The simultaneous existence of low-value health care and underutilization of high-value care are global problems. Health technology reassessment (HTR) aims to optimize the value for money of technologies already in use within health care. Identifying candidate interventions for HTR remains challenging. Therefore, we tested a novel method to identify candidate outpatient prescription drugs for HTR through practice variation.
Methods
We used administrative data for all publicly funded outpatient prescriptions dispensed to persons aged 65 or older in Alberta in 2023. Through quantitative comparison of funnel plots for Anatomic Therapeutic Chemical (ATC) classes at the fourth level stratified by prescriber specialty, variation in prescription dispensation rates between prescribers was used to estimate three outcomes: the number of prescribers affected, the number of patients affected, and the potential budgetary impact. We ranked combinations of ATC class and prescriber specialty in descending order for each outcome, with use above and below the mean considered separately.
Results
We analyzed data on 17.5 million dispensations, encompassing more than 8,000 prescribers and approximately 600,000 patients. The top ATC class–prescriber specialty combinations for each outcome showed high similarity above and below control limits while exhibiting minimal overlap between outcomes.
Conclusions
Our method successfully identified ATC class–prescriber specialty combinations with marked variation in use, for potential advancement through the HTR process. Depending on the perspective of those undertaking HTR of prescription drugs, different outcomes may be useful in technology prioritization. To make the ATC class–prescriber specialty combinations actionable, future efforts should focus on exploring the patients affected.
The global mental health (GMH) field aims to equitably improve mental health and well-being everywhere. This article reviews persistent common challenges hindering sustained, high-quality delivery of mental health and psychosocial support (MHPSS). Our focus is on programming that is funded or implemented by external organizations, typically universities or international non-governmental organizations from high-income countries. It is a consensus statement of MHPSS practitioners, programmers and researchers working for these organizations and some who are locally based who observe these programs in action. We comment on progress to date, barriers and recommendations for change and the importance of promoting sustained integration of MHPSS into health and social service systems through a comprehensive, recovery-oriented system of care. We call for prioritizing often-neglected issues (e.g., stigma, severe mental health conditions and neurodevelopmental conditions), strengthening workforce training and supervision and monitoring and evaluation systems to ensure program quality. The continued dominance of the Global North in shaping GMH programming priorities remains a concern. We advocate for a greater involvement of local workers and communities in agenda-setting for programs, culturally grounded implementation and long-term capacity building. Evidence-based practices must be met with contextual relevance, and comprehensive guidelines for sustained support are needed for development settings. For persistent funding challenges, we recommend clearer funder objectives, investment in in-house mental health expertise and funder coordination with prioritization of complementary programming. These recommendations are essential to realizing equitable, comprehensive, evidence-based and contextually grounded GMH programming.
Early Positive Approaches to Support (E-PAtS) is a co-produced group intervention supporting family carers of children (0–5 years) with additional developmental needs. This study compared online (n = 10) and in-person (n = 11) groups to investigate whether mode of delivery was associated with different outcomes. Participants were 98 family carers reporting on their mental well-being, self-efficacy, child symptoms and knowledge pre and post intervention. Generalised estimating equations compared outcomes between groups, controlling for group cluster effects.
Results
Mental well-being improved significantly across both groups (d = 0.47, 95% CI: 0.30, 0.63), as did an in-session measure concerning mechanisms of change (d = 1.28, 95% CI: 0.97, 1.59) and all other assessed outcomes. There were no significant differences in measured outcomes between online and in-person groups.
Clinical implications
Establishing the equivalence of in-person with online groups is an important first step for improving service reach and support access for families of children with additional developmental needs.
The recent conflict in the Middle East posed unprecedented threats, with hundreds of long-range ballistic missiles launched toward Israel, targeting military and civilian facilities including hospitals. Organizational and logistic actions were taken in Sheba Medical Center, both pre-emptively and during the conflict, preparing for a mass casualty incident while maintaining routine medical care to the population and maintaining safety of patients and staff. These included discharging patients and increasing home hospitalizations, ward evacuations, transferring patients to protected areas, and classifying patients by their vulnerability and status of protection, accelerated structural adaptation of underground spaces to house patients, and construction of a tented field hospital underground. An effective command and control system was in place to monitor protective status, and an ethical committee was convened to assist in decision-making. These measures enabled continued delivery of emergency and medical care under fire while ensuring the safety of patients and staff.
The crystal structure of resmetirom heminonahydrate Form CSI has been solved and refined using synchrotron X-ray powder diffraction data and optimized using density functional theory techniques. Form CSI had been described previously as a dihydrate. Resmetirom heminonahydrate Form CSI crystallizes in space group P-1 (#2) with a = 11.3094(23), b = 15.158(6), c = 16.570(7) Å, α = 67.405(13), β = 74.425(7), γ = 69.526(7)°, V = 2,427.2(4) Å3, and Z = 4 at 298 K. The crystal structure consists of layers of resmetirom molecules parallel to the bc-plane. These layers are separated by water-rich layers also parallel to the bc-plane. A strong N–H···O links the two resmetirom molecules. The equivalent amino group in the other molecule acts as a donor to a water molecule. A number of C–H···O and C–H···N hydrogen bonds also contribute to the lattice energy. Water molecules act as donors to both O and N in the resmetirom molecules. The structure is more complicated than a hydrogen-bonded framework of resmetirom molecules with water in the pores. The powder pattern has been submitted to the International Centre for Diffraction Data (ICDD) for inclusion in the Powder Diffraction File™ (PDF®).
The weekly iron and folic acid supplementation (WIFAS) programme is a school-based initiative designed to reduce iron and folate deficiency anaemia among adolescent girls. In Ethiopia, donor-supported pilot programmes are implementing WIFAS in schools, but evaluations of its demand- and supply-side barriers and facilitators remain limited. This study aimed to explore these barriers and facilitators in the Sidama Region of Ethiopia. An exploratory qualitative study collected data from ten WIFAS-targeted schools using key informant interviews (KIIs) and focus group discussions (FGDs). Participants included purposively selected school directors, WIFAS-trained teachers, health centre heads, district health and education office nutrition focal points, and regional health and education bureau delegates. Ten FGDs were conducted with adolescent girls and their mothers. Thematic analysis was performed using Open Code software to identify emerging themes. This study identified low awareness of iron and folic acid (IFA) tablets, negative community perceptions, fear of side effects, supplementation interruptions due to school closures, and inadequate coverage as key barriers. Structural barriers included poor programme coordination, weak supply chain management, and water scarcity. Facilitators included free IFA tablet distribution, health extension workers’ involvement in awareness campaigns, positive testimonials, local leader support, training, and regular supervision. The WIFAS programme implemented in schools of the Sidama Region faces programmatic and structural barriers. However, facilitators like free IFA distribution, health extension worker involvement, and community support offer opportunities for improvement. These findings highlight the need for addressing barriers while leveraging existing facilitators for enhancing programme success.
The actuator line model (ALM) is an approach commonly used to represent lifting and dragging devices like wings and blades in large-eddy simulations (LES). The crux of the ALM is the projection of the actuator point forces onto the LES grid by means of a Gaussian regularisation kernel. The minimum width of the kernel is constrained by the grid size; however, for most practical applications like LES of wind turbines, this value is an order of magnitude larger than the optimal value that maximises accuracy. This discrepancy motivated the development of corrections for the actuator line, which, however, neglect the effect of unsteady spanwise shed vorticity. In this work we develop a model for the impact of spanwise shed vorticity on the unsteady loading of an aerofoil modelled as a Gaussian body force distribution, where the model is applicable within the regime of unsteady attached flow. The model solution is derived both in the time and frequency domain and features an explicit dependence on the Gaussian kernel width. We verify the model with ALM-LES for both pitch steps and periodic pitching. The model solution is compared withTheodorsen theory and validated with both computational fluid dynamics using body fitted grids and experiment. It is concluded that the optimal kernel width for unsteady aerodynamics is approximately $40\,\%$ of the chord. The ALM is able to predict the magnitude of the unsteady loading up to a reduced frequency of $k\approx 0.2$.
We report experiments in a long tank showing that transverse Benjamin–Feir instability of Stokes waves can lead to a significant energy transfer into oscillations across the tank. We observe frequency downshift in the long-term evolution of Stokes waves essentially when significant energy is transferred to narrow-banded transverse modes. Experiments with Stokes waves are often carried out with wavelengths that are not long compared with the width of the tank, permitting transverse instabilities to be excited. With insufficient resolution of measurements across the tank, transfer of energy to transverse modes can be misinterpreted as dissipation. Our experiments suggest that the frequency downshift depends as much on energy-preserving transverse modulations of type I as it does on damping or wave breaking. Broad-banded unstable modulations of type II do not imply downshift.
Evidence suggests that psychological preparedness is associated with better physical disaster preparation, lower mental health impacts, and improved recovery efforts following a disaster. This analysis provides preliminary insights into factors that may be associated with better psychological preparedness and explores the relationship between psychological preparedness and mental health among Australian rural residents.
Methods
A cross-sectional survey design was used to collect data regarding factors associated with Psychological Preparedness.
National online study where participants were community members from MMM1 to MMM6 (N = 388). This study used the Awareness, Anticipation and Management (AAM) subscale from the Psychological Preparedness for Disaster Threat Scale (PPDTS).
Results
Individuals who live in an Australian rural area were 11.59 times more likely to have experienced a disaster, compared to those in a metropolitan area. Significant associations were found between higher PPDTS scores and male sex, age, rural location, having experienced a disaster, and specific physical preparedness items. Individuals with elevated levels of depression, anxiety, and stress had statistically significantly lower PPDTS scores.
Conclusions
Investment in psychological preparedness for natural disasters may assist in preventing mental health exacerbation post disasters. This is particularly important in rural communities, which are more likely to experience disasters but have limited mental health support.
Despite omics technologies gaining traction in clinical settings, particularly in oncology, challenges persist in their widespread adoption due to the pre-requisite robust evidence supporting efficacy and cost-effectiveness. This study aims to explore the experiences of organizations working in the health technology assessment (HTA) field in evaluating omics technologies, with a particular focus on the adoption and application of specific assessment frameworks.
Methods
We conducted a global survey to gather insights into current practices and frameworks used in HTA evaluations of omics technologies.
Results
We gathered responses from thirty-nine participants representing organizations across twenty-nine countries and five continents. Among them, 51 percent (n = 20) reported experience in evaluating omics technologies, including multi-omics tests for early disease detection, biomarker-based cancer diagnostics, and advanced genomic sequencing techniques. Only three organizations employed specific assessment frameworks: the Adelaide Health Technology Assessment Agency in Australia, the Netherlands Cancer Institute, and the Andalusian HTA Agency in Spain. These frameworks address key evaluation aspects such as analytical and clinical validity, clinical and personal utility, organizational impact, and ethical, legal, and social implications of omics technologies.
Discussion
Despite their relevance, the limited adoption of tailored frameworks highlights the need for more structured and context-specific approaches to facilitate the integration of omics technologies into healthcare systems. Collaborative efforts among stakeholders, including patients, healthcare providers, policymakers, and industry representatives, are crucial for devising robust evaluation strategies addressing the complexities of omics technologies comprehensively.
In this seminal study, Peter J. Katzenstein drags the analysis of world politics from the Newtonian humanism of the nineteenth century into a new post-Newtonianism of the twenty-first. The key concept is entanglement. By examining differences in context, process, and language, Katzenstein specifies how risk and uncertainty intertwine. Three deeply researched case studies – finance and political economy, nuclear crisis politics and war, and global warming and AI – support his original arguments. A chapter on power further illustrates the risk-uncertainty conundrum. Entanglements in World Politics calls for humility and eclectic pragmatism, emphasizing the unity of knowledge of the natural and humanistic sciences and the complementarities of science and religion. Katzenstein's engaging writing and innovative approach make this a must-read for anyone interested in the complexities of global politics. Following Russia's annexation of Crimea in 2014, the recent conflicts in Ukraine and the Middle East signals the return of geopolitics. This book challenges conventional approaches that ignore border change, arguing that geopolitics is driven by nationalism and focusing on how nationalism transforms the state. Using geocoded historical maps covering state borders and ethnic groups in Europe, the authors' spatial approach shows how, since the French Revolution, nationalism has caused increasing congruence between state and national borders and how a lack of congruence increased the risk of armed conflict. This macroprocess is traced from early modern Europe and widens the geographic scope to the entire world in the mid-twentieth century. The analysis shows that the risk of conflict may be increased by how nationalists seeking to revive past golden ages and restore their nations' prestige respond to incongruent borders. 'In this rich history of everyday encounters between US soldiers and Chinese civilians, Chunmei Du explores their entangled relations from the end of World War II to the founding of the People's Republic of China. Drawing upon official, popular and personal accounts from both countries, Du examines the sensorial, material, and symbolic exchanges that took place between GIs and ordinary Chinese people-stall vendors, pedestrians, rickshaw pullers, 'Jeep girls,' and suspected thieves. Using ethnographic material of new parents’ encounters with welfare workers during the process of claiming and receipt of universal family entitlements in Denmark and Romania, this article proposes the concept of bureaucratic translation. Drawing on Latourian conceptual foundations, we show that the communication of bureaucratic information is not only symbolically loaded, but invariably in need of ‘translation’. We highlight five interrelated processes of meaning-making parents have to engage in. Despite the universalism of entitlements, parents experience information offered by welfare workers as specialised knowledge that they should not legitimately be expected to have good command of. Their contestation stems from the tension between the helping ethos of universalist programmes and the inadequacies and insufficiencies of bureaucratic information offered. Bureaucratic translation illuminates the complexities of ‘learning costs’ underpinning administrative burden from citizens’ perspective, flagging difficulties even for the bureaucratically least challenging social programmes.

We present a Late Pleistocene paleoecological record from King Island in western Bass Strait, Tasmania, and compare this to existing records from the eastern Bass Strait islands to improve our understanding of the region’s paleoecology and paleoclimatology. Vegetation change across the region followed similar trajectories during the late glacial–Middle Holocene, characterized by homogeneous warming and wetting trends. Spatial divergence occurred during the Middle Holocene when sea level rose, and different drivers began influencing western and eastern Bass Strait islands. In eastern Bass Strait, Middle Holocene sea-level rise caused replacement of woodland by coastal heathland, while in the west, a drier period accompanied by fires transformed forests to forest–scrub. The comparative analysis suggests that Westerly driven climatic anti-phasing was pronounced at higher latitudes of Tasmania during the late glacial–Early Holocene. A combination of weak Leeuwin Current, positive Indian Ocean Dipole (IOD), and El Niño–Southern Oscillation (ENSO) intensification contributed to Middle Holocene aridity across Bass Strait. Strong Westerlies and negative IOD phases led to greater regionalization of rainfall across western Bass Strait during the Late Holocene, while ENSO intensification drove rainfall declines in eastern Bass Strait. These findings provide new insights into the complexity of Late Pleistocene environmental dynamics across southeast Australia.
