are scarce in other areas of the Mediterranean Sea is remarkable, such as the crustacean decapods Bythocaris cosmetops and Pagurus mbizi, Atlantic species with no records in the Mediterranean Sea, and the bathyal molluscs Poromya granulata and Alvania testae, collected at shallow depths. The spatial distribution of faunal assemblages was mainly related to depth and percentage of gravel and clay according to the canonical correspondence analysis. The geographic location of the area, the heterogeneity of soft bottoms and the presence of upwellings in the area may favour the high biodiversity found in the studied soft bottoms. This study increases the scarce knowledge of the circalittoral fauna of sedimentary habitats of the Alboran Sea, providing a baseline for the management of this interesting SCI and for the EU Marine Strategy Framework Directive.
Using data collected by the Ontario HIV Treatment Network Cohort Study, the primary goal of this study was to describe the socio-demographic characteristics of a sample of older people (age 50 and over) from Ontario (n = 1,129) living with HIV/AIDS (PHAs). The secondary goal was to compare four sub-samples of older PHAs: women (10.6%), heterosexual men (16.7%), gay men (65.8%), and bisexual men (6.9%). These groups differed significantly by age, education, income, nationality, race, and time spent living with HIV. Compared to other groups, gay and bisexual men (GBM) reported lower HIV stigma, disclosure stigma, negative self-image stigma, and concern with public attitude stigma. GBM also reported higher mental health quality of life, lower depression scores, and lower maladaptive coping scores. These findings suggest that services for older PHAs, including training for HIV and gerontological providers, may need to cater to the specific needs of these sub-populations.
To compare BMI with abdominal skinfold thickness (ASF), waist circumference and waist-to-height ratio in the prediction of insulin resistance (IR) in prepubertal Colombian children.
Design
We calculated age- and sex-specific Z-scores for BMI, ASF, waist circumference, waist-to-height ratio and three other skinfold-thickness sites. Logistic regression with stepwise selection (P = 0·80 for entry and P = 0·05 for retention) was performed to identify predictors of IR and extreme IR, which were determined by age- and sex-specific Z-scores to identify the ≥ 90th and ≥ 95th percentile of homeostasis model assessment (HOMAIR), respectively. We used receiver operating characteristic curves to compare the area under the curve between models.
Setting
Bucaramanga, Colombia.
Subjects
Children (n 1261) aged 6–10 years in Tanner stage 1 from a population-based study.
Results
A total of 127 children (seventy girls and fifty-seven boys) were classified with IR, including sixty-three children (thirty-three girls and thirty boys) classified with extreme IR. Only ASF and BMI Z-scores were retained as predictors of IR by stepwise selection. Adding ASF Z-score to BMI Z-score improved the area under the curve from 0·794 (95 % CI 0·752, 0·837) to 0·811 (95 % CI 0·770, 0·851; P for contrast = 0·01). In predicting extreme IR, the addition of ASF Z-score to BMI Z-score improved the area under the curve from 0·837 (95 % CI 0·790, 0·884) to 0·864 (95 % CI 0·823, 0·905; P for contrast = 0·01).
Conclusions
ASF Z-score predicted IR independent of BMI Z-score in our population of prepubertal children. ASF and BMI Z-scores together improved IR risk stratification compared with BMI Z-score alone, opening new perspectives in the prediction of cardiometabolic risk in prepubertal children.
The risk of developing cardiovascular diseases is known to begin before birth and the impact of the intrauterine environment on subsequent adult health is currently being investigated from many quarters. Following our studies demonstrating the impact of hypoxia in utero and consequent intrauterine growth restriction (IUGR) on the rat cardiovascular system, we hypothesized that changes extend throughout the vasculature and alter function of the renal artery. In addition, we hypothesized that hypoxia induces renal senescence as a potential mediator of altered vascular function. We demonstrated that IUGR females had decreased responses to the adrenergic agonist phenylephrine (PE; pEC50 6.50 ± 0.05 control v. 6.17 ± 0.09 IUGR, P < 0.05) and the endothelium-dependent vasodilator methylcholine (MCh; Emax 89.8 ± 7.0% control v. 41.0 ± 6.5% IUGR, P < 0.001). In IUGR females, this was characterised by increased basal nitric oxide (NO) modulation of vasoconstriction (PE pEC50 6.17 ± 0.09 IUGR v. 6.42 ± 0.08 in the presence of the NO synthase inhibitor N-nitro-l-arginine methyl ester hydrochloride (l-NAME; P < 0.01) but decreased activated NO modulation (no change in MCh responses in the presence of l-NAME), respectively. In contrast, IUGR males had no changes in PE or MCh responses but demonstrated increased basal NO (PE pEC50 6.29 ± 0.06 IUGR v. 6.42 ± 0.12 plus l-NAME, P < 0.01) and activated NO (Emax 37.8 ± 9.4% control v. −0.8 ± 13.0% plus l-NAME, P < 0.05) modulation. No significant changes were found in gross kidney morphology, proteinuria or markers of cellular senescence in either sex. In summary, renal vascular function was altered by hypoxia in utero in a sex-dependent manner but was unlikely to be mediated by premature renal senescence.
We have previously shown that adult rat offspring born intrauterine growth restricted (IUGR) as a result of a prenatal hypoxic insult exhibit several cardiovascular characteristics that are compatible with common manifestations of chronic iron toxicity. As hypoxia is one of the major regulators of iron absorption and metabolism, we hypothesized that hypoxia-induced IUGR offspring will have long-term changes in their ability to regulate iron metabolism leading to myocardial iron deposition and induction of myocardial oxidative stress. Pregnant Sprague Dawley rats were randomized to control (n = 8) or maternal hypoxia (11.5% oxygen; n = 8) during the last 6 days of pregnancy. At birth, litters were reduced to eight pups (four male and four female). At 4 or 12 months of age, offspring were euthanatized and samples (blood and myocardium) were collected. In only the male offspring, IUGR and aging were associated with an increase in myocardial markers of oxidative stress such as oxidized/reduced glutathione ratio and malondialdehyde. Aged male IUGR offspring also exhibited interstitial myocardial remodeling characterized by myocyte loss and disrupted extracellular matrix.Contrary to our hypothesis, however, neither IUGR nor aging were associated with changes in any systemic or local markers of iron metabolism. Our results suggest that hypoxic insults leading to IUGR produce long-term effects on the levels of oxidative stress and connective tissue distribution in the myocardium of male but not female offspring.
Objectives: Immediate post-mastectomy breast reconstruction (IBR) is a procedure that has proven advantages, but it also entails risks. The aim of this study was to identify risk factors for reconstruction failure.
Methods: A review was made of all the IBR carried out at a general hospital from 2002 to 2009. Retrospective information was obtained about postoperative complications and the characteristics of patients and treatments applied. The minimum follow-up period was 9 months. Cox's regression analysis was performed on the variables related to the reconstruction failure requiring the removal of the prosthesis, with an explanatory model in which all the study variables were introduced and a predictive model that contained only the variables known before the intervention.
Results: A total of 115 IRB interventions carried out on 112 women with breast cancer were analyzed. The mean follow-up period was 25.5 months. In sixty cases (52.2 percent), there were no complications; in sixteen cases (13.9 percent) minor complications appeared, and in 39 (33.9 percent) the complications were moderate. In twenty-six cases (22.6 percent), a reconstruction failure occurred. Cox's regression model revealed that the reconstruction failures were related to the patient's age (Hazard Ratio 1.08), to neoadjuvant chemotherapy (HR 6.24) and to postoperative tamoxifen (HR 3.10). The predictive model included the age of the patient (HR 1.05) and the use of neoadjuvant chemotherapy (HR 5.11).
Conclusions: A significant proportion of the patients receiving IBR developed reconstruction failure. Multivariate analysis identified three variables related to this complication, two of which were known before the intervention.
