Individual responses to stress are highly heterogeneous, resulting in diverse psychopathological outcomes. This variability poses challenges for traditional diagnostic frameworks and underscores the need for a transdiagnostic approach to guide interventions. This study aimed to identify distinct phenotypes within a stress-exposed population and to characterize their biological profiles using a multimodal machine learning framework.
Methods
A total of 809 stress-exposed adults (mean age 40.5 ± 8.74 years; 53.7% female) underwent clinical, laboratory, and structural MRI assessments. Data-driven clustering of clinical variables identified phenotypes, followed by machine learning classifiers trained on neuroimaging and laboratory data to predict phenotype membership. SHapley Additive exPlanations (SHAP) analysis was used to identify key biological features distinguishing each phenotype.
Results
Three phenotypes were identified: a multi-risk group (n = 321) characterized by prominent depression, anxiety, and sleep disturbances; an alcohol-related risk group (n = 226) with high alcohol misuse and minimal comorbidity; and a resilient low-risk group (n = 262). Machine learning models accurately classified these phenotypes, indicating distinct biological profiles. SHAP analysis revealed phenotype-specific signatures: the multi-risk phenotype was associated with frontal-subcortical structural alterations and dysregulated cortisol, whereas the alcohol-related risk phenotype was characterized by frontal-insular structural alterations and metabolic abnormalities.
Conclusions
This study demonstrates the stratification of stress-exposed individuals into clinically and biologically distinct phenotypes. By integrating multimodal data with machine learning, we identified phenotype-specific neurobiological and metabolic profiles that extend beyond conventional diagnostic frameworks. These findings support a transdiagnostic, data-driven approach to improve risk stratification and inform personalized interventions in stress-exposed populations.
Substances, such as cannabis, alcohol, and nicotine, have each been associated with an increased risk of mental health concerns in adolescence. These substances are often taken concurrently and less is known about the effects of polysubstance use. We investigated the relationship between polysubstance use and psychopathology in adolescence including internalising, externalising behaviours, and psychotic experiences (PE). As a secondary analysis, we examined gender differences.
Methods:
A sample of 4,404 15–16-year-old adolescents in 40 schools in Ireland was examined. Our exposure was single substance use (cannabis, alcohol, and nicotine) and polysubstance use within the past month. The outcome of interest was psychopathology, measured using the Strengths & Difficulties Questionnaire, and the Adolescent Psychotic Symptoms Scale. Associations were explored using mixed-effects logistic models.
Results:
Adjusted odds ratios were calculated to examine the associations between polysubstance use and each outcome, compared to substance-naive participants. A significant dose response relationship was observed, with increasing substance use associated with higher odds of general psychopathology (aOR range: 1.65–5.09), internalising behaviours (aOR range: 1.18–2.06), externalising behaviours (aOR range: 1.68–5.79), and PE (aOR range: 1.60–3.13). Females engaging in polysubstance use exhibited the poorest mental health outcomes.
Conclusion:
This study highlights the strong associations between adolescent polysubstance use and emerging mental health problems. The findings reinforce the need for mental health services to take polysubstance use into consideration during intervention. Tailored public health messaging is required, acknowledging polysubstance use as a marker for poor mental health among adolescents and young adults.
Predicting and controlling the transport of colloids in porous media is essential for applications ranging from contaminant remediation to drug delivery. In these complex environments, solute gradients are ubiquitous and could drive diffusiophoretic particle migration, yet their impact on macroscopic colloid dispersion remains poorly understood. Here we combine experiments and simulations to quantify how diffusiophoresis alters the spreading of a colloidal blob in a two-dimensional ordered/disordered porous medium. A joint blob of colloids and salt at high concentration is introduced into a medium filled with salt at low concentration and advected by a background flow. Intuition suggests that when colloids are attracted towards or repelled from the solute-rich blob, dispersion should be suppressed or enhanced, respectively. Instead, we observe the opposite trend: longitudinal dispersion is enhanced in the attractive case, whereas dispersion is suppressed in the repulsive case. Numerical simulations reveal that this striking reversal arises from diffusiophoretic exchange of particles between slow and fast streamlines, which we capture using a minimal two-layer model of coupled fast and slow plug flows. Finally, we probe how geometric disorder in the medium modulates this mechanism. Our results demonstrate that diffusiophoresis can strongly modulate macroscopic dispersion of colloids in porous media with implications for transport in subsurface and biological environments.
Despite evidence of disparities in care received by Black communities, limited interventions exist to address them. The SEE ME training is a co-produced intervention using video testimonials to improve the care of Black individuals with psychosis within NHS early intervention in psychosis (EIP) services. This study explored mental health professionals’ experiences of the SEE ME training and its impact on addressing racial inequalities within EIP services. Semi-structured interviews were conducted with 21 mental health professionals, and the data were analysed with thematic analysis.
Results
Thematic analysis identified six superordinate themes and 17 subthemes, encompassing the importance of adopting clients’ perspectives, prioritising individuals’ voices, enabling reflection, fostering shared humanity and creating psychological safety.
Clinical implications
This is the first qualitative study exploring the impact of SEE ME training. Findings offer valuable insights for improving anti-racist practice and service development in mental healthcare.
We study the reverse mathematics of characterization theorems of regular countable second countable (CSC) spaces. We prove that arithmetic comprehension is equivalent over $\mathbf {RCA}_0$ to every $T_3$ CSC space being metrizable, and we characterize the $T_3$ spaces which are metrizable over $\mathbf {RCA}_0$. We show that Lynn’s theorem for CSC spaces can be carried out in $\mathbf {ACA}_0$, namely that every zero-dimensional separable space is homeomorphic to the order topology of a linear order. We also show that arithmetic comprehension is equivalent to every $T_2$ compact CSC space being well-orderable. From general topology, we know that the locally compact $T_2$ CSC spaces are the well-orderable CSC spaces, and that the $T_3$ scattered CSC spaces are the completely metrizable CSC spaces. We show that these characterizations and a few others are equivalent to arithmetic transfinite recursion over $\mathbf {RCA}_0$. We also find a few statements that are equivalent to $\Pi ^1_1$ comprehension. In particular we show that every $T_3$ CSC space has a Cantor–Bendixson rank and that every $T_3$ CSC space is the disjoint union of a scattered space and dense in itself space are equivalent to $\Pi ^1_1$ comprehension over $\mathbf {RCA}_0$.
To investigate whether socioeconomic deprivation is associated with residual or recurrent cholesteatoma following primary mastoidectomy.
Methods
A retrospective cohort study included all patients undergoing primary mastoidectomy for cholesteatoma in National Health Service Tayside between 2012 and 2020. Socioeconomic status was assessed using the Scottish Index of Multiple Deprivation 2020 and divided into quintiles. Patients were stratified into paediatric (≤16 years) and adult (>16 years) groups. Recurrence/residual rates across Scottish Index of Multiple Deprivation quintiles were compared using Fisher’s exact tests, and time to recurrence was analysed using Kaplan–Meier survival curves.
Results
One hundred eighty-four patients were included (59 children, 125 adults). Recurrence/residual occurred in 39 per cent of children and 24 per cent of adults. In children, higher deprivation was significantly associated with risk of recurrence/residual disease (p = 0.0013). No significant association was observed in adults.
Conclusion
Socioeconomic deprivation is linked to higher recurrence/residual in children, highlighting disparities in postoperative outcomes and the importance of targeted follow-up.
This article reads the post-moratorium boom in historical whaling fiction against shifting debates over the legitimacy of Japanese whaling. Taking Itō Jun’s Kyogei no umi (2013) as example, I draw on critical heritage studies and Svetlana Boym’s theory of nostalgia to show how the novel reconstructs Edo-period whaling as a morally coherent, patriarchal community built on self-sacrifice and masculine heroism. In doing so, the novel aligns with cultural-nationalist framings of whaling, enhancing its symbolic value as a marker of Japanese identity. The article thus shows how such narratives participate in the discursive work that sustains whaling in contemporary Japan.
Superswells and domes – large-scale, persistent interface deformations – are key features in geophysical and multiphase systems driven by thermal convection. These structures often emerge in layered convective systems, such as Earth’s mantle or two-fluid industrial processes, where buoyancy contrasts and fluid properties create complex interface dynamics. However, the physical mechanisms governing the formation of these structures remain poorly understood. Here, we use high-resolution direct numerical simulations of two-layer Rayleigh–Bénard convection to investigate how the Prandtl number ($\textit{Pr}$) and buoyancy number ($B$) govern transitions in interfacial morphology. At a fixed Rayleigh number ($\textit{Ra}$), we identify three distinct regimes: stratified convection at high $B$, where compositional buoyancy suppresses interfacial motion; an interfacial fragmentation regime at low $\textit{Pr}$ and $B$, driven by inertial–thermal buoyant balance; and a transitional dome-forming regime within a $\textit{Pr}$-dependent range, where viscous and buoyant forces deform the interface into coherent, superswell-like structures. A theoretical phase diagram predicts these transitions and agrees well with simulation results. These findings provide mechanistic insight into interfacial phase behaviour in thermally driven flows, offering a framework that links geophysical surface features with underlying convective dynamics and informing the control of interfacial morphology in complex multiphase systems.
We introduce action-driven flows for causal variational principles, being a class of nonconvex variational problems emanating from applications in fundamental physics. In the compact setting, Hölder continuous curves of measures are constructed by using the method of minimizing movements. As is illustrated in examples, these curves will in general not have a limit point, due to the nonconvexity of the action. This leads us to introducing a novel penalization which ensures the existence of a limit point, giving rise to approximative solutions of the Euler-Lagrange equations. The methods and results are adapted and generalized to the causal action principle in the finite-dimensional case. As an application, we construct a flow of measures for causal fermion systems in the infinite-dimensional situation.
Adults with intellectual disabilities who display behaviours that challenge (BtC) are more prone to poor health.
Aims
This study seeks to evidence the long-term health outcomes for those who display BtC.
Method
We conducted a longitudinal cohort study of adults with intellectual disabilities aged ≥18 years in England, using data from the Clinical Practice Research Datalink Aurum (January 2003 to December 2023) linked to Hospital Episode Statistics and Office for National Statistics. Main outcome measures were annual health checks, general practitioner referrals, emergency visits, out-patient attendance, in-patient admissions and mortality.
Results
Among 83 166 adults with intellectual disabilities (mean age 38.6 years), 18.5% had a record of BtC, with similar sociodemographic distributions to those without BtC, but higher rates of physical and mental health comorbidities and uptake of annual health checks. A total of 72.5% of participants with BtC were receiving psychotropic medication(s). Adults with BtC had higher rates of mental health out-patient attendance (odds ratio: 1.42, 95% CI 1.33–1.52) and in-patient admissions (incidence rate ratio (IRR): 1.19, 95% CI 1.09–1.29), but consistently lower rates of physical health out-patient attendance (IRR = 0.81, 95% CI 0.78–0.84) and in-patient admissions (IRR = 0.77, 95% CI 0.74–0.79), after adjusting for demographic and clinical characteristics. BtC were not associated with mortality after adjustment for comorbidities (hazard ratio: 0.97, 95% CI 0.93–1.00).
Conclusions
This longitudinal study not only corroborated the markedly elevated burden of physical and mental health comorbidities among individuals displaying BtC, but also indicated that repeated efforts to improve health outcomes have yielded minimal measurable benefit over time. The apparent absence of progress is likely underpinned by a combination of insufficiently effective or poorly tailored interventions and wider systemic constraints that limit the capacity of services to respond to the complex needs of this population.
Classical capillary–gravity wave theories incorporate surface tension through the free-surface dynamic boundary condition but assume a flat equilibrium interface, thereby excluding the spatially varying geometry of equilibrium menisci. Motivated by experiments on meniscus-modified wave transmission past surface-piercing barriers with pinned contact lines, we develop a linearised two-dimensional framework incorporating the equilibrium meniscus profile into wave propagation and scattering. Linearisation about a prescribed meniscus yields modified kinematic and dynamic boundary conditions with spatially varying coefficients determined by the local meniscus height, slope and curvature. Solved via the finite-element method, the model captures observed variations in transmission as a function of meniscus height and contact angle. For a 15 Hz wave in deep water, the model shows that the transmission coefficient varies on the order of 0.1 for each 1 mm increase in capillary rise, provided the meniscus remains single valued. Computed energy-flux fields provide a physical interpretation: depressed menisci redirect energy away from the barrier, while raised menisci guide energy through the elevated fluid region to enhance coupling. Local dispersion analysis further clarifies how meniscus geometry influences phase evolution and wave structure. This framework establishes a consistent free-surface formulation for waves on spatially varying interfaces, providing insight into wave–meniscus coupling and enabling future extensions to nonlinear, three-dimensional and multi-valued configurations.
As an increasing number of psychiatrists complete training in the UK and move into post-qualification specialist practice in Canada, challenges can arise with adaptation and professional recognition. In this reflection, we examine our experiences over the past 5 years working in and delivering undergraduate and postgraduate medical education in Canada. Patients recognise the unique value of UK psychiatry training; however, this training can also present challenges. We explore some of these issues across the domains of undergraduate medical education, postgraduate psychiatry education and post-qualification specialist practice, using a comparative narrative.
Higher-education institutions have long relied on a broad range of internal and external stakeholders, including academic staff, administrators, governmental representatives, donors and industry. In recent decades, this stakeholder landscape has diversified, driven by higher-education reforms that have intensified competition, strengthened accountability and heightened expectations that universities respond to societal needs. At the same time, geopolitical tensions and growing societal distrust in academic institutions have underscored the need to reassess how stakeholders shape governance and strategic directions of universities. Against this backdrop, this article examines the changing role of stakeholders in higher-education governance through the lens of the stakeholder theory, focusing on their power, legitimacy and urgency. While traditional actors remain central, emerging and previously overlooked stakeholders – including students, early career academics, organized academic labour and EdTech platforms – are increasingly playing a role in higher-education decision making. The article explores the changing influence of these actors and what these shifts imply for institutional autonomy and academic freedom.
Yeast species have several adaptations that enable them to survive in harsh environments. These adaptations include biofilm formation, where the secretion of extracellular polymeric substances can protect the cells from a hostile environment, or, under nutrient-limited conditions, pseudohyphal or hyphal growth, where the colony can send out long tendrils to explore the environment and seek nutrients. Recently, we observed a spiral colony morphology emerge in an isolate of the hyphae-forming yeast Magnusiomyces magnusii (M. magnusii) grown under laboratory conditions. We use an off-lattice agent-based model (ABM) that simulates colony development to investigate the hypothesis that bias in the angle between successive hyphal segments causes the spiral morphology. The model involves biologically motivated rules of hyphal extension, with key model parameters including the colony size at the onset of hyphal filaments, and the angle between the penultimate and the apical segments. Using one example of an experimentally grown colony, we use a sequential neural likelihood method to perform likelihood-free Bayesian inference to infer the model parameters. Our results indicate a mean angle between hyphal segments of ${2.3}^{\circ } [{1.1}^{\circ }, {3.6}^{\circ }]$ (95% credible interval). To confirm the model’s applicability to colony growth, we use biologically feasible parameter values to yield morphologies observed in M. magnusii experiments.
This study aims to investigate the awareness, practices, needs, and challenges of hospital-based health technology assessment (HB-HTA) in China’s tertiary public hospitals, drawing on perspectives from both hospital management and clinical practice.
Methods
Semi-structured interviews were conducted with seventeen participants representing six major tertiary public hospitals in China. Respondents included vice presidents, clinical department directors, and heads of pharmacy. The questionnaire explored four dimensions consistent with China’s national performance appraisal framework: medical quality, operational efficiency, sustainable development, and satisfaction. Data were analyzed using NVivo 14 for qualitative responses and Excel 2021 for quantitative metrics.
Results
The findings revealed that 82.35 percent (14/17) of hospitals lacked dedicated HB-HTA teams, with decisions made primarily by coordination between Medical Affairs Offices (28.89 percent, 13/45) and Pharmacy Departments (22.22 percent, 10/45). Safety (28.26 percent, 13/46), effectiveness (23.91 percent, 11/46), and cost-effectiveness (19.57 percent, 9/46) were prioritized criteria, yet only 29.41 percent (5/17) of institutions used formal HB-HTA tools. Divergence persisted between administrators’ organizational priorities and clinicians’ patient-centered perspectives. Despite barriers reported by 62.8 percent (27/43) of respondents’ institutions, 88.24 percent (15/17) expressed an urgent or recognized need for HB-HTA implementation.
Conclusions
For effective HB-HTA implementation in China, addressing capacity deficits and reconciling stakeholder perspectives is essential. A project-based strategy, supported by government real-world data platforms and interdisciplinary teams, is recommended for contextually-appropriate evaluation frameworks.
This study explores the psychosocial consequences of recurrent environmental displacement among river-nomadic (Bede) communities in Bangladesh, emphasizing how cyclical mobility, ecological degradation, and social marginalization shape mental health and resilience. A qualitative phenomenological approach was adopted, analyzing twelve in-depth interviews and one focus group discussion from three Jamuna River settlements (2025 monsoon season) via NVivo 14. We identified six themes: income-driven relocation, environmental hazards as triggers, cumulative physical and emotional tolls, negotiation/reputation as safety mechanisms, culturally rooted coping, and aspirations for stability. Participants described chronic uncertainty alongside strong communal solidarity and faith-based coping. Findings highlight that displacement among river nomads is a continuous, multifaceted psychosocial exposure rather than an isolated event. Sustainable interventions should integrate mobile, community-based mental health and psychosocial support (MHPSS) services with environmentally informed livelihood programs and inclusive governance. This research contributes empirical evidence to the discourse on climate-related migration, emphasizing the need for culturally grounded, mobility-sensitive approaches for marginalized riverine populations.
To halt and reverse the ongoing biodiversity crisis, substantial changes in human behaviour will be required. One important challenge in achieving such changes is the ‘extinction of experience’ – the progressive loss of direct human interactions with nature (hereafter ‘personalised ecologies’). Diminished personalised ecologies can erode emotional ties to nature, weaken pro-conservation attitudes and reduce engagement in the diverse actions needed to support biodiversity. Although the extent of this decline has increasingly been documented, far less attention has been paid to the processes through which it unfolds over time, particularly across generations. In this Perspective, we examine how the extinction of experience may become entrenched within societies. We propose a conceptual framework, drawing on ideas from behavioural genetics, to explore how personalised ecologies could be transmitted across generations through genetic, environmental and interactive pathways. We then use this framework to consider why, once initiated, the extinction of experience may persist and intensify over time. Finally, we outline potential strategies to help disrupt this cycle of disconnection and to foster more positive, self-reinforcing trajectories of human–nature interactions. Achieving this will likely require coordinated efforts across sectors, including conservation organisations, education systems, urban planning and public health initiatives.
Archaeologists are increasingly integrating the analysis of ancient DNA into cemetery studies; however, while such efforts highlight relatedness and descent, biological kinship forms only one aspect of social organisation in human societies. Here, the authors demonstrate this using the Tamir necropolis in Mongolia (100 BC–AD 100) as a case study. Employing a pipeline that combines linear discriminant analysis, machine-learning classifiers and cultural phylogenetics, they interrogate the cultural and social dynamics that structured the cemetery, disentangling the influence of genetic relatedness, social status and cultural tradition and offering a powerful framework for the investigation of social organisation in ancient populations.
Let G be a finite group and let p be a prime. In this paper, we prove a strengthened version of Brauer’s height zero conjecture for the principal p-block of G that takes the action of a certain group of Galois automorphisms into account. This answers a conjecture recently proposed by Malle, Moretó, Rizo and Schaeffer Fry. We then use this to obtain a structural result which can be seen as a Galois version of the Itô–Michler theorem.
