Viscous fingering instabilities, common in confined environments such as porous media or Hele-Shaw cells, surprisingly also occur in unconfined, non-porous settings as revealed by recent experiments. These novel instabilities involve free-surface flows of dissimilar viscosity. We demonstrate that such a free-surface flow, involving a thin film of viscous fluid spreading over a substrate that is prewetted with a fluid of higher viscosity, is susceptible to a similar type of novel viscous fingering instability. Such flows are relevant to a range of geophysical, industrial and physiological applications from the small scales of thin-film coating applications and nasal drug delivery to the large scales of lava flows. In developing a theoretical framework, we assume that the intruding layer and the liquid film over which it flows are both long and thin, the effects of inertia and surface tension are negligible, and both layers are driven by gravity and resisted by viscous shear stress so that the principles of lubrication theory hold. We investigate the stability of axisymmetric similarity solutions, describing the base flow, by examining the growth of small-amplitude non-axisymmetric perturbations. We characterise regions of instability across parameter space and find that these instabilities emerge above a critical viscosity ratio. That is, a fluid of low viscosity intruding into another fluid of sufficiently high viscosity is susceptible to instability, akin to traditional viscous fingering in a porous medium. We identify the mechanism of instability, compare with other frontal instabilities and demonstrate that high enough density differences suppress the instability completely.
The obesity prevalence among the Chinese population has increased more than three-fold over the last twenty years.(1) A cross-sectional study involving 15.8 million Chinese adults revealed that in 2023 one in every two Chinese adults was categorised as overweight or obese.(2) Chinese individuals face a higher health risk of developing chronic disease at comparable Body Mass Index levels to Caucasians.(3) Thus, it is urgent to address the increasing obesity prevalence in this population. People’ s perception towards obesity is likely to play an important role in influencing their weight management behaviour.(4) For Chinese people, the unique Confucianism ideology, collectivist society, and modern mass media information exposure may have a profound influence on how Chinese people develop their perceptions about weight issues. It is therefore valuable to understand Chinese people’s obesity-related perceptions as this may inform more culturally appropriate interventions for weight management. To the authors’ knowledge, this is the first bilingual systematic review which aimed to synthesise published literature exploring the perceptions of Chinese adults towards overweight and obesity. Four English databases and two Chinese databases (Medline, APA PsycINFO, Social Science Premium Collection, Scopus, Wanfang Chinese Journal Database, and Chinese Electronic Periodical Services) were searched from inception to 17 Jun 2023. Studies were included if they were published in English or Chinese, investigated perceptions towards overweight and obesity, and focused on Chinese adults living in or outside of Mainland China. All retrieved citations were screened by two reviewers independently. A narrative synthesis method was employed. Forty-eight studies (n = 20 in English, n = 21 in simplified Chinese, n = 7 in Traditional Chinese) were included, involving n= 73910 participants. Females were more often researched. Also, the overall sample consisted more younger and more highly educated people, with 22 studies including solely university students. The collective quality or risk of bias of the studies was suboptimal. Three overarching themes emerged from this review: (i) Chinese adults tend to misperceive their weight status, largely stemming from limited awareness of BMI and societal norms idealizing thinness, leading to high dissatisfaction with body weight and high motivation to pursue weight loss. Females perceived a stricter weight ideal than males; (ii) Despite a pronounced motivation for weight management, study participants lacked practical knowledge regarding sustainable strategies. Non-scientific weight-loss methods and unproductive perceptions were prevalent among this population; (iii) Chinese adults perceived dealing with obesity as a lonely journey, influenced by the pervasive negative societal attitude towards obesity and the prevailing mindset emphasizing personal responsibility for weight-related issues. This review provides a comprehensive profile of perceptions towards obesity among Chinese adults over the last two decades and discusses methodology implications and future research directions.
While early intervention in psychosis (EIP) programs have been increasingly implemented across the globe, many initiatives from Africa, Asia and Latin America are not widely known. The aims of the current review are (a) to describe population-based and small-scale, single-site EIP programs in Africa, Asia and Latin America, (b) to examine the variability between programs located in low-and-middle income (LMIC) and high-income countries in similar regions and (c) to outline some of the challenges and provide recommendations to overcome existing obstacles.
Methods
EIP programs in Africa, Asia and Latin America were identified through experts from the different target regions. We performed a systematic search in Medline, Embase, APA PsycInfo, Web of Science and Scopus up to February 6, 2024.
Results
Most EIP programs in these continents are small-scale, single-site programs that serve a limited section of the population. Population-based programs with widespread coverage and programs integrated into primary health care are rare. In Africa, EIP programs are virtually absent. Mainland China is one of the only LMICs that has begun to take steps toward developing a population-based EIP program. High-income Asian countries (e.g. Hong Kong and Singapore) have well-developed, comprehensive programs for individuals with early psychosis, while others with similar economies (e.g. South Korea and Japan) do not. In Latin America, Chile is the only country in the process of providing population-based EIP care.
Conclusions
Financial resources and integration in mental health care, as well as the availability of epidemiological data on psychosis, impact the implementation of EIP programs. Given the major treatment gap of early psychosis in Africa, Latin America and large parts of Asia, publicly funded, locally-led and accessible community-based EIP care provision is urgently needed.
We examine the gravity-driven flow of a thin film of viscous fluid spreading over a rigid plate that is lubricated by another viscous fluid. We model the flow over such a ‘soft’ substrate by applying the principles of lubrication theory, assuming that vertical shear provides the dominant resistance to the flow. We do so in axisymmetric and two-dimensional geometries in settings in which the flow is self-similar. Different flow regimes arise, depending on the values of four key dimensionless parameters. As the viscosity ratio varies, the behaviour of the intruding layer ranges from that of a thin coating film, which exerts negligible traction on the underlying layer, to a very viscous gravity current spreading over a low-viscosity, near-rigid layer. As the density difference between the two layers approaches zero, the nose of the intruding layer steepens, approaching a shock front in the equal-density limit. We characterise a frontal stress singularity, which forms near the nose of the intruding layer, by performing an asymptotic analysis in a small neighbourhood of the front. We find from our asymptotic analysis that unlike single-layer viscous gravity currents, which exhibit a cube-root frontal singularity, the nose of a viscous gravity current propagating over another viscous fluid instead exhibits a square-root singularity, to leading order. We also find that large differences in the densities between the two fluids give rise to flows similar to that of thin films of a single viscous fluid spreading over a rigid, yet mobile, substrate.
Cognitive impairment, a major determinant of poor functioning in schizophrenia, had limited responses to existing antipsychotic drugs. The limited efficacy could be due to regional differences in the dysregulation of the dopamine system. This study investigated striatal and peripheral dopaminergic makers in schizophrenia and their relationship with cognitive impairment.
Methods
Thirty-three patients with schizophrenia and 36 age- and sex-matched healthy controls (HC) participated. We evaluated their cognitive performance, examined the availability of striatal dopamine transporter (DAT) using single-photon emission computed tomography with 99mTc-TRODAT, and measured plasma levels of dopaminergic precursors (phenylalanine and tyrosine) and three branched-chain amino acids (BCAA) that compete with precursors for brain uptake via ultra-performance liquid chromatography.
Results
Schizophrenia patients exhibited lower cognitive performance, decreased striatal DAT availability, and reduced levels of phenylalanine, tyrosine, leucine, and isoleucine, and the ratio of phenylalanine plus tyrosine to BCAA. Within the patient group, lower DAT availability in the left caudate nucleus (CN) or putamen was positively associated with attention deficits. Meanwhile, lower tyrosine levels and the ratio of phenylalanine plus tyrosine to BCAA were positively related to executive dysfunction. Among all participants, DAT availability in the right CN or putamen was positively related to memory function, and plasma phenylalanine level was positively associated with executive function.
Conclusions
This study supports the role of dopamine system abnormalities in cognitive impairment in schizophrenia. The distinct associations between different dopaminergic alterations and specific cognitive domain impairments suggest the potential need for multifaceted treatment approaches to target these impairments.
To optimize flapping foil performance, in the current study we apply deep reinforcement learning (DRL) to plan foil non-parametric motion, as the traditional control techniques and simplified motions cannot fully model nonlinear, unsteady and high-dimensional foil–vortex interactions. Therefore, a DRL training framework is proposed based on the proximal policy optimization algorithm and the transformer architecture, where the policy is initialized from the sinusoidal expert display. We first demonstrate the effectiveness of the proposed DRL-training framework, learning the coherent foil flapping motion to generate thrust. Furthermore, by adjusting reward functions and action thresholds, DRL-optimized foil trajectories can gain significant enhancement in both thrust and efficiency compared with the sinusoidal motion. Last, through visualization of wake morphology and instantaneous pressure distributions, it is found that DRL-optimized foil can adaptively adjust the phases between motion and shedding vortices to improve hydrodynamic performance. Our results give a hint of how to solve complex fluid manipulation problems using the DRL method.
The presence of a penicillin allergy label in a patient’s medical chart is associated with negative clinical and economic outcomes. Given that less than 10% of reported reactions are truly immunoglobulin E-mediated, removal of unverified penicillin allergy labels is a public health priority and an area of ongoing implementation research. The Veterans Health Administration (VHA) is the largest integrated healthcare system in the United States, with almost 9 million veterans currently enrolled. However, studies analyzing the impact of the penicillin allergy label in this population are limited to single facilities and largely focus on short-term outcomes of allergy documentation correction, usage of β-lactams, and avoidance of antibiotic-related side effects. Broader, national VHA studies focusing on health outcomes and costs are lacking. As with non-VHA facilities, penicillin allergy evaluations are limited owing to the absence of formal allergy/immunology services at most VHA facilities. Pharmacy-driven screening and referral for clinic-based penicillin skin testing is a promising and frequently discussed modality in the literature, but its scalability within the VHA is not yet proven. Broader, evidence-based strategies that can be adapted to the available resources of individual VHA facilities, including those without on-site access to allergy providers, are needed.
Oncological and palliative care patients face unique stressors which increase their risk of developing depression and anxiety. Cognitive behavioural therapy (CBT) and mindfulness has established success in improving this population’s mental health. Traditional face-to-face psychotherapy is costly, has long wait lists, often lacks accessibility, and has strict scheduling, each of which can make attending psychotherapy physically, mentally, and financially out of reach for oncological and palliative patients. Web-based CBT (e-CBT) is a promising alternative that has shown efficacy in this and other patient populations.
Objectives
To quantify the efficacy of online CBT and mindfulness therapy in oncological and palliative patients experiencing depression and anxiety symptoms.
Methods
Participants with depression or anxiety related to their diagnosis were recruited from care settings in Kingston, Ontario, and randomly assigned to 8 weekly e-CBT/mindfulness modules (N= 25) or treatment as usual (TAU; N=24). Modules consisted of CBT concepts, problem-solving, mindfulness, homework, and personalised feedback from their therapist through a secure platform (Online Psychotherapy Tool- OPTT) Participants completed PHQ-9 and GAD-7 in weeks 1, 4, and 8. (NCT04664270: REB# 6031471).
Results
Significant decreases in PHQ-9 and GAD-7 scores within individuals support the hypothesis of efficacy. At this time, 10 e-CBT/mindfulness and 12 TAU have completed the study. Decreases in PHQ-9 and GAD-7 scores within e-CBT group support the hypothesis of efficacy. Specifically, PHQ-9 scores decreased over the 3 repeated measures (ANOVA, 2 groups, 3 repeated measures and the decrease in GAD-7 scores was similarly large)
Conclusions
As hypothesized, the results suggest that e-CBT/mindfulness therapy is an affordable, accessible, and efficacious mental health treatment for this population. The virtual, asynchronous delivery format is particularly appropriate given the unique barriers.

Vortex shedding in the wake of a cylinder in uniform flow can be suppressed via the application of a porous coating; however, the suppression mechanism is not fully understood. The internal flow field of a porous coated cylinder (PCC) can provide a deeper understanding of how the flow within the porous medium affects the wake development. A structured PCC (SPCC) was three-dimensionally printed using a transparent material and tested in water tunnel facilities using flow visualisation and tomographic particle image velocimetry at outer-diameter Reynolds numbers of $Re = 7 \times 10^{3}$ and $7.3 \times 10^{4}$, respectively. The internal and near-wall flow fields are analysed at the windward and mid-circumference regions. Flow stagnation is observed in the porous layer on the windward side and its boundary is shown to fluctuate with time in the outermost porous layer. This stagnation region generates a quasi-aerodynamic body that influences boundary layer development on the SPCC inner diameter, that separates into a shear layer within the porous medium. For the first time via experiment, spectral content within the separated shear layer reveals vortex shedding processes emanating through single pores at the outer diameter, providing strong evidence that SPCC vortex shedding originates from the inner diameter. Velocity fluctuations linked to this vortex shedding propagate through the porous layers into the external flow field at a velocity less than that of the free stream. The Strouhal number linked to this velocity accurately predicts the SPCC vortex shedding frequency.
The target backsheath field acceleration mechanism is one of the main mechanisms of laser-driven proton acceleration (LDPA) and strongly depends on the comprehensive performance of the ultrashort ultra-intense lasers used as the driving sources. The successful use of the SG-II Peta-watt (SG-II PW) laser facility for LDPA and its applications in radiographic diagnoses have been manifested by the good performance of the SG-II PW facility. Recently, the SG-II PW laser facility has undergone extensive maintenance and a comprehensive technical upgrade in terms of the seed source, laser contrast and terminal focus. LDPA experiments were performed using the maintained SG-II PW laser beam, and the highest cutoff energy of the proton beam was obviously increased. Accordingly, a double-film target structure was used, and the maximum cutoff energy of the proton beam was up to 70 MeV. These results demonstrate that the comprehensive performance of the SG-II PW laser facility was improved significantly.
The objective of this paper is to present the movement mechanisms of transport aircraft response to severe clear-air turbulence to obtain the loss of control prevention for pilot training in IATA – Loss of Control In-flight (LOC-I) program. The transport aircraft in transonic flight is subjected to severe clear-air turbulence, resulting in a sudden plunging motion with the abrupt change in flight attitude and gravitational acceleration. The comparative analyses of the flight environment and aircraft response to severe clear-air turbulence for two four-jet aircraft are studied. The one with a larger dropped-off altitude during the plunging motion will be chosen to construct the movement mechanism. The nonlinear unsteady aerodynamic model of the chosen transport is established through flight data mining and the fuzzy-logic modeling of artificial intelligence technique based on post-flight data. The crosswind before the turbulence encounter will easily induce a rolling motion and then the sudden plunging motion during the turbulence encounter. The influences of the varying vertical wind and crosswind on loss of control are presented. To formulate preventive actions, the situation awareness of varying crosswind encountering for the operational pilot will be studied further in the future. The present study is initiated to examine the possible mitigation concepts of accident prevention for the pilot training course of IATA – Loss of Control In-flight (LOC-I) program.
The great demographic pressure brings tremendous volume of beef demand. The key to solve this problem is the growth and development of Chinese cattle. In order to find molecular markers conducive to the growth and development of Chinese cattle, sequencing was used to determine the position of copy number variations (CNVs), bioinformatics analysis was used to predict the function of ZNF146 gene, real-time fluorescent quantitative polymerase chain reaction (qPCR) was used for CNV genotyping and one-way analysis of variance was used for association analysis. The results showed that there exists CNV in Chr 18: 47225201-47229600 (5.0.1 version) of ZNF146 gene through the early sequencing results in the laboratory and predicted ZNF146 gene was expressed in liver, skeletal muscle and breast cells, and was amplified or overexpressed in pancreatic cancer, which promoted the development of tumour through bioinformatics. Therefore, it is predicted that ZNF146 gene affects the proliferation of muscle cells, and then affects the growth and development of cattle. Furthermore, CNV genotyping of ZNF146 gene was three types (deletion type, normal type and duplication type) by Real-time fluorescent quantitative PCR (qPCR). The association analysis results showed that ZNF146-CNV was significantly correlated with rump length of Qinchuan cattle, hucklebone width of Jiaxian red cattle and heart girth of Yunling cattle. From the above results, ZNF146-CNV had a significant effect on growth traits, which provided an important candidate molecular marker for growth and development of Chinese cattle.
The high-altitude landscape of western Tibet is one of the most extreme environments in which humans have managed to introduce crop cultivation. To date, only sparse palaeoeconomic data have been reported from this region. The authors present archaeobotanical evidence from five sites (dating from the late first millennium BC and the early first millennium AD) located in the cold-arid landscape of western Tibet. The data indicate that barley was widely grown in this region by c. 400 BC but probably fulfilled differing roles within local ecological constraints on cultivation. Additionally, larger sites are characterised by more diverse crop assemblages than smaller sites, suggesting a role for social diversity in the development of high-altitude agriculture.
There is evidence that some red supergiants (RSGs) experience phases of episodic mass-loss. These episodes yield more extreme mass-loss rates, further stripping the envelope of the RSG, significantly affecting the further evolution towards the final collapse of the star. Mass lost through RSG outbursts/superwinds will flow outwards and form dust further out from the surface and this dust may be detected and modelled. Here, we aim to derive the surface properties and estimate the global properties of Mid-IR bright RSGs in the Magellanic Clouds. These properties will then be compared to evolutionary predictions and used for future spectral energy distribution fitting studies to measure the mass-loss rates from present circumstellar dust.
Informal carers play an essential role in the care of individuals with Parkinson’s disease (PD). This role, however, is often fraught with difficulties, including emotional, physical, and financial. Coping styles and relationship quality have been hypothesized to influence the impact of stressors. The aim of this study is to examine the relationship between carers’ coping style, relationship quality, and carer burden.
Design:
Cross-sectional.
Participants:
Thirty-nine PD patient carer dyads were included in the study.
Measurements:
Participants completed self-rated questionnaires including the Dyadic Adjustment Scale, Zarit Burden Interview, and Brief Coping Orientation to Problems Experienced Inventory.
Results:
Correlational analyses found significant and positive correlation between carer burden and all three coping styles (problem-focused, emotion-focused, and dysfunctional). There was also a moderate association between carers’ perceived relationship quality and satisfaction and carer burden. Regression analyses found that carer’s gender, severity of PD, relationship quality, emotion-focused, and dysfunctional coping styles did not predict carer burden. Conversely, problem-focused coping style predicted carer burden.
Conclusion:
The results highlight that there is no perfect way to react and care for a loved one and serves as important information for practitioners who design and implement interventions.
The epidemic of tuberculosis has posed a serious burden in Qinghai province, it is necessary to clarify the epidemiological characteristics and spatial-temporal distribution of TB for future prevention and control measures. We used descriptive epidemiological methods and spatial statistical analysis including spatial correlation and spatial-temporal analysis in this study. Furthermore, we applied an exponential smoothing model for TB epidemiological trend forecasting. Of 43 859 TB cases, the sex ratio was 1.27:1 (M:F), and the average annual TB registered incidence was 70.00/100 000 of 2009–2019. More cases were reported in March and April, and the worst TB stricken regions were the prefectures of Golog and Yushu. High TB registered incidences were seen in males, farmers and herdsmen, Tibetans, or elderly people. 7132 cases were intractable, which were recurrent, drug resistant, or co-infected with other infections. Three likely cases clusters with significant high risk were found by spatial-temporal scan on data of 2009–2019. The exponential smoothing winters' additive model was selected as the best-fitting model to forecast monthly TB cases in the future. This research indicated that TB in Qinghai is still a serious threaten to the local residents' health. Multi-departmental collaboration and funds special for TB treatments and control are still needed, and the exponential smoothing model is promising which could be applied for forecasting of TB epidemic trend in this high-altitude province.
During emergency responses, public health leaders frequently serve in incident management roles that differ from their routine job functions. Leaders’ familiarity with incident management principles and functions can influence response outcomes. Therefore, training and exercises in incident management are often required for public health leaders. To describe existing methods of incident management training and exercises in the literature, we queried 6 English language databases and found 786 relevant articles. Five themes emerged: (1) experiential learning as an established approach to foster engaging and interactive learning environments and optimize training design; (2) technology-aided decision support tools are increasingly common for crisis decision-making; (3) integration of leadership training in the education continuum is needed for developing public health response leaders; (4) equal emphasis on competency and character is needed for developing capable and adaptable leaders; and (5) consistent evaluation methodologies and metrics are needed to assess the effectiveness of educational interventions.
These findings offer important strategic and practical considerations for improving the design and delivery of educational interventions to develop public health emergency response leaders. This review and ongoing real-world events could facilitate further exploration of current practices, emerging trends, and challenges for continuous improvements in developing public health emergency response leaders.
Global health disasters are on the rise and can occur at any time with little advance warning, necessitating preparation. The authors created a comprehensive evidence-based Emergency Preparedness Training Program focused on long-term retention and sustained learner engagement.
Method:
A prospective observational study was conducted of a simulation-based mass casualty event training program designed using an outcomes-based logic model. A total of 25 frontline healthcare workers from multiple hospital sites in the New York metropolitan area participated in an 8-hour immersive workshop. Data was collected from assessments, and surveys provided to participants 3 weeks prior to the workshop, immediately following the workshop, and 3 months after completion of the workshop.
Results:
The mean percentage of total knowledge scores improved across pre-workshop, post-workshop and retention (3 months post-workshop) assessments (53.2% vs. 64.8% vs. 67.6%, P < 0.05). Average comfort scores in the core MCI competencies increased across pre-workshop, post-workshop and retention self-assessments (P < 0.01). Of the participants assessed at 3 months retention (n = 14, 56%), 50.0% (n = 7) assisted in updating their hospital’s emergency operations plan and 50.0% (n = 7) pursued further self-directed learning in disaster preparedness medicine.
Conclusions:
The use of the logic model provided a transparent framework for the design, implementation, and evaluation of a competency-based EPT program at a single academic center.
Few studies have derived data-driven dietary patterns in youth in the USA. This study examined data-driven dietary patterns and their associations with BMI measures in predominantly low-income, racial/ethnic minority US youth. Data were from baseline assessments of the four Childhood Obesity Prevention and Treatment Research (COPTR) Consortium trials: NET-Works (534 2–4-year-olds), GROW (610 3–5-year-olds), GOALS (241 7–11-year-olds) and IMPACT (360 10–13-year-olds). Weight and height were measured. Children/adult proxies completed three 24-h dietary recalls. Dietary patterns were derived for each site from twenty-four food/beverage groups using k-means cluster analysis. Multivariable linear regression models examined associations of dietary patterns with BMI and percentage of the 95th BMI percentile. Healthy (produce and whole grains) and Unhealthy (fried food, savoury snacks and desserts) patterns were found in NET-Works and GROW. GROW additionally had a dairy- and sugar-sweetened beverage-based pattern. GOALS had a similar Healthy pattern and a pattern resembling a traditional Mexican diet. Associations between dietary patterns and BMI were only observed in IMPACT. In IMPACT, youth in the Sandwich (cold cuts, refined grains, cheese and miscellaneous) compared with Mixed (whole grains and desserts) cluster had significantly higher BMI (β = 0·99 (95 % CI 0·01, 1·97)) and percentage of the 95th BMI percentile (β = 4·17 (95 % CI 0·11, 8·24)). Healthy and Unhealthy patterns were the most common dietary patterns in COPTR youth, but diets may differ according to age, race/ethnicity or geographic location. Public health messages focused on healthy dietary substitutions may help youth mimic a dietary pattern associated with lower BMI.
To investigate the molecular epidemiology of methicillin-susceptible Staphylococcus aureus (MSSA) in infants in a neonatal intensive care unit (NICU) using whole-genome sequencing.

Universal S. aureus screening was done using a single swab obtained from the anterior nares, axilla, and groin area of infants in the NICU on a weekly basis. Core genome multilocus sequence type (cgMLST) analysis was performed on MSSA isolates detected over 1 year (2018–2019).
Results:
In total, 68 MSSA-colonized infants were identified, and cgMLSTs of 67 MSSA isolates were analyzed. Overall, we identified 11 cgMLST isolate groups comprising 39 isolates (58%), with group sizes ranging from 2 to 10 isolates, and 28 isolates (42%) were unrelated to each other or any of the isolate groups. Cases of infants colonized by MSSA were scattered throughout the 1-year study period, and isolates belonging to the same cgMLST group were typically detected contemporaneously, over a few weeks or a few months. Overall, 13 infants (19.7%) developed MSSA infections: bacteremia (n = 3), wound infection (n = 5), conjunctivitis (n = 4), and cellulitis (n = 1). We detected no association between these clinically manifest infections and specific cgMLST groups.
Conclusions:
Although MSSA isolates in infants in a NICU showed high diversity, most were related to other isolates, albeit within small groups. cgMLST facilitates an understanding of the complex transmission dynamics of MSSA in NICUs, and these data can be used to inform better control strategies.
