Conventional understanding and research regarding prognostic understanding too often focuses on transmission of information. However, merely overcoming barriers to patient understanding may not be sufficient. In this article the authors provide a more nuanced understanding of prognostic awareness, using oncological care as an overarching example, and discuss factors that may lead to prognostic discordance between physicians and patients. We summarize the current literature and research and present a model developed by the authors to characterize barriers to prognostic awareness. Ultimately, multiple influences on prognostic understanding may impede acceptance by patients even when adequate transfer of information takes place. Physicians should improve how they transmit prognostic information, as this information may be processed in different ways. A model of misunderstandings in awareness, ranging from patient understanding to patient belief, may be useful to guide future discussions. Future decision-making studies should consider these many variables so that interventions may be created to address all aspects of the prognostic disclosure process.
Civic engagement is increasingly relevant for healthy and active ageing and addressing social exclusion among older people. Current research focuses primarily on formal volunteering, overlooking other ways older people contribute to their families and communities. This study addresses these gaps by recognising civic engagement as multi-dimensional – including associational engagement, informal care-giving, formal volunteering, digital engagement and formal/informal political engagement – and exploring activity combinations among older individuals. Using data from the 2016 European Quality of Life Survey (33 European countries), it examines the civic engagement of 9,031 individuals aged 65+. Descriptive analysis maps their multi-dimensional civic engagement, while latent class analysis identifies distinct engagement profiles and explores which activities are combined. It also investigates the socio-structural and social capital resources associated with each profile. Findings reveal that 32 per cent of older individuals are not engaged in civic activities. Among the civically engaged, five profiles emerge, illustrating varied engagement across multiple activities. Many older people (35.8 per cent) combine several civic activities, albeit in different combinations. Informal care-giving can be found in all profiles; and for a large part of the population, it is their only civic activity, while another profile displays older Europeans engaged in several activities simultaneously. Higher levels of socio-structural resources are associated with greater diversity in civic engagement in later life. Interventions and policies therefore must consider the diverse circumstances and preferences of older people and valorise and include all forms of multi-dimensional civic engagement, including informal care-giving, in policy making.
Isolated multi-MeV $\gamma$-rays with attosecond duration, high collimation and beam angular momentum (BAM) may find many interesting applications in nuclear physics, astrophysics, etc. Here, we propose a scheme to generate such $\gamma$-rays via nonlinear Thomson scattering of a rotating relativistic electron sheet driven by a few-cycle twisted laser pulse interacting with a micro-droplet target. Our model clarifies the laser intensity threshold and carrier-envelope phase effect on the generation of the isolated electron sheet. Three-dimensional numerical simulations demonstrate the $\gamma$-ray emission with 320 attoseconds duration and peak brilliance of $9.3\times 10^{24}$ photons s${}^{-1}$ mrad${}^{-2}$ mm${}^{-2}$ per 0.1$\%$ bandwidth at 4.3 MeV. The $\gamma$-ray beam carries a large BAM of $2.8 \times 10^{16}\mathrm{\hslash}$, which arises from the efficient BAM transfer from the rotating electron sheet, subsequently leading to a unique angular distribution. This work should promote the experimental investigation of nonlinear Thomson scattering of rotating electron sheets in large laser facilities.
The Sustainable Development Goals (SDG) are at the core of the development agenda. Despite their wide adoption, it is still unclear the extent to which they can provide insights on environmental sustainability. The paper presents an assessment of the potential of the indicators used in the SDGs to track environmental sustainability. The results show that only a few SDG indicators describe the state of the environment, and those that do so, do not, generally, have science-based targets that describe whether environmental sustainability conditions are met. The latter aspect should be reinforced in framework that will replace the SDGs after 2030.
Technical summary
The Sustainable Development Goals (SDG) are at the core of the development agenda. Despite their wide adoption, it is still unclear whether they can be used to monitor environmental sustainability, if this is to be understood from a strong sustainability perspective. The paper presents an assessment of the adequacy of the indicator sets used by United Nations, Eurostat, OECD, and the Sustainable Development Solutions Network for strong sustainability monitoring. The results show that most environmental indicators do not have science-based environmental standards that reflect whether natural capital meets environmental sustainability conditions, thereby preventing their use as strong sustainability indicators. While meeting the SDGs would likely contribute to improving environmental performance, on their own they are not adequate to monitor progress toward it. Complementary scientifically grounded metrics are needed to track the underlying state of natural capital that provides non-substitutable functions. The strong sustainability dimension within the SDGs will need to be strengthened in post-2030 sustainable development monitoring framework.

Patients from religious minorities can face unique challenges reconciling their beliefs with the values that undergird Western Medical Ethics. This paper explores homologies between approaches of Orthodox Judaism and Islam to medical ethics, and how these religions’ moral codes differ from the prevailing ethos in medicine. Through analysis of religious and biomedical literature, this work examines how Jewish and Muslim religious observances affect decisions about genetic counseling, reproductive health, pediatric medicine, mental health, and end-of-life decisions. These traditions embrace a theocentric rather than an autonomy-based ethics. Central to this conception is the view that life and the body are gifts from God rather than the individual and the primacy of community norms. These insights can help clinicians provide care that aligns Muslim and Jewish patients’ health goals with their religious beliefs and cultural values. Finally, dialogue in a medical context between these faith traditions provides an opportunity for rapprochement amidst geopolitical turmoil.
Persistently rising atmospheric greenhouse gas concentrations challenge dominant Liberal hopes that science and multilateralism might deliver rational, global climate outcomes. Emerging Realist climate approaches that take geopolitics and national interests more seriously have yet to explore Morgenthau’s concern that ‘scientism’ – exaggerated faith in scientific rationality to solve political problems – would lead to disastrous underestimations of power and irrationality. Recently, Realists have mooted ‘solar geoengineering’ designs as a ‘lesser evil’ option to deliberately cool the Earth independently of emissions reductions. However, assessments of solar geoengineering prospects barely factor in Realist concerns, focusing instead on idealised scientific modelling of bio-physical effects and Liberal governance scenarios. To explore how geoengineering techno-science would be ‘translated’ into security assessments, geopolitical logics were elicited through interviews and group discussions with (mainly Arctic-oriented) national security professionals. Security experts reframe solar geoengineering in three significant ways: (a) from a climate ‘global public good’ to a source of geopolitical leverage and disruption; (b) from a risk-reduction tool to a potential source of distrust and escalation; and (c) from a knowledge-deficit problem solvable by more research, to a potential disinformation vector. This expands Realist scholarship on climate change and identifies serious risks to ongoing scientific and commercial pursuit of such technologies.
For several applications there are advantages in writing turbulent flow equations in a coordinate frame aligned with the streamlines and several two-dimensional examples of this approach have appeared in the literature. In this paper, we extend this approach to general three-dimensional flows. We find that, in any flow that has a component of its vorticity aligned in the streamline direction, congruences of its streamlines do not form integrable manifolds. This limits the development of a streamline coordinate description of such flows, although some useful results can still be obtained. However, in the case of general three-dimensional complex-lamellar flows, where the mean velocity and mean vorticity are everywhere orthogonal, a complete streamline coordinate description can be derived. Furthermore, we show that general complex-lamellar flows are a good approximation to boundary layers and thin free shear layers. We derive the underlying true coordinate system for such flows, where the orthogonal coordinate surfaces are two stream surfaces and a modified potential surface. From this we obtain physical equations, where flow variables have the same dimensions they would have in a Cartesian coordinate frame. Finally, we show that rational approximations to these equations, which describe small-perturbation flows, contain some terms that have been ignored in previous applications and we detail some practical applications of the theory in modelling and analysis.
The discourse of tragedy has significant value in a military context, reminding us of the temptations of hubris, the prevalence of moral dilemmas, and the inescapable limits of foresight. Today, however, this discourse is drawn upon too heavily. Within the tragicized politics of nuclear and drone violence, foreseeable and solvable problems are reconceptualized as intractable dilemmas, and morally accountable agents are reframed as powerless observers. The tragedy discourse, when wrongly applied by policymakers and the media, indulges the very hubris the tragic recognition is intended to caution against. This article clarifies the limits of “tragedy” in the context of military violence and argues for a renewed focus on political responsibility.
This paper investigates the impact of corporate expansion in the alcohol retail industry on small-scale retailers. Our empirical approach exploits the timing of corporate entries across U.S. states that do not have a state monopoly to examine entry effects based on proximity to incumbent retailers. The analysis, drawing on a comprehensive dataset of U.S. alcohol retailers from 2000 to 2020, reveals that corporate entries within a 1-mile radius positively impact the employment and revenue of nearby small-scale retailers, with these effects intensifying over time and being more pronounced in metropolitan markets. Despite these localized impacts, the overall market structure and firm behavior remain largely unaffected.
In this article I analyze Salvador Allende’s economic program and policies. I argue that the explosion of inflation during his administration (above 1,500% on a six-month annualized measure) was predictable, and I show that the government’s response to it was political. I postulate that runaway inflation generated major disaffection among the middle class and that that unhappiness paved the way to Pinochet’s coup d’état in 1973.
Cannabis use and familial vulnerability to psychosis have been associated with social cognition deficits. This study examined the potential relationship between cannabis use and cognitive biases underlying social cognition and functioning in patients with first episode psychosis (FEP), their siblings, and controls.
Methods
We analyzed a sample of 543 participants with FEP, 203 siblings, and 1168 controls from the EU-GEI study using a correlational design. We used logistic regression analyses to examine the influence of clinical group, lifetime cannabis use frequency, and potency of cannabis use on cognitive biases, accounting for demographic and cognitive variables.
Results
FEP patients showed increased odds of facial recognition processing (FRP) deficits (OR = 1.642, CI 1.123–2.402) relative to controls but not of speech illusions (SI) or jumping to conclusions (JTC) bias, with no statistically significant differences relative to siblings. Daily and occasional lifetime cannabis use were associated with decreased odds of SI (OR = 0.605, CI 0.368–0.997 and OR = 0.646, CI 0.457–0.913 respectively) and JTC bias (OR = 0.625, CI 0.422–0.925 and OR = 0.602, CI 0.460–0.787 respectively) compared with lifetime abstinence, but not with FRP deficits, in the whole sample. Within the cannabis user group, low-potency cannabis use was associated with increased odds of SI (OR = 1.829, CI 1.297–2.578, FRP deficits (OR = 1.393, CI 1.031–1.882, and JTC (OR = 1.661, CI 1.271–2.171) relative to high-potency cannabis use, with comparable effects in the three clinical groups.
Conclusions
Our findings suggest increased odds of cognitive biases in FEP patients who have never used cannabis and in low-potency users. Future studies should elucidate this association and its potential implications.
To investigate the association between food insecurity (FI) and diet quality in private sector service workers.
Design:
Data were collected via electronic questionnaires (2019) and the national register data (2018–2019). FI was measured using the Household Food Insecurity Access Scale (HFIAS) and diet quality using an FFQ and a modified Healthy Food Intake Index (mHFII). The associations between HFIAS and mHFII were studied using ANOVA and ordinal regression analysis.
Setting:
Cross-sectional survey and register data for all municipalities in Finland in 2018–2019.
Participants:
Individuals (n 6435) belonging to the Finnish Service Union United. The members are predominantly women and work mainly in retail trade, tourism, restaurant and leisure services, property maintenance and security services.
Results:
Overall diet quality, measured by mHFII, was significantly lower in those experiencing severe FI than in those who were food secure (8·0 v. 9·1). Additionally, those with severe FI were less likely to have higher (more optimal) scores in sugar-sweetened beverages (OR: 0·67), fibre-rich grains (OR: 0·79), vegetables (OR: 0·54), fruits and berries (OR: 0·61), vegetable oil (OR: 0·80), fish (OR: 0·65), milk (OR: 0·89) and nuts and seeds (OR: 0·66) than food-secure participants. Severe FI was associated with higher odds for less frequent consumption of red and processed meat (OR: 1·15, a higher score represents less frequent consumption).
Conclusions:
Severe FI was linked to both lower overall diet quality and suboptimal consumption of several food groups. Individuals experiencing severe FI may be predisposed to accumulating dietary risk factors for chronic diseases.
Mass Casualty Incidents (MCIs) pose significant challenges to health care systems, especially regarding emergency preparedness and response. This study aims to analyze the epidemiological characteristics and burden of MCIs in Spain from 2014 to 2022, focusing on the type, frequency, and impact of these incidents on public health and emergency services.
Methods
A population-based retrospective observational study examined MCIs in Spain between January 2014 and December 2022. Data were collected from various emergency services. Incidents involving 4 or more victims requiring medical assistance and ambulance mobilization were included. The study categorized MCIs into 5 types: road traffic accidents, fires and explosions, chemical poisonings, maritime accidents, and others.
Results
A total of 1618 MCIs resulting in 8556 victims were identified, averaging 15 (95% CI, 11-19) incidents per month, with 79% due to road traffic accidents and 13% to fires and explosions, which also had the highest average of 7.6 victims per incident. Despite maritime accidents comprising only 1.9% of incidents, they had the highest fatality rate. MCIs were more frequent on weekends, in January and July, and between 3:00 PM and 9:00 PM. The average response time was 38 minutes, with 35% of victims sustaining severe injuries.
Conclusions
Despite a slight decrease in annual MCIs from 2014 to 2022 in Spain, the trend is not statistically significant. The study highlights the need for a national registry and standardized data collection to enhance emergency preparedness and response planning and facilitate the reduction of the MCI burden.
The identified victim effect is the phenomenon in which people tend to contribute more to identified than to unidentified victims. Kogut and Ritov (Journal of Behavioral Decision Making, 18(3), 157–167, 2005) found that the identified victim effect was limited to a single victim and driven by empathic emotions. In a pre-registered experiment with an online U.S. American MTurk sample on CloudResearch (N = 2003), we conducted a close replication and extension of Experiment 2 from Kogut and Ritov (Journal of Behavioral Decision Making, 18(3), 157–167, 2005). The replication findings failed to provide empirical support for the identified single victim effect hypothesis since we found no evidence of differences in willingness to contribute when comparing a single identified victim to a single unidentified victim (η2p = .00, 90% CI [0.00, 0.00]), and no indication for the target article’s interaction between singularity and identifiability (original: η2p = .062, 90% CI [0.01, 0.15]; replication: η2p = .00, 90% CI [0.00, 0.00]). Extending the replication to conduct a conceptual replication of Kogut and Ritov (Organizational Behavior and Human Decision Processes, 104(2), 150–157, 2007), we investigated a boundary condition of the effect—group belonging. We found support for an ingroup bias in helping behaviors and indications for empathic emotions and perceived responsibility contributing to this effect. We discuss differences between our study and the target article and implications for the literature on the identified victim effect.
During the Cold War, logical rationality – consistency axioms, subjective expected utility maximization, Bayesian probability updating – became the bedrock of economics and other social sciences. In the 1970s, logical rationality underwent attack by the heuristics-and-biases program, which interpreted the theory as a universal norm of how individuals should make decisions, although such an interpretation is absent in von Neumann and Morgenstern’s foundational work and dismissed by Savage. Deviations in people’s judgments from the theory were thought to reveal stable cognitive biases, which were in turn thought to underlie social problems, justifying governmental paternalism. In the 1990s, the ecological rationality program entered the field, based on the work of Simon. It moves beyond the narrow bounds of logical rationality and analyzes how individuals and institutions make decisions under uncertainty and intractability. This broader view has shown that many supposed cognitive biases are marks of intelligence rather than irrationality, and that heuristics are indispensable guides in a world of uncertainty. The passionate debate between the three research programs became known as the rationality wars. I provide a brief account from the ‘frontline’ and show how the parties understood in strikingly different ways what the war entailed.
The Atlantic Forest is one of the most threatened biomes globally. Data from monitoring programs are necessary to evaluate the conservation status of species, prioritise conservation actions and to evaluate the effectiveness of these actions. Birds are particularly well represented in citizen-collected datasets that are used worldwide in ecological and conservation studies. Here, we analyse presence-only data from three online citizen science datasets of Atlantic Forest endemic bird species to evaluate whether the representation of these species was correlated with their global threat status, range and estimated abundance. We conclude that even though species are over- and under-represented with regard to their presumed abundance, data collected by citizen scientists can be used to infer species distribution and, to a lesser degree, species abundance. This pattern holds true for species across global threat status.
“Imperial Crucible” tells the story of the Aluminum Company of America (Alcoa) from the company’s founding in Pittsburgh in 1888 through the 1950s. Although scholars have long contended that American multinational corporations played a pivotal role in the industrialization of the United States, the building of a global working class, and the transformation of European empires, they have tended to see these stories as distinct, rather than interconnected. In contrast, Imperial Crucible focuses on a single firm to draw together the political-economic, working-class, and imperial history of American business. What the industrialists behind Alcoa built, I argue, was not a multinational but a transimperial corporation.
