Timely access to endovascular treatment (EVT) for ischaemic stroke patients is critical for optimal outcomes, but Canada’s size and population distribution create barriers to access. EVT is mostly available in tertiary centres located in large urban cities, and patients that arrive at intravenous thrombolysis (IVT)-only stroke centres need to be transferred for EVT.
Methods:
Geographic modelling of access to an IVT-only centre and an EVT-capable centre was conducted for Canada. Canada was divided into small grid sections. Drive times from the centre of each grid section to the closest stroke centres and the population of each grid section were obtained. The onset to paramedic arrival time and on-scene time were assumed to be 30 and 30 minutes, respectively. In the suboptimal and optimal scenarios, the door-in-door-out (DIDO) times were 150 minutes and 45 minutes, respectively. The poor access regions and population were calculated for onset to thrombolysis at 4.5 hours and to EVT-capable centre arrival for EVT within 6 and 3 hours.
Results:
The results show 99.37% of the population having access to thrombolysis within 4.5 hours. However, with a suboptimal DIDO time, 13.6% (5.2 million people) and 42.7% (16.2 million people) do not have access to EVT within 6 and 3 hours, respectively. With an efficient DIDO time, an additional 5.6% (2.1 million people) and 15.7% (6.0 million people) have access to EVT within 6 and 3 hours, respectively.
Conclusion:
There is an imperative to reduce DIDO times to an ambitious median of 45 minutes to ensure optimal access to EVT across Canada.
Chapter 7 transitions to the use of failure arguments in international law, examining how they have been used to justify expanding the authority of international bodies. It begins with an analysis of the UN’s Uniting for Peace Resolution and examines the role of failure arguments in the 1990s discourse on state failure and development cooperation. This chapter highlights the challenges of invoking arguments from failure within an international framework, where constitutional principles like separation of powers are less clearly defined. Drawing on Part I, the chapter emphasizes that failure arguments suppose a quasi-constitutional context including expectations of mutual support and cooperation. This only rarely exists in international settings, with the United Nations being one of the exceptions in this regard. The chapter also points to the need to consider political power structures when dealing with arguments from failure, noting the neo-colonial undertones of such arguments in some cases.
The influence of the People’s Republic of China (PRC) on the law and legal order of the Hong Kong Special Administrative Region might seem not to be a case of inter-Asian law because it occurs within a single jurisdiction. Yet, Beijing has employed a wide range of means to shape Hong Kong’s legal order, ranging from making or interpreting PRC law for Hong Kong, to mandating or pressing for local lawmaking in Hong Kong, to more diffuse influences on Hong Kong’s legal order and its context. Chinese influence has made Hong Kong law less liberal and democratic and more like the PRC’s. The China–Hong Kong case shows the spectrum of modes of inter-Asian legal influence, the complexity of the relationship between transplants or exports of legal models and legal influence, and the issues that lie ahead in an era of possible competition between China and the United States/the West for legal influence.
Textbooks continue to serve as essential central repositories of knowledge for postgraduate education in paediatric cardiology, despite the widespread availability of digital learning tools. Recent studies confirm that trainees still value textbooks for their structure, depth, and accessibility, while also requesting improved pedagogy and organisation. In preparing the 5th edition of Anderson’s Pediatric Cardiology, our editorial team implemented several deliberate educational innovations. These included (1) re-focusing towards our target audience, the congenital cardiac trainee and practicing congenital cardiologist; (2) adoption of a consistent lesion-based framework, termed the “red line,” linking anatomy, physiology, and clinical features; (3) expansion of concise visual elements such as tables, figures, algorithms, and flowcharts; and (4) incorporation of clear learning objectives throughout. These modifications were informed by feedback from trainees, educators, and practicing providers, balancing the desires and needs of the modern learner, and firmly grounded in principles of cognitive and educational science. This paper outlines the rationale, design, and pedagogical implications of these innovations within the broader context of modern medical education.
To address challenges in the real-world implementation of digital health for mental healthcare in Nigeria, this study conducted a process evaluation of five World Health Organization-recommended digital tools within a state-wide primary health care program in Lagos. Employing a convergent mixed-methods design across five facilities, we measured implementation fidelity through observation and platform analytics, and assessed stakeholder perceptions via validated surveys and interviews. The findings revealed a sharp divergence in success. Administrative tools that streamlined workflows, such as drug stock notification and automated client reminders, achieved high fidelity (>90% adherence). In contrast, clinical tools that altered provider–patient interactions, including a decision support app and a client helpline, demonstrated low fidelity (<66% adherence). Qualitative analysis attributed this gap to the successful tools’ seamless workflow integration versus the clinical tools’ disruption of practice and introduction of perceived professional and liability risks. The study concludes that digital health adoption is determined less by technological sophistication than by its integration into human systems. Scaling these innovations effectively requires prioritizing tools that align with existing workflows and developing a supportive policy ecosystem to address the professional concerns of frontline health workers.
In the framework of the common objective of this volume, this chapter focuses on the technological element –expressed in AI– which is usually part of the definition of remote work. This chapter discusses how AI tools shape the organization and performance of remote work, how algorithms impact remote workers rights and how trade unions and workers can harness these powerful instruments to improve working and living conditions. Three hypotheses are considered. First, that AI systems and algorithmic management generate a de facto deepening of the subordinate position of the worker. Second, that this process does not represent technological determinism but instead the impact of human and institutional elements. And finally, that technological resources usually are more present in remote work than in traditional work done at the workplace. These hypotheses and concerns are addressed in several ways: by contextualizing the issue over time, through a multi-level optic centered on the interactions of different levels of regulation, by examining practical dimensions and finally by exploring the implications for unions and worker agency.
We tend to think that we are prima facie morally entitled to determine the course of our own lives to some degree, and to make our own decisions about matters that are personal to us. Dworkin speaks of our “right to make decisions about the character of [our] lives”. Feinberg suggests that we plausibly have a personal domain over which we are “sovereign” and hence where we “alone” have the final say about “what is to happen”. And Akhlaghi defends the idea that we have a pro tanto or defeasible moral right to “autonomous self-making” – viz. a pro tanto moral right to autonomously decide to make certain “transformative choices” that will influence how our lives will go and who we will become.
A kitten’s early experiences have lasting effects on adult behaviour and welfare. Domestic cat (Felis silvestris catus) breeders play an important role in shaping these outcomes through their knowledge and understanding of early environment and socialisation. This study aimed to investigate Swedish pedigree cat breeders’ knowledge, attitudes and current socialisation practices. An online survey was distributed via social media to active Swedish breeders of pedigree cats (n = 133). The data were analysed primarily using descriptive statistics with several areas for improvement being identified. More breeders stated the importance of multiple people handling the kittens than they applied in practice. Significantly more breeders reported that it was important for kittens to interact with other companion animals than they demonstrated in practice. Gaps in knowledge were identified regarding the optimal age for socialisation, handling quantity, and the heritability of traits relating to the approach to new experiences. Breeders generally expressed positive attitudes towards socialisation and wished they could have prioritised it more. These findings have important implications for animal welfare, as insufficient early socialisation can increase the risk of fear-related behaviours and reduced overall cat welfare later in life. In conclusion, this study, although based on a convenience sample, provides an important first step towards understanding and improving kitten socialisation practices in Sweden and elsewhere.
This concluding chapter synthesizes the book’s findings and addresses broader trends such as declining trust in state institutions, governance crises, and the expanding role of constitutional law and judicial review. These trends not only elevate expectations of courts to address systemic issues but also increase calls for legal flexibility in the interest of bringing about certain outputs, including through arguments based on failure. The chapter cautions against the potential slide into authoritarianism that such flexibility could enable, emphasizing the need for careful and restrained application of these arguments in public law. It ends by highlighting the need for more North-South comparison and exchange to help address those challenges.
This chapter asks whether there is anything we might productively characterize as an Inter-Asian approach to religion–state relations. I use the example of the Essential Religious Practices (ERP) Doctrine as a window into this analysis. The ERP Doctrine offers the best-case argument for the existence of an Inter-Asian approach to religion–state relations because, after its initial articulation by the Indian Supreme Court, it has been widely influential within South and Southeast Asia. I use two of the contexts where ERP analysis has been influential – Malaysia and Sri Lanka – to show how there has indeed been significant conceptual migration within Asia with regard to religious freedom jurisprudence. The ERP Doctrine’s travels are clearly reflected in the flow of jurisprudential ideas and via robust campus-court exchanges. At the same time, differences in the theoretical networks and sociopolitical contexts within which the ERP Doctrine has traveled prevent it from constituting a homogenous and hermetically sealed Inter-Asian approach.
Accurately predicting the melting of encapsulated phase-change materials (PCMs) is essential for optimising thermal energy storage (TES) systems, especially when natural convection dominates at high-Rayleigh-number conditions. This study conducts a pore-scale study on the constrained melting of spherical PCM capsules, using a multiple-relaxation-time lattice Boltzmann method for the thermal flow, combined with an immersed boundary method for the solid–liquid interface. A novel ray-based phase identification scheme is introduced to resolve concave phase boundaries under strong convection, thereby improving the model accuracy in high-Rayleigh-number simulations. The model is validated against analytical, numerical and experimental benchmarks, showing superior capability and accuracy. For constrained PCM melting, the melting behaviour is reproduced, and effects of boundary temperature ($T_b$), initial subcooling ($\Delta T_s$) and capsule size ($l_z$) are examined with a fixed Prandtl number ($\textit{Pr}=59.76$). Higher $T_b$ accelerates melting, whereas $\Delta T_s$ has only minor effects. Reducing $l_z$ shortens the melting time due to the smaller PCM volume, but increases the dimensionless melting time by suppressing natural convection and shifting the melting process from convection- to conduction-dominated regimes. Accordingly, a critical capsule size $l_{z,c}$ is identified, below which conduction governs the melting process. A unified Rayleigh number of $Ra_c\approx 1.9\times 10^4$ is obtained for all $l_{z,c}$ under varying $T_b$, serving as a universal threshold between the two melting regimes. For predicting liquid fraction evolutions in both conduction- and convection-dominated regimes, two empirical correlations are proposed via dimensional analysis. These findings advance the understanding of constrained PCM melting and support TES system optimisation across diverse operating conditions.
Contrary to conventional wisdom, external freedom is ambiguous in Kant’s Rechtslehre. On the one hand, external freedom can refer to freedom in the external use of choice. On the other, external freedom can refer to the kind of independence encoded in our one innate right. Recognizing the ambiguity allows us not only to see the truth in the various and incompatible ways in which commentators have understood external freedom, but also to see more clearly external freedom’s relationship to autonomy (or internal freedom). In turn, better understanding the relationship between internal and external freedom sheds light on the relationship between Kant’s political philosophy and his moral philosophy, which are often (and wrongly) assumed to be discontinuous.
Microbial protein feed (MPF), produced through precision fermentation, offers a promising pathway toward a more sustainable agri-food sector by reducing reliance on conventional feed sources. This emerging cellular technology can help conserve natural resources, lower greenhouse gas emissions, and address the growing global demand for protein alternatives. Similar to other biotechnology innovations, the realization of MPF’s potential benefits dependents heavily on a supportive regulatory environment. This study examines the regulatory frameworks of the United States, the European Union, China, and Australia to assess how pre-market regulations shape the development of MPF and to identify specific legal challenges faced by manufacturers. Across these jurisdictions, there are no specific laws or regulations for MPF products. Their oversight falls under general feed and GMO-related legislation, creating regulatory gaps, ambiguity in product assessments, and procedural inefficiencies that are further exacerbated by overlapping regulatory authorities and generic labeling requirements. Such challenges not only slow the commercialization of MPF products but also impede broader efforts to develop more sustainable agri-food systems. Our analysis indicates that targeted regulatory reforms are needed to streamline pre-market approval pathways and improve risk assessment standards. By adopting dedicated regulatory approaches and strengthening pre-market consultation mechanisms with developers, these jurisdictions will be better positioned to advance environmentally responsible protein production and accelerate the transition to a more sustainable agri-food sector.
Despite initial hopes that advances in information technology would spread and deepen democracy around the world, new platforms for communicating have instead provided opportunities for the weakening of democracy. Social media, website hosting, messaging apps, and related technologies provide easy and cheap ways for micro-actors such as individuals and small groups (in addition to more traditional state and non-state actors) to wield soft power for antidemocratic purposes. This chapter probes how the malign version of soft power works by attracting targets through flows of information that seduce and trick audiences with mis- and disinformation as well as with divisive and hateful messaging. Focusing on malign soft power and how it is wielded through control of information flows (content, velocity, and access) provides a framework for assessing how cyber-enabled antidemocratic efforts take form and how new actors emerge.
It is increasingly acknowledged that the transition from animal- to plant-based proteins confers substantial benefits for both human health and environmental sustainability. This review provides a critical examination of the necessity to consider not only the degree of processing applied to plant protein-based foods, particularly those derived from legume seeds, but also the nutritional quality of the resulting products. While it is well established that plant proteins exhibit a lower environmental footprint compared with their animal-derived counterparts, their digestibility is often reduced, primarily due to the matrix composition of raw ingredients. To improve protein digestibility and reduce the antinutrient content in these raw materials, a variety of processing methods are used, encompassing thermal and biological treatments (e.g. extrusion and fermentation, respectively). However, processing methods may also introduce unintended adverse effects, including protein oxidation and formation of undesirable compounds. Therefore, this review explores the paradox of sustainable processing, where process conditions may simultaneously confer environmental advantages while compromising nutritional quality. In this context, food classification systems such as NOVA are critically examined, highlighting the absence of an evidence-based system that integrates both processing conditions and product formulation in the classification of plant-based foods. Current approaches categorizing plant-based options as ultra-processed foods without accounting for the nutritional quality of their ingredients, and their ultimate digestibility, may mislead consumers and discourage the consumption of nutritionally adequate plant-based alternatives. Finally, the review emphasizes the need for the adoption of processing technologies that address the sustainability challenge and the nutritional quality of plant protein-based foods.
